Skull-base trauma: neurosurgical perspective.

Trauma to the cranial base can complicate craniofacial injuries and lead to significant neurological morbidity, related to brain and/or cranial nerve injury. The optimal management involves a multidisciplinary effort. This article provides the neurosurgeon's perspective in management of such trauma using a 5-year retrospective analysis of patients sustaining skull-base trauma. The salient features of anterior and middle skull-base (temporal bone) trauma are summarized, and the importance of frontal basilar trauma as well as brain injury is evident. With these injuries, all cranial nerves (exce

Lysosome-Bursting Calcium Carbonate Nanoparticles Amplify Immunogenic Cell Death via Synergistic Phototherapy and Calcium Overload for Immune Checkpoint Blockade.

OBJECTIVE: An undetectable postoperative serum cortisol has been regarded as a definition of cure in Cushing's disease. However, we noted disease recurrence amongst patients with Cushing's disease despite undetectable postoperative cortisol levels, and this led us to audit our data. We have also previously assessed surgical outcome for acromegaly and microprolactinoma for a single surgeon. The aims of this study were two-fold: (i) to investigate the treatment and surgical outcome of patients with Cushing's disease. In particular, we wished to compare the data with outcome for other pituitary t

Rare Diseases of the Orbit.

This article provides an overview of rare orbital diseases. Congenital malformations, inflammatory diseases, benign and malignant neoplasias are described. Although it represents a relatively small area of the body the orbit contains multiple different tissues. Therefore, a great variety of diseases can be found within the orbital space. That is the reason, why both the completeness and the level of detail in the description of particular diseases must be somewhat limited. Nevertheless, clinical manifestations, important aspects of diagnosis, treatment strategies, and, when specific data are a

A rare case of diffuse large B-cell lymphoma of the frontal sinus and rapid review of literature.

INTRODUCTION: Primary lymphoma of the paranasal sinuses, especially involving the frontal sinus, is exceedingly rare. Extranodal non-Hodgkin lymphoma (NHL) located in the head and neck frequently poses diagnostic challenges because its nonspecific symptoms can be mistaken for benign issues such as rhinosinusitis. AIM: This case report seeks to emphasize the challenges in diagnosis, the aggressive characteristics, and the effective treatment of an uncommon manifestation of diffuse large B-cell lymphoma (DLBCL) primarily affecting the frontal and ethmoid sinuses. CASE PRESENTATION: We present

Recurrent Mandibular Ewing sarcoma in an 11-year-old boy: a rare pediatric case and review of the literature.

RATIONALE: Mandibular Ewing sarcoma is exceptionally rare in children and frequently mimics common odontogenic infections. This overlap leads to delayed diagnosis and potentially rapid tumor progression. Early recognition and appropriate multimodal management are critical for improving outcomes. PATIENT CONCERNS: An 11.5-year-old Iranian boy of Middle Eastern ethnicity presented with progressive left mandibular swelling and pain for 1 month, unresponsive to antibiotic therapy previously prescribed for presumed dental abscess. DIAGNOSIS: Imaging (CT, MRI) revealed an aggressive destructive ma

Clinical features and outcomes of four patients with invasive fungal sinusitis.

OBJECTIVE: The frequency of invasive fungal sinusitis (IFS) has increased in recent years with the use of steroids, onset of diabetes mellitus, and the administration of antibacterial agents. We report on the clinical features and outcomes of four patients with IFS involving the cavernous sinus and orbit. Prognostic factors facilitating an early diagnosis are described, and the usefulness of combination therapy involving systemic administration of antifungal agents and surgical intervention is discussed. METHODS: We treated four patients with IFS between March 2003 and November 2007 at Ehime

BACKGROUND: CRANIAL (CSF Rhinorrhoea After Endonasal Intervention to the Skull Base) is a prospective multicenter observational study seeking to determine 1) the scope of skull base repair methods used and 2) corresponding rates of postoperative cerebrospinal fluid (CSF) rhinorrhea in the endonasal transsphenoidal approach (TSA) and the expanded endonasal approach (EEA) for skull base tumors. We sought to pilot the project, assessing the feasibility and acceptability by gathering preliminary data. METHODS: A prospective observational cohort study was piloted at 12 tertiary neurosurgical units

Pituitary metastasis: a clinical overview.

The pituitary gland is an unusual site for metastatic spread and has been associated with a poor prognosis. Clinical presentation is variable but can include visual field defects, cranial nerve palsies, anterior pituitary dysfunction and/ or diabetes insipidus. Management options include surgery or radiotherapy, chemotherapy/immunotherapy or a conservative approach. The pituitary should not be overlooked as a site for metastasis in patients with known cancer and can be the first presentation of neoplastic disease in some patients. Given that patients are now living longer with cancer, clinicia

Selective extradural anterior clinoidectomy for supra- and parasellar processes. Technical note.

Removal of the anterior clinoid process (ACP) facilitates radical removal of tumors or radical neck clipping of aneurysms in the supra- and parasellar regions by providing a wide operative exposure of the internal carotid artery (ICA) and the optic nerve and by reducing the need for brain retraction. Over a period of 3 years, anterior clinoidectomy was performed in 40 patients, 30 of whom harbored aneurysms (18 of the ICA and 13 of the basilar artery [one patient had two aneurysms]) and 10 of whom had tumors (four large pituitary tumors, four craniopharyngiomas, and two sphenoid ridge meningio

Olfactory Neuroblastoma-A Challenging Fine Line between Metastasis and Hematology.

Developing in a limited space, rare tumors located at the nose and paranasal sinuses are sometimes difficult to diagnose due to their modest clinical presentation, which is uncorrelated with anatomopathological diversity. This limits the preoperative diagnosis without added immune histochemical study; for that reason, we present our experience with these tumors with the intention of raising awareness. The patient included in our study was investigated by our department through clinical and endoscopic examination, imaging investigations, and an anatomic-pathological study. The selected patient

Mucormycosis during Imatinib treatment: case report.

UnlabelledPhiladelphia chromosome positive acute lymphoblastic leukemia is classified as a very high-risk group and it requires an intensive chemotherapy regimen associated with tyrosine-kinase inhibitors and allogeneic hematopoietic stem cell transplant from related or unrelated HLA matched donor. Most times, intensive chemotherapy regimens are associated with prolonged and profound pancytopenia when the risk of invasive fungal infection increases. After Candida and Aspergillus species, Mucormycosis is the third frequent fungal infection in hematology patients and it is associated with a redu

Anterior petrosal approach for petroclival solitary plasmacytoma.

BACKGROUND: Primary solitary plasmacytoma (PSP) of the skull base is a rare localized monoclonal plasma cell dyscrasia with normal or low plasma cell infiltration. Differentiating from other skull base tumors based on radiologic findings is difficult due to nonspecific features. PSP has a better prognosis after surgical resection and adjuvant radiotherapy, unless the tumor progresses to multiple myeloma (MM). Nonetheless, 50-60% progress to MM within a median time of 2 years. Gross total resection (GTR) for PSP is controversial for improving overall survival. However, if the lesion is easily a

Malignant Melanoma Bone Metastasis With Unknown Primary Site: A Case Report and Literature Review.

IntroductionCases of malignant melanoma first identified in bone are relatively uncommon. Approximately 4.3% of the patients will have metastasis, among which 23% will have metastasis to the bone.Case presentationThis article presents a case involving malignant melanoma with a metastatic lesion located at the distal femur, which was the initial symptom observed. The patient was a 21-year-old male who was admitted to the hospital due to intermittent pain in his lower left thigh. Imaging studies indicated bone destruction in the distal left femur, and pathological and immunohistochemical confirm

Extramedullary disease in multiple myeloma.

When clonal plasma cells grow at anatomic sites distant from the bone marrow or grows contiguous from osseous lesions that break through the cortical bone, it is referred to as extramedullary multiple myeloma (EMD). EMD remains challenging from a therapeutic and biological perspective. The pathogenetic mechanisms are not completely understood and it is generally associated with high-risk cytogenetics which portends poor outcomes. There is a rising incidence of EMD in the era of novel agents, likely a reflection of longer OS, with no standard treatment approach. Patients benefit from aggressive

Osteitis Fibrosa Cystica: The Hideous Face of Hyperparathyroidism.

Osteitis fibrosa cystica (OFC), the skeletal scourge of advanced hyperparathyroidism, unveils itself as a relentless destroyer of bone architecture, leaving behind a landscape of fragility and deformity. In the shadow of chronic kidney disease, where mineral imbalances reign, this rare but devastating entity emerges as a silent tormentor. We present the case of a hemodialysis patient who fell victim to this sinister complication, manifesting with excruciating bone pain, pathological fractures, and radiographic and histological evidence of cystic osteolytic lesions. Biological assessments revea

[Transcranial microsurgical decompression of the optic canal in surgical treatment of meningiomas of the sellar region].

RATIONALE: When removing the meningiomas of the sellar region, there is always a risk of visual impairment for various reasons, in particular, as a result of traction damage to the optic nerve. Decompression of the optic canal increases nerve mobility during tumor manipulation. In cases of meningioma growing into the canal, its decompression often seems necessary. AIM: Evaluation of the effectiveness and risks of performing decompression of the optic canal. MATERIALS AND METHODS: The study included patients with meningiomas of the parasellar location, who underwent surgical treatment at the

A review on clinical cases of rare mold infections and their antifungal treatment strategies.

In recent years, with the growing population of immunocompromised individuals, infections caused by rare environmental molds have become increasingly prevalent and clinically significant. Due to frequent misdiagnosis and difficult clinical management, the overall disease burden of such infections poses a major challenge in clinical practice. This article aims to fill the knowledge gap by reviewing cases of rare mold infections from January 2020 to February 2026, analyzing the susceptible populations, identification techniques, antifungal susceptibility testing, and antifungal treatment strateg
