Do we really need to pay attention to the less common crystals?

In addition to monosodium urate, calcium pyrophosphate dihydrate, and apatite crystals, a wide variety of less common crystals, artifacts, and, occasionally, some unidentified birefringent materials may be seen in synovial fluids in association with acute or chronic arthritis. These unusual crystals and their associated musculoskeletal manifestations may be confused with the more common crystals and their manifestations and sometimes may provide the clue for systemic disease because they may present with a pseudovasculitis syndrome. Important advances in understanding the molecular aspect and

Fine needle aspiration biopsy of peripheral congenital bronchial cyst.

Bronchial cysts usually occur centrally near the mediastinum, but may present as a peripheral "coin" lesion requiring distinction from other causes of coin lesions of the lung. A case is reported in which fine needle aspiration (FNA) biopsy was used to make the diagnosis of such a peripheral bronchial cyst. FNA produced an opaque opalescent fluid; cytologic study of the smears showed abundant bipolar birefringent needle-shaped crystals an a few macrophages. Neither special staining of some smears nor cultures of part of the aspirate revealed the presence of any organism. The cytologic diagnosi

The effect of prenatal indium chloride exposure on chondrogenic ossification.

Daily indium chloride doses of control (0) or 400 mg/kg were administered orally to pregnant Sprague-Dawley (SD) rats by gavage, on d 20 of gestation. Indium concentration was determined in the maternal and fetal blood, livers, kidneys, skulls, and femurs by atomic absorption spectrometry. Further groups of pregnant rats were treated with control (0) or 400 mg/kg indium chloride orally, during the whole gestation period. The fetuses were examined on d 21 of gestation, using histological and histochemical methods. Four hours after the administration indium concentration was found to be signific

Atypical Tumoral Presentation of Calcium Pyrophosphate Deposition Disease: A Case Report.

CASE: A 55-year-old man presented with a history of forefoot pain and swelling. Radiographs revealed a mass with internal calcifications and osseous erosion of the fifth metatarsophalangeal bone. The mass was isointense to muscle on T1-weighted magnetic resonance imaging (MRI) and hyperintense on T2-weighted MRI. A biopsy was performed, and intraoperatively, the lesion appeared as chalky white material, which under polarized light microscopy was composed of weakly positively birefringent rhomboid crystals, leading to a diagnosis of tophaceous pseudogout. CONCLUSION: Tophaceus pseudogout shoul

Laryngeal teflonoma identified by Fourier-transform infrared microspectroscopy after forensic autopsy: an interesting tool for foreign material identification in forensic cases.

Forensic pathologists are sometimes confronted with microscopic foreign bodies mixed in with soft tissues surrounding wounds and which are thus difficult to identify. This identification, however, could be primordial in investigating a crime and in determining the weapon used. A case of a fatal respiratory distress syndrome due to conjoining suicidal drug intoxication and laryngeal obstruction by a voluminous foreign body giant cell granuloma is presented. The classical histological examination showed exogenous particles in the vocal cord tumor with birefringent qualities. Their analysis with

Cholesteryl ester storage disease in a young child presenting as isolated hepatomegaly treated with simvastatin.

Cholesteryl ester storage disease (CESD) is an autosomal recessive disorder resulting from lysosomal acid lipase deficiency and is usually characterized by hepatomegaly and hyperlipidemia. This paper reports a two-year-old boy who had hepatosplenomegaly, hyperlipidemia and hypertransaminasemia determined incidentally. The liver biopsy sample was orange-yellow in appearance. Microscopically, microvesicular steatosis and birefringent crystals were seen in liver biopsy. The diagnosis of CESD was confirmed by the reduced human acid lipase activity in peripheral leukocytes. Simvastatin therapy was

Prospective study of morphology and phagocytosis of synovial fluid monosodium urate crystals in gouty arthritis.

Crystals can be identified in synovial fluids (SF) in the absence of acute inflammation as well as during acute gouty arthritis. We examined gouty joint effusions to evaluate whether observable properties of the crystals correlate with the clinically detectable inflammation. Higher SF leukocyte counts and more crystal phagocytosis were confirmed during more severe arthritis. We found no differences between crystal sizes, numbers of shapes in severe versus mild inflammation. Some patients with severe acute gout had few crystals or only punctate birefringent fragments. Factors other than those w

Nosematosis of the cornea. Case report, including electron microscopic studies.

A 26-year-old woman underwent enucleation of a blind, painful eye because of a perforated corneal ulcer. Histopathologic examination of the eye disclosed an acute, necrotizing keratitis surrounding myriad small (2.5 to 3 x 4.5 to 5 micrometer), oval, faintly visible structures. Under polarized light, they were partially birefringent and stained intensely positive with the acid-fast technique. The organisms were located mainly within the cytoplasm of histiocytes in the deep corneal lamellae. They were interpreted as a protozoa of the genus Nosema. The diagnosis was confirmed by electron microsc

Watery Diarrhea Caused by AA-Amyloid Deposition Mimicking Collagenous Colitis in Antisynthetase Syndrome-Lupus Overlap.

Collagenous colitis-like amyloid deposition in the gastrointestinal tract is an uncommon pattern of gastrointestinal amyloidosis. This pattern is usually associated with AA-amyloid. The recognition of this pattern is difficult for an unaware pathologist. We report collagenous colitis-like deposition of AA amyloid in a 45-year-old woman at the time of autopsy. The woman suffered from antisynthetase syndrome-systemic lupus erythematosus overlap syndrome-was on continued immunosuppression and presented with fever, watery diarrhea, and vomiting. The amyloid showed a band-like congophilic subepithe

Halide Ion-Directed Symmetry Control in π-Conjugated Molecular Crystals.

Two halide-containing π-conjugated molecular crystals, (C5H7BNO2)Cl and (C5H7BNO2)Br, were synthesized using pyridine-4-boronic acid and halide ions of differing ionic radii. (C5H7BNO2)Cl adopts a noncentrosymmetric (NCS) structure and exhibits SHG efficiency comparable to KH2PO4, along with a wide UV transparency window, making it promising for UV NLO applications. In contrast, (C5H7BNO2)Br crystallizes in a centrosymmetric (CS) structure, exhibiting enhanced birefringence. Structural and theoretical analyses reveal that the halide ions influence molecular packing and coplanarity, thereby tun

Systemic amyloid A amyloidosis in a captive Cape hyrax (Procavia capensis).

Amyloidosis is a disease caused by amyloid deposition and has been reported in various animal species; however, reports on Afrotherian species, including Cape hyraxes, are extremely limited. In this report, we describe the case of a 4-year-old captive Cape hyrax affected by amyloidosis. Histological examination revealed amyloid deposits in the liver, spleen, kidneys, and heart, which stained orange-red with Congo red and exhibited typical birefringence under polarized light. Mass spectrometry-based proteomic analysis and immunohistochemistry identified serum amyloid A in the amyloid deposits.

2,8-dihyroxyadenine (DHA) crystalline nephropathy: A case report.

Adenine phosphoribosyltransferase (APRT) deficiency is a rare autosomal disorder with extremely variable presentation. The disease spectrum ranges from completely asymptomatic to 2,8-dihydroxyadenine (DHA) stones to massive deposition of DHA crystals leading to DHA crystalline nephropathy. We report a case of a 45-year-old woman who presented with acute kidney injury and recurrent vomiting. Kidney biopsy revealed precipitation of brown crystals in tubular lumina with acute tubular injury with characteristic birefringence on polarizing light, confirming the unexpected diagnosis of DHA crystalli

Time Translation Symmetry Breaking in an Isolated Spin-Orbit-Coupled Fluid of Light.

We study the interplay between intrinsic spin-orbit coupling and nonlinear photon-photon interactions in a nonparaxial, elliptically polarized fluid of light propagating in a bulk Kerr medium. We find that in situations where the nonlinear interactions induce birefringence, i.e., a polarization-dependent nonlinear refractive index, their interplay with spin-orbit coupling results in an interference between the two polarization components of the fluid traveling at different wave vectors, which entails the breaking of translation symmetry along the propagation direction. This phenomenon leads to

Conjunctival Amyloidosis: A Report of Two Cases with Review of Literature - 2000-2020.

Amyloidosis is a complex multisystem disorder characterized by deposition of an aberrant protein in tissues and results in disruption of the normal organ function. Localized amyloidosis is a rare disorder. It commonly affects the head-and-neck region, and only 4% of these lesions are encountered in the orbital region. Hence, conjunctival amyloidosis is a very rare entity. It is thought to be a manifestation of local immunologic disorders. Amyloidosis of conjunctiva is more often localized with no other systemic features. Here, we present two cases of unilateral conjunctival amyloidosis, one wi

Localized amyloidosis affecting the lacrimal sac managed by endoscopic surgery: A case report.

BACKGROUND: Here we present a rare case of localized amyloidosis involving the nasolacrimal duct and lacrimal sac which was managed by endoscopic surgery. CASE SUMMARY: A 50-year-old man whose medical history included bilateral ventricular fold and vocal cord amyloidosis complained of bilateral epiphora. Magnetic resonance imaging revealed a neoplasm within the nasolacrimal sac. Characteristic positivity for Congo red staining and birefringence under a polarized microscope proved the diagnosis of amyloidosis. Dacryocystorhinostomy via an endoscope obtained a favorable result. A one-year follo

Lichen amyloidosis in a dark skinned patient.

Lichen amyloidosis is a primary localized cutaneous amyloidosis without systemic involvement, characterized by a persistent pruritic eruption of multiple discrete hyperkeratotic papules. The etiology is unknown, but chronic irritation of the skin has been proposed as an etiological factor. We herein report a typical case of lichen amyloidosis in a dark skinned patient. Physical examination revealed slightly shiny, brownish and fine uniform papules approximately 1 cm in diameter, with no accompanying macular lesions. Biopsy specimens taken from some of these papules on the legs showed small glo

Primary diffuse tracheobronchial amyloidosis in a dog.

Primary diffuse tracheobronchial amyloidosis was diagnosed at necropsy in an intact male Akita dog aged 11 years, a non-productive chronic cough having been the only related clinical sign. Histologically, eosinophilic hyalinized deposits were found as a band in the lamina propria underneath the epithelium of the trachea and bronchi. When stained with Congo red, apple-green birefringence was observed in the deposits viewed with polarized light. The amyloid did not lose sensitivity to Congo red staining after incubation with potassium permanganate, indicating that it was of the AL (amyloid light

Cytologic detection of Cryptococcus neoformans in cerebrospinal fluid. Rapid screening methods.

Yeasts of Cryptococcus neoformans in cerebrospinal fluid (CSF) were studied using polarized light and fluorescence microscopy. Ultraviolet-induced fluorescence of wet-fixed, Papanicolaou-stained smears and polarization of air-dried, Diff-Quik or Leishman-stained preparations are complementary methods that allow for rapid screening for and immediate identification of Cryptococcus in cytologic preparations of CSF.

Comparative polarized-light and electron microscopic studies of changes in the structure of the intercalate disc following various experimental lesions.

The intercalate disc was studied following various traumatizing interference (cyanide, carbon monoxide poisoning, intermittent coronary perfusion, nitrogen perfusion, intermittent hypoxic loading, direct electric trauma). The most frequent alterations were the separation, vesicular transformation and loss of electron-dense material of the fascia adherens and occludens of the junctions. In extreme situations the lysis of filaments joining to the junctions and the rupture of the membrane was observed. Total desintegration of the intercalate disc was rarely encountered probably due to the high ad
