Physiological incidents during 39 years of hypobaric chamber training in Japan.

BACKGROUND: Hypobaric chamber training for military aircrew is very important for flight safety. Since we began hypobaric training in our laboratory in 1960, some trainees have suffered physiological incidents. This study will characterize the physiological incidents during hypobaric chamber training at the Japan Air Self-Defense Force (JASDF). METHODS: All available training records from 1960-1998 were reviewed and the frequency of physiological incidents counted and analyzed. RESULTS: There were 29,677 trainees and 58,454 exposures. Overall frequency of physiological incidents was 6.3%. Ph

Cardiac problems associated with dysbarism.

Patient salvage rates depend on the extent of injury, speed and appropriateness of treatment, and the patient's age and state of health in addition to other variables. Undertreatment of DII may result in significant morbidity and associated costs. If there is any doubt regarding whether a patient might benefit from treatment, a trial at pressure is indicated. Among those with AGE who reach recompression within 5 minutes, the death rate may be only 5%, and those who survive have little morbidity. Among those whose recompression delay increases to 5 hours, however, mortality approaches 10%, and

Effects of fluid loss on the physiology of closed-circuit rebreather divers after 100- and 45-metre dives.

INTRODUCTION: Diving induced immersion diuresis predisposes divers to dehydration. Dehydration is considered a risk factor for decompression sickness (DCS) but there is very little evidence to prove it. Dehydration also potentially modifies venous gas emboli (VGE) formation and impairs endothelial function. The purpose of this study was to report the effects of fluid loss during a dive on the diver's physiology. METHODS: Nine divers performed a 45 metre fresh water (mfw) and a 100 mfw dive with predetermined dive profiles. Body weight was measured before and after the dive. Post-dive detectio

[Enlarged operations for atypical lombo-sciatic pain from disk origin (author's transl)].

In certain forms of atypical sciatic pain the surgical treatment of the affection cannot be accomplished exclusively upon the disk. This study was done on 120 patients operated upon during a two year period. The major technics used had an action upon the articular processes and facets with or without removal of the disk. In 60 cases a desensibilisation of the region was equally done. The principal reason of the surgical treatment remains still to permit the decompression of the sick root. In certain circumstances a more important surgical treatment is required: laminectomy is performed in case

Knowledge-level assessment of commercial divers: a focus on fundamental decompression procedures.

Commercial diving, a specialized profession encompassing various underwater tasks for industries such as construction and maintenance, exposes divers to unique hazards and challenges. Fundamental diving knowledge, encompassing the understanding of diving theories (DTs) and decompression procedures (DCPs), plays a pivotal role in enhancing diving safety. This study aims to assess the knowledge level of professional commercial divers regarding fundamental DTs and DCPs. The findings reveal notable disparities in knowledge levels among divers based on their qualifications, service sectors and prof

The role of persistent foramen ovale and other shunts in decompression illness.

A persistent foramen ovale (PFO) and other types of right-to-left shunts are associated with neurological, cutaneous and cardiovascular decompression illness (DCI). A right-to-left shunt is particularly likely to be implicated in causation when these types of DCI occur after dives that are not provocative. It is believed that venous nitrogen bubbles that form after decompression pass through the shunt to circumvent the lung filter and invade systemic tissues supersaturated with nitrogen (or other inert gas) and as a result there is peripheral amplification of bubble emboli in those tissues. Appr

The new insights of hyperbaric oxygen therapy: focus on inflammatory bowel disease.

Inflammatory bowel diseases (IBD), with an increasing incidence, pose a significant health burden. Although there have been significant advances in the treatment of IBD, more progress is still needed. Hyperbaric oxygen therapy (HBOT) has been shown to treat a host of conditions such as carbon monoxide poisoning, decompression sickness, and gas gangrene. In the last few years, there has been an increase in research into the use of HBOT as an adjunct to conventional treatment for IBD. Related research has shown that HBOT may exert its therapeutic effects by decreasing oxidative stress, inhibitin

A diver with immersion pulmonary oedema and prolonged respiratory symptoms.

Immersion pulmonary oedema (IPE) is particularly associated with an excessive reaction to exercise and/or cold stress. IPE usually resolves without recompression therapy within a day or two. Herein we report a diver diagnosed with IPE, in whom symptoms persisted for five days. A 58-year-old man presented with sudden onset of dyspnoea, cough and haemoptysis after surfacing. He was an experienced diving instructor with a history of moderate mitral valve regurgitation. While IPE was diagnosed and oxygen administered, respiratory symptoms deteriorated, and serum C-reactive protein elevated. No evi

Hearing loss in diving--a study amongst Navy divers.

Despite the commonly observed condition of middle and inner ear barotrauma among divers, there is little evidence of insidious and permanent development of sensorineural hearing loss associated with diving. An audiometric survey was performed on a group of 120 divers and 166 non divers from the Royal Malaysian Naval Base in Lumut, West Malaysia between July to December 1991. The results of this survey revealed that insidious development of high frequency sensorineural hearing loss may be associated with diving. At frequencies 4000, 6000 and 8000 Hz the divers had higher mean hearing levels tha

Non-traumatic ischaemic myelopathy: a review of 25 cases.

The causes of ischaemic myelopathy are described in 25 patients. Nine developed following surgical manipulation or traumatic laceration of the aorta, 1 following intercostal artery ligation, 3 following aortic aneurysm dissection, 2 following myocardial infarction and/or cardiac arrest, 7 in the absence of any specifically identifiable predisposing factors, and 3 in association with decompression sickness. The degree of clinical recovery was greater among those with incomplete spastic (as opposed to complete flaccid) paralysis and among those in whom sensory loss below the level of injury was

Persistent (patent) foramen ovale assessment and closure for the general physician.

One-quarter of adults have a persistent (patent) foramen ovale (PFO). Right-to-left shunting through a PFO can facilitate paradoxical thromboembolism, cryptogenic stroke, paradoxical gas embolism, decompression sickness, arterial hypoxaemia and migraine with aura. Atrial septal defects and pulmonary shunts are also associated with these diseases, and investigation must differentiate them from a PFO. The detection and assessment of PFOs and other right-to-left shunts uses ultrasound techniques augmented with intravenous injection of bubble contrast. When diseases are attributed to a PFO, the PF

Facial baroparesis: a critical differential diagnosis for scuba diving accidents--case report.

Facial nerve baroparesis is a rare and potentially under-reported complication of scuba diving. A diver, after surfacing from a shallow dive, developed isolated left-sided facial palsy accompanied by pain and decreased hearing in the left ear. No other signs or symptoms attributable to a scuba diving accident were detected. Forty minutes later, he heard a "pop" in the affected ear, after which all symptoms quickly resolved. Repeat neurological and ear examinations were normal. He showed no residual or new symptoms 24 hours later. The differential diagnosis of facial neurological deficit after

Neurologic complications of scuba diving.

Recreational scuba diving has become a popular sport in the United States, with almost 9 million certified divers. When severe diving injury occurs, the nervous system is frequently involved. In dive-related barotrauma, compressed or expanding gas within the ears, sinuses and lungs causes various forms of neurologic injury. Otic barotrauma often induces pain, vertigo and hearing loss. In pulmonary barotrauma of ascent, lung damage can precipitate arterial gas embolism, causing blockage of cerebral blood vessels and alterations of consciousness, seizures and focal neurologic deficits. In patien

Dysbaric osteonecrosis in Turkish sponge divers.

Skeletal radiographs were performed to determine the prevalence of dysbaric osteonecrosis (DON) in 51 Turkish sponge divers. DON was correlated with the diver's age and experience, maximum diving depth, and decompression sickness (DCS). Thirty-six of the 51 divers had radiographic evidence of one or more lesions, for a prevalence of 70.6% DON. Proximal humerus was the most effected site. Type B (head, neck, and shaft) lesions were most common, comprising 63.6% of all DON lesions. Type A (juxta-articular) lesions were observed in all 10 divers who complained of painful motion of their shoulder

The pathophysiologic role of fat in dysbaric osteonecrosis.

Dysbaric osteonecrosis (DON) can occur in humans and sheep after a single hyperbaric air exposure with inadequate decompression. The authors hypothesize that DON does not result from primary embolic or compressive effects of nitrogen bubbles on the osseous vasculature, but by secondary injury to the marrow adipose tissue by rapidly expanding nitrogen gas that triggers local, and possibly systemic, intravascular coagulation. A 28-year-old scallop diver remained at a depth of 92 feet in sea water for 4.5 hours on surface-supplied compressed air. Decompression sickness occurred after a no-stop as
