Effects of permissive hypercapnia on intraoperative cerebral oxygenation and early postoperative cognitive function in older patients with fragile brain function during the non-acute phase undergoing laparoscopic colorectal surgery: A randomized controlled trial.

BACKGROUND AND PURPOSE: Older adults with non-acute fragile brain function (NFBF) may be particularly susceptible to perioperative disturbances in cerebral oxygenation and postoperative neurocognitive decline. Permissive hypercapnia (PHC) may enhance cerebral oxygenation, but its effects in this population remain unclear. We examined whether PHC-based ventilation improves intraoperative regional cerebral oxygen saturation (rSO2) and early postoperative cognitive outcomes in older patients with NFBF undergoing elective laparoscopic colorectal surgery. METHODS: In this single-center, single-bli

Association of target-controlled versus manually controlled infusion with postoperative recovery outcomes in elderly cardiac surgery patients: an international multi-cohort study.

BACKGROUND: Target-controlled infusion (TCI) optimizes drug delivery and hemodynamic stability using individualized pharmacokinetic models. We compared the effects of TCI versus manually controlled infusion (MCI) of propofol and remifentanil on postoperative recovery in elderly cardiac surgery patients. METHODS: An international retrospective cohort study was conducted using data from Union Hospital and the INSPIRE dataset. Elderly patients (≥65 years) receiving TCI were matched 1:1 with those receiving MCI using propensity scores to adjust for confounding variables. The primary outcome was t

Dual-threshold plasma Aβ oligomerization for postoperative delirium risk stratification.

BACKGROUND: Postoperative delirium is common in older surgical patients, but simple blood tests to identify risk are lacking. Plasma amyloid-β oligomers measured by multimer detection (MDS-OAβ) may reflect neurodegenerative vulnerability. METHODS: We enrolled 101 patients aged ≥65 years undergoing elective orthopaedic surgery with general anaesthesia. Blood was drawn preoperatively and at first delirium diagnosis or on postoperative Day 4 if no delirium. MDS-OAβ was quantified blinded. Delirium was assessed daily on postoperative Days 1-3 (DRS-R-98 and DSM). Propensity-score matching on APOE

Challenging Medical and Surgical Management of Ectopic Parathyroid Hormone (PTH) Adenomas in the Elderly.

Ectopic parathyroid adenomas (EPAs) are rare and difficult to localize, making surgical cures challenging on the first attempt. However, the management of EPAs in the literature on the geriatric population is limited. Herein, we report two elderly cases of EPAs in atypical locations and challenging management with a mini review of the literature. An 82-year-old English male presented with confusion and constipation, and his tests confirmed primary hyperparathyroidism (PHPT). However, IV fluids and IV bisphosphonate treatment did not treat his hypercalcemia and imaging investigations could not

Benchmarking institutional geriatric hip fracture management: a prelude to a care quality improvement initiative.

BACKGROUND: Fractures around the hip in older adults have increased in the last two decades, and the numbers are projected to rise over the next 30 years with estimates that half of them will occur in Asia. Proximal hip fractures should be operated within 48 h of injury to prevent poor outcomes. This study aims to benchmark current hip fracture care using quality improvement tools of care structure, care processes, and outcomes in a tertiary care hospital in Eastern India and determine the evidence-practice gaps and barriers to implementing the six best practices that reduce mortality and morb

The association of peri-operative scores, including frailty, with outcomes after unscheduled surgery.

Postoperative hospital stay is longer for frail, older patients, who are more likely to experience prolonged postoperative morbidity and reduced long-term survival. We recorded in-hospital mortality, morbidity and length of stay for 164 patients aged at least 65 years after unscheduled surgery. We evaluated pre-operative frailty with the 7-point Clinical Frailty Scale: 81 patients were 'not vulnerable' (frailty score 1-3) and 83 were 'vulnerable or frail' (frailty score ≧ 4), with mean (SD) ages of 74.7 (7.5) years vs. 79.4 (8.3) years, respectively, p < 0.001. Within 30 postoperative days 8/1
