Computer-assisted diagnosis techniques (dermoscopy and spectroscopy-based) for diagnosing skin cancer in adults.

BACKGROUND: Early accurate detection of all skin cancer types is essential to guide appropriate management and to improve morbidity and survival. Melanoma and cutaneous squamous cell carcinoma (cSCC) are high-risk skin cancers which have the potential to metastasise and ultimately lead to death, whereas basal cell carcinoma (BCC) is usually localised with potential to infiltrate and damage surrounding tissue. Anxiety around missing early curable cases needs to be balanced against inappropriate referral and unnecessary excision of benign lesions. Computer-assisted diagnosis (CAD) systems use ar

Better imagined: Neural correlates of the episodic simulation boost to prospective memory performance.

Episodic simulation is an adaptive process that can support goal-directed activity and planning success. We investigated the neural architecture associated with the episodic simulation improvement to the likelihood of carrying out future actions by isolating the brain regions associated with this facilitation in a prospective memory paradigm. Participants performed a lexical decision task by making word/non-word judgments, with rarely occurring prospective memory target words requiring a pre-specified manual response. Prior to scanning, participants were given exposure to two lists of prospect

Generalization in category learning: the roles of representational and decisional uncertainty.

Effective generalization in a multiple-category situation involves both assessing potential membership in individual categories and resolving conflict between categories while implementing a decision bound. We separated generalization from decision bound implementation using an information integration task in which category exemplars varied over two incommensurable feature dimensions. Human subjects first learned to categorize stimuli within limited training regions, and then, during fMRI scanning, they also categorized transfer stimuli from new regions of perceptual space. Transfer stimuli di

Oscillations in human orbitofrontal cortex during even chance gambling.

Evaluating value and risk as well as comparing expected and actual outcomes is the crux of decision making and reinforcement based learning. In this study, we record from stereotactic electroencephalograph depth electrodes in a human subject in numerous areas in the brain. We focus on the lateral and medial orbitofrontal cortex while they perform a gambling task involving betting on a high card. Preliminary time-frequency analysis shows modulations in the 5-15 Hz band that is well synced to the different events of the task. These oscillations increase in both high betting scenarios as well as

Parcellating the neuroanatomical basis of impaired decision-making in traumatic brain injury.

Cognitive dysfunction is a devastating consequence of traumatic brain injury that affects the majority of those who survive with moderate-to-severe injury, and many patients with mild head injury. Disruption of key monoaminergic neurotransmitter systems, such as the dopaminergic system, may play a key role in the widespread cognitive dysfunction seen after traumatic axonal injury. Manifestations of injury to this system may include impaired decision-making and impulsivity. We used the Cambridge Gambling Task to characterize decision-making and risk-taking behaviour, outside of a learning conte

Optimisation of the reflux-symptom association statistics for use in infants being investigated by 24-hour pH impedance.

BACKGROUND AND AIM: pH-impedance monitoring is used to diagnose symptomatic gastroesophageal reflux (GER) based on symptom association probability (SAP). Current criteria for calculation of SAP are optimised for heartburn in adults. Infants, however, demonstrate a different symptom profile. The aim of the present study was to optimise criteria for calculation of SAP in infants with GER disease. PATIENTS AND METHODS: Ten infants referred for investigation of symptomatic reflux were enrolled. GER episodes were recorded using a pH-impedance probe, which remained in place for 48 hours. Discrepancy between clinical staging through bimanual palpation and pathological staging after cystectomy.

OBJECTIVES: In muscle invasive bladder cancer (MIBC), careful clinical staging is essential for patient counseling and decision-making. Bimanual palpation (BP) is an integral part and guideline advice of clinical staging. Until now, however, the value of BP has never been studied. With this study, we aim to determine the accuracy of clinical staging through BP. METHODS: Detailed clinical data were collected from a population-based series of 1,409 patients with MIBC, diagnosed between 1989 and 2005, in the region of the Comprehensive Cancer Centre East in The Netherlands. Selected were all pat

Interactive decision support for risk management: a qualitative evaluation in cancer genetic counselling sessions.

Genetic counselling for inherited susceptibility to cancer involves communication of a significant amount of information about possible consequences of different interventions. This study explores counsellors' attitudes to computer software designed to aid this process. Eight genetic counsellors used the software with actors playing patients. Clinicians' rating of expected patient satisfaction, content, accuracy, timeliness, format, overall value, ease of use, effect on the patient-provider relationship and effect on clinician's performance were evaluated via qualitative and quantitative analy

Environment Classification for Robotic Leg Prostheses and Exoskeletons Using Deep Convolutional Neural Networks.

Robotic leg prostheses and exoskeletons can provide powered locomotor assistance to older adults and/or persons with physical disabilities. However, the current locomotion mode recognition systems being developed for automated high-level control and decision-making rely on mechanical, inertial, and/or neuromuscular sensors, which inherently have limited prediction horizons (i.e., analogous to walking blindfolded). Inspired by the human vision-locomotor control system, we developed an environment classification system powered by computer vision and deep learning to predict the oncoming walking

Artificial Intelligence and Advanced Melanoma: Treatment Management Implications.

Artificial intelligence (AI), a field of research in which computers are applied to mimic humans, is continuously expanding and influencing many aspects of our lives. From electric cars to search motors, AI helps us manage our daily lives by simplifying functions and activities that would be more complex otherwise. Even in the medical field, and specifically in oncology, many studies in recent years have highlighted the possible helping role that AI could play in clinical and therapeutic patient management. In specific contexts, clinical decisions are supported by "intelligent" machines and th

A Narrative Review on Clinical Applications of fNIRS.

Functional near-infrared spectroscopy (fNIRS) is a relatively new imaging modality in the functional neuroimaging research arena. The fNIRS modality non-invasively investigates the change of blood oxygenation level in the human brain utilizing the transillumination technique. In the last two decades, the interest in this modality is gradually evolving for its real-time monitoring, relatively low-cost, radiation-less environment, portability, patient-friendliness, etc. Including brain-computer interface and functional neuroimaging research, this technique has some important application of clini

Retained guidewire mimicking an aortic intimal flap diagnosed with point-of-care ultrasound - A case report and review of the literature.

Isolated abdominal aortic dissection is a rare but potentially fatal condition. Timely diagnosis and management are crucial to reduce mortality and possible complications. Though computed tomography (CT) is the diagnostic imaging modality of choice, point-of-care ultrasound (PoCUS) holds certain promises in diagnosing aortic dissection. PoCUS is readily available at the bedside, has high specificity and sensitivity, and is useful, especially in unstable cases or when advanced imaging access is limited or time-consuming. PoCUS can also aid in risk stratification and assist in clinical decision-

POST-HOC EXPLAINABILITY OF BI-RADS DESCRIPTORS IN A MULTI-TASK FRAMEWORK FOR BREAST CANCER DETECTION AND SEGMENTATION.

Despite recent medical advancements, breast cancer remains one of the most prevalent and deadly diseases among women. Although machine learning-based Computer-Aided Diagnosis (CAD) systems have shown potential to assist radiologists in analyzing medical images, the opaque nature of the best-performing CAD systems has raised concerns about their trustworthiness and interpretability. This paper proposes MT-BI-RADS, a novel explainable deep learning approach for tumor detection in Breast Ultrasound (BUS) images. The approach offers three levels of explanations to enable radiologists to comprehend

Intestinal Obstruction: Evaluation and Management.

Acute intestinal obstruction occurs when the forward flow of intestinal contents is interrupted or impaired by a mechanical cause. It is most commonly induced by intra-abdominal adhesions, malignancy, and herniation. The clinical presentation generally includes nausea, emesis, colicky abdominal pain, and cessation of passage of flatus and stool, although the severity of these clinical symptoms varies based on the acuity and anatomic level of obstruction. Abdominal distension, tympany to percussion, and high-pitched bowel sounds are classic findings. Laboratory evaluation should include a compl

Pelvic lymph node mapping in prostate cancer: examining the impact of PSMA PET/CT on radiotherapy decision-making in patients with node-positive disease.

INTRODUCTION: Prostate Specific Membrane Antigen (PSMA) imaging with Positron Emission Tomography (PET) plays a crucial role in prostate cancer management. However, there is a lack of comprehensive data on how PSMA PET/CT (Computed Tomography) influences radiotherapeutic decisions, particularly in node-positive prostate cancer cases. This study aims to address this gap by evaluating two primary objectives: (1) Mapping the regional and non-regional lymph nodes (LNs) up to the aortic bifurcation and their distribution using conventional methods with CT compared to PSMA PET/CT, and (2) assessing

Use of artificial intelligence as an innovative donor-recipient matching model for liver transplantation: results from a multicenter Spanish study.

BACKGROUND & AIMS: There is an increasing discrepancy between the number of potential liver graft recipients and the number of organs available. Organ allocation should follow the concept of benefit of survival, avoiding human-innate subjectivity. The aim of this study is to use artificial-neural-networks (ANNs) for donor-recipient (D-R) matching in liver transplantation (LT) and to compare its accuracy with validated scores (MELD, D-MELD, DRI, P-SOFT, SOFT, and BAR) of graft survival. METHODS: 64 donor and recipient variables from a set of 1003 LTs from a multicenter study including 11 Spani
