Wartime neurosurgical experience in Lebanon, 1982-85. II: Closed craniocerebral injuries.

This report presents 64 patients who sustained military-related closed craniocerebral injuries during the Lebanon conflict of 1982-85, all of whom underwent CT scanning at the initial assessment. Of these, 59% required surgery for removal of hematomas, depressed fractures and for monitoring intracranial pressure, in addition to intensive care management of elevated intracranial pressure and associated insults. CT scan revealed brain concussion only in 23%, depressed fracture in 9%, brain contusion alone in 17%, extracerebral hematomas in 17%, intracerebral hematomas in 11%, and diffuse axonal

Classification of Sport-Related Head Trauma: A Spectrum of Mild to Severe Injury.

OBJECTIVE: To identify the types of injuries the human brain incurs as a result of traumatic forces applied to the cranium. In athletic events and endeavors, the full spectrum of intracranial hemorrhages in various compartments, raised intracranial pressure, and diffuse nonhemorrhagic damage may be seen. In this review, we describe these serious injuries and the more common mild traumatic brain injury in their clinical presentations and relate concussion classification to the overall picture of traumatic brain injury. METHODS: Our cumulative experience with athletic injuries, both at the catas

Trends in mortality rates and correlations between intracranial injuries and external causes: A Japanese population study.

The age-standardized incidence of head trauma in 2016 was 369 per 100,000 people worldwide. The Western Pacific region, including Japan, had the highest incidence. This study aimed to extract ICD-10 code data for intracranial injury (S06) and external causes of morbidity and mortality (V01-Y89), analyze their characteristics and interrelationships, and contribute to these diseases' prevention, treatment, and prognosis. The number of deaths according to injury type and external cause type of intracranial injury published by the Japanese government was statistically analyzed using JoinPoint, and

The Pathophysiology of Traumatic Brain Injuries and the Rationale Behind Creatine Supplementation as a Potential Therapy: A Review.

Traumatic brain injuries (TBI) are detrimental to the brain in a variety of ways. Mild traumatic brain injuries (mTBI) are concussions; these are common events that disrupt typical brain functioning and send millions of patients to seek acute care each year globally. Despite the frequency of mTBIs, clinicians have few tools, pharmacologic and nonpharmacologic, to promote recovery and alleviate symptoms. After a TBI, complex biomolecular signaling, diffuse axonal stretching, and glutamate excitotoxicity occur, along with other pathological sequelae. Creatine has been shown to improve cognitive

New magnetic resonance imaging techniques for the evaluation of traumatic brain injury.

Although current computerized tomography (CT) and magnetic resonance imaging (MRI) techniques have shown great utility in diagnosing various aspects traumatic brain injury, damage resulting from mild diffuse brain injury often goes undetected with these procedures. Newly developed MRI techniques, including magnetization transfer imaging (MTI) and diffusion-weighted imaging (DWI), have been proposed to have enhanced sensitivities for identifying damage induced by both diffuse and focal brain injury. Results from recent initial studies with experimental models of brain injury suggest that MTI ma

Evaluation of three animal models for concussion and serious brain injury.

Three animal models were evaluated in this study involving head impacts of the rat, including the Marmarou drop-weight and two momentum-exchange techniques. In series 1, 36 Wistar rats were hit on the side of the free-moving head using Marmarou's 450 g impact mass at 4.4, 5.4, and 6.3 m/s. Head acceleration was measured and injuries were observed. The 6.3-m/s side impact resulted in no deaths, no skull fractures, infrequent contusions, and some injuries consistent with diffuse axonal injury. In series 2, 57 Marmarou drop-weight tests were conducted to study head biomechanical responses. Marmar

Paroxysmal sympathetic hyperactivity associated with posterior reversible encephalopathy syndrome.

Paroxysmal sympathetic hyperactivity (PSH) causes paroxysms of increased sympathetic hyperactivity in the setting of acquired brain injury. Posterior reversible encephalopathy syndrome (PRES) is a clinico-radiological syndrome characterised by cortical/subcortical vasogenic oedema in multiple brain areas. Patients present with seizures, encephalopathy and headaches. In this article, we report a patient who presented in status epilepticus with a hypertensive peak and decreased level of consciousness. Brain imaging showed diffuse hyperintensities with beading intracranial vessels. Then, the pati

Transorbital penetrating brain injury by branchlet: a rare case.

Penetrating injury of the brain and skull is uncommon, representing about 0.4% of head injuries. With advances in radiological techniques such as high-resolution and reconstruction computed tomography (CT), assessment of injuries is more accurate and easier. In this article, we report the case of a 46-year-old man presenting with head injury after a branchlet had penetrated through the right orbit into the brain. CT scan of the brain revealed diffuse subarachnoid hemorrhage, intraventricular hemorrhage, and mild obstructive hydrocephalus. CT scan of the brain with reconstruction revealed that

'Concussion' is not a true diagnosis.

In current usage ‘concussion’ describes a clinical presentation, but does not identify the underlying pathological process and therefore cannot be considered a true diagnosis. However, mounting evidence indicates diffuse axonal injury as a likely pathological substrate for concussion, thereby providing a framework to develop true diagnostic criteria.

Magnetic resonance imaging after 'diffuse' nonmissile head injury. A neurobehavioral study.

The diagnosis of diffuse brain injury is considered when computed tomography provides no evidence of an intracranial mass lesion in acute nonmissile head injury. Magnetic resonance imaging (MRI) of a young woman who had sustained severe diffuse brain injury five years earlier disclosed multifocal lesions involving the frontal, temporal, parietal, and occipital lobes. We report the results of serial neurobehavioral assessment for clinical correlation with the brain lesions visualized by MRI. Pending confirmation of our findings in a series of patients studied by MRI during the acute and chronic

Sensitivity of Diffusion Tensor Imaging for Assessing Injury Severity in a Rat Model of Isolated Diffuse Axonal Injury: Comparison with Histology and Neurological Assessment.

Diffuse axonal brain injury (DAI) is a common, debilitating consequence of traumatic brain injury, yet its detection and severity grading remain challenging in clinical and experimental settings. This study evaluated the sensitivity of diffusion tensor imaging (DTI), histology, and neurological severity scoring (NSS) in assessing injury severity in a rat model of isolated DAI. A rotational injury model induced mild, moderate, or severe DAI in male and female rats. Neurological deficits were assessed 48 h after injury via NSS. Magnetic resonance imaging, including DTI metrics, such as fractiona

Comparison of computer tomography and magnetic resonance imaging scans on the third day of life in term newborns with neonatal encephalopathy.

OBJECTIVE: Our goal was to compare the patterns of brain injury detected by computed tomography, conventional MRI (T1- and T2-weighted sequences), and diffusion-weighted MRI in a cohort of term newborns with neonatal encephalopathy studied uniformly with all 3 modalities on the third day of life. METHODS: Term newborns (> or =36 weeks' gestation) admitted to our center with neonatal encephalopathy were scanned with computed tomography, MRI, and diffusion-weighted MRI at 72 (+/-12) hours of life (n = 48). Each modality was scored independently of the other with previously validated scoring sys

Penetrating Brain Injury through a Pre-existing Skull Defect: Case Report.

Introduction: Penetrating brain trauma represents about 0,4% of traumatic brain injuries. In the literature, there is a number of case studies and case-series on non-missile penetrating injuries management, but reports on penetrating brain injuries through a craniectomy are lacking. Case presentation:A 44-year-old male was transferred to the Emergency Room in a comatose state after being stabbed on the head by a sharp object at an area where a previous decompressive craniectomy took place years ago. Computed tomography of the head showed an extensive intracerebral hemorrhage with intraventricu

Patterns of damage in the mature neonatal brain.

Patterns of damage in the mature neonatal brain can be subdivided into focal, multifocal and diffuse. The main cause of diffuse brain damage in the term newborn is hypoxic-ischaemic encephalopathy (HIE). HIE is still the major recognized perinatal cause of neurological morbidity in full-term newborns. MRI offers today the highest sensitivity in detecting acute anoxic injury of the neonatal brain. Conventional acquisition techniques together with modern diffusion techniques can identify typical patterns of HIE injury, even in the early course of the disease. However, even though highly suggesti

High-field proton magnetic resonance spectroscopy of a swine model for axonal injury.

A miniature swine model for diffuse brain injury has recently been developed that replicates the inertial loading conditions associated with rotational acceleration during automotive accidents. The swine model induces diffuse axonal pathology without macroscopic injury such as contusions and hematomas, thus affording a unique opportunity to study axonal injury with noninvasive techniques such as magnetic resonance imaging (MRI) and spectroscopy (MRS). In the present study, we evaluated this diffuse injury model with proton MRS, in vivo, using a high-field (4.0-T) MR scanner, since MRS has been

Pathology of head trauma.

This article reviews the essential primary and secondary injuries attributable to traumatic brain injury (TBI) which causes one third of all injury deaths in the United States. Motor vehicle crashes, falls, assaults, guns, sports, and recreational activities are the major causes of TBI. Secondary peak incidences of TBI occur in infants and children and the elderly. Conditions that increase risk for accidents include alcoholism, prior head injury, prior meningitis, seizure disorders, mental retardation, and psychiatric disorders. However, gunshot wounds to the head are steadily increasing and s

Rostral brain axonal injury in congenital central hypoventilation syndrome.

Brain injury underlying the state-related loss of ventilatory drive, autonomic, cognitive, and affective deficits in congenital central hypoventilation syndrome (CCHS) patients appears throughout the brain, as demonstrated by magnetic resonance (MR) T2 relaxometry and mean diffusivity studies. However, neither MR measure is optimal to describe types of axonal injury essential for assessing neural interactions responsible for CCHS characteristics. To evaluate axonal integrity and partition the nature of tissue damage (axonal vs. myelin injury) in CCHS, we measured water diffusion parallel (axia

Traumatic brain injury in the developing rat pup: studies of ICP, PVI and neurological response.

Diffuse brain swelling is a common complication in young victims of a seven head injury but, there is a lack of data on relevant models of injury. We produced diffuse brain injury in 21 day old Lewis rat pups (N = 33) by modifying a recently established weight-drop-model. The trauma threshold, neurological response, histological changes, intracranial pressure (ICP), and arterial blood pressure (ABP) were determined. In addition, the pressure-volume-index (PVI) was measured 15 min before, 2 min, and 1 h after brain injury. In the 1 m/100 g group 4 of 5 rats died, whereas in the 0.5 m/100 g only
