Intracochlear Pressure Measurements Quantify Overall Noise Exposure to the Inner Ear During Mastoidectomy.

INTRODUCTION: The acoustic exposure patients endure during otologic surgery is unclear, as previous studies have not captured the effects of vibrational noise on the inner ear. To fill this knowledge gap, we examined inner ear noise directly via intracochlear pressure changes during mastoidectomy and cochleostomy. METHODS: Inner ear stimulation was measured using fiber optic pressure sensors inserted into the cochlea of cadaveric heads using a transcanal approach. A tympanomeatal flap was elevated, cochleostomies were made into scala vestibuli and tympani, sensors were placed and secured to t

Similarities and Differences Between Vestibular and Cochlear Systems - A Review of Clinical and Physiological Evidence.

The evoked response to repeated brief stimuli, such as clicks or short tone bursts, is used for clinical evaluation of the function of both the auditory and vestibular systems. One auditory response is a neural potential - the Auditory Brainstem Response (ABR) - recorded by surface electrodes on the head. The clinical analogue for testing the otolithic response to abrupt sounds and vibration is the myogenic potential recorded from tensed muscles - the vestibular evoked myogenic potential (VEMP). VEMPs have provided clinicians with a long sought-after tool - a simple, clinically realistic indic

[Hearing screening in newborn infants. Comparative studies and cost analysis with different instruments].

BACKGROUND: The necessity of screening examinations in newborns today cannot be denied. Up to now, it has only been achieved in a few countries to introduce a general hearing screening for newborns. METHODS: We examined 100 newborns (200 ears) at their third day of life. All ears were evaluated with the TEOAE-screening-device ECHOSCREEN. In addition, we carried out BERA screening examinations with the ALGO PORTABLE or with the EVOFLASH on 100 of these 200 ears. As a reference method, we utilized TEOAE-examinations with the ILO-88 for all ears. RESULTS: 4.5% of the 200 ears examined with the

Age-related hearing loss: the status of Schuknecht's typology.

PURPOSE OF REVIEW: Recent developments in age-related hearing loss (ARHL) are reviewed with an emphasis on their relation to the framework advocated by Schuknecht. More than a classification scheme, Schuknecht's typology incorporates testable hypotheses about the bases of ARHL. Since there is presently no widely accepted competing framework, research in this area should be aimed at supporting, modifying, or replacing Schuknecht scheme. Only recently has our understanding of cellular changes and gene/environment interactions in ARHL achieved the level needed for hypothesis-driven experiments in

[Sound sensations produced by electric stimulation of the structures of the middle ear and the tympanic chord].

Certain facts must be clearly understood before any attempt is made to analyse the sensations produced by electrical stimulation of the structures of the middle ear. Firstly, when the current passes from an electrode to the skin or to any polarised structure whatsoever, the point at which the electrode is applied and the skin behave like the two plates of a condenser microphone. Electrical vibrations are transformed into ordinary acoustic resonances which can travel through air or bone. These acoustic resonances are fairly pure in comparison with electrical vibrations, judged by the standards

Receptor potentials of lizard hair cells with free-standing stereocilia: responses to acoustic clicks.

Receptor potentials of single hair cells in the free-standing region of the basilar papilla of the anaesthetized alligator lizard were measured intracellularly with micropipettes. Stimuli were primarily acoustic pulses (clicks) delivered to the tympanic membrane. The receptor potential was independent of click repetition rate for the range 10-150 clicks/s. This property is presumed to be the basis of the rate independence of the extracellular cochlear microphonic potential. The receptor potential wave-form consisted of a fast oscillatory component (or oscillation) superimposed on a usually pos

Blebs in inner and outer hair cells: a pathophysiological hypothesis.

INTRODUCTION: The ototoxic effects of cisplatin include loss of outer hair cells, degeneration of the stria vascularis and a decrease in the number of spiral ganglion cells. Scanning microscopy has shown balloon-like protrusions (blebs) of the plasma membrane of inner hair cells following cisplatin administration. The present study was undertaken to identify the possible role of inner and outer hair cell blebs in the pathogenesis of cisplatin-induced ototoxicity. MATERIALS AND METHODS: Twenty-five guinea pigs were injected with cisplatin and their hearing tested at different time-points, befo

ATP-gamma-S shifts the operating point of outer hair cell transduction towards scala tympani.

ATP receptor agonists and antagonists alter cochlear mechanics as measured by changes in distortion product otoacoustic emissions (DPOAE). Some of the effects on DPOAEs are consistent with the hypothesis that ATP affects mechano-electrical transduction and the operating point of the outer hair cells (OHCs). This hypothesis was tested by monitoring the effect of ATP-gamma-S on the operating point of the OHCs. Guinea pigs anesthetized with urethane and with sectioned middle ear muscles were used. The cochlear microphonic (CM) was recorded differentially (scala vestibuli referenced to scala tympa

Intensity-dependent peak shift in cochlear transfer functions at the cellular level, its elimination by sound exposure, and its possible underlying mechanisms.

Our systematic study of cochlear transfer functions has confirmed earlier results that, in a normal cochlea, the cochlear AC responses at any given cochlear location do not have a fixed best frequency at which the response is maximal. The best frequency depends on sound intensity, shifting to lower frequencies as the intensity is increased. This phenomenon may account for the so called 'half-octave shift' of maximum cochlear damage relative to the frequency of the damaging sound observed in studies of auditory noise exposure. Our experimental results combined with the results of others and wit

The effect of blood flow promoting drugs on cochlear blood flow, perilymphatic pO(2) and auditory function in the normal and noise-damaged hypoxic and ischemic guinea pig inner ear.

The effect of blood flow promoting drugs, such as hydroxyethyl starch (HES) either of low or high molecular weight (HES 70, HES 200), pentoxifylline, ginkgo biloba, naftidrofuryl and betahistine, and various combinations of the drugs was studied in unexposed and noise-exposed (broad-band noise, bandwidth 1-12 kHz, 106 dB SPL, 30 min) guinea pigs. The results were compared without therapy and placebo (isotonic saline, NaCl). The cochlear blood flow (CoBF) and the partial pressure of oxygen in the perilymph (PL-pO(2)) were continuously and simultaneously recorded over a period of 210 min. In add

[Hearing preservation and tinnitus following removal of acoustic neurinomas].

Thirty-five cases of unilateral acoustic neurinomas were analyzed with special reference to the postoperative eighth cranial nerve function. An additional three cases of bilateral acoustic neurinomas associated with neurofibromatosis were also analyzed. Out of a total of 40 neurinomas in all, 38 cases were retrospectively reviewed. The thirty-five cases of unilateral acoustic neurinomas were summarized as follows. The patients' age ranged from 23 to 69 years old. The tumor size varied as follows; 7 cases were confined to the internal acoustic meatus, 4 cases were 20 mm or less in their maximum

Effects of acute lead acetate exposure on adult guinea pigs: electrophysiological study of the inner ear.

The effects on humans of lead acetate exposure may involve the cranial nerves, since vertigo and sensory neuronal deafness have been reported in lead workers; however, there exist only a few reports concerning the dose effects of lead acetate both on the cochlea and the eighth cranial nerve. The effects of lead acetate on the cochlea and the eighth nerve were investigated systematically using cochlear microphonics (CM), whole-nerve action potential (AP), and endocochlear potential (EP) in guinea pigs (male albino Hartley). Guinea pigs were injected with 2 ml of a 1% solution of lead acetate (2

Otoacoustic Emissions.

Otoacoustic emissions are low-intensity sounds that are produced in the cochlea and transmitted through the middle ear apparatus to the ear canal. They can be detected and extracted from the background noise in the ear canal through the use of a sensitive microphone and selective filtering or averaging techniques. The technical aspects of emission recording are very similar to those associated with the detection and capture of auditory evoked potentials. Emissions provide an acoustic link to a physiological window through which we can view the auditory periphery using frequency-specific stimul

Ganglioside GM3 is essential for the structural integrity and function of cochlear hair cells.

GM3 synthase (ST3GAL5) is the first biosynthetic enzyme of a- and b-series gangliosides. Patients with GM3 synthase deficiency suffer severe neurological disability and deafness. Eight children (ages 4.1 ± 2.3 years) homozygous for ST3GAL5 c.694C>T had no detectable GM3 (a-series) or GD3 (b-series) in plasma. Their auditory function was characterized by the absence of middle ear muscle reflexes, distortion product otoacoustic emissions and cochlear microphonics, as well as abnormal auditory brainstem responses and cortical auditory-evoked potentials. In St3gal5(-/-) mice, stereocilia of outer

Persistent Auditory Nerve Damage Following Kainic Acid Excitotoxicity in the Budgerigar (Melopsittacus undulatus).

Permanent loss of auditory nerve (AN) fibers occurs with increasing age and sound overexposure, sometimes without hair cell damage or associated audiometric threshold elevation. Rodent studies suggest effects of AN damage on central processing and behavior, but these species have limited capacity to discriminate low-frequency speech-like sounds. Here, we introduce a new animal model of AN damage in an avian communication specialist, the budgerigar (Melopsittacus undulatus). The budgerigar is a vocal learner and speech mimic with sensitive low-frequency hearing and human-like behavioral sensiti

Ototoxic effects and mechanisms of loop diuretics.

Over the past two decades considerable progress has been made in understanding the ototoxic effects and mechanisms underlying loop diuretics. As typical representative of loop diuretics ethacrynic acid or furosemide only induces temporary hearing loss, but rarely permanent deafness unless applied in severe acute or chronic renal failure or with other ototoxic drugs. Loop diuretic induce unique pathological changes in the cochlea such as formation of edematous spaces in the epithelium of the stria vascularis, which leads to rapid decrease of the endolymphatic potential and eventual loss of the

Does repeated hyperbaric exposure to 4 atmosphere absolute cause hearing impairment? Study in Guinea pigs and clinical incidences.

HYPOTHESIS: Direct pressure applied on the inner ear cannot induce hearing loss. BACKGROUND: Three possible causes have been described in the literature for inner ear permanent lesions during scuba diving: pressure imbalance between the middle ear and the external ear, appearance of microbubbles in the internal ear, and direct effect of pressure on the inner ear. We seek to determine whether this last factor can be involved. METHODS: We submitted two groups of guinea pigs previously implanted with an electrode in the round window to a protocol of air diving in a hyperbaric chamber. Eardrums
