BACKGROUND: As population-based genomic screening (PGS) programs are increasingly implemented, standardized, evidence-based approaches are needed to support decision-making about which genetic findings should be returned based on clinical actionability. The Clinical Genome Resource (ClinGen) Actionability Framework, which guides decision-making on the return of monogenic secondary findings, was adapted to create a framework for the context of PGS in adults.
METHODS: ClinGen convened a group of experts in public health, medical genetics and genomics, genetic counseling, population screening, h

I attended the NSIGHT Ethics and Policy Advisory Board's meeting on sequencing newborns as a research associate in a joint apprenticeship between the University of California, San Francisco, Institute for Human Genetics and the university's Program in Bioethics. But I also came to the meeting with a deeply personal perspective: I had spent nearly my entire childhood in search of a diagnosis and therefore was eager to hear the board's discussion on how to ethically include genomic sequencing early in life. Genomic sequencing in the newborn period could have helped me avoid my diagnostic odyssey

The history of sexually transmitted diseases is thought to date back to earliest times, and many ancient texts describe conditions that may be those of syphilis and gonorrhea, which at one time were thought to be the same disease. A main figure in the research in this area was Albert Ludwig Sigesmund Neisser, who discovered the gonococcus in 1879 and later produced the most comprehensive account of experimental syphilis ever published. This article provides a brief biography of Albert Neisser, focusing on his discoveries in the area of infectious diseases, the so-called Neisser-Hansen controve

Management of newborns with disorders of sex development (DSD), especially in deciding the need for a sex assignment surgery, is a complex matter. It is associated with many bioethical issues, such as concerns about the rights and welfare of the newborns and the reliability of parents' consent to the paternalistic disposition of physicians in making the best decisions. This paper, containing interviews with six medical experts and three religious' experts, aims to raise awareness of the multidisciplinary approach, which uses a combination of medicine, religion, and ethics in managing children

Controlled organ donation after circulatory death (DCD) has recently been revived in the UK, as part of attempts to increase organ donation rates. The re-introduction of DCD has subsequently become the focus of bioethical controversy, since it necessitates intervening in the care of dying patients to obtain quality donor organs. Transplant policy responses to these concerns have generated new legal and ethical guidelines to address uncertainties around DCD, producing claims that the UK has overcome' the ethical challenge of DCD. In contrast, by drawing on Lynch's call to 'respecify' ethics, th

BACKGROUND: Physician participation in medical aid in dying (MAID) is commonly framed as a binary choice between participation and objection. Emerging evidence suggests physician involvement is nuanced, with levels of engagement shaped by ethical beliefs, professional responsibilities, and organizational structure. Improved understanding of how physicians make these decisions is needed to inform policy, clinical practice, and discussions about conscience-based practices.
METHODS: We conducted a qualitative descriptive study of 20 physicians practicing in US states where MAID is legal. We used

BACKGROUND: When dietary intakes are insufficient, oral nutritional supplements (ONS) are valuable allies to ensure effective nutritional care for patients with cancer. ONS benefits in preventing and treating malnutrition, improving clinical outcomes and quality of life and reducing healthcare costs are well supported by the literature. Moreover, the advent of novel ONS formulas, enriched with additional components (eg immune system modulation), might further improve outcomes in patients with cancer. Yet, a critical question remains: why are ONS, under clinical supervision, not fully covered f

The Czech debate on euthanasia illustrates with particular clarity how bioethical questions are inseparably intertwined with law, politics, and public discourse. Over the past decade, the issue has generated an exceptional number of monographs, creating a dense intellectual framework for reflection on dignity, autonomy, and end-of-life decision-making. Despite sustained scholarly engagement and repeated legislative initiatives, all proposals to legalize euthanasia have been rejected. The Constitutional Court has clarified that, while personal autonomy deserves respect, no constitutional right

BACKGROUND: A variety of terms and attitudes surround palliative sedation (PS) with little research devoted to hospice and palliative care (HPC) clinicians' perceptions and experiences with PS. These factors may contribute to the wide variability in the reported prevalence of PS.
OBJECTIVE: This study was designed to better identify hospice and palliative care (HPC) clinician attitudes toward, and clinical experiences with palliative sedation (PS).
METHODS: A 32-question survey was distributed to members of the American Academy of Hospice and Palliative Medicine (n = 4678). The questions exp

The litigation brought by the Estate of Henrietta Lacks against Thermo Fisher Scientific, Ultragenyx Pharmaceutical, and Novartis Pharmaceuticals reframes benefit-sharing from voluntary institutional ethics into an enforceable legal duty. By pleading a single equitable claim - unjust enrichment - the Estate seeks to bypass the property and informed-consent doctrines that have defeated comparable tissue-appropriation suits, while accepting distinctive risks involving remoteness, limitations, causation, and apportionment. This commentary examines the strategy's strengths and vulnerabilities thro

This symposium includes twelve personal narratives from parents of children with Down syndrome. These narratives foster better understanding of these parents' experiences of learning that their child has Down syndrome, navigating sometimes labrynthine medical, educational, and social services bureaucracy, and contending with an implicitly-and sometimes explicitly-ableist culture. Four commentaries on these narratives are also included, authored by experts and scholars in bioethics, disability studies, health and science communication, and public advocacy. The goal of this symposium is to call

Psychiatry is distinguished from other fields of medical expertise and bears a particular kind of responsibility, namely the treatment of persons incapable of informed consent per se. The History of psychiatry shows that much too often inhuman abuse was happening in psychiatric facilities. An ethics of psychiatry therefore requires a reliable and stable foundation for values that allow justifying normative claims embracing both characteristics. Such a basic foundation already exists in form of the pluralistic and international recognition of human dignity. We argue that human dignity does and

Some bioethicists have argued in favor of a sliding scale notion of competence, paternalistically requiring greater competence in relation to more significant risk. I argue against a sliding scale notion, taking issue with the positions of Allen E. Buchanan and Dan W. Brock, Ian Wilkes, and Joel Feinberg. Rejecting arguments that a sliding scale is supported by legal cases, by ordinary usage, and by fallible judgments about competence, I argue in favor of greater evidence of competence when risk is greater. Two clinical cases are examined, both involving amputation, to show that my fixed conce

Spina bifida is a neurodevelopmental disorder that results in a broad range of disability. Over the last few decades, there have been significant advances in diagnosis and treatment of this condition, which have raised concerns regarding how clinicians prognosticate the extent of disability, determine quality of life, and use that information to make treatment recommendations. From the selective treatment of neonates in the 1970s, to the advent of maternal-fetal surgery today, the issues that have been raised surrounding spina bifida intervention invoke principles of medical bioethics such as

A recent decision of the Italian Constitutional Court has legitimized for the first time heterologous artificial insemination. On April 9(th), 2014 the "Consulta" declared unconstitutional the sections of law n. 40 of 19 February 2004 (Judgment n. 162/2014), which prohibited heterologous fertilization: a) section 4 paragraph 3 "It is forbidden the use of techniques of medically assisted procreation of the heterologous type"; b) section 9, paragraphs 1 and 3, which include the prohibition of the disclaimer of paternity and the anonymity of the mother; c) section 12, paragraph 1, which includes

In this essay, I examine the question of whether it is possible that the encounter with the other could be mediated such that the interval of distance would lose its determining power. I reflect on some instances of extraordinary corporeality, most particularly the phenomenon of conjoined twins, in order to problematize the relation between subjects as they are embodied. Where the normative body is supposedly marked out by the closed boundaries of the skin, the figuration of the anomalous body as potentially leaky and contaminatory positions it as being of prime concern to both medical and cul

Since the first successful lung transplants in humans were done in the 1980s, lung transplantation has become an established treatment for end-stage pulmonary disease. Because the access to transplantable organs is limited and unpredictable, rules that guide the allocation of lungs for transplants have emerged. Such rules are governed not only by medical and bioethical necessities, but also by local traditions, legislation, and practical circumstances. Therefore, there may be significant differences between the organ allocation practices in various parts of the world. In this brief communicati

Law n. 40/2004 regulated Medically Assisted Procreation (MAP) in Italy. The case described in this article is part of the many judicial cases that have affected this legislation. Following the marital separation process, the former husband withdrew consent to the implantation of already formed embryos in the uterus of his former wife who turned to the judiciary to obtain permission to initiate such a procedure. This paper consists of an analysis of this judicial process and deals with medical-legal and bioethical issues. In contrast to what is stated in Law n. 40/2004, the subsequent Law n. 21
