Trans-Nasion-Complex Approach for Endoscopic Modified Lothrop Procedure: Conception, Anatomy, and Technique.

BACKGROUND: The endoscopic modified Lothrop procedure (EMLP) is an important procedure used to address frontal and anterior skull-base lesions. Two techniques were established, namely, the inside-out approach and the outside-in approach. The former technique take the frontal recess and the first olfactory filament (FOF) as key landmarks while the latter use the FOF as posterior boundary. In some cases, however, these two landmarks are not available. Therefore, we supplement the outside-in approach and named it trans-nasion-complex approach (TNCA) for EMLP that can be performed without locating

An 8-year-old boy with a Pott's puffy tumor.

An 8-year old boy with a history of trauma, sinusitis and a swelling of the frontal bone with somnolence was diagnosed with a Pott's puffy tumor (PPT). Minimal invasive surgical intervention was performed together with a strict regimen of antibiotic therapy. In this case debridement of the frontal bone was not necessary. Serial X-ray imaging of the skull showed complete ossification of the frontal bone lesion. Early diagnosis using thorough radiological evaluation is necessary to effectuate the proper therapeutic approach. For this reason, a patient with a forehead swelling and a history of tr

Definitive reconstruction of frontal asymmetry in an adolescent with anterior plagiocephaly due to unilateral coronal craniosynostosis using a hybrid osteo-titanium technology based on an individualized approach: A case report.

BACKGROUND: Unilateral coronal craniosynostosis causes three-dimensional craniofacial asymmetry. Although correction is usually performed during infancy, some patients present in adolescence with an untreated or persistent deformity. We describe an individualized hybrid reconstruction combining a patient-specific titanium implant with autologous bone. CASE DESCRIPTION: A 15-year-old girl with untreated left unilateral coronal craniosynostosis presented with marked frontal and fronto-orbital asymmetry. Three-dimensional computed tomography demonstrated left frontal retrusion, orbital asymmetry

[Cephalography in Thalassemic patients].

This article reported a study of cephalography in 60 Thalassemic patients between the age of 11-16 years old. There were 8 beta-thalassemia, 35 beta-thalassemia/Hb E and 17 Hb H diseases. Sixty age-matched controlled patients were used for comparison. It was found that 11 thalassemic patients (18.2%) had showed absence of frontal sinus, only 3 cases had showed absence of maxillary sinus, 38 cases (63.3%) had thickening of frontal bone and parietal bone, mostly in the anterior region. Twenty-two cases (36.6%) had fine granular pattern of bone, five cases (8.3%) had hair on end appearance and 44

Hiding Within the Cracks: Case Report of Rare Sutural Bone Found at the Nasion.

Pathology such as skull fractures can be misdiagnosed in the presence of anatomical variations. One variant that has had little description in the literature are the sutural bones associated with the nasal bones. Herein, we describe a case of a rare sutural bone at the nasion, between the bones of the right nasal, frontal, and maxillary frontal process. To our knowledge, this is the first report of such a variant bone in this location, and such it should be considered by clinicians when evaluating patients for pathology in this region.

Giant epidermoid cyst as an unusual presentation of hypoglobus and ptosis.

A 24-year-old man presented with a large right frontal epidural epidermoid cyst that resulted in right hypoglobus and right upper eyelid ptosis. CT and MRI revealed a bone-destroying mass involving the right frontal bone and eroding through the right orbital roof. Complete surgical removal was performed through a frontal craniotomy. After surgery, there was near-complete resolution of the hypoglobus and ptosis.

Radiological relationship of the ethmoid slit with the frontal sinus and the anterior ethmoidal artery and applications to the frontal sinus drillout.

PURPOSE: To compare two types of CT acquisition parameters: CT scan of the facial bone and CT scan of the sinuses, for studying the ethmoidal slit and its relationship with the frontal sinus and anterior ethmoidal artery. MATERIALS AND METHODS: Retrospective study of 145 scans of the sinuses and 79 of the facial bones performed between 2012 and 2016. On each scan, the visibility of the ethmoidal slits, their length, their distance from the ethmoidal artery, and their relationship with the anterior and posterior wall of the frontal sinus were studied. RESULTS: The ethmoidal slit was better vi

Adaptation of the cranium to spring cranioplasty forces.

BACKGROUND: During spring-assisted cranioplasty, the spring transmits forces through adjacent cranium. We have previously demonstrated that the ectocranial-endocranial thickness of cranial sutures increases significantly over time in the presence of continuous spring forces. We wished to investigate if cranial bone showed similar adaptational responses. METHODS: New Zealand white rabbits were randomized into a treatment group [a spring was placed across a posterior frontal suture (PFS) suturectomy and a control group (PFS suturectomy)]. Animals (n = 6) were euthanized from each group at 4, 7,

Anatomic guidelines for the placement of external references for maxillary repositioning.

OBJECTIVE: External reference points, particularly Kirschner pins (K-wire), placed in the region of the nasion have been shown to improve the accuracy of maxillary vertical repositioning. Although no complications associated with this technique have been reported, there is a potential for injury to the anterior cranial fossa or frontal sinus. The purpose of this study was to measure the shortest distance from the nasion to the anterior cranial fossa and from the nasion to the frontal sinus. These measurements were used to establish anatomic guidelines governing safe placement of external refer

OBJECTIVE: To investigate the development of the metopic suture in fetuses with holoprosencephaly at 11 + 0 to 13 + 6 weeks of gestation. METHODS: Three-dimensional (3D) ultrasound was used to measure the height and gap between the frontal bones in 200 normal fetuses and in nine fetuses with holoprosencephaly at 11 + 0 to 13 + 6 (median, 12) weeks of gestation. RESULTS: In the 200 normal fetuses, the height of the frontal bones increased significantly with gestation from a mean of 2.5 mm (5(th) and 95(th) centiles: 1.9 mm and 3.3 mm) at 11 weeks to 6.1 mm (5(th) and 95(th) centiles: 4.6 mm a

Pott puffy tumor appearing with a frontocutaneous fistula.

Pott puffy tumor is defined as a forehead-localized swelling with overlying subperiosteal abscess and osteomyelitis of the frontal bone. It is an uncommon disease entity and more rarely reported with its frontocutaneous fistula. We report the unusual case of Pott puffy tumor appearing with a frontocutaneous fistula. We performed a frontal sinus surgery using a combined endonasal and percutaneous approach and frontal bone reconstruction with a resorbable mesh plate and bone cement. During a follow-up 1 year after the surgery, there was no recurrence, and we achieved excellent result from the ae

The pressure exerted by mucoceles in the frontal sinus. An experimental study in the cat.

The pathophysiologic process that results in bone erosion from frontal sinus mucoceles has long been a mystery. Traditionally, bone erosion has been attributed to pressure. However, no scientific evidence has yet been presented to support this hypothesis. To answer this question, seven cats had a mucocele created in one frontal sinus, with the other sinus left as a control. A manometer was inserted into each frontal sinus. Pressure readings were taken over a 7-month period. Pressures on the mucocele side were higher than on the control side and were sufficient to erode bone. Bone was thicker o

The tenacity of the frontal sinus mucosa.

A number of persons in other surgical disciplines express a blatant disregard for the potential dangers of frontal sinus mucosal entrapment. Although bone removal is universally recommended by the otolaryngologic proponents of ablative procedures in the frontal sinus, no comprehensive explanation has been proffered to justify this dictum. To this end, 27 cats had simple excision of the frontal sinus mucosa to see if it is necessary to bur away the inner table of bone to completely eliminate all vestiges of mucosa. Mucosal regrowth was seen in 72%, and suppurative infection, in 17% of the sinus

Modified Lynch procedure for chronic frontal sinus diseases: rationale, technique, and long-term results.

The modified Lynch operation (Neel-Lake) differs in several ways from the operation described by Lynch. The operation begins with an intranasal anterior ethmoidectomy. The agger nasi cells are removed by curetting forward between the frontal process of the maxilla and the septum. The middle turbinate, normal-appearing mucosa of the frontal-ethmoid complex, and frontal process of the superior maxilla are preserved. Bone removal is limited in most cases to the anterior floor of the frontal sinus, a portion of the lacrimal bone, and the bone over the anterior ethmoid cells. Another important diff

Piano technique for frontal sinus setback: a technical note.

Frontal sinus setback is a procedure used for addressing severe protrusion of the frontal area. Various surgical techniques have been developed over time to address frontal sinus abnormalities with varying etiologies and severity. The present article introduces the piano technique as a novel method for frontal sinus setback with less aggressiveness and complexity compared to other available methods, while being efficient in providing substantial long-term improvements in aesthetic outcomes. In this method, the anterior wall of the frontal sinus is divided into numerous rectangular bony segment

Late complication of the frontobasal trauma.

The injuries of the frontobasal region are always a great challenge to a surgeon, especially the management of bone defects of the frontal region. The authors present a patient with late, 33-year postaccident complication, which had been surgically treated, and whose frontal bone defect had been reconstructed with methyl methacrylate. Clinical examination and computed tomography confirmed signs of previous operation and presence of an infection and alloplastic material. Specific for this case was challenge to manage chronic infection and reestablish integrity of the skull in the frontal region

Osteoplastic flap approach versus orbitotomy in case of orbitofrontal cholesterol granuloma.

A 44-year-old man developed a slow-growing painless left superolateral orbital mass that extended into the frontal sinus with a complaint of ptosis. Magnetic resonance imaging revealed a heterogenous hyperintense lesion confined to the left frontal bone and superior orbit. The osteoplastic frontal sinus approach was performed to drain supraorbital cholesterol granuloma cyst and for curetting the capsule. Orbitofrontal cholesterol granuloma characteristically arises in the diploe of the superolateral frontal bone. The traditional approach for a primarily orbitofrontal cholesterol granuloma is t
