In Gaucher disease (GD), genetic deficiency of acid β-glucosidase leads to accumulation of its substrate glucosylceramide (GlcCer) and glucosylsphingosine (GlcSph). Lipid-laden cells, most prominently seen as macrophages engorged with GlcCer and GlcSph-laden lysosomes, trigger chronic metabolic inflammation and multisystemic phenotypes. Among the pleiotropic effects of inflammatory cascades, the induction of glucosylceramide synthase accentuates the primary metabolic defect. First-line therapies for adults with GD type 1 include Enzyme Replacement Therapy (ERT) and eliglustat Substrate Reducti

This report describes an open-label, nonrandomized, prospective evaluation of the effects of angiotensin-converting enzyme inhibitor and angiotensin receptor blocker therapy on patients who have Fabry disease and also received enzyme replacement therapy with agalsidase-beta, given at 1 mg/kg body wt every 2 wk. Previous placebo-controlled phase III and phase IV trials with agalsidase-beta demonstrated clearing of globotriaosylceramide from vascular endothelia but little effect on proteinuria or progressive loss of kidney function in patients with Fabry disease and severe chronic kidney disease

Mucopolysaccharidosis type Ⅰ was a rare inherited metabolic disorder caused by a deficiency of alpha-L-iduronidase, resulting in the incomplete degradation of glycosaminoglycans that accumulate in lysosomes, leading to tissue damage and organ dysfunction. The main treatment options for mucopolysaccharidosis type Ⅰ were hematopoietic stem cell transplantation and enzyme replacement therapy. Patients who did not receive early replacement therapy often struggle to survive into adulthood, and cases of enzyme replacement therapy initiated in adulthood were extremely rare. This article summarized an

Hyperuricemia, caused by purine metabolism disorders, is associated with gout, kidney stones, and cardiovascular disease. While urate oxidase (UOX) therapy reduces uric acid by converting it to allantoin, the generation of hydrogen peroxide (H2O2) limits its safety and efficacy due to oxidative stress. To overcome this limitation, we developed a modular enzyme assembly that colocalizes UOX with catalase (CAT), an H2O2-degrading enzyme, via engineered coiled-coil motifs. Complementary motifs bearing site-specific cysteine residues were designed to promote disulfide-stabilized heterodimerization

Acute kidney injury (AKI) presents a recalcitrant pathological network where metabolic dysfunction and oxidative stress are intrinsically coupled requiring more than single-target intervention. Herein, we report the rational design of UL-Gel, a core-shell compartmentalized nanozyme-metabolic enzyme cascade microreactor for synergistic lactate depletion and reactive oxygen species scavenging. This system is constructed via enzyme-driven interfacial radical gelation, where lactate oxidase-generated H2O2 is locally relayed to the UIO-66 core to initiate polymerization, whereas elevated pathologic

CLN1 Batten disease is caused by mutations in the CLN1 gene which codes for the lysosomal enzyme palmitoyl-protein thioesterase-1 (PPT1). Disease progression is marked by intellectual and motor deterioration, seizures, vision loss, and early mortality. There are no approved treatments for this severe pediatric condition. We describe the development and characterization of recombinant human PPT1 (rhPPT1) suitable for use as a clinical enzyme replacement therapy in CLN1 Batten patients. rhPPT1 displays similar mannose-6-phosphate receptor (M6PR)-dependent uptake kinetics in neuronal cell lines f

AIMS: Enzyme replacement therapy (ERT) for mucopolysaccharidosis II (MPS II) requires long-term, weekly intravenous infusions often lasting over 3 h each time, which can burden paediatric patients and caregivers and negatively affect their quality of life and treatment compliance. The aim of this study was to assess whether shortening infusion duration impacts the long-term efficacy and safety of ERT.
METHODS: A post hoc analysis was conducted using 260 weeks of clinical data from 27 Japanese patients with MPS II who received pabinafusp alfa at a dose of 2.0 mg/kg/week during a Phase II/III c

Enzyme-photosensitizer (PS) conjugates hold great promise for clinical treatment of cancer and infectious diseases via catalysis-augmented photodynamic therapy (PDT). Compared to covalent coupling, physical binding utilizing noncovalent interactions provides a simple and nondestructive strategy to combine PS with enzymes. However, the mechanism of enzyme-PS physical combination remains largely unknown, and physically bonded enzyme-PS conjugates are rarely reported. Here, we systematically investigate the interacting behaviors of representative enzymes with one of the most popular PS of chlorin

Harnessing the significant buildup of lactic acid (LA) within the tumor microenvironment (TME) for metabolic manipulation presents a promising avenue for cancer treatment. Nevertheless, single-agent therapies often fail to address the complex and varying needs of TME heterogeneity, posing a substantial scientific hurdle in oncology. In this context, we employ asymmetric mesoporous silica nanoparticles (AMS NPs) as delivery vehicles, simultaneously loading them with zinc‑cobalt‑manganese ferrite nanoparticles (ZCMF NPs), lactate oxidase (LOX), and doxorubicin (DOX). We enhance tumor-targeting t

Hypophosphatasia (HPP) is a hereditary disorder characterized by impaired bone mineralization caused by decreased tissue-nonspecific alkaline phosphatase (TNSALP) activity. Specifically, HPP is caused by a loss-of-function variant in the ALPL gene encoding TNSALP. Although genotype-phenotype correlations have been described, phenotypic differences have been reported in patients with the same variants, even within families. The proband, a girl, was suspected to have in utero fractures of the long bones, suggestive of osteogenesis imperfecta. No respiratory impairment was observed after birth; h

BACKGROUND: Dietary peptides are increasingly linked to inflammatory gastrointestinal diseases, exemplified by coeliac disease. Coeliac disease is caused by an acquired immune response to proline- and glutamine-rich gluten peptides, which bottleneck proteolysis and provide substrates for immune recognition. Enzyme therapies aim to eliminate gluten immunogenic peptides as an adjunct to gluten-free diet.
AIMS: To investigate overlooked aspects of enzyme development given difficulties in translating preclinical efficacy into clinical benefit.
METHODS: We assessed mode-of-action, target organ an

BACKGROUND: Rheumatoid arthritis (RA) is a chronic autoimmune disease characterized by persistent synovial inflammation and joint destruction. Insufficient apoptosis of fibroblast-like synoviocytes (FLSs) significantly contributes to the pathogenesis of RA by promoting hyperplasia and inflammatory cytokine secretion. Celastrol (CLT), a pentacyclic triterpene derived from Tripterygium wilfordii, has shown promise in inducing apoptosis via endoplasmic reticulum (ER) stress-mediated pathways. However, its clinical application is limited by poor solubility and off-target effects. Targeted delivery

Rationale: Childhood nephrotic syndrome (NS) is a serious disease affecting the health and quality of life of children, which is characterized by a series of pathophysiological changes due to the increased permeability of the glomerular membrane to plasma proteins. Low renal drug distribution and inefficient cellular uptake, resulting from cellular dysfunctions of filtration and internalization, are the main barriers to drug treatment in childhood NS, leading to deterioration in nephropathy. However, efficient therapeutic methods against childhood NS are still lacking in clinic. Methods: This

OBJECTIVES: Gaucher disease (GD) is a lysosomal storage disease caused by glucocerebrosidase (GCase) enzyme deficiency. Gaucher cells transformed from the macrophages by progressive sphingolipid accumulation and infiltrate bone marrow, spleen, liver, and other organs. The accumulation of substrate causes inflammation, compromised cellular homeostasis, and disturbed autophagy. It has been hypothesized that this proinflammatory state of GD leads cytokines and chemokines release. As a result of inflammatory process, the cellular dysfunction caused by disruption of cellular signaling, organelle dy

Gaucher disease (GD) is a lysosomal storage disorder with glucocerebroside accumulation in the macrophages. The disease is divided into three types based on neurocognitive involvement with GD1 having no involvement while the acute (GD2) and chronic (GD3) are neuronopathic. The non-neurological symptoms of GD3 are well treated with enzyme replacement therapy (ERT) which has replaced hematopoietic stem cell transplantation (HSCT). ERT is unable to prevent neurological progression as the enzyme cannot cross the blood-brain barrier. In this retrospective study, we report the general, neurocognitiv

This open-label, phase 1/2 study (JMACCT CTR JMA-IIA00350) evaluated the efficacy and safety of intracerebroventricular idursulfase beta in patients with mucopolysaccharidosis II (MPS II). Herein, we report the 100-week results. Six patients with severe MPS II aged 23-65 months were enrolled. Idursulfase beta (increasing from 1 to 30 mg between weeks 0 and 24, followed by a 30-mg final dose) was administered intracerebroventricularly once every 4 weeks using an implanted cerebrospinal fluid (CSF) reservoir; intravenous administration of idursulfase was also continued throughout the study.
