Multimodal Fusion of Echocardiogram Images and Electronic Medical Records for Heart Disease Screening: Retrospective Algorithm Development and Validation Study.

BACKGROUND: Echocardiography is a fundamental imaging modality for the diagnosis of heart disease (HD), but its interpretation remains operator-dependent and lacks standardized, data-driven decision support. Although artificial intelligence has improved image-based diagnosis, the added value and interpretability of integrating routinely collected electronic medical records (EMRs) with echocardiogram (ECHO) for large-scale screening remain underexplored. OBJECTIVE: This study aims to develop an explainable artificial intelligence framework that integrates multimodal data, including ECHO images

Longitudinal Changes in Thyroid Hormones and Serum Albumin Following CABG, AVR, and PCI: An Exploratory Electronic Medical Record Study.

BACKGROUND: Surgical trauma disrupts hormone networks, but the duration required for these systems to recover remains unclear. We hypothesize that significant perioperative stress would trigger protracted abnormalities of the thyroid axis extending past 28 days. METHODS: This retrospective exploratory study analyzed opportunistically obtained thyroid-related laboratory values (free T3 [FT3], free T4 [FT4], and thyroid-stimulating hormone [TSH]) and serum albumin from electronic medical records of patients undergoing CABG, AVR, or PCI between 2017 and 2022. Preprocedural baseline values were c

Estrogen exposure from modern contraceptives and vascular risk in women with migraine: A nationwide electronic medical record database study.

BackgroundMany guidelines list migraine with aura (MwA) as a contraindication to estrogen-containing combined hormonal contraceptives (CHCs) due to vascular risks. However, current evidence is based on small sample studies with potential influence by confounding factors. Additionally, few studies have examined the vascular risk associated with modern CHCs with lower dose estrogen, particularly in relation to aura status. The present study aims to investigate the vascular risk of modern CHCs in women with migraine with and without aura.MethodsWe used a de-identified electronic medical record da

Exploration of chronic kidney disease screening, diagnosis and management in Australian general practice using electronic medical record data.

BACKGROUND: CKD is a common but under-recognised condition that places significant burden on the individual and the health system globally. Our study applied a set of primary care quality indicators originally developed and validated using Canadian primary care data for screening, diagnosis and monitoring of CKD. These indicators were then applied to a large primary care dataset to assess CKD detection and management practices in Australia. METHODS: We used de-identified data from the Patron repository, which contains data extracted from electronic medical records (EMRs) in Australian general

Electron beam monitoring of a modified conventional medical accelerator with a portable current transformer system traceable to primary electrical standards.

BACKGROUND: Ultra-high dose rate radiotherapy (UHDR) delivers therapeutic doses at rates >40 Gy/s in a fraction of a second, aiming to enhance the therapeutic ratio through the FLASH effect. The substantial increase in UHDR beam current poses serious challenges for conventional active dosimeters. Integrating current transformers (ICT) offer a nondestructive solution for accurate monitoring, enabling the type of fast transient readout that will be crucial for UHDR treatment verification. PURPOSE: The aim of this study is to build and characterize a clinically deployable ICT system and to devel

Phenotyping people with a history of injecting drug use within electronic medical records using an interactive machine learning approach.

People with a history of injecting drug use are a priority for eliminating blood-borne viruses and sexually transmissible infections. Identifying them for disease surveillance in electronic medical records (EMRs) is challenged by sparsity of predictors. This study introduced a novel approach to phenotype people who have injected drugs using structured EMR data and interactive human-in-the-loop methods. We iteratively trained random forest classifiers removing important features and adding new positive labels each time. The initial model achieved 92.7% precision and 93.5% recall. Models maintai

Risk of circulatory diseases associated with proton-pump inhibitors: a retrospective cohort study using electronic medical records in Thailand.

BACKGROUND: Proton-pump inhibitors (PPIs) are prescribed to treat gastric acid-related diseases, while they may also have potential risks to population health. Recent studies suggested that a potential mechanism explaining the association between PPIs and cardiovascular diseases (CVD) includes the inhibition of the nitrate-nitrite-nitric oxide (NO) pathway. However, previous observational studies showed controversial results of the association. In addition, the inhibition of the NO pathway due to PPIs use may lead to peripheral vascular diseases (PVD); however, none of the studies explore the

Integration of Cirrhosis Best Practices Into Electronic Medical Record Documentation Associated With Reduction in 30-Day Mortality Following Hospitalization.

BACKGROUND: Hospital admissions for patients with cirrhosis continue to increase. In New York City, 25% to 30% of hospitalized cirrhotics are readmitted within 30 days. Rehospitalization is associated with increased mortality, poor quality of life, and financial burden to patients, hospitals, and payers. Preventable readmissions are partially accounted for by a well-documented quality gap between evidence-based guidelines for cirrhosis management and real-world adherence to these recommendations. METHODS: We performed a prospective cohort study that compared outcomes among cirrhotic patients
