Asthma and geographical altitude: an inverse relationship in Mexico.

Motivated by the great asthma-prevalence variation throughout the world, we analyzed the rate variation of medical services due to asthma in the 32 Mexican states at the Instituto Mexicano del Seguro Social (approximately 24 million insured subjects). In 1993, a total of 406,036 services were due to asthma, and state rates ranged from 53 to 476 x 10,000 insured subjects. A direct correlation (p < 0.05) was found for single-room house, ascariasis, and rubella rates, but the strongest (inverse) correlation was found for geographical altitude (r = -0.73, p < 10(-5)). Thus, in Mexico, geographical

Effect of caffeine on submaximal exercise performance at altitude.

The purpose of this study was to determine if caffeine (CAF) could enhance exercise performance at high altitude (HA). Eight males (17 to 24 years) performed two submaximal endurance tests to exhaustion (ETX) while cycling at approximately 80% of their altitude-specific maximal aerobic power during each of three phases: 1) sea level (SL); 2) after 1 h (acute) at 4,300 m; and 3) after 2 weeks (chronic) at 4,300 m. Subjects received either CAF (4 mg.kg-1) or a placebo drink 1 h prior to each ETX bout at each phase in a double-blind crossover design. ETX was little affected during CAF treatment a

Arachnoid nodules in the lungs of high altitude Indians.

BACKGROUND: Nodules of cells showing a striking histological similarity to those of arachnoid villi have previously been found closely adjacent to pulmonary venules in several diseases associated with alveolar hypoxia or pulmonary oedema including mitral stenosis, plexogenic pulmonary arteriopathy, pulmonary thromboembolism, and chronic obstructive pulmonary disease. METHODS: Histological sections of the lungs of seven adult native highlanders from La Paz (3600 m) were examined. RESULTS: Arachnoid nodules were found in the lungs of one Aymara and one Mestizo Indian. CONCLUSIONS: These bodie

Nutritional aspects of health and performance at lowland and altitude.

One of the most important nutritional goals amongst athletes is to maintain adequate energy and fluid balance, since these are subject to relatively rapid changes and are directly related to performance and health. This may especially be the case when exercise intensity is high. Furthermore, when due to exercise and environmental stress food and fluid intake become depressed. In such conditions there may be a dramatic increase in the utilization of carbohydrate (CHO), fluid, and in some instances protein. These increased requirements may then not be covered. Insufficient replacement of CHO may

The effect of altitude on umbilical cord blood gases.

OBJECTIVE: Analysis of umbilical cord blood acid-base status has proven useful in the immediate care and resuscitation of the newborn and provides an objective measure of the intrapartum fetal environment. Our study was designed to determine whether there are any significant differences in the acid-base status of neonates delivered at a relatively high altitude of 6000 ft compared with sea-level controls. METHODS: Our study population consisted of 300 women who had undergone spontaneous vaginal delivery after uncomplicated labor; 150 delivered at an institution located at an elevation of 5900

Neonatal septicaemia in an African city of high altitude.

Ninety-nine cases of neonatal septicaemia prospectively seen over a 3-year period in a large cosmopolitan African city of high altitude is presented. An incidence of 6.5 per 1000 live births was noted. Though the most important pathogens were Klebsiella spp. and Staphylococcus aureus, Citrobacter difficile and Alkalegenes faecalis were the pathogens associated with a high mortality rate. Low birth weight infants were significantly more affected. The overall mortality rate was 27.3 per cent. The commonest predisposing perinatal factors were birth asphyxia and prolonged rupture of fetal membrane

[Human life at high altitudes: myths and realities].

Life at high altitude presents very interesting biological and medical aspects. For countries such as the Andeans, it also has socio-economical implications. The most important towns of Bolivia are situated between 3,000 and 4,850 m. It's to say that a great percentage of the population is permanently staying there. Moreover owing to the improvement of the roads and transport, an extensive migration (professional life, business, tourism, sport) developed some years ago. At 3,000 to 4,850 m, an oxygen arterial pressure (PaO2) between 40 and 70 mm Hg is resulting from the environmental hypoxia.

Altitude insomnia: studies during an expedition to the Himalayas.

During an expedition to the Himalayas, we studied the sleep and respiration of six climbers. Three ingested acetazolamide (500 mg) daily throughout the climb and the other three ingested placebo. At high altitude (4,150-4,846 m), each subject ingested temazepam (10 mg) for one night and placebo for another. Acetazolamide improved sleep above 2,750 m, but it is uncertain whether this was due to sedation or to improvements in arterial oxygen saturation. Sleep was markedly disturbed in all subjects above 4,000 m. Temazepam improved sleep, and in subjects taking acetazolamide, it reduced sleep-ons

Control of breathing in Sherpas at low and high altitude.

Sherpas are well known for their physical performance at extreme altitudes, yet they are reported to have blunted ventilatory responses to acute hypoxia and relative hypoventilation in chronic hypoxia. To examine this paradox, we studied ventilatory control in Sherpas in comparison to that in Westerners at both low and high altitude. At low altitude, 25 Sherpas had higher minute ventilation, higher respiratory frequency, and lower end-tidal carbon dioxide tension than 25 Westerners. The hypoxic ventilatory response of Sherpas was found to be similar to that in Westerners, even though long alti

Effects of simulated altitude on O2 transport in dogs.

The effects of exposure to simulated altitude on skeletal muscle and on the cardiocirculatory system rhave been studied on awake, conscious, dogs (16-39 kg) at rest and during moderate treadmill exercise at 15% incline. Measurements were obtained in Denver at a PB of 635 mm Hg and after 3 weeks of continuous exposure to a PB of 435 mm Hg using a hypobaric chamber. At both levels of PB the dogs showed marked panting, associated with pulmonary hyperventilation and low PaCO2's (28 mm Hg at 635 and 17 mm Hg at 435 mm Hg). Resting VO2's were high (12.4 and 11.4 ml/min-kg) reflecting the high respir

Further studies on pulmonary oedema of high altitude. Abnormal responses to hypoxia of men who had developed pulmonary oedema at high altitude.

101 Indian soldiers, 57 of whom had developed pulmonary oedema of high altitude (POHA) and 44 who had not developed this condition in spite of being at high altitudes for over 2 years, were investigated for observing the differences, if any, in their reaction to acute hypoxic stress. Each subject was made to breathe a 10% hypoxic mixture for 5 min. Haemodynamic parameters like pulmonary artery pressure (systolic, diastolic and mean), brachial artery pressure, wedge pressure, cardiac output, minute ventilation, arterial oxygen saturation and oxygen uptake before and at the end of hypoxic breath

Clinical and Pathophysiological Features of High-altitude Pulmonary Edema in the Japanese Population: A Review of Studies on High-altitude Pulmonary Edema in Japan.

High-altitude pulmonary edema (HAPE) is a life-threatening, noncardiogenic pulmonary edema that occurs in unacclimatized individuals rapidly ascending to high altitudes above 2,500 m above sea level. Until the entity of HAPE was first identified in a case report published in Japan in 1966, the symptoms of severe dyspnea or coma occurring in climbers of the Japan Alps were incorrectly attributed to pneumonia or congestive heart failure. The Shinshu University Hospital serves as the central facility for rescuing and treating patients with HAPE in the region. Over the past 50 years, a series of s

The Complexity of Diagnosing High-Altitude Pulmonary Edema: A Case Report and Review of the Differential Diagnosis of Greater Than Expected Hypoxemia at Altitude.

High-altitude pulmonary edema (HAPE) is a common presumptive diagnosis for a patient who experiences significant dyspnea and cyanosis at altitude. In this study, we present a case of a 58-year-old woman who was initially diagnosed with HAPE, although further evaluation revealed the presence of two underlying contributors to her significant hypoxemia at altitude. We discuss the medical workup for causes of greater than expected hypoxemia at altitude and the role some relevant medical comorbidities may play.

Preparation for high altitude expedition and changes in cardiopulmonary and biochemical laboratory parameters with ascent to high altitude in transplant patients and live donors.

High-altitude climbing has many risks, and transplant recipients should discuss the associated risks and means of preparation with their physicians. This study aimed to help prepare athletic transplant donors and recipients for mountain climbing and was designed to evaluate physical performance and changes in cardiopulmonary and biochemical laboratory parameters of transplant recipients and donors in extreme conditions of high altitude. Ten subjects-six liver transplant recipients, two liver donors, and one kidney transplant recipient and his donor-were selected for this expedition to Island P

Altitude-induced plasticity of respiratory central-peripheral chemoreflex interaction.

Stable breathing depends on the integrated action of central and peripheral chemoreceptors, but the nature of this interaction in humans and its physiological importance remain controversial. Previous studies reported additive, hypo-additive and hyper-additive responses across species, preparations and protocols. Here, we used a novel method to quantify peripheral chemoreflex sensitivity across graded levels of central chemoreceptor activation to test whether central-peripheral chemoreceptor interaction in awake humans is fixed or state dependent. At low altitude, the form of interaction varie
