Edge Computing for Environment-Based Locomotion Modes Prediction and Terrain Features Calculation in Lower Limb Prostheses.

Wearable lower-limb prostheses benefit from environment-aware control, yet strict on-device constraints (size and compute) make real-time perception and decision-making challenging. We present an edge-computing framework that integrates two inertial measurement units with an event-triggered array LiDAR sensor to enable synchronous locomotion modes prediction and terrain features calculation. Joint kinematics from the IMUs dynamically trigger LiDAR scans at critical gait phases, reducing LiDAR active sensing time while preserving forward terrain information for decision-making. The complete pip

Angioinvasive mucormycosis in burn intensive care units: A case report and review of literature.

BACKGROUND: Mucormycosis is a rare, rapidly progressive and often fatal fungal infection. The rarity of the condition lends itself to unfamiliarity, delayed treatment, and poor outcomes. Diagnosis of fungal infections early enough to enable appropriate treatment occurs in less than half of affected patients. CASE SUMMARY: An 11-year-old girl with a history of 15% total body surface area scald burns involving both lower limbs progressed to develop angioinvasive mucormycosis. This further led to a thrombosis of the right external iliac artery and vein and rapidly progressive necrosis of surroun

High-Grade Chondrosarcoma of the Proximal Phalanx: an Unusual Case of a Rare Entity.

Chondrosarcoma of the hand is a rare disease, but is one of the more common malignancies of the hand. Biopsies and imaging are a fundamental step in determining correct diagnosis, grading and selection for best treatment. We describe the case of a 77-year-old male complaining of a painless swelling in the proximal phalanx of the third ray of left hand. A biopsy was performed and the histology revealed a G2 chondrosarcoma. The patient underwent III ray amputation with metacarpal bone disarticulation and sacrifice of the radial digit nerve of the fourth ray. Definitive histology revealed grade 3

Femoral Fracture Secondary to a Gunshot Wound Leading to Chronic Expanding Hematoma with Osteomyelitis - An Unusual Presentation of a Pseudotumour: A Case Report.

A chronic expanding hematoma (CEH) is a rare clinicopathologic entity that may simulate the clinical and radiologic presentation of soft tissue sarcomas. Etiology has been attributed to repeated exudation and bleeding from capillaries in granulation tissue, resulting in a gradually enlarging mass. A 51-year-old male presented with a large thigh mass following a gunshot wound one year prior. Diagnostic imaging revealed a large complex mass with cystic areas overlying cortical erosions in the femoral diaphysis suggestive of osteomyelitis versus a primary aggressive new growth. Biopsy confirmed C

Austwickiosis in Captive African Spurred Tortoises (Geochelone sulcata) Co-infected with Cryptosporidium ducismarci.

Dermatophilosis has been described in many animals since it was first reported in 1915. In 2010, the genus and species associated with chelonid dermatophilosis was reclassified as Austwickia chelonae. Here we discuss a series of four submissions consisting of eight juvenile African spurred tortoises (Geochelone sulcata) from a large breeding facility. Submissions began in December 2016 and continued to June 2017 and then again in November 2018. Clinical signs were originally noted in December of 2015 and consisted of facial swelling and mandibular necrosis that led to complete disarticulation

The i-LIMB hand and the DMC plus hand compared: a case report.

The i-LIMB hand is a novel upper limb myoelectric prosthetic hand with several joints in the fingers and thumb. This study aimed to determine whether this new device had more functionality than a more conventional myoelectric prosthetic hand with only a single joint between the thumb and two fingers. Therefore, a 45-year-old man with a wrist disarticulation used the i-LIMB hand and the widely used Dynamic Mode Control hand (DMC plus hand) in a test procedure that covered all functional levels of the International Classification of Function (ICF). Functional outcomes of the i-LIMB seemed to be

Scapulothoracic dissociation secondary to major shoulder trauma.

Three skeletally immature patients with scapulothoracic dissociation were reviewed. A 5-year-old child's arm, caught in a conveyor belt, led to complete upper extremity amputation. Multiple fractures, muscular damage, and cutaneous and subcutaneous tissue disruption (degloving) were present throughout the avulsed extremity. Replantation was considered, but not carried out because of these extensive injuries. He was treated with a myoelectric prosthesis. Two older boys had scapulothoracic dissociation (one open, one closed) associated with clavicular diaphyseal fractures following blunt trauma.

The Madura foot: an "innocent foot mycosis"?

With the increased movement of the world population, acquaintance with the clinical picture of the Madura foot is of growing importance beyond its original endemic areas. The characteristic triad of symptoms consists of indurated swelling, multiple sinus tracts with purulent discharge filled with grains and localization at the foot. An increasing number of new etiologic agents are recognized today. For a better choice of therapy an adequate diagnostic procedure is essential ; a deep biopsy for histology appears to give a more substantial contribution to identification of the causal organism th

Functional outcome following traumatic upper limb amputation and prosthetic limb fitting.

Nineteen consecutive patients underwent traumatic upper limb amputation for nonreconstructible or replantible upper limb injury at a Level I trauma center over a 9-year-period. Eleven amputations were at the transradial level, five were transhumeral, and three were shoulder disarticulation. Eighteen patients underwent prosthetic limb fitting. Fifteen of the 18 initially underwent preparatory prosthetic limb fitting within 30 days following amputation with a body-powered, cable-driven prosthesis. Seventeen of the 18 achieved sufficient proficiency with their prostheses to allow them to return t

Through-knee amputation in high-risk patients with vascular disease: indications, complications and rehabilitation.

During a 10-year period 104 patients (mean age 72 years) had 106 through-knee amputations. Indications for surgery were: limb gangrene, 67 (64 per cent); ischaemic ulceration, 22 (21 per cent); rest pain, 9; knee contractures, 6. Thirty patients had had previous unsuccessful vascular reconstructive surgery and five had had a failed femoral embolectomy. The through-knee disarticulation used lateral skin flaps. The mortality was 21 (20 per cent). Of the 83 survivors, 59 (71 per cent) underwent uncomplicated primary wound healing; 36 (43 per cent) of the survivors were unsuitable for rehabilitati

Phlegmasia Cerulea Dolens and Pulmonary Embolism in a Patient With NSCLC Receiving Dual EGFR/MET Targeted Therapy: A Case Report.

Although venous thromboembolism is common in NSCLC, phlegmasia cerulea dolens, the most severe form of deep vein thrombosis, is exceedingly rare. We describe a 43-year-old man with metastatic EGFR-mutated NSCLC who developed extensive phlegmasia cerulea dolens and pulmonary embolism while receiving dual targeted therapy with osimertinib and tepotinib for acquired MET amplification. He presented with painful swelling of all limbs progressing to cyanosis and venous gangrene. Imaging confirmed widespread venous thrombosis and pulmonary embolism. Despite thrombectomy, fasciotomy, and therapeutic h

Acute Postoperative Pain Management After Major Limb Amputation in a Pediatric Patient: A Case Report.

PURPOSE: Although total prevention of phantom pain is difficult, pediatric patients requiring amputation benefit from an individualized combination of analgesic techniques for phantom pain reduction using a multimodal and interprofessional approach. This is especially useful in the event a single therapy is ineffective for total pain reduction, and may ultimately lead to a reduction in chronic pain development. DESIGN: Case report with multimodal and interprofessional approach. METHODS: A 16-year-old patient with synovial sarcoma underwent a right hemipelvectomy and hip disarticulation. The

[Synchronous primary double: condrosarcoma and sarcoma high grade fusocelular associated with Maffucci syndrome].

Maffucci syndrome is characterized by the presence of multiple enchondromes and hemangiomas that can affect soft tissues and other organs. The risk of malignant transformation of lesions is 100% during the life of the individual, with chondrosarcoma being the most frequently associated malignant tumor. We present the case of a 44-year-old man diagnosed with Maffucci syndrome who developed a synchronous double primary: chondrosarcoma and high-grade multicenter fusocellular sarcoma of scapular and tricipital region, was treated with disarticulation interscapule-thoracic, presented accelerated pr

Dystrophic Epidermolysis Bullosa.

Epidermolysis bullosa is a rare inherited blistering disease with an incidence of 8-10 per million live births. Dystrophic epidermolysis bullosa is a type of epidermolysis bullosa caused by mutation in type VII collagen, COL7A1. There are 14 subtypes of dystrophic epidermolysis bullosa and 400 mutations of COL7A1. Electron microscopy is the gold standard diagnostic test but expensive. Immunofluorescence study is a suitable diagnostic alternative. Trauma prevention along with supportive care is the mainstay of therapy. Squamous cell carcinoma develops at an early age in epidermolysis bullosa th

A feasibility trial of a prefabricated immediate postoperative prosthetic limb system.

In this feasibility trial, 32 consecutive patients undergoing transtibial or knee disarticulation amputation had application of a prefabricated immediate postoperative prosthesis in the operating room following lower extremity amputation. Partial weightbearing was initiated on the first postoperative day. Twenty-nine underwent primary amputation as a consequence of nonsalvageable gangrene or diabetic foot infection. Others were performed at the time of wound closure following traumatic amputation for a crushed extremity and one was performed for an infected nonunion of the tibia and fibula. Se

Therapy in unilateral sirenomelia: report of one case.

A 17-month-old male infant was referred to us with a unilateral sirenoid malformation in 1970. Besides urogenital and lumbosacral vertebral defects, the right knee was flexed 160 degrees, and the hip externally rotated and abducted. The thigh and calf were connected by a soft tissue bridge with the heel fixed to the ischial region. Arteriography showed that the foot and lower leg were supplied by the profunda femoris artery through the soft tissue bridge. The lower leg had only a very small supply from the popliteal artery. Therefore, an amputation by disarticulation at the knee joint was perf

Bicycle injury pattern among children in rural India.

205 children treated with bicycle injuries during the four year period 1979-1982 at General Hospital, Sangli, in rural Western Maharashtra were reviewed. Amputation of the right toe was the most common injury comprising 74% of all the injuries. This occurred in children carried as passengers on the rear carrier of the bicycle, who hang their legs on either side of the spoke wheel. The right big toe of the passenger is then trapped by the chain above, the spokes at the extreme inner end, horizontal fork below and the free wheel posteriorly. Peddling of the bicycle in a reverse direction followe
