Bridging therapy with axicabtagene ciloleucel for large B-cell lymphoma: results from the US Lymphoma CAR-T Consortium.

During the manufacturing period of autologous chimeric antigen receptor (CAR) T-cell therapy, patients may experience a decline in their condition due to cancer progression. In this study, we investigated the impact of bridging therapy (BT) on the outcome of patients with relapsed/refractory large B-cell lymphoma who received antilymphoma treatment between leukapheresis and axicabtagene ciloleucel (axi-cel) infusion. We conducted our analysis using data from the multicenter US Lymphoma CAR-T Consortium, with a median follow-up of 33 months (range, 4.3-42.1). Out of the 298 patients who underwe

Liver organoids cocultured on decellularized native liver scaffolds as a bridging therapy improves survival from liver failure in rabbits.

The present study aimed to develop viable liver organoids using decellularized native liver scaffolds and evaluate the efficacy of human liver organoid transplantation in a rabbit model of cirrhosis. Liver organoids were formed by coculture of hepatocyte-like cells derived from the human-induced pluripotent stem cells with three other cell types. Twelve 3-mo-old New Zealand White Rabbits underwent a sham operation, bile duct ligation, or biliary duct ligation followed by liver organoid transplantation. Liver organoid structure and function before and after transplantation were evaluated using

Use of Extracorporeal Membrane Oxygenation as Bridge to Replacement Therapies in Cardiogenic Shock: Insights From the Extracorporeal Life Support Organization.

BACKGROUND: There has been increasing use of extracorporeal membrane oxygenation (ECMO) as bridge to heart transplant (orthotopic heart transplant [OHT]) or left ventricular assist device (LVAD) over the last decade. We aimed to provide insights on the population, outcomes, and predictors for the selection of each therapy. METHODS: Using the Extracorporeal Life Support Organization Registry between 2010 and 2019, we compared in-hospital mortality and length of stay, predictors of OHT versus LVAD, and predictors of in-hospital mortality for patients with cardiogenic shock that were bridged wit

Bridging the Gap: A Pilot Program to Understand and Meet the Needs of Pediatric Patients and Families as They Transition Off Cancer-Directed Therapy.

Background: With improved curative therapies, over 80% of children and adolescent/young adults diagnosed with cancer are expected to live into adulthood. This population is at risk for increased morbidity and early mortality and requires ongoing health care and surveillance for late effects of treatment. This pilot study assessed the acceptability of a structured medical visit at the completion of cancer-directed therapy as well as patient/family's knowledge of diagnosis and other aspects of care. Method: Patients/families who were 0 to 6 months from completion of cancer-directed therapy atten

Bridging-therapy with intravenous recombinant tissue plasminogen activator improves functional outcome in patients with endovascular treatment in acute stroke.

BACKGROUND: Although endovascular treatment for proximal cerebral vessel occlusion is very effective, it remains controversial if intravenous thrombolysis (IVT) prior to endovascular treatment is superior compared to endovascular treatment alone. In this study we compared functional outcomes and recanalization rates of endovascularly treated stroke patients with and without bridging IVT. METHODS: Patients with acute large artery occlusion within the anterior and posterior cerebral circulation eligible for intraarterial revascularization with and without prior IVT were included in this monocen

Is mechanically bridging patients with a failing cardiac graft to retransplantation an effective therapy? Analysis of the United Network of Organ Sharing database.

BACKGROUND: The results of bridging patients with cardiac allograft failure to retransplantation (ReTx) with mechanical circulatory support (MCS) have not been well studied. The United Network of Organ Sharing (UNOS) database was used to analyze outcomes of patients successfully bridged with MCS to cardiac ReTx. METHODS: Of 1,690 cardiac ReTx identified in the UNOS database from October 1987 to July 2011, 149 (8.8%) were bridged to ReTx with MCS. RESULTS: Patients bridged to ReTx with MCS had a poorer survival than patients not bridged (p < 0.0001). ReTx after ventricular assist device (VAD)

Phage therapy for multidrug-resistant pulmonary infections: bridging precision matching, lung delivery, host immunity, and clinical translation.

Chronic pulmonary infections caused by multidrug-resistant (MDR) bacteria are a growing global health problem, and the declining effectiveness of conventional antibiotics increases the need for alternative antimicrobial strategies. Unlike broad-spectrum antibiotics, bacteriophages are highly specific, and their use therefore offers not only antibacterial activity but also the possibility of reshaping the airway microbial ecosystem in a targeted manner. In this review, we reframe phage therapy as a precision microbiome-modulating strategy for chronic pulmonary infections. We first review precis

Outcomes of Bridging Therapy in Patients With Relapsed/Refractory Large B Cell Lymphoma Receiving Third-Line CAR T.

Bridging therapy (BT) is frequently used in patients with relapsed/refractory (R/R) large B-cell lymphoma (LBCL) undergoing chimeric antigen receptor T cell (CAR T) therapy, but the optimal BT approach remains uncertain. We evaluated BT strategies and their associations with response and survival outcomes. Accordingly, we included patients with R/R LBCL evaluated for third-line or later CD19-directed CAR T at Princess Margaret Cancer Center from 2017 to 2024 (follow-up to 01/2025). BT modalities included chemotherapy, polatuzumab-based chemotherapy, radiotherapy, corticosteroids, and none. We

Photodynamic Therapy as an Adjunct During Stent-Bridged Neoadjuvant Management of Obstructive Colon Cancer: A Case-Based Narrative Review.

Malignant left-sided large-bowel obstruction remains a high-risk presentation that often requires urgent decompression. Self-expanding metal stents (SEMS) can convert an emergency to an elective operation and may create a window for staging, multidisciplinary planning, and (in selected patients) neoadjuvant systemic therapy. Photodynamic therapy (PDT) is a light-activated, locally delivered ablative modality that has been used historically for palliation or local control of colorectal and pelvic recurrences, but its role in contemporary management of obstructing colon cancer remains investigat

AI-Guided Cognitive Behavioral Therapy for Depression and Anxiety: Bridging the Mental Health Treatment Gap Through Digital Psychiatry.

Background: Depression and anxiety disorders remain among the leading contributors to global disability and represent a major public health challenge. Although evidence-based psychotherapies are available, access to treatment remains limited due to structural, economic, geographical, and workforce-related barriers. Digital mental health interventions have emerged as scalable approaches to reducing this treatment gap, with artificial intelligence (AI)-guided cognitive behavioral therapy (CBT) representing a rapidly developing and clinically relevant extension of digital psychotherapy. Bridging Clinical Trials to Real-World Clinical Practice: TCHP Neoadjuvant Therapy Outcomes in HER2-Positive Breast Cancer.

PURPOSE: Trastuzumab-based neoadjuvant therapy (NAT) is standard for HER2-positive breast cancer (HER2+ BC). The TCHP regimen (docetaxel, carboplatin, trastuzumab, pertuzumab) demonstrated efficacy in clinical trials; however, real-world evidence remains limited. This study evaluates the effectiveness and safety of TCHP in routine clinical practice at King Hussein Cancer Center (KHCC). PATIENTS AND METHODS: This retrospective study included 161 patients with HER2+ BC who received at least one cycle of TCHP between January 2022 and October 2024. The primary endpoint was pathological complete r

Impact of Disease Biology and Bridging Strategy on Outcomes After CAR-T Cell Therapy in Relapsed/Refractory Multiple Myeloma.

Bridging therapy is commonly administered prior to B-cell maturation antigen (BCMA)-directed chimeric antigen receptor (CAR)-T cell therapy in relapsed/refractory multiple myeloma (RRMM) to maintain disease control during the manufacturing period. However, the optimal choice and intensity of bridging therapy remain unclear, and it is debated whether outcomes are primarily influenced by bridging strategy or underlying disease biology. We aimed to evaluate the impact of different bridging strategies on outcomes after BCMA-directed CAR-T cell therapy in RRMM, while accounting for disease biology,

Bridging Therapy and Risk of Bleeding and Thrombosis in Continuous-Flow Left Ventricular Assist Device Patients: A Quasi-Experimental Study.

Bridging with low-molecular-weight heparin (LMWH) is recommended in continuous-flow left ventricular assist device (CF-LVAD) patients during subtherapeutic international normalized ratios (INRs). We aimed to assess the risk of adverse events during bridging in patients implanted at Leiden University Medical Center between 2010 and 2024. Incidence rates and hazard ratios of major bleeding, thromboembolic events, neurologic complications, and death with 95% confidence intervals (95% CI) were estimated by time-dependent Cox regression. Using a regression discontinuity design, we mimicked a trial

CNS bridging radiotherapy achieves rapid cytoreduction before CAR T-cell therapy for aggressive B-cell lymphomas.

Chimeric antigen receptor (CAR) T-cell therapy (CART) for central nervous system lymphoma (CNSL) is a promising strategy, yet responses are frequently not durable. Bridging radiotherapy (BRT) is used for extracranial lymphoma in which it can improve CART outcomes through cytoreduction of high-risk lesions. We hypothesized that BRT would achieve similar, significant cytoreduction before CART for CNSL (CNS-BRT). We identified patients with CNSL with non-Hodgkin B-cell lymphoma who received CNS-BRT before commercial CART. Cytoreduction from CNS-BRT was calculated as change in lesion size before C
