[Endoscopic therapy of acute and chronic pancreatitis].

Endoscopic therapy is valuable for both acute and chronic pancreatitis. Early endoscopic papillotomy appears, in the case of a severe course of acute biliary pancreatitis, to be advantageous. Endoscopic drainage can be considered in cases of acute fluid retention and necrosis as well as subacute, non-healing pancreatitis or cyst development. By acute chronic pancreatitis with strictures or bile duct stones, papillotomy, dilation and stent insertion can lead to an improvement in pain symptoms. An improvement in endo- or exocrine function, however, is not expected. Studies on the endoscopic ther

[Results of laparoscopic cholecystectomy for acute cholecystitis in elderly patients].

OBJECTIVES: The aim of this retrospective study was to evaluate the feasibility and the morbidity of laparoscopic cholecystectomy for acute cholecystitis in elderly patients. METHODS: Among 891 consecutive patients who underwent cholecystectomy, 151 had acute cholecystitis. Fifty three patients of > or = 70 years of age (group 1) were compared to 98 younger patients (group 2). Analysis was made in "intention to treat" so directly open cholecystectomies during the same period were also included. RESULTS: Elderly patients had a lower success rate of laparoscopic treatment (52.8% versus 70.4%;

Advances in biliary stone management: Latest-generation extracorporeal shock wave lithotripsy vs laser lithotripsy for difficult bile duct stones.

BACKGROUND: Extracorporeal shock wave lithotripsy (ESWL) and laser lithotripsy (LL) are established alternatives for the management of difficult common bile duct (CBD) stones. However, there is limited evidence regarding the efficacy and safety of the latest-generation Dornier Delta III lithotripter. In particular, evidence on the clinical performance of the Dornier Delta III lithotripter is scarce. AIM: To evaluate and compare the efficacy and safety of ESWL performed with the Dornier Delta III and of LL using a single-operator cholangioscope with specific focus on stone clearance rates, num

Case report: Lemmel syndrome-aggravated by consumption of fatty food.

INTRODUCTION AND IMPORTANCE: One of the very rare causes of obstructive jaundice, Lemmel's syndrome, is caused due to biliary mechanical compression by a juxtapapillary diverticulum. It is defined as obstructive jaundice due to juxtapapillary diverticulum in the absence of cholelithiasis, periampullary tumours, or other detectable obstacles. It is an extremely rare syndrome; therefore, studies need to be done for proper diagnosis and management. CASE PRESENTATION: Our patient, a 73-year-old woman, gave a history of recent acute right hypochondriac pain that had subsided the day before. Lab in

Portal vein injury following endoscopic retrograde cholangiopancreatography: A case report.

Endoscopic retrograde cholangiopancreatography (ERCP) has been a widely used procedure in the diagnosis and treatment of various pancreaticobiliary disorders. Although widely considered a safe procedure, ERCP is associated morbidity and occasional mortality. The most common complications include acute pancreatitis, hemorrhage, and duodenal perforation. Portal vein cannulation is a rare complication of ERCP. We described a case of placement of an endoscopic biliary stent in the portal vein during ERCP and sphinc-terotomy. A 54-year-old female patient underwent laparoscopic cholecystectomy with

The role of a sequencing-based clinical intestinal screening test in patients at high-risk for Clostridium difficile and other pathogens: a case report.

BACKGROUND: Hospitalization and antibiotic treatment can put patients at high risk for Clostridium difficile infection, where a disturbance of the gut microbiome allows for Clostridium difficile proliferation and associated symptoms, including mild, moderate, or severe diarrhea. Clostridium difficile infection is challenging to treat, often recurrent, and leads to almost 30,000 annual deaths in the USA alone. Here we present a case where SmartGut™, an at-home, self-administered sequencing-based clinical intestinal screening test, was used to identify the presence of Clostridium difficile in a

The anocavernosal erectile dysfunction syndrome. II Anal fissure and erectile dysfunction.

A previous study has demonstrated that the bulbocavernosus muscle (BCM) is a part of the external anal sphincter (EAS) [Shafik, Arch Androl, 1999]. It aids erection by compressing the penile bulb and the dorsal penile vein, and acts as a 'suction-ejection' pump in the ejaculatory process. Being a part of the EAS, the BCM is assumed to be involved in the different EAS pathologies. A recent study showed that erectile (ED) and ejaculatory dysfunction in 16 men with fecal incontinence (FI) after an anal fistula operation was cured after sphincteroplasty [Shafik, in press]. Anatomy variant symptomatic of the duplication of the common bile duct type IV.

INTRODUCTION: Duplicity of the common bile duct (BCBD) is an unusual congenital disorder. CASE REPORT: A 80-year-old woman with duplication of the common bile duct with retrograde endoscopic cholangiopancreatography (ERCP) who did not resolve the symptoms. DISCUSSION: Our case is a variant of type IV to the classification of DCBC. The MR cholangiography and presurgical ERCP allows assessment of the bile ducts, their caliber, and assessment of abnormalities. The treatment before DCBC will depend on the clinic and the type of opening of the CBCA. CONCLUSIONS: It is important to perform a pre-

Ballooning of the papilla during contrast injection: the semaphore of a choledochocele.

BACKGROUND: Choledochocele is a rare abnormality involving the intramural segment of the common bile duct. ERCP is essential to demonstrate a choledochocele. The aim of this study was to investigate the duodenoscopic and cholangiographic findings during ERCP. METHODS: Over a 4-year period, 17 symptomatic patients (8 men, 9 women; age range 45 to 83 years) were identified as having choledochoceles. The diagnosis of choledochocele was made by both duodenoscopic and cholangiographic findings. RESULTS: As a characteristic duodenoscopic finding, the enlarged bulging papilla was noted in 8 patient

Surgeon-Performed Endoscopic Retrograde Cholangiopancreatography in a Rural Trinidad Hospital: A 10-Year Review of Practice and Outcomes.

BACKGROUND: Endoscopic retrograde cholangiopancreatography (ERCP) is a key therapeutic modality for biliary and pancreatic diseases. While outcomes from tertiary centres are well documented, there is limited published data describing ERCP practice in Caribbean settings. This study evaluated the patient characteristics, indications, procedural outcomes, and complications of surgeon-performed ERCPs at a rural hospital in Trinidad. METHODS: A retrospective observational study was conducted of all patients who underwent attempted or completed ERCP between 1 January 2015 and 31 December 2025, perf

Development and validation of a novel risk score for predicting post-ERCP pancreatitis in patients with choledocholithiasis: a retrospective study.

INTRODUCTION: Post-endoscopic retrograde cholangiopancreatography (ERCP) pancreatitis (PEP) remains the most common serious complication of ERCP, with variable incidence across patient populations. Choledocholithiasis patients represent a distinct subgroup with unique risk profiles, yet existing prediction models lack indication-specific validation. We aimed to develop and internally validate a practical risk score for predicting PEP in patients undergoing ERCP for choledocholithiasis. METHODOLOGY: This single-center retrospective cohort study included 500 consecutive patients who underwent th

Analysis of risk factors associated with endoscopic retrograde cholangiopancreatography for patients with liver cirrhosis: a multicenter, retrospective, clinical study.

BACKGROUND: Endoscopic retrograde cholangiopancreatography (ERCP) is the endoscopic modality of choice for the treatment of biliary and pancreatic diseases. However, patients with cirrhosis, particularly those with decompensated cirrhosis, are believed to be at increased risk for complications associated with ERCP. There is a paucity of literature describing the outcomes of ERCP for patients with cirrhosis. This study aimed to investigate the outcomes of ERCP for cirrhosis patients, especially adverse events, and evaluated its safety and efficacy. METHODS: We performed a multicenter, retrospe

Efficacy and safety of therapeutic ERCP in patients with cirrhosis: a large multicenter study.

BACKGROUND AND AIMS: Patients with cirrhosis may be less than optimal candidates for ERCP because of underlying ascites, coagulopathy, encephalopathy, and other problems. Although the risks of surgery in patients with cirrhosis are well known, few data are available regarding ERCP in patients with cirrhosis. We performed a retrospective, multicenter study of ERCP in patients with cirrhosis to evaluate outcomes, efficacy, and safety. METHODS: Multicenter retrospective study. RESULTS: A total of 538 ERCP procedures were performed on 328 patients with cirrhosis. A total of 229 patients had Chil

Efficacy and safety of endoscopic retrograde cholangiopancreatography (ERCP) performed in patients with Periampullary duodenal diverticula (PAD).

BACKGROUND AND AIM: Periampullary diverticula (PAD) are found in 9-32% of patients who undergo endoscopic retrograde cholangiopancreatography (ERCP). PAD are acquired lesions which are rare in patients <40 years, but increasing with age. Several endoscopic studies have revealed an association between PAD and common bile duct (CBD) stones. The presence of a papilla located in the diverticula is also frequent (6.8-54.9%) and represents a restrictive factor for successful cannulation. MATERIALS AND METHODS: A retrospective analysis has been made of data related to the patients with PAD treated a
