The Expanding Spectrum of Disease Treated by the Transnasal, Transsphenoidal Microscopic and Endoscopic Anterior Skull Base Approach: A Single-Center Experience 2008-2015.

INTRODUCTION: The transsphenoidal approach was initially developed in neurosurgical practice as an operative approach to the pituitary gland. The introduction of the operating endoscope has improved the versatility of the transsphenoidal approach, broadening the spectrum of lesions that can be treated effectively with this operative strategy. METHODS: We performed a retrospective review of all patients who underwent transnasal, transsphenoidal operations at Brigham and Women's Hospital from April 2008 to February 2015 and categorized each case by pathologic diagnosis. RESULTS: A total of 792

Thymic lymphoid hyperplasia with multilocular thymic cysts diagnosed before the Sjögren syndrome diagnosis.

BACKGROUND: Thymic lymphoid hyperplasia is often present with myasthenia gravis as well as other autoimmune diseases such as systemic lupus erythematosus and rheumatoid arthritis. Of the 4 cases of thymic lymphoid hyperplasia associated with Sjögren syndrome that have been reported, no case with a thymic lesion diagnosis that led to the diagnosis of Sjögren syndrome has been reported. We herein report a case of thymic lymphoid hyperplasia with multilocular thymic cysts, diagnosed before Sjögren syndrome. CASE PRESENTATION: A 37-year-old Japanese woman had an approximate 5-cm anterior mediasti

[The role of tomography (X-ray and magnetic resonance) in the diagnosis and determination of the extension of tumors in the roof of the third ventricle].

We describe the CT scan and MR appearance of neoplastic lesions affecting the roof of the third ventricle. The neoplastic lesions of this region may be of neuroepithelial origin (sub-ependymoma, colloid cyst), meningeal cell origin (meningiomas), metastatic or primary localization of lymphoproliferative tumors (lymphomas) and dysembryogenetic origin (epidermoid tumor). CT scan and MR imaging are useful techniques for diagnosis and planning of surgical or medical treatment. The primary role of these imaging techniques, is discussed in the quantification of extension, biologic malignancy and ana

Histological description of tomographic images of Taenia solium cysticerci in pig brains.

In the computed tomography (CT) scans of five pigs with neurocysticercosis (Taenia solium), interesting lesions produced by the larvae were selected. The pigs were euthanized and the corresponding lesions in their brains identified and processed for histopathology. It was found that the hypodense areas in CT scans corresponded to vesicular larvae that are presumably viable. The hyperdense areas were either solid in nature or had formed a rim around a hypodense center and in the brain slice they were either solid granulomas or colloidal cysts with inflammatory reaction in the periphery, perivas

Granulocyte colony-stimulating factor-producing cutaneous angiosarcoma with leukaemoid reaction arising on a burn scar.

BACKGROUND: Better understanding of embryology, histopathology and genetics of dysraphic conditions have lead to an expansion of this concept to entities with a similar microscopic appearance (e.g. enterogenous cysts, colloid cyst of the III-rd ventricle) or rated among neoplasms (e.g. mature teratoma), creating a certain conceptual confusion. Currently the diagnosis of "teratoma" is being substituted by "enterogenous cyst" or "teratomatous cyst". AIM OF PAPER: Clarification of concepts in this field and presentation of the experience of the Department of Neurosurgery of the Children's Memori

Chronic adult hydrocephalus due to uncommon causes.

The authors report on four unusual cases of chronic adult hydrocephalus extracted from a large personal series of patients with hydrocephalic dementia. A syndrome resembling that of normal pressure hydrocephalus was due to the following aetiologies in these patients: Paget's disease of the skull, unruptured cerebral arteriovenous malformation, colloid cyst of the third ventricle, and cerebromeningeal cysticercosis. All these patients exhibited disturbances in cerebrospinal fluid dynamics at the time of study, and were considerably improved by CSF shunting. The value of the continuous CSF press

Visualizing thyroid health: a pictorial journey through 2017 ACR TI-RADS and common thyroid pathologies.

With the advent of high-resolution ultrasonography (HRUS), more thyroid nodules are being detected than ever before, and they are being identified at an earlier stage. It poses a challenge for radiologists and clinicians in deciding what to do next. Most nodules are benign and require no follow-up and intervention. Even highly suspicious nodules can be followed up, if the size is small. Variations in HRUS interpretation among radiologists are common, with frequent misidentifications between spongiform and solid-cystic lesions, hypoechoic and very hypoechoic nodules, and microcalcification and

Role of intraoperative computed tomography scanner in modern neurosurgery - An early experience.

BACKGROUND: Intraoperative imaging addresses the limitations of frameless neuronavigation systems by providing real-time image updates. With the advent of new multidetector intraoperative computed tomography (CT), soft tissue can be visualized far better than before. We report the early departmental experience of our intraoperative CT scanner's use in a wide range of technically challenging neurosurgical cases. METHODS: We retrospectively analyzed the data of all patients in whom intraoperative CT scanner was utilized. Out of 31 patients, 24 (77.4%) were cranial and 8 (22.6%) spinal cases. Th

Tumours of the third ventricle.

Primary intraventricular tumors, including colloid cysts, choroid plexus papillomas, and ependymomas have their attachment within the IIIrd ventricle and stay within its confines. Secondary intraventricular tumours arise adjacent to the IIIrd ventricle, often in the suprasellar region, and include craniopharyngiomas, optic nerve gliomas, and meningiomas. Approaches to the ventricle include subfrontal, subtemporal, anterior transventricular, anterior and posterior transcallosal, and transtentorial routes. Memory problems have been noted with anterior dorsal methods, implicating damage to one or

Intracranial Cysts: A Single-Institution Experience With 27 Surgically Managed Cases.

Introduction Intracranial cysts (ICs) are rare pathologies that are often found incidentally during radiological examinations. They may appear in various brain regions and are categorized as normal, congenital, traumatic, or tumor-associated variants. ICs can be asymptomatic or cause symptoms, such as headaches, visual impairments, or seizures, depending on their size and location. Severe complications include obstructive hydrocephalus, loss of consciousness, and intracranial bleeding. Surgical excision is the most accepted type of management in most ICs. Objectives This study aimed to evaluat

A rare case of a concurrent large thyroglossal duct cyst with a base of tongue haemangioma.

An 85-year-old lady presented with a large midline neck mass. After 8 years of steady growth, the previously asymptomatic mass began to cause stridor and dysphagia. The patient's comorbidities included a previous partial glossectomy for haemangioma of the tongue, chronic obstructive pulmonary disease, congestive cardiac failure and obesity (body mass index >30). CT neck revealed the midline mass was cystic in nature, most likely a thyroglossal duct cyst. This mass was closely related to an angiomatous malformation involving the tongue, floor of mouth and left parotid. Fine needle aspiration cy

Meesmann's corneal dystrophy: ultrastructural features.

Ultrastructural studies were done on a cornea obtained at the time of lamellar keratoplasty from a patient with the clinical diagnosis of Meesmann's corneal dystrophy. Light microscopy showed in the corneal epithelium the typical tiny cysts containing cellular debris and a homogeneous substance that reacted with periodic acid and Schiff's reagent and stained with Hale's colloidal iron; as well, the basement membrane was markedly thickened. Electron microscopy revealed that the cysts had a corrugated or microvillous wall, consistent with acantholysis. The epithelial cells were rich in glycogen,

Thyroglossal duct cyst: a cytopathologic study of 26 cases.

Thyroglossal duct cyst (TDC), or embryologic remnants of thyroid gland, is a common congenital anomaly. TDC may cause a midline neck mass, which occasionally may become infected, and rarely gives rise to carcinoma. As a number of other nonneoplastic and neoplastic lesions can cause cystic masses in the neck, we explored the role of fine-needle aspiration (FNA) in making a preoperative diagnosis of TDC for a more accurate and timely clinical intervention. Twenty-six cases of TDC were identified from the cytopathology files of The Johns Hopkins Hospital in a 15-yr period (1990-2004). Material wa

Some approaches to treatment of patients with thyroid nodular diseases in the Semipalatinsk region of Kazakhstan.

It was determined by experts that in the region adjacent to the Semipalatinsk Nuclear Test Site thyroid nodular prevalence was significantly associated with radiation dose from nuclear weapons testing. The medical rehabilitation of patients is of special practical and scientific importance in this region. Some patients have contraindications for surgery and radioiodine or refuse them for various reasons. Percutaneous intranodular injection therapy by "Paoscle" (PIITP) was used for treatment of patients with benign thyroid nodular diseases. The study group included a total of 107 patients (main

Involution of the chicken bursa of Fabricius: a light microscopic study with special reference to transport of colloidal carbon in the involuting bursa.

The involution of the bursa of Fabricius in White Leghorn chickens and the transport of colloidal carbon in the involuting bursa were studied on light microscopy. In chickens, from newly hatched to 23.5 weeks of age, the bursa was weighed, its histology was described, and the mitoses in the cortical and medullary compartments of the lymphoid follicles were counted. Decrease in bursal weight and in the number of mitoses were observed between 10 and 16 weeks of age. During week 17.5, the follicle-associated epithelium began to lose its endocytic capability, and mucin droplets appeared in the fol
