BACKGROUND: A comprehensive insight into the effect of longstanding diabetes mellitus and neuropathy on foot joint kinetics during walking is lacking. Our goal was to assess the in-vivo kinetics of major foot joints in persons with diabetes.
METHODS: Three groups, matched for age, sex and walking speed were recruited in this study: 1) people with diabetic peripheral neuropathy, 2) people with diabetes without peripheral neuropathy, and 3) control subjects without diabetes. Participants were instrumented with retroreflective markers on both feet and lower limbs and underwent a barefoot gait an

This study introduces a new technique to measure bone size and shape. A three-dimensional laser scan was taken of the talus, navicular, medial cuneiform, and first metatarsal from 107 skeletons of known age and sex. The bones were analyzed for differences in bone morphology between the sexes and the ability of each bone to contribute to the adducted position of the first metatarsal. Linear measurements showed that male bones were larger than female bones. Measurements of articular surfaces suggested that female bones had the potential for more movement to occur in the direction of adduction, p

BACKGROUND: Middle facet subluxation (MFS) of the subtalar joint is a key indicator of peritalar subluxation in progressive collapsing foot deformity (PCFD). Although its assessment relies on weightbearing computed tomography (WBCT), it remains unclear which weightbearing radiographic (WBR) measurements correlate with MFS. We hypothesize that radiographic parameters assessing hindfoot alignment are correlated with MFS.
METHODS: Seventy-three feet diagnosed with PCFD were evaluated using WBR and WBCT. Five parameters were measured in both modalities: talonavicular coverage (TNC) angle, hindfoo

BACKGROUND AND OBJECTIVES: Progressive Collapsing Foot Deformity (PCFD), a multifactorial disorder, involves the synergistic failure of dynamic restraints, primarily the posterior tibial tendon (PTT), and static restraints, chiefly the calcaneonavicular ligament (SL). The necessity of routinely reconstructing the SL complex in the early, flexible stages of the disease is a significant clinical debate. To clarify this, this study aimed to assess whether measurable macro-structural changes, specifically alterations in the ultrasonographic thickness of the supramedial fasciculus of the calcaneonav

Progressive collapsing foot deformity results from degeneration of the ligaments and posterior tibial tendon (PTT). Our understanding of the relationship between their failures remains incomplete. We sought to improve this understanding through computational modeling of the forces in these soft tissues. The impact of PTT and ligament failures on force changes in the remaining ligaments was investigated by quantifying ligament force changes during simulated ligament and tendon cutting in a validated finite element model of the foot. The ability of the PTT to restore foot alignment was also eval

BACKGROUND: Historical concept of flatfoot as posterior tibial tendon dysfunction (PTTD) has been questioned. Recently, the consensus group published a new classification system and recommended renaming PTTD to Progressive Collapsing Foot Deformity (PCFD). The new PCFD classification could be effective in providing comprehensive information on the deformity. To date, there has been no study reporting intra- and interobserver reliability and the frequency of each class in PCFD classification.
METHODS: This was a single-center, retrospective study conducted from prospectively collected registry

BACKGROUND: Adult acquired flatfoot deformity is characterized by a progressive functional deficit of the foot that leads to an eversion of the subtalar joint complex with heel valgus, abduction of the forefoot and collapse of the medial arch. In the case of a flexible deformity, a joint-preserving operative reconstruction is advisable, which should correct all elements of the deformity. A calcaneal lengthening osteotomy can correct excessive abduction of the forefoot, which can be measured by the amount of talar head uncoverage visible on AP weight-bearing x‑rays of the foot.

BACKGROUND: The objective of this study was to evaluate the correlation between Weightbearing CT (WBCT) markers of pronounced peritalar subluxation (PTS) and MRI findings of soft tissue insufficiency in patients with flexible Progressive Collapsing Foot Deformity (PCFD). We hypothesized that significant correlation would be found.
METHODS: Retrospective comparative study with 54 flexible PCFD patients. WBCT and MRI variables deformity severity were evaluated, including markers of pronounced PTS, as well as soft tissue degeneration. A multiple regression analysis and partition prediction model

Charcot-Marie-Tooth disease (CMT) causes disabling cavovarus foot deformity. Orthopaedic surgery is performed in severe cases; however few studies have investigated whether surgery improves health outcomes during childhood. This study investigated the impact of cavovarus surgery on validated physical, functional, parent/self-reported and biomechanical measures in 21 consecutive patients (mean age at surgery 12.5 years, SD 2.7) evaluated before and after surgery (mean duration 15.7 months, SD 5.9), and compared to natural history data from 206 children with CMT. Measures from the CMT Pediatric

BACKGROUND: Pedal biphalangism, which was also defined as symphalangism, is seen at a frequency that cannot be ignored; however, no study can be found in the literature evaluating biphalangism in normal population in comparison to those who have foot disorders. The aim of this study was to evaluate the incidence of the pedal fifth toe symphalangism in normal population and in patients with foot deformity including hallux valgus, pes planus, pes cavus, and pes equinovarus. We hypothesized that pedal fifth toe symphalangism may be a predisposing factor or an accompanying structural variation for

This pictorial review focuses on basic procedures performed within the field of podiatric surgery, specifically for elective reconstruction of the midfoot and rearfoot with focus on the flatfoot deformity. Our goal is to demonstrate objective radiographic parameters that surgeons utilize to initially define the deformity, lead to procedure selection, and judge post-operative outcomes. We hope that radiologists will employ this information to improve their assessment of post-operative radiographs following reconstructive foot surgeries. First, relevant radiographic measurements are defined and

INTRODUCTION: A successful adjusted treatment algorithm for the correction of cavovarus foot deformity requires soft-tissue balancing procedures, in particular total split posterior tibial tendon transfer (T-SPOTT), in combination with adjunctive corrective procedures depending on the degree of deformity.
STEP 1 SURGICAL PREPARATION: Place the patient in a supine position and follow a standard aseptic surgical disinfection and draping protocol, allowing access to the iliac crest.
STEP 2 STEINDLER RELEASE OF THE PLANTAR APONEUROSIS: Use a medial approach to access and transect the plantar apo

BACKGROUND: Arthrodesis of the naviculocuneiform (NC) joints is not a common procedure, as it is perceived by many to be less reliable or less predictable than arthrodesis of proximal or distal joints in the medial column. There is a subset of patients with planovalgus feet, cavovarus feet, and degenerative arthritis who also have an apex of deformity at the NC joints in whom fusion is indicated. The surgical technique, fusion rates, and deformity correction data for NC fusion in planovalgus feet are evaluated in this report.
METHODS: Twenty-eight patients (33 feet) who underwent surgery betw

BACKGROUND: Radiographic charted values are widely applied in the assessment of paediatric flat foot and as quantitative markers of intervention success. Nearly all literature pertaining to the reliability of these measures relates to the adult foot. In view of the importance placed on these charted values we assessed the inter and intra-rater reliability of 10 key radiographic measures in children aged 7-14 years.
METHODS: We present the reliability of these measures in terms of intra class correlation and standard error based on repeated measures of 50 radiographs across five independent tr

BACKGROUND: Neglected or inadequately treated rigid congenitally deformed feet in older children are a nightmarish challenge for the child, the parents, and the orthopaedic surgeon. Because of the multiplicity of spatial deformities exhibited by these feet and legs, it was hypothesized that correction using the Taylor spatial frame (TSF) would decrease morbidity, facilitate correction, and minimize treatment time in children from remote regions with extremely rigid deformed feet.
METHODS: Recent experience with the management of 11 such feet (Dimeglio type IV) in 9 children with an average ag

Twelve patients (17 feet) with progressive neuromuscular planovalgus foot deformity underwent an extra-articular subtalar arthrodesis with the combined Batchelor-Grice procedure. All patients were ambulant and received conservative treatment with braces and special orthopaedic shoes before the operation. Postoperatively the feet were immobilized in short leg casts for 6 weeks. Evaluation was based on the appearance of the feet, the clinical symptoms and radiographic measurements. Solid fusion and sustained correction took place in all feet. Foot appearance and mobility of the patients were imp

BACKGROUND Secondary polycythemia is a potential complication of an erythropoietin-secreting renal cell carcinoma. Increased red blood cell mass can elevate blood viscosity, which can impair blood flow, making individuals susceptible to vaso-occlusive events. One of the serious potential complications of a hyper-viscous state is ischemic stroke. CASE REPORT We present the case of a patient who was brought to the Emergency Department with right-sided extremity weakness and slurred speech consistent with acute ischemic stroke. MRI showed acute infarct involving the left corona radiata and poster

Although equinus deformity in children with cerebral palsy is often corrected with surgery, postsurgical recurrence of the deformity is not uncommon. In order to isolate factors that may be related to its recurrence, 31 studies were evaluated. Data from nine articles indicated that children younger than approximately 7 years of age at the time of surgery had a higher risk of recurrence than children who were older at the time of surgery. Recurrence rates may be understated in studies including less than a minimum of 4-5 years of patient follow-up. Lower recurrence rates are documented in diple

PURPOSE OF THE STUDY: Much work has focused on the short-term outcome after external fixation with the Ilizarov apparatus for treatment of severe recurrent or neglected foot deformity. No series has provided sufficient follow-up to allow an analysis of long-term stabilization.
MATERIAL AND METHODS: We reviewed a series of 17 patients (21 feet) treated with the Ilizarov apparatus between 1988 and 1998 and reexamined after a mean 6-year follow-up. We assessed the risk of degradation of the initial result and quantified the need for secondary arthrodesis surgery. This series included 18 congenit
