Trekking Poles to Aid Multiple Sclerosis Walking Impairment: An Exploratory Comparison of the Effects of Assistive Devices on Psychosocial Impact and Walking.

BACKGROUND: Walking dysfunction is reported by two-thirds of persons with multiple sclerosis (MS). Assistive devices are frequently recommended to improve walking; however, it is uncommon to consider their psychosocial impact, although many users abandon their assistive devices. The psychosocial impact, walking, balance, and fatigue associated with three assistive devices were compared to guide clinical decision making. METHODS: Twenty-five persons with MS (median Expanded Disability Status Scale score, 4.0; range, 2.5-6.0) who reported walking difficulty were trained in the use of three assi

The cost-effectiveness of recommended adjunctive interventions for knee osteoarthritis: Results from a computer simulation model.

OBJECTIVE: To estimate the potential lifetime health gains, healthcare costs, and cost-effectiveness of recommended adjunctive treatments for knee osteoarthritis delivered in addition to established core treatments, relative to core treatment only, from the perspective of the New Zealand (NZ) healthcare sector. DESIGN: Recommended adjunctive knee osteoarthritis treatments were identified in clinical practice guidelines. Evidence of effectiveness was sourced from existing systematic reviews and meta-analyses. Treatment costs were calculated by applying local reference prices to estimated resou

Development of a community mobility skills course for people who use mobility devices.

OBJECTIVE: We assessed mobility device skills in a lived-in environment on a community mobility skills course (CMSC) and related those skills to previously demonstrated skills in a controlled environment on an inside mobility skills course (IMSC). METHOD: Six mobility device users were selected from 91 adults who had previously completed the IMSC. Each of the following device groups was represented: power wheelchair, manual wheelchair, scooter, cane, crutch, and walker. CMSC tasks were modified from IMSC tasks and designed using materials in the community. RESULTS: All participants completed

Trunk kinematics in hemiplegic gait and the effect of walking aids.

OBJECTIVE: To establish baseline measurements of trunk movements during hemiplegic gait, to assess the relationship between trunk movements and walking ability, and to investigate the effect of walking aids on the trunk movements. METHOD: Twenty subjects with a chronic hemiplegia from a stroke, who could walk independently, were recruited. Lateral and vertical movements of the pelvis, and symmetry of these movements were measured using CODA (a three-dimensional movement analysis system) as the subjects walked at their own pace without an aid. They were also tested as they walked with a stick

Ankle-foot orthoses: effect on energy expenditure of gait in spastic diplegic children.

This study examined the effect of conventional plastic bilateral ankle-foot orthoses (AFOs) on the energy expenditure of ambulation in spastic diplegic cerebral palsy patients. Eighteen subjects (aged 8.3 +/- 2.83 years) ambulated five minutes at freely chosen velocities with and without their AFOs. Heart rate (beats per minute) and distance (meters) ambulated were recorded for each minute. A physiologic cost index was calculated by determining the ratio of walking heart rate to velocity [(HRamb-HRrest) divided by meters per minute]. The mean physiologic cost indexes for the last three minutes

Endovascular Aortic Repair for Isolated Infrarenal Aortic Stenosis.

An 87-year-old man with severe intermittent claudication and lower limb pain at rest was referred to our hospital for examination. Computed tomography (CT) angiography imaging showed a severe stenosis in the infrarenal abdominal aorta. The ankle brachial indices (ABIs) were 0.62 (right) and 0.60 (left). Endovascular aortic repair was performed with distal protection. The postoperative course was uneventful, and postoperative CT showed no stenosis or distal embolization. The ABI improved to the normal values of 1.02 (right) and 1.13 (left). The patient was followed up as an outpatient without a

Bilateral knee arthrodesis in a patient with common variable immunodeficiency.

Patients with common variable immunodeficiency can present with debilitating arthritis. We present the case of a 42-year-old man with bilateral knee arthritis who underwent a right total knee arthroplasty that subsequently became infected. Five months after resection arthroplasty, his right leg spontaneously fused in extension, but his left knee was limited to an arc of motion between 90° and 110°. At the patient's request, he underwent a noninstrumented arthrodesis of the left knee. The patient now has bilateral arthrodeses and ambulates with a cane. While arthroplasty may be attempted in suc

Mesothelioma in an agricultural community of India: a clinicopathological study.

Mesothelioma is an uncommon neoplastic condition. Its association with asbestos exposure is well established, but it occurs even in non-industrial workers, and naturally there must be some other factors in its aetiology than asbestos exposure. In this report, five patients with primary mesothelioma, all belonging to a rural agricultural community with no chance of any asbestos exposure, have been documented. There was one common interesting observation, in that all five patients were associated with sugar-cane farming or an allied trade. Whether this observation is coincidental or has any aeti

Complication of Middle Cerebral Artery Aneurysm Surgery: An M2 Tear at the Neck, Managed with a Salvage M2-to-M2 In Situ Bypass.

An unexpected rupture at the aneurysm neck, with or without adjacent arterial injury or compromise of distal branches during microsurgical clipping, can be a challenging surgical problem to resolve. In this presented case of a neurologically intact 65-year-old woman, elective clipping of an unruptured right middle cerebral artery bifurcation aneurysm was complicated by an unexpected M2 tear at the neck, involving the origin of the frontal M2. Attempts to seal the tear directly, using various techniques, failed; therefore, it was ultimately managed with sacrifice of the vessel and a salvage sid

Maggot therapy for wound debridement in a traumatic foot-degloving injury: a case report.

A 43-year-old man was treated for a traumatic degloving injury to his foot with a transmetatarsal amputation and wound care because of the extensive soft tissue loss. After biweekly sharp debridements in the office for 4 weeks, very minimal skin ingrowth was noted. Maggot therapy was then implemented for a 48-hour treatment and a second treatment for 72 hours to help reduce excessive fibrosis and to painlessly debride the tissues to expose the granular base. Daily dressing changes for the next 6 weeks successfully allowed complete wound closure without any additional interventions. During the

Functional capabilities of lower limb amputees.

Five hundred lower limb amputees were evaluated by personal interview at the Artificial Limb Centre, Pune, India. Information thus obtained on activities of daily living and functional capabilities with their prostheses was analysed. The analysis showed that 55% of the amputees were totally independent, 40% had to use a crutch or cane and only 5% were solely dependent on a wheelchair for ambulation. Further, it was also confirmed that as age increased, functional independence decreased, and that below-knee amputees were more independent than above-knee and bilateral amputees. When compared wit

Citrus mealybug (Hemiptera: Pseudococcidae) movement and population dynamics in an arbor-trained vineyard.

The citrus mealybug, Planococcus citri (Risso) (Hemiptera: Pseudococcidae), is the main grapevine pest in vineyards in some countries, such as Spain and Brazil. In Galician vineyards (northwestern Spain), mealybug population levels are low because the accumulated degree-days are lower than in other grapevine-growing areas. The main problem caused by mealybugs is the transmission of viruses, even at low infestation levels. The active period of citrus mealybug in the study vineyard lasted from July until December, with an important movement peak at the end of July and August and a lower peak in

In-toeing in children with type I osteogenesis imperfecta: an observational descriptive study.

BACKGROUND: Osteogenesis imperfecta is an autosomal-dominant disorder of the connective tissue. Also known as brittle bone disease, it renders those affected susceptible to fractures after minimal trauma. Therefore, it is important to minimize the risk of falls and subsequent fractures in patients with this disease. In-toeing is a common condition in children that can result from various pathologic entities, including anteversion, internal tibial torsion, and metatarsus adductus. These conditions can result in frequent tripping and other functional problems. METHODS: A descriptive study was u

Tropical spastic paraparesis treated with Combivir (lamivudine-zidovudine).

Tropical spastic paraparesis (TSP) or human T-cell leukemia virus-type 1 (HTLV-I)-associated myelopathy is caused by human T-lymphotropic virus type 1. It is a slow, progressive spastic paraparesis with significant morbidity and causing profound repercussions on quality of life. No therapies have been found to persistently improve the outcome in these patients. We present a patient with HTLV-1-associated myelopathy/TSP (HAM/TSP) who was treated with Combivir (lamivudine-zidovudine, GlaxoSmithKline, London, UK). She was walker-dependent for several years but, soon after treatment with lamivudin

Long-term assessment of nano and bulk copper compound exposure in sugarcane (Saccharum officinarum).

As interest in the use of copper-based nanomaterials in agriculture continue to increase, research into their exposure effects must expand from short-term, high exposure studies to long-term studies at realistic concentrations. Long-term studies can better elucidate the implications of copper nanomaterial exposure by allowing plants to mature and adapt to higher copper concentrations. In this study, sugarcane plants were grown to maturity in large nursery pots using soils amended with one of the following treatments: Kocide 3000 (Cu(OH)2), a nano-sized CuO (nCuO), a bulk-sized CuO (bCuO), copp

Walking Aid Use in Canada: Prevalence and Demographic Characteristics Among Community-Dwelling Users.

BACKGROUND: Mobility limitations represent the third most prevalent cause of disability, affecting more than 1.9 million community-dwelling Canadians. Walking aids are often prescribed to reduce the impacts of mobility impairments. There are limited data on walking aids since 2004. OBJECTIVE: The objectives of this study were to investigate the prevalence of walking aid use in Canada and to explore demographic characteristics among users of walking aids. DESIGN: The design used was a secondary analysis of a cross-sectional national survey. METHODS: Data were obtained from the 2012 Canadian

An unusual presentation of Brown-Sequard syndrome.

We report the case of a man with Brown-Sequard syndrome following a fall in which he sustained fractures of the thoracic and lumbosacral spine. Despite characteristic neurological findings, the diagnosis was delayed due to the absence of history of penetrating spinal trauma and incorrect attribution of unilateral-like weakness and numbness to lumbosacral trauma. A directed history and examination revealed that the patient was stabbed in the back with a penknife while leaving a bus and stepped down onto a paralyzed leg, which collapsed beneath him. The patient was given an antibiotic, underwent

Individual joint contribution to body weight support in the affected lower limb during walking in post-stroke hemiplegia.

OBJECTIVE: Patients with post-stroke hemiplegia have difficulty with body weight support during walking. However, it is unclear which intra-limb strategy for body weight support tends to predominate, and how the intra-limb strategy is related to gait function. Support moment and individual joint contribution to support moment are the parameters that reflect intra-limb strategy for body weight supporting. The aim of this study was to test whether support moment and individual joint contributions differed between post-stroke subjects with different gait function. METHOD: Laboratory gait analysi
