Long-term predictors for psychological outcome of pre-symptomatic testing for late-onset neurological diseases.

This longitudinal study aimed at determining predicting variables for middle and long-term psychological disturbance due pre-symptomatic testing (PST) for two late-onset neurological diseases, Huntington disease (HD) and TTR (transthyretin protein) familial amyloid polyneuropathy (FAP) Val30Met (now classified as Val50Met). 196 clinical records of persons who performed PST at least three years ago and answered to the two stages of evaluation (before PST and least 3 years after disclosure of results) were analysed. For this purpose, regression analysis was performed, showing that the Positive S

[Bullous dermatitis self-induced].

In three patients, two women aged 37 and 20 years and a man aged 46 years, a blistering skin condition was observed. Histopathological examination of the skin lesions revealed a blister level just beneath the stratum granulosum, which is proof of a friction blister. The patients were suffering from bullous dermatitis artefacta or bullous pathomimia. This is a psychiatric disorder characterised by rubbing skin blisters but denying self-infliction. Early diagnosis is important to prevent unnecessary drug therapy and chronic morbidity. The typical histology supports the doctor in his suspected di

Psychiatric and psychometric issues in acné excoriée.

Psychiatric and psychometric issues were assessed in 12 acné excoriée patients. Two patients exhibited a DSM-III-R dysthymia and 2 patients a DSM-III-R personality disorder, which were considered as associated comorbidities. Most patients reported concern for being unattractive and a compulsive urge to manipulate their skin. However, 8 out 12 patients did not fulfill criteria for any DSM-III-R disorder. In particular, we could not find evidence for the assumption of underlying obsessive-compulsive disorders or body image disorders in acné excoriée.

Hyperthyroidism in adolescents.

The term 'hyperthyroidism' refers to a form of thyrotoxicosis due to inappropriate high synthesis and secretion of thyroid hormone(s) by the thyroid. The leading cause of hyperthyroidism in adolescents is Graves' disease (GD); however, one should also consider other potential causes, such as toxic nodular goitre (single or multinodular), and other rare disorders leading to excessive production and release of thyroid hormones. The term 'thyrotoxicosis' refers to a clinical state resulting from inappropriate high thyroid hormone action in tissues, generally due to inappropriate high tissue thyro

Comment on a Multicenter Retrospective Descriptive and Analytical Study of 129 Cases of Trichoteiromania.

We have read Ramos Costa et al.'s publication "Trichoteiromania: A multicentre retrospective descriptive and analytical study of 129 cases" in the Journal of the European Academy of Dermatology, and comment on some critical issues related to the nosology, nomenclature, and methodology. The term trichoteiromania was originally coined for the rubbing of the hair due to an underlying psychiatric disorder without further specifications. Based on case observations of patients with trichoteiromania, it was later reported that in contrast to trichotillomania that represents an obsessive-compulsive di

Factitious diarrhoea.

Gastroenterologists often have to deal with patients with chronic diarrhoea. The vast majority will suffer from functional bowel syndrome. A few will present with a clear-cut organic disease. If chronic diarrhoea remains unexplained after extensive testing, if daily stool volume is high (greater than 500 g/24 h) and if the patient is female, factitious diarrhoea should be considered. Melanosis coli on sigmoidoscopy, cathartic colon on barium enema examination, a positive room search and chemical detection of the presence of specific laxatives in urine or stool may lead to the correct diagnosis

Brain single photon emission computed tomography findings in depressive pseudodementia patients.

BACKGROUND: Recently, there have been studies suggesting that depressive pseudodementia would include early-stage dementing disorder. Through the comparison of the 99mTc-HMPAO single photon emission computed tomography (SPECT) image of depressive pseudodementia subjects, healthy comparison subjects, depressed subjects free of cognitive impairment, and dementia of Alzheimer's type (DAT) subjects, we aimed to see part of pathophysiology of the depressive pseudodementia of elderly patients. METHODS: Study subjects consisted of seven patients with depressive pseudodementia, seven healthy comparis

A stepwise diagnostic framework for recurrent unexplained fever in adults: an evidence-based review with focus on Latin America.

BACKGROUND AND METHODS: Recurrent fever is a complex clinical presentation that poses significant diagnostic challenges, particularly when no clear etiology emerges despite extensive investigation. This integrative narrative review provides a comprehensive overview of recurrent fever and proposes a practical diagnostic flowchart focused on the most frequent etiologies, with particular emphasis on the Latin American and Brazilian contexts. Literature from MEDLINE/PubMed, LILACS, and SciELO was reviewed to summarize evidence on epidemiology, differential diagnosis, and diagnostic strategies. RE

Diagnoses made in a secondary care "fits, faints, and funny turns" clinic.

AIMS: To investigate the diagnoses made for children referred to a "fits, faints, and funny turns" clinic. METHODS: Prospective study of 380 children referred to a dedicated secondary care clinic over an eight year period. RESULTS: Twenty three per cent of children were given a final diagnosis of one of the childhood epilepsies, with 48% of these having a specific epilepsy syndrome. Syncope was the commonest cause of a non-epileptic event (syncope and reflex anoxic seizures comprised 100/238, 42%) but there were a wide variety of other causes. Fifty three events (14%) were unclassified and m

How to manage autoimmune hyperthyroidism (Graves' disease): from differential diagnosis to emerging immunological therapies.

Graves' disease is an autoimmune disorder and represents the most prevalent cause of hyperthyroidism. Its pathogenesis involves a complex interplay of immunological mechanisms, with a central role attributed to stimulating autoantibodies directed against the thyroid-stimulating hormone receptor (TSHR) expressed on thyroid follicular cells. The differential diagnosis of hyperthyroidism is of paramount importance, including other causes of thyrotoxicosis such as autonomously functioning thyroid adenoma, toxic nodular goitre, various forms of inflammatory or destructive thyroiditis and factitious

[Secretan's syndrome: myth or pathomimia?].

UNLABELLED: Secretan's syndrome is a rare condition involving generally trauma-induced hard edema of the dorsal aspect of the hand. The cause is poorly understood but factitious trauma is often suspected. CASE REPORT: A 42-year-old woman presented with a fortuitous edema on the back of the right hand. The minimally depressible edema was associated with moderately intense mechanical pain. Routine laboratory tests were normal. An extensive imaging work-up (bone x-ray of the hand and wrist, bone scintigraphy, computed tomography phlebography, lymphoscintigraphy, magnetic resonance imaging) was e

Factitious vesicocutaneous fistula: an enigma in diagnosis and treatment.

A case report of a patient with a recurrent factitial vesicocutaneous and a tensor fasciae latae musculocutaneous flap were employed in reconstruction. Difficulty in diagnosis and treatment of this patient with factitial disease is emphasized. Excessive hospitalization and cost reflect these difficulties. A life-threatening disease was modified by planned reconstruction coordinated with antidepressant medication and psychotherapy. Treatment centered on avoiding insight and maintaining denial ASA defense mechanism. The transference of self-mutilation in the suprapubic region to the donor thigh

"Excited delirium syndrome": is it a cause of death?

Excited delirium syndrome (EDS) has become a controversial and vexing forensic issue due to its association with restraint and sudden unexpected death. Although some authorities and jurisdictions recognised EDS as a cause of death there is no consensus among the medical community in this regard. The overlapping nature of the spectrum of antemortem behaviours and signs with many natural disease processes complicates this issue further. We describe two deaths which initially presented as EDS-like behaviour during restraint. In the first case, the deceased was travelling on a long distance flight

Electroencephalographic sleep in late-life neuropsychiatric disorders.

Late-life depression and dementia of the Alzheimer's type both have profound, although different, effects on electroencephalographic (EEG) sleep patterns. Thus, while depression is associated with REM sleep disinhibition and extreme sleep fragmentation (e.g., sleep onset REM periods and early morning awakenings), Alzheimer's disease is associated with deficits in the production of phasic activity during sleep (e.g., rapid eye movements and K-complexes) and with increased rates of sleep-disordered breathing. These differences have been shown to be reliable in large numbers of patients during th

Automatic and effortful memory processes in depressed persons.

Clinical lore has held that depression results in memory dysfunction, particularly in older adults. Some believe that memory loss due to depression is indistinguishable from an organic dementia and label such dysfunction pseudodementia. Previous literature has inconclusively supported the relation between depression and memory deficits. This research assessed three groups of subjects: (a) 30 depressed patients, (b) 20 psychiatric controls, and (c) 30 normal controls. Dependent memory tasks were designed to vary along the automatic and effortful memory encoding continuum defined by Hasher and Z

Psychopharmacological therapies in dermatology.

When patients with psychodermatologic disorders present in clinic, the dermatologist can refer them to psychiatrists or other mental health care professionals. However, it is often the case that these patients will refuse a psychiatric referral because they either do not believe they have a disorder of psychiatric nature or they feel there is societal stigma associated with psychiatric illness. Therefore, it is essential for dermatologists to understand the common classifications for psychodermatological cases and to know how to optimally treat these patients with pharmacotherapy. The intent o

[Pathway to diagnosis and real-life experience of patients believing they are affected by "chronic Lyme disease"].

OBJECTIVES: Chronic Lyme disease is a subject of scientific and social controversy in both Europe and the United States. The aim of our study was to analyze the pathway to diagnosis of patients believing they were affected by the disease, and to describe their real-life experience. METHOD: A qualitative study was performed with 13 patients declaring themselves to be affected by chronic Lyme disease. Interviews were analyzed by 2 general medical practice interns, supervised by a general practitioner with a diploma in socio-anthropology and an infectious diseases specialist. RESULTS: Internet

Where Reality and Fantasy Collide-Prolonged Fever to Munchausen Syndrome by Proxy.

Background: Munchausen syndrome by proxy (MSBP) or factitious disorder imposed on another (FDIA) is a bizarre psychiatric entity, consisting of the fabrication of symptoms and alteration of laboratory tests by a caregiver. It is considered a serious form of child abuse. Alarm signs are frequent medical visits and strange symptoms that are never objectified during hospitalization. Methods: This case sets a bright light on how difficult the diagnosis and management of FDIA is and the severe consequences this disease has on a defenseless child. Results (Case report): A boy who is 3 years 8 months
