Assessing a decade of public health preparedness: progress on the precipice?

September 11 and the subsequent anthrax attacks marked the beginning of significant investment by the federal government to develop a national public health emergency response capability. Recognizing the importance of the public health sector's contribution to the burgeoning homeland security enterprise, this investment was intended to convey a "dual benefit" by strengthening the overall public health infrastructure while building preparedness capabilities. In many instances, federal funds were used successfully for preparedness activities. For example, electronic health information networks,

Integrating Artificial Intelligence and Multicenter Real-World Evidence to Advance Precision Dosing: A Critical Perspective Review.

BACKGROUND: Precision dosing integrates patient-specific factors to optimize pharmacotherapy. However, current evidence often relies on limited single-center studies, restricting generalizability. METHODS: A narrative review of literature on real-world evidence, pharmacogenetics, biomarkers, artificial intelligence (AI), and privacy-preserving data strategies relevant to precision dosing was conducted. RESULTS: Single-center data are insufficient for robust precision-dosing evidence because of small subgroups and local practice biases. Multicenter privacy-preserving systems enable richer, mo

Artificial intelligence in central nervous system autoimmune diseases: from clinical decision support to mechanistic discovery and translation.

Central nervous system (CNS) autoimmune diseases, including multiple sclerosis (MS), neuromyelitis optica spectrum disorder (NMOSD), autoimmune encephalitis (AE), and myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD), remain difficult to diagnose, stratify, and treat because clinical phenotypes overlap, biomarkers are imperfect, and the disease course requires longitudinal observation. Artificial intelligence (AI) is increasingly being evaluated to integrate clinical, imaging, immunological, and molecular data for decision-making in neuroimmunology. Clinical applications

Blockchain-enabled identity management for IoT: a multi-layered defense against adversarial AI.

The growing deployment of the Internet of Things (IoT), especially in critical infrastructure, has increased the need for identity systems that are scalable and robust against attacks. However, existing centralized systems have fundamental weaknesses, especially where adversaries use artificial intelligence (AI)-based techniques, such as generative spoofing, model poisoning, and deepfakes to create fake identities. In this paper, we present a novel blockchain-based IoT security system that combines decentralized identity verification, zero-knowledge proofs, Byzantine-resistant federated learni

Intelligent diagnostics: AI-augmented microfluidic devices for non-invasive and minimally invasive disease biomarker monitoring.

Decentralised point-of-care testing (POCT) is driving a paradigm shift towards non-invasive and minimally invasive continuous health monitoring. While alternative biofluids, such as saliva, sweat, tears, exhaled breath condensate, and interstitial fluid (ISF) offer a patient-centric window into systemic physiology, their clinical utility remains constrained by ultra-low biomarker concentrations, severe matrix complexity, high analytical noise, and ambiguous correlations with established blood reference ranges. This review examines how micro-total analysis systems (µTAS), augmented by advanced

Microbial inoculants for soil restoration: a risk-proportional stewardship framework integrating strain-resolved genomics and adaptive governance.

Global soil degradation and increasing reliance on chemical inputs threaten agricultural sustainability, driving interest in microbial inoculants as tools for soil restoration. These biological products have the potential to enhance nutrient cycling, improve soil structure, and support plant resilience, but their environmental release raises important safety and stewardship considerations. Here, we propose a risk-proportional framework for the responsible deployment of microbial inoculants grounded in release-based stewardship. The framework integrates genome-resolved strain identification, ex

VERTIcal Grid lOgistic regression with Confidence Intervals (VERTIGO-CI).

Federated learning of data from multiple participating parties is getting more attention and has many healthcare applications. We have previously developed VERTIGO, a distributed logistic regression model for vertically partitioned data. The model takes advantage of the linear separation property of kernel matrices of a dual space model to harmonize information in a privacy-preserving manner. However, this method does not handle the variance estimation and only provides point estimates: it cannot report test statistics and associated P-values. In this work, we extend VERTIGO by introducing a n
