Atypical Multiple Sclerosis Lesions or Progressive Multifocal Leukoencephalopathy Lesions: That Is the Question.

Progressive multifocal leukoencephalopathy (PML) is a serious infective disease of the central nervous system that may occur in case of severe immunosuppression or after some treatment for multiple sclerosis (MS) with natalizumab, dimethyl fumarate, and fingolimod. In these case reports, we highlight the importance of differential diagnosis between PML and MS lesions in order to provide rapidly the best treatment option, by discussing the finding of brain (magnetic resonance imaging) MRI suggestive for PML in 2 MS patients, one treated with dimethyl fumarate and the other during natalizumab wi

Short-term brain atrophy evolution after initiation of immunotherapy in a real-world multiple sclerosis cohort.

BACKGROUND AND PURPOSE: In multiple sclerosis (MS), brain atrophy measurements have emerged as an important biomarker reflecting neurodegeneration and disability progression. However, due to several potential confounders, investigation of brain atrophy in clinical routine and even in controlled clinical studies can be challenging. The aim of this study was to investigate the short-term dynamics of brain atrophy development after initiation of disease-modifying therapy (DMT) in a "real-world setting." METHODS: In this retrospective study, we included MS patients starting DMT (natalizumab, fing

The activation of nuclear factor-E2-related factor 2 (Nrf2)/heme oxygenase-1 (HO-1) signaling blunts cholestasis-induced liver and kidney injury.

Cholestasis is a severe clinical complication that severely damages the liver. Kidneys are also the most affected extrahepatic organs in cholestasis. The pivotal role of oxidative stress has been mentioned in the pathogenesis of cholestasis-induced organ injury. The activation of the nuclear factor-E2-related factor 2 (Nrf2) pathway is involved in response to oxidative stress. The current study was designed to evaluate the potential role of Nrf2 signaling activation in preventing bile acids-induced toxicity in the liver and kidney. Dimethyl fumarate was used as a robust activator of Nrf2 signa

Succination inactivates gasdermin D and blocks pyroptosis.

Activated macrophages undergo a metabolic switch to aerobic glycolysis, accumulating Krebs' cycle intermediates that alter transcription of immune response genes. We extended these observations by defining fumarate as an inhibitor of pyroptotic cell death. We found that dimethyl fumarate (DMF) delivered to cells or endogenous fumarate reacts with gasdermin D (GSDMD) at critical cysteine residues to form S-(2-succinyl)-cysteine. GSDMD succination prevents its interaction with caspases, limiting its processing, oligomerization, and capacity to induce cell death. In mice, the administration of DM

Expression of STAT- and T-cell-related genes in women with first-line treatment of relapsing-remitting multiple sclerosis.

Relapsing-remitting multiple sclerosis is associated with changes in Jak/STAT pathways in immune cells, but the influence of disease-modifying drugs on these pathways is poorly understood. The aim of this study was to evaluate the impact of first-line disease-modifying drugs used in treatment of RRMS on expression of the STAT pathway and T-cell-related genes in the blood and on serum concentrations of sgp130 and TGF-β1 in women, as well as on the level of phosphorylated STAT3 and STAT5 proteins in T cells of untreated patients and heathy controls. Expression of STAT1, STAT3, STAT5A, STAT5B, SO

Fumarate Signaling in Cardiovascular Disease: Therapeutic Potential and Pathologic Pitfalls of DMF/MMF and FH1 Deficiency.

Fumarate is increasingly recognized as a metabolic signal with dual effects in cardiovascular disease. Pharmacologic doses of dimethyl fumarate (DMF) and its metabolite monomethyl fumarate (MMF) transiently activate Nrf2 and HCAR2 pathways, offering antioxidant, anti-inflammatory, and antifibrotic benefits. These include reduced endothelial activation, macrophage foam cell formation, and vascular remodeling in atherosclerosis, ischemia-reperfusion injury, hypertension, and diabetic cardiomyopathy. In contrast, sustained fumarate accumulation-due to fumarate hydratase 1 (FH1) loss-drives protei

Fumaric Acid Esters as Modulators of Ocular Inflammation and Angiogenesis.

Ocular diseases leading to irreversible vision loss are driven by convergent pathologic processes, including chronic inflammation, oxidative stress, and angiogenesis. Despite therapeutic advances, corneal neovascularization, age-related macular degeneration, diabetic retinopathy, and glaucoma remain major causes of vision impairment. Conventional therapies for these diseases pose challenges related to suboptimal response rates and long-term inefficacy, highlighting the need for treatments that target upstream mechanisms shared across ocular pathologies. Fumaric acid esters (FAEs), particularly

Fumarate and Its Esters: Physiology, Pathology, Molecular Mechanisms, Diseases, and Therapy.

Fumarate is a critical intermediate of the tricarboxylic acid cycle that, beyond its metabolic role, has emerged as an important regulator of cellular signaling. Fumaric acid esters (FAEs), including dimethyl fumarate and monomethyl fumarate, have been utilized as therapeutic agents for immune-mediated diseases and are being explored for broader applications. Accumulating evidence indicates that both fumarate and FAEs modulate diverse biological processes through mechanisms such as protein succination and metabolic reprogramming. In this review, we discuss the current understanding of fumarate

Patient-reported disability progression outcomes among patients with multiple sclerosis: Results of an outcomes-based agreement.

BackgroundOutcomes-based agreements (OBAs) are agreements between payers and manufacturers in which payment for medications is tied to patient outcomes. These contracts aim to measure the value of prescription medications on predefined clinical indicators in real-world patient populations. OBAs are gaining traction in the United States as the health care industry shifts from volume-based to value-based care. Multiple sclerosis (MS) is an appealing therapeutic area for OBAs because of its prevalence, high cost of medications, and multiple effective therapeutic options.ObjectiveTo describe findi
