Bipolar transfer of the pectoralis major muscle for restoration of elbow flexion in 29 cases.

BACKGROUND: This study evaluated the functional outcomes of bipolar pedicled pectoralis major (PM) transfer to restore elbow flexion. METHODS: We retrospectively reviewed 29 transfers in 28 patients with a mean age of 31.2 years (range, 5-65 years). The loss of elbow flexion was due to brachial plexus palsy in 24 patients, elbow flexors necrosis in 4, and poliomyelitis in 1. The entire PM muscle was mobilized and fixed proximally to the coracoid process. Intraoperative positioning and postoperative immobilization of the shoulder and the elbow flexed at 60° and 120°, respectively, allowed dire

Correlation between morphometry of the suprascapular notch and anthropometric measurements of the scapula.

The concept of the study was to find the correlation between the morphometry of the suprascapular notch and basic anthropometric measurements of the human scapula. The measurements of the human scapulae included: morphological length and width, maximal width and length projection of scapular spine, length of acromion, and maximal length of the coracoid process. The glenoid cavity was measured in two perpendicular directions to evaluate its width and length. The width-length scapular and glenoid cavity indexes were calculated for every bone. In addition to standard anthropometric measurements t

Position of scapula and clavicle in acute acromioclavicular joint dislocations: depressed scapula or elevated distal clavicle?

BACKGROUND: Increased coracoclavicular distance due to acute acromioclavicular joint (ACJ) instability is often described as a pseudoelevation of the clavicle due to inferior hanging of the scapula, while the distal clavicle remains in its position. The aim of this study was to analyze whether the elevation of the distal clavicle, depression of the scapula, or both are associated with vertical instability and to evaluate the impact of weighted stress radiographs on the clavicle and scapular position in acute ACJ instabilities. METHODS: The cohort consisted of 505 patients (f = 52, m = 453; me

[Injuries of the acromio- and sternoclavicular joint--surgical or conservative treatment?].

At the acromioclavicular (AC) joint we distinguish between horizontal instability caused by damage to the AC ligament from vertical instability caused by damage to the coracoclavicular liagments. The most common mechanism of injury is direct force resulting from a fall onto the point of the shoulder. The injury is classified according to the amount of damage brought about by a given force. Horizontal and vertical instability have to be evaluated by special radiographic views. Types I and II are treated by a sling worn for a few days and the application of ice bags. In type III injuries the pat

[Complete cervical rib. Possible neurovascular implications of the upper limb].

The thoracic outlet syndrome (TOS) is caused by compression of the brachial plexus or subclavian artery or vein in the region of the neck and shoulder girdle. The neurovascular bundle may be compressed at multiple sites: costoclavicular space, interscalene triangle, insertion of the pectoralis minor into the coracoid process. More than 90% of the patients present with neurologic symptoms: pain, paraesthesias or arm and hand weakness and 10% also have vascular problems. The diagnosis of TOS is always difficult and depends on careful clinical study of patients. For the neurological type of TOS,

Osteology of the pectoral girdle and forelimb of Siriusgnathus niemeyerorum, a Late Triassic traversodontid cynodont from Brazil.

Traversodontids are non-mammaliaform cynodonts, herbivorous to omnivorous, that lived during the Triassic. In the Late Triassic, some members of this group increased in body size relative to other cynodont lineages and evolved postcranial adaptations associated with locomotion. The Late Triassic Siriusgnathus niemeyerorum is one such traversodontid. Originally described in 2018 based on cranial material, the species has since been the focus of considerable advances in our understanding of its skull anatomy; however, its postcranial skeleton remains undescribed. The present study examined the p

An Unusual Exostosis of the Scapula Mimicking a Glenoid-Like Structure in Snapping Scapula Syndrome.

INTRODUCTION: Snapping scapula syndrome is an unusual condition characterized by an audible, grating, and snapping sound upon shoulder movement, often observed in young adults. Clinical severity ranges from gentle friction sounds to louder grating noises or crepitus, which may or may not be associated with pain and limited motion. Multiple etiologies include congenital malformations such as Sprengel's deformity, exostosis, subscapular mass, and scapulothoracic bursitis. CASE PRESENTATION: A 28-year-old right-handed male presented a 12-year history of crepitus and snapping of the right scapula

Short Expandable-Wing Suture Anchor for Osteoporotic and Small Bone Fixation: Validation in a 3D-Printed Coracoclavicular Reconstruction Model.

Suture anchors are widely used for tendon and ligament repair, but their fixation strength is compromised in osteoporotic bone and limited bone volume such as the coracoid process. Existing designs are prone to penetration and insufficient cortical engagement under such conditions. In this study, we developed a novel short expandable-wing (SEW) suture anchor (Ti6Al4V) designed to enhance pull-out resistance through a deployable wing mechanism that locks directly against the cortical bone. Finite element analysis based on CT-derived bone material properties demonstrated reduced intra-bone displ

The Functional Outcome of Open Reduction and Internal Fixation of Unstable Floating Shoulder Injuries: A Prospective Study with Good Short-Term and Medium-Term Outcome.

INTRODUCTION: The term "superior shoulder suspension complex (SSSC)" coined by Goss is a bone and soft tissue stable ring made up of clavicular-acromioclavicular strut, the three-process scapular body and the clavicular-coracoclavicular ligamentous-coracoid (C-4) linkage. Disruption at any one site of the SSSC complex makes the other site unstable in a vicious cycle altering the functional anatomy and muscular forces resulting in a "floating shoulder injury" (FSI). The purpose of this study is to evaluate the functional outcome after open reduction and internal fixation of both clavicle and sc

A case of metastatic renal cell carcinoma to the ovary.

A 52-year-old woman had a pathological fracture of the right femur. On histopathological examination bone metastasis from renal cell carcinoma was suspected. Abdominal computed tomography showed a heterogeneous mass (9.1 x 7.8 x 6.5 cm) in the left kidney and a cystic multilocular mass (12 x 10 cm) in the pelvis. Bone scintigraphy revealed an abnormal uptake in the left coracoid process, right third rib, and right distal femur and proximal tibia. Clinical diagnosis was left renal cancer with multiple bone metastases (cT2NOM1, stage IV) and a right ovarian tumor. We performed left radical nephr

Displaced scapular fractures: indication and long-term results of open reduction and internal fixation.

Displaced scapular fractures are often found in polytraumatized patients. In emergency treatment they assume a minor role. Advances in dealing with severely injured patients in most instances allow us to perform an operation on the fractured scapula within the first 2 weeks after injury. A differentiated approach is necessary as exclusively conservative treatment does not always bring about good results. From 1981-1991 we performed open reduction and internal fixation (ORIF) in 25 patients with displaced fractures of the scapula. The long-term results could be assessed in 20 patients after an

A case report and literature review of pediatric multifocal melorheostosis in a unilateral limb.

INTRODUCTION AND IMPORTANCE: Melorheostosis (Léri-Joanny syndrome) is a rare, nonhereditary, congenital sclerotic bone dysplasia with an estimated prevalence of 0.9 per million. Its enigmatic etiology and distinctive "melting wax" radiological appearance warrant detailed case reporting to enhance understanding of diagnosis and management, particularly in pediatric populations. CASE PRESENTATION: A 9-year-old male presented with incidental right upper limb bone abnormalities discovered post-trauma. Asymptomatic without pain/swelling, he had no relevant family history. The medial coracoclavicular ligament: anatomy, biomechanics, and clinical relevance-a research study.

BACKGROUND: The medial coracoclavicular ligament (MCCL), is a structure that shows defined morphologic and histologic features. However, little attention has been paid to the MCCL to date. This study was conducted to (1) determine whether the MCCL is a constant structure, (2) analyze its mechanical properties, and (3) determine its possible role in acromioclavicular (AC) stability. METHODS: AC joints, lateral coracoclavicular ligaments (LCCLs; conoid and trapezoid), and MCCLs were dissected in 30 fresh frozen upper limbs. In 6 of these specimens, we performed a sequential sectioning following
