Contact lenses for children.

Contact lenses for children are used to correct a variety of conditions. This paper reviews fitting of soft, silicone elastimer, and rigid gas-permeable contact lenses for therapeutic treatment of aphakia, anisometropia, myopia, hyperopia, esotropia, irregular astigmatism, and nystagmus. Cosmetic custom designs are described for the treatment of aniridia, albinism, amblyopia, and corneal disfigurement. Working with children and gaining their trust requires inventiveness and patience. Prescribing contact lenses for them requires developing good cooperation between the child, the parents, and th

Corneal vascularization.

Corneal vascularization is an important clinical event that requires careful management. Vessel penetration can serve to heal a threatening inflammatory process, cause opacification and loss of vision, induce graft rejection, or serve as a warning of corneal distress. The latter condition is particularly common in contact lens wearers. Practitioners should be aware of the subtle signs of early neovascularization, and work toward its remission. Acute vessel penetration is a difficult clinical situation to manage. Quiescent vessels in the cornea must be explained by history and managed with eith

Diagnosis and primary care management of tear film deficiencies.

The tear film is a complex physical and biochemical structure. It functions to lubricate and protect the ocular surface from a variety of insults. Disorders of the tear film can lead to patient complaints as well as observable clinical signs. Combining clinical observations with a knowledge of the dynamic interaction of the tear film and the ocular surface should allow the practitioner to develop a management plan for symptomatic patients. This article details the many tests available for assessing tear film and ocular surface integrity, and discusses means of managing complications.

Infectious and inflammatory diseases.

The systemic conditions described in this paper are (1) sarcoidosis, (2) tuberculosis, (3) toxoplasmosis, and (4) histoplasmosis. The lung is the primary target organ of granulomatous involvement for each of these disorders. This and other similarities may confuse the diagnostic picture; however, appropriate understanding of clinical, epidemiologic, pathophysiologic, immunologic, radiologic, and serologic features assists in clearly establishing a diagnosis. The goals of this paper are to help the clinician develop a working knowledge of these four conditions, as well as an understanding of wh

Extraocularmotor dysfunction associated with tumors.

Intracranial and extracranial tumors can be the cause of extraocularmotor dysfunction. In approximately 19% of reported cases, paralysis of the third, fourth, or sixth cranial nerves is due to neoplastic growth; the sixth nerve is the most commonly affected. Tumors can produce extraocular muscle dysfunction by directly affecting the extraocular muscle; directly affecting the third, fourth, or sixth cranial nerves; directly affecting the supranuclear extraocular movement areas; indirectly affecting any of the above by metastasis from a tumor in another part of the body; and indirectly affecting

Angle recession.

Angle recession is a relatively common result of ocular injury, which, if extensive, creates a noteworthy risk of elevated intraocular pressure (IOP) and glaucoma. The unilateral elevation in IOP is usually seen long after the injury has occurred. The diagnosis is often made based on clinical suspicion, either because of information obtained during the history-taking or from signs of ocular trauma observed during the examination. The treatment of angle-recession glaucoma is in many ways similar to that of primary open-angle glaucoma, although miotic drugs and argon laser trabeculoplasty are co

Hyphema.

Hyphema is a potentially sight-threatening sequela of blunt trauma. Delayed healing time, poor visual outcome, and complications such as corneal blood staining, anterior and posterior synechiae, increased intraocular pressure, and glaucomatous optic atrophy are most often associated with hemorrhage filling more than one half of the anterior chamber. Rebleeds are most likely 3 to 5 days following injury, in children, in blacks (particularly if they have sickle cell disease), and in persons who have ingested aspirin or other antiplatelet compounds. Treatment of hyphema is controversial, and medi

Injury to the cornea.

Ocular trauma represents one of the most frequent and challenging clinical presentations confronting the primary eye care provider. The degree of damage to the ocular tissues can range from mild to severe. It is imperative that the managing practitioner be able to rapidly assess the level of injury and take appropriate steps to minimize ocular morbidity, thereby maximizing the return of normal visual function. Corneal injuries can be divided into three major categories: mechanical, chemical, and radiant energy. This discussion will deal with the general management of mechanical types of injuri

Optic nerve trauma.

Trauma to the optic nerve may be direct, such as from a penetrating object, or indirect, which may result despite lack of direct contact of an object with the nerve. Although indirect injury initially causes no change in the appearance of the nerve head, within a matter of weeks optic atrophy will be manifest. The pathophysiology of nerve damage is incompletely understood. Management is controversial; steroid therapy has been advocated, as has surgical decompression of the nerve. Indirect injuries affecting the optic nerve may also result from torsional rotation of the globe (avulsion) and fro

Orbital fractures.

The optometrist is often the first practitioner to see patients with ocular trauma and associated orbital fractures. Trauma is now the second leading cause of blindness in the United States. Early and accurate diagnosis of orbital fractures is required to avoid compromising the binocular vision and cosmetic appearance of the patient. With an understanding of the characteristic clinical presentations of fractures and a knowledge of current imaging techniques, these disorders can be accurately diagnosed, managed, or referred by the optometrist.

New antibacterial drugs for topical ophthalmic use.

Three new antibacterial drugs have recently been approved for topical ophthalmic use. Ciprofloxacin (Ciloxan) and norfloxacin (Chibroxin) are fluoroquinolones with very broad activity spectra; both have been found to be safe and effective for the treatment of conjunctivitis. Ciprofloxacin, in the commercially available concentration (0.3%), has also been found to be a safe and effective treatment for corneal ulcers. Trimethoprim-polymyxin B (Polytrim) is a new combination product that has been used successfully for the treatment of conjunctivitis. Use of these three new drugs will significantl

Topical carbonic anhydrase inhibitors: a new perspective in glaucoma therapy.

Oral carbonic anhydrase inhibitors have been used extensively in the treatment of all types of glaucoma. These agents reduce intraocular pressure by suppressing the rate of aqueous humor formation. Despite their efficacy as ocular hypotensive agents, these agents are not always used for protracted therapy because of significant systemic side effects. Consequently, topically active carbonic anhydrase inhibitors have been a goal of glaucoma researchers for many years. Recently, a new class of carbonic anhydrase inhibitors has entered clinical trials as ocular hypotensive agents. The thienothiopy

Chemical treatment of verrucae.

Viral warts such as verrucae vulgaris, verrucae plana, and cutaneous horns, when located in close proximity to the eye, can be the cause of chronic, mild conjunctivitis. Removal by chemical cautery using bichloroacetic acid, or by simple excision for pedunculated verrucae, can produce a welcome reduction in symptoms. Chemical cautery may also be employed for the cosmetic treatment of xanthelasma. The proper techniques for cautery and for excision, as well as the evaluation of patients for complications, are discussed.

Surgical management of glaucoma.

Surgery plays an important role in the treatment of glaucoma. For the angle-closure glaucomas, surgery is the primary mode of treatment to restore the normal route of aqueous out of the eye. For the open-angle glaucomas, surgery is usually a secondary treatment modality, indicated after medical therapy fails to halt glaucomatous damage or does not reduce the intraocular pressure to an acceptable level for an individual's eye.

A new look at an old disease.

Despite 200 years of effort, much about glaucoma remains a mystery, and numerous critical questions remain unanswered. Are there subgroups of primary open-angle glaucoma, with different etiologies, risk factors, clinical presentations, and prognoses? What criteria should be adopted for important diagnostic tests, such as automated perimetry, to reduce the likelihood of variability? Is medical management of the disease truly efficacious? Should laser trabeculoplasty become the initial surgery of choice? Current answers to these and other important questions are reviewed in this survey of the ma

Microbiological aspects of extended-wear soft contact lenses.

Corneal ulcers are the most devastating complication associated with extended-wear soft contact lenses. The increasing prevalence of microbial keratitis associated with the wear of these lenses is a major concern for the optometrist. Studies of extended-wear and daily-wear contact lens patients strongly suggest that extended-wear patients face a greater risk of ulcerative keratitis than other cosmetic contact lens wearers. This discussion addresses the potential risks of extended-wear, with emphasis on ulcerative keratitis, the likely causes for these increased risks, and what the optometrist

Complications of extended wear.

Extended wear has been an attractive modality for many contact lens wearers. Despite the associated benefits of convenience and comfort, however, there have been many physiological complications reported for hydrogel extended-wear lenses, and a lesser number for rigid gas-permeable extended-wear lenses. This review describes the physiological as well as the mechanical complications of extended-wear lenses, for both soft lens and rigid lens materials, including etiology, clinical presentation, and management strategies.

Adverse effects of antiglaucoma medications.

Most patients with relatively early glaucomatous damage can be satisfactorily managed with topically or orally administered medications. These drugs, however, can sometimes cause adverse ocular or systemic effects. In this paper the fundamental pharmacologic activity of antiglaucoma medications is reviewed, and appropriate strategies to prevent or manage drug-induced side effects are considered.

The secondary glaucomas.

Glaucoma resulting from secondary congenital, ocular, or systemic conditions represents a formidable diagnostic and management challenge for optometrists. This paper discusses the pathogenesis, diagnosis, and treatment of six common forms of the disease: exfoliation syndrome, pigmentary glaucoma, neovascular glaucoma, lens-induced glaucoma, glaucoma accompanied by ocular inflammation, and trauma-induced glaucoma. For each of these conditions, clinical experience, observational skills, and an understanding of the mechanisms by which elevated intraocular pressure may result are prerequisites for
