Understanding caregivers' and community influencers' perspectives on the barriers to childhood immunisation in Northern Nigerian States with public-private partnerships in routine immunisation programme.

BACKGROUND: Despite investments by the Nigerian government and international organisations in childhood immunisation to combat child mortality, coverage in many northern states remains below the national average, thereby increasing the risk of vaccine-preventable diseases. This paper examines the barriers to immunisation in six states in northeast and northwest Nigeria, which have the lowest vaccination coverage rates in the country. METHOD: We conducted 24 focus group discussions (FGDs) with mothers/caregivers and community influencers who collaborate with health workers to provide routine i

Getting to the Point: A Community-Designed, Low-Barrier Hepatitis C Testing and Treatment Program for People Who Inject Drugs in Rural America.

BACKGROUND: People who inject drugs (PWID) have a higher risk of contracting hepatitis C (HCV) than the general population, but these individuals are often poorly served by traditional healthcare systems. The elimination of HCV as a threat to public health relies on the treatment of this population. Novel care models designed with input from PWID may help to better align care to the needs of PWID. METHODS: We designed and implemented a community-based, point-of-care testing program for HCV delivered by a syringe service program, combined with facilitated access to a healthcare provider, care

Barriers to the Provision of Preventive Care to People Living with Mental Health Conditions: Self-Report by Staff Working in an Australian Community Managed Organisation.

People living with mental health conditions experience a reduced life expectancy largely due to a higher prevalence of chronic diseases. Addressing health risk behaviours, including tobacco smoking, inadequate nutrition, harmful alcohol consumption, and physical inactivity (SNAP), through the provision of preventive care, is recommended to reduce this burden. Community Managed Organisations (CMOs) may play an important role in providing preventive care to consumers with mental health conditions, however, few studies have examined preventive care provision in CMO settings; and no studies have c

Impact of sunflower seed oil versus mustard seed oil on skin barrier function in newborns: a community-based, cluster-randomized trial.

BACKGROUND: Natural vegetable oils are widely used for newborn massage in many low resource settings. Animal models indicated that sunflower seed oil (SSO) can accelerate skin barrier recovery following damage, while other oils, including mustard oil (MO), may cause further skin barrier damage. The objective was to compare the effects of two SSO and MO used for routine massage on skin integrity in premature and full-term neonates. METHODS: This community-based cluster randomized controlled trial included 995 neonates assigned to full body massage with sunflower seed oil (SSO, intervention) or

The "Safety Net" of Community Care: Leveraging GIS to Identify Geographic Access Barriers to Texas Family Planning Clinics for Homeless Women Veterans.

The Veterans Healthcare Administration (VHA) is developing a civilian referral system to address specialty access issues to VHA healthcare. Homeless women Veterans may not have the resources to navigate referral systems when travel to VHA Medical Centers (VAMCs) is limited, especially for family planning needs. Recent Texas legislation restricted funding to civilian, publically-funded family planning clinics, limiting comprehensive services. This study's goal was to assess geographic availability of VAMCs and family planning clinics for homeless Texan women Veterans. We identified 3,246 Texan

Understanding Systems Change in Early Implementation of Housing First in Canadian Communities: An Examination of Facilitators/Barriers, Training/Technical Assistance, and Points of Leverage.

We present interim findings of a cross-site case study of an initiative to expand Housing First (HF) in Canada through training and technical assistance (TTA). HF is an evidence-based practice designed to end chronic homelessness for consumers of mental health services. We draw upon concepts from implementation science and systems change theory to examine how early implementation occurs within a system. Case studies examining HF early implementation were conducted in six Canadian communities receiving HF TTA. The primary data are field notes gathered over 1.5 years and evaluations from site-sp

Breaking barriers to completing genetic testing for inherited breast cancer among at-risk Black women using a community-based participatory research approach.

National guidelines from the US Preventive Services Task Force and the National Comprehensive Cancer Network recommend the use of family-health-history (FHH)-based risk assessment tools to guide genetic testing (GT) among women with an increased risk of inherited cancer and inform personalized cancer risk management. Prior research has focused on attitudes toward and decisions about initial uptake of GT in Black patients but little is known about the factors that impact the subsequent completion of GT after they have already provided consent. Using a community-based participatory research (CBP

Identifying barriers and potential solutions to improve equitable access to community eye services in central Kenya: a rapid exploratory sequential mixed methods study.

BACKGROUND: Recent research has found that less than half of people identified with an eye problem in Meru county's screening programme were able to access care, with younger adults being the least likely to receive the care they needed. We aimed to interview and survey members of this 'left-behind' group to explore barriers and identify potential solutions using a rapid mixed-methods approach. METHODS: First, we conducted interviews to explore perceptions of barriers and potential solutions. Next, we asked a representative sample to rank the suggested solutions by likely impact. Finally, we

Exploring facilitators and barriers to early TB case finding at private community pharmacies in Kampala, Uganda using the Consolidated Framework for Implementation Research (CFIR).

BACKGROUND: Tuberculosis (TB) remains one of the leading global infectious diseases killer, with Uganda among the countries bearing the highest TB/HIV burden. The World Health Organization’s (WHO) ambitious End TB strategy by 2030 emphasizes the strong involvement of private healthcare providers in TB efforts. In line with this, Uganda has adopted the WHO’s public-private mix (PPM) model. This study explored the facilitators and barriers to engaging private community pharmacies in the early detection of TB cases in Kampala, Uganda. DESIGN/METHODS: We conducted a qualitative study at five priva

Barriers and opportunities to bridge between hospital and community via rehabilitation exercises for people with disabilities: multi-ministerial R&D efforts in South Korea.

People with disabilities often experience limited participation in community-based exercise activities aimed at promoting health. The concept that "Exercise is Medicine" is widely acknowledged across societies and historical periods. However, there is a notable discontinuity between hospital-based and community-based health promotion efforts for people with disabilities. This article discusses multi-ministerial research and development (R&D) efforts in South Korea to address this issue, emphasizing the need for transitional strategies to successfully bridge the service gap. As the first collab

Analysis of the Costs and Reach of Transportation Services to Reduce Barriers to Opioid Use Disorder (OUD) Treatment: Evidence from the Kentucky HEALing Communities Study.

BACKGROUND: Medications for opioid use disorder (MOUD) have been proven to be the most effective method for treating individuals with opioid use disorder (OUD). Individuals who are retained on methadone and buprenorphine treatment have a decreased risk of mortality when compared to others with OUD. However, many individuals in need of MOUD face transportation barriers that prevent them from having the requisite access to treatment. METHODS: As part of the evidence-based practices (EBPs) implemented throughout the HEALing (Helping to End Addiction Long-term®) Communities Study (HCS), strategie

Systematic review of barriers to and enablers of tuberculosis diagnosis, notification, and intervention for designing customised intervention package to minimise 'missing millions' in tribal communities of India.

BACKGROUND: Tribal communities in India experience a very high burden of tuberculosis (TB), estimated at 7030 per million. The diagnosis and notification gaps are substantial, partly due to the geographical remoteness of these populations. Within an overarching study to design an intervention for finding the 'missing millions' among tribal communities, we conducted a systematic review to identify the barriers and enablers of tuberculosis diagnosis and notification, with the aim of developing a contextually relevant intervention. METHODS: We searched PubMed, Embase and Web of Science using ter

A qualitative analysis of the barriers and enablers faced by Australian rural general practitioners in the non-pharmacological management of congestive heart failure in community dwelling patients.

BACKGROUND: Congestive heart failure (CHF) is a significant health problem in Australia, and disproportionately affects rural Australians. Management of CHF in Australia is heavily centred around the general practitioner (GP). Australian and international literature indicates there is a gap between current and best practice in CHF management. There is little known about the non-pharmacological aspects of management, or CHF management in a rural Australian context. This study aimed to identify what Australian GPs practicing in the Northern Rivers Region of New South Wales, Australia, perceived
