Sustaining Engagement in HIV Prevention Program Among Adolescents and Young Adults (AYA) in Nigeria: Longitudinal Secondary Analysis.

BACKGROUND: Adolescent and young adult (AYA; 14-24 years old) engagement in HIV programs is often transient, and socioeconomically marginalized AYA are less engaged. Few studies have examined whether engagement can be sustained over time in HIV prevention programs. This study examines correlates of sustained AYA engagement over 24 months in an HIV prevention program in Nigeria. METHODS: We conducted a secondary data analysis of longitudinal data from an HIV prevention program among AYA in 32 local communities in Nigeria from 2021 to 2023. In the study, participants were invited to join crowds

Daily PrEP and event-driven PrEP adherence for men who have sex with men in China.

BACKGROUND: The HIV incidence among Chinese men who have sex with men (MSM) has still been high over the past decade. Despite HIV pre-exposure prophylaxis (PrEP) being an effective prevention strategy, few studies have examined PrEP uptake and adherence in China. This study examined PrEP initiation and adherence among Chinese MSM enrolled in a multi-site PrEP demonstration trial. METHODS: Three-month data for this study were drawn from a PrEP demonstration project in Guangzhou and Wuhan, China. The project aimed to increase the engagement of MSM in China for HIV/STI screening, HIV risk reduct

Medial Patellofemoral Ligament Reconstruction in Adolescents: What Parents Care About.

INTRODUCTION: Numerous techniques exist for pediatric medial patellofemoral ligament (MPFL) reconstruction (MPFL-R). Pediatric orthopaedic surgeons and patient families must navigate choices between allograft and autograft, incision type, and surgery cost. While previous research has found similar surgical outcomes among different MPFL-R techniques, minimal data exists on what patients and their families prefer. To engage in shared decision-making (SDM), pediatric orthopaedic surgeons must understand family preferences, including between incision type and cost. We conducted a study utilizing c

Loss Aversion and Current, Former, and Never-Smoking Status.

INTRODUCTION: Loss aversion (LA) is a bias in decision-making wherein potential losses have a greater influence on choices than equivalent gains. Such a bias may protect individuals from harm. Consistent with this idea, lower LA has been measured in individuals who endorse current cigarette smoking as well as other substance use compared to controls matched on important sociodemographic variables (age, gender, and educational attainment). The goal of the present study was to systematically replicate and extend this association between LA and smoking status by comparing those meeting criteria f

Community-engaged mHealth intervention to increase uptake of HIV pre-exposure prophylaxis (PrEP) among gay, bisexual and other men who have sex with men in China: study protocol for a pilot randomised controlled trial.

INTRODUCTION: The large number of key populations in China who would benefit from HIV pre-exposure prophylaxis (PrEP) in the context of limited health system capacity and public awareness will pose challenges for timely PrEP scale-up, suggesting an urgent need for innovative and accessible interventions. This study aims to develop and pilot test a theory-informed, tailored mobile phone intervention that was codeveloped by young gay men, HIV clinicians and public health researchers to increase engagement in PrEP education and initiation among Chinese gay, bisexual and other men who have sex wit

Conservative and surgical modalities in the management of paediatric parapneumonic effusion and empyema: a protocol for a living systematic review and network meta-analysis.

INTRODUCTION: Parapneumonic effusion and empyema are common complications of paediatric pneumonia. Acceptable treatment modalities for large parapneumonic effusions include antibiotics alone or in conjunction with surgical interventions. Clear guidelines on the best treatment approach are lacking and mostly based on evidence prior to widespread pneumococcal conjugate 13-valent vaccination (PCV-13). METHODS AND ANALYSIS: A living systematic review and network meta-analysis will be performed comparing the five treatment modalities: (1) antibiotics alone; (2) chest tube drainage without fibrinol

A mixed-methods approach to understanding overdose risk-management strategies among a nationwide convenience sample.

BACKGROUND: This mixed-methods study compared drug use histories between individuals who had a reported non-fatal overdose to individuals who did not and described the overdose risk-management strategies as they were experienced by a sample of people who use drugs (PWUD). Shifts from non-medical prescription opioid use (NMPOU) to the use of heroin and synthetic opioids have been implicated in national increases in overdose incidences in the United States. However, relatively limited data exists regarding the narrative experiences of the evolving overdose risk management strategies among a nati

Retrospective screening of new psychoactive substances (NPS) in post mortem samples from 2014 to 2021.

Systematic retrospective processing of previously analysed biological samples has been proven to be a valuable tool in the search for new drugs (e.g. new psychoactive substances (NPS)) and for quality assessment in clinical and forensic toxicology. In a previous study, we developed a strategy for retrospective data-analysis using a personalized library of synthetic cannabinoids, designer benzodiazepines and synthetic opioids obtained from the crowdsourced database HighResNPS (https://highresnps.com). In this study, the same strategy was employed for the compounds within the groups of NPS that

Towards activity-based exposure measures in spatial analysis of pedestrian-motor vehicle crashes.

BACKGROUND: Although numerous efforts have been devoted to exploring the effects of area-wide factors on the frequency of pedestrian crashes in neighborhoods over the past two decades, existing studies have largely failed to provide a full picture of the factors that contribute to the incidence of zonal pedestrian crashes, due to the unavailability of reliable exposure data and use of less sound analytical methods. METHODS: Based on a crowdsourced dataset in Hong Kong, we first proposed a procedure to extract pedestrian trajectories from travel-diary survey data. We then aggregated these data

Early Definitive Fusion Versus Magnetic Growing Rods in "Tweeners": What Do Parents Prefer?

INTRODUCTION: In treating early-onset scoliosis (EOS), pediatric orthopaedic surgeons and families must often choose between growth-friendly interventions such as magnetically controlled growing rods (MCGR) and early definitive posterior spinal fusion (PSF). While each strategy has associated pros and cons, little is understood about the specific preferences of patients' families. Addressing potential nuances in these preferences is crucial for meaningful shared decision-making (SDM). To better understand the factors influencing family decisions, we conducted a choice-based conjoint (CBC) anal

Rickettsia spp. in finnish ixodid ticks.

BACKGROUND: Rickettsia spp. are intracellular, arthropod-borne bacteria pathogenic to humans and animals. The clinical manifestations of rickettsial infections range from mild to severe, but diagnosis is sometimes missed owing to mild symptoms or empirical antibiotic treatment for suspected tick-borne diseases. This study aimed to determine the prevalence of Rickettsia spp. in ticks across Finland, explore possible regional variations, identify the infecting Rickettsia species, and assess rickettsial exposure of certain human patients. METHODS: We analysed 5101 ticks from 20 locations, collec

Reassurance Techniques Do Not Significantly Impact Confidence in Biosimilars for Psoriasis: A Survey of a Convenience Sample of Individuals with Self-Identified Psoriasis.

INTRODUCTION: Biosimilars are underutilized, and negative perceptions may hinder their acceptance by patients. Psychologic interventions have not been extensively studied in the context of alleviating biosimilar hesitancy. The objective of this study was to assess the effectiveness of psychologic interventions on biosimilar confidence. METHODS: Following institutional review board (IRB) approval, 1285 subjects with self-reported psoriasis were recruited using Amazon Mechanical Turk, an online crowdsourcing platform. Participants were randomized to one of ten groups. Group A started with a hyp

High stakes: how legal cannabis pricing impacts the illicit market.

BACKGROUND: Although nearly 80% of Americans reside in a county with a legal, regulated cannabis dispensary, the majority of cannabis consumed in the U.S. is still obtained from unregulated, illicit sources. Illicit cannabis carries greater health risks and is generally less preferred than regulated cannabis, yet its lower price makes it attractive to many customers. However, comparative research on price differentials between the legal and illicit markets across states is scarce. Establishing cannabis price benchmarks is essential to ensure that legal markets are competitively priced and can

Paired-Comparison Versus Cross-Sectional Approaches for Assessing Longitudinal Speech Change in Parkinson's Disease Following Deep Brain Stimulation of the Subthalamic Nucleus.

PURPOSE: Longitudinal changes in perceptual speech outcomes are typically derived from independent judgments at each time point rather than side by side within-speaker comparisons. This study evaluated a paired-comparison method for detecting longitudinal speech change in Parkinson's disease (PD) across three perceptual constructs (intelligibility, listener effort, and severity) relative to a cross-sectional method for detecting speech changes in the same cohort. METHOD: We collected recordings of the Speech Intelligibility Test at a baseline session and 12-month follow-up from speakers with

Ticks and Tick-Borne Pathogens Encountered by Dogs and Cats: A North European Perspective.

Climate change is increasing the worldwide burden of tick-borne diseases (TBDs). Dramatic increases in human cases of borreliosis have been reported during the past few decades, including from Finland, located in North Europe. As human exposure to ticks carrying pathogens is increasing, so likely is exposure of dogs and cats. However, feline or canine TBD cases are not notifiable. Likewise, no combined databases of cases exist in Finland, hindering assessment of related trends. Here, we utilize crowdsourced tick samples to reveal how commonly and to which species of TBPs dogs and cats are expo
