[Intraoprative identification and monitoring of motor structures of the brain stem].

The purpose of this work was to evaluate the method of identification of motor structures of the bottom of the IV ventricle in order to find the zone through which it is possible to safely approach the bulky formation and to assess the efficacy of subsequent monitoring of these structures for the prevention of their intraoperative injury. Fourteen patients with bulky tumors of the caudal portions of the brain stem aged 4 to 57 were examined. Motor structures of the brain stem (nerves VII or IX, X, and XII, depending on the site of injury) were identified by direct electrostimulation of the sur

Hyperfractionated radiotherapy and polychemotherapy in brain stem tumors in children.

Between October 1989 and January 1991 five children with brain stem tumors were treated with sequential chemo- and radiotherapy. The polychemotherapy consisted of procarbazine, ifosfamide, etoposide, methotrexate, cisplatin and cytosine arabinoside. Locally, hyperfractionated radiotherapy was delivered at a total dose of 63.8 Gy (1.1 Gy twice daily, 10 fractions per week). After a median observation time of 11.8 (range 4-23) months from diagnosis three children are alive and without evidence of tumor progression. Two patients died from tumor progression 11 and 16 months respectively after init

Epidermoid cysts of the brain stem. Report of three cases.

The authors report the cases of three patients with epidermoid cysts which insinuated themselves into the brain stem. In all three patients, the tumor occupied the pons, although in one it was predominantly located in the medulla. The cyst contents and nonadherent tumor capsule were removed in all three patients, but no attempt was made to remove tumor densely adherent to the brain stem. One patient's cyst was removed in one operation, but maximal resection in the other two required two operations. After surgery, sixth nerve function completely returned in one patient; another patient had a st

Lesions of the brain stem: assessment by magnetic resonance imaging.

Ninety-six magnetic resonance imaging (MRI) studies demonstrating solitary brain stem lesions were reviewed in order to establish distinguishing features between tumours and other lesions. Histological confirmation of the radiological diagnoses was obtained in 33% of patients. The morphology of the lesion rather than its signal characteristics was the most useful feature in differential diagnosis, except after haemorrhage. However, areas of abnormal T2 recovery time were significantly more extensive than areas of abnormal T1 recovery time in tumours at presentation. In other lesions and in tum

[Primary hematoma of the brain stem: considerations apropos of a case].

The case of a 48 year-old woman presenting with a primary pontine hematoma is reported. The clinical evolution simulated a brain stem tumor and the diagnosis of hematoma was not established before the operation. The preoperative neurological deficit improved except for a facial nerve palsy. These hematomas are thought to be due to rupture of "cryptic" arteriovenous malformations and should be differentiated from those secondary to systemic hypertension. The diagnosis of brain stem hematomas should be considered in any case of brain stem lesion, especially in young and normotensive patients.

[Topography of cholinergic neurons in the brain stem of the human fetus].

Cholinacetyltransferase (ChAT) activity has been studied in 56 nuclei of the cerebral trunk in human fetuses at the age of 6-8 lunar months. Cytoplasmic and synaptic ChAT activity has been revealed and three types of neurons for cholinergic synaptic transmission has been distinguished. There are only cholinergic-noncholinoceptive neurons in five macrocellular nuclei of the cranial nerves. In 25 nuclei (paravicellular, reticular, pigmented, sensitive nuclei of the cranial nerves, nuclei of the funiculi posterior and some other switching centres) there are only noncholinergic-cholinoceptive neur

Microsurgery of teratoma and germinoma involving the diencephalon and the brain stem.

The authors encountered 95 cases of germinoma (two-cell pattern pinealoma) and teratoma involving the third ventricle and the pineal region from 1962 to 1980. Germinoma is radiosensitive. However, removal (extensive or total) of the tumour followed by postoperative irradiation showed better results than simple radiotherapy. In the recent 62 cases, 5-year and 10-year survival rates were 80.2% and 69.3% respectively (in children under 15 years, both were 86.9%). Teratoma is not radiosensitive and requires operation. For operating on a pineal germinoma, either an infratentorial supracerebellar ap

The Implication of Hypocretin in Drug Abuse and Arousal in the Brain Stem.

Hypocretin (orexin, Hcrt) neurons located in the lateral hypothalamus (LH) project widely into the brain and are thus responsible for the physiological action of the hypocretin complex. Hypocretin is involved in both arousal and addiction, and brainstem areas such as the locus coeruleus (LC), paragigantocellularis (PGi), and dorsal raphe (DR) contribute to these functions. In the present review, we focus on the effect of Hcrt on drug abuse and arousal in the brainstem.

Brain stem encephalitis is a rare complication of COVID-19.

Here, we describe the clinical phenotype of SARS-CoV-2-related CNS disease and evaluate the SARS-CoV-2 antibody index as a tool to differentiate between a direct (viral) and indirect etiology. Out of >4000 hospitalized patients with COVID-19, we included 13 patients with neurological symptoms with suspicion of neuroinflammation. On clinical grounds, eight were classified as having a possible/probable relationship between neurological symptoms and COVID-19. A clinically distinctive phenotype of brainstem and cerebellar symptoms was seen in 6/8 patients. As we found a positive SARS-CoV-2 antibod

Botulinum Toxin Type A: Assessing The Effects on The Brain Stem.

BACKGROUND: In this study, our aim is to investigate the possible effects of Botulinum toxin type A administrations in the early and late periods on the brain stem. METHODS: Eighteen white New Zealand rabbits were used in this study with the subjects being divided into three groups. Group I received 0.05 mL sterile saline to the left anterior auricular muscles. Group II and III were injected with Botulinum toxin type A (Botox, Allergan) to the left anterior auricular muscles. Group II was sacrificed 5 days after application and Group III was sacrificed 12 weeks after application; brain stem t

Electrical Guidance of Human Stem Cells in the Rat Brain.

Limited migration of neural stem cells in adult brain is a roadblock for the use of stem cell therapies to treat brain diseases and injuries. Here, we report a strategy that mobilizes and guides migration of stem cells in the brain in vivo. We developed a safe stimulation paradigm to deliver directional currents in the brain. Tracking cells expressing GFP demonstrated electrical mobilization and guidance of migration of human neural stem cells, even against co-existing intrinsic cues in the rostral migration stream. Transplanted cells were observed at 3 weeks and 4 months after stimulation in

Rebleeding and Outcome in Patients with Symptomatic Brain Stem Cavernomas.

PURPOSE: We sought to evaluate the long-term functional outcomes and identify the potential risk factors for rebleeding in patients with brain stem cavernous malformations (BCMs) who presented with hemorrhages and were surgically or conservatively treated and prospectively monitored. METHODS: From January 1990 to July 2015, we included patients with first hemorrhagic episodes secondary to single BCMs. Modified Rankin score (mRS) was used for neurological status assessment. Univariate and multivariate regression statistics were used to identify the risk factors for rebleeding. RESULTS: A tota

Entracking as a Brain Stem Code for Pitch: The Butte Hypothesis.

The basic nature of pitch is much debated. A robust code for pitch exists in the auditory nerve in the form of an across-fiber pooled interspike interval (ISI) distribution, which resembles the stimulus autocorrelation. An unsolved question is how this representation can be "read out" by the brain. A new view is proposed in which a known brain-stem property plays a key role in the coding of periodicity, which I refer to as "entracking", a contraction of "entrained phase-locking". It is proposed that a scalar rather than vector code of periodicity exists by virtue of coincidence detectors that
