Endoprosthesis is a method allowing a valuable substitution of bone defects (articular ends of long tubular bones, both articular ends of the bones making the articulation, whole long tubular bones with articulations). The employment of endoprostheses is expedient for the substitution of defects of the breast bone, pelvis, hand bones as well. Endoprosthesis in combination with preoperative radiotherapy and post-operative modern chemotherapy allows preserving the extremities in certain patients with osteogenic sarcoma, chondrosarcoma, reticulosarcoma, chondrosarcoma and solitary plasmacytoma. Two different series of patients with burst injuries of the hand were retrospectively reviewed: one series included 23 men, 15-55 years old, referred to the emergency department for the first-instance assessment of injuries of the right hand (13 patients) and left hand (10 patients). The other series included 44 patients (42 men and 2 women, aged 7-61 years) referred to our department for second-instance examinations: the latter patients had burst injuries involving only the hands in 43 cases and both the hand and the foot in one case. We report the radiologic patterns of traumatic bone injuri

Chondrosarcoma is the second-most common primary malignant bone tumor but chondrosarcoma of small bones of the hand is extremely rare, representing less than 2% of all cases, with exceedingly rare skin metastases. Cutaneous metastases of chondrosarcoma represent less than 3% of all cutaneous metastases. According to PubMed, there are only four previous case reports of cutaneous metastases originating from chondrosarcoma of small bones of the hand. We present an additional case of cutaneous metastases of phalangeal chondrosarcoma with a unique immunophenotype.

Three small infants whose mothers had received oral anticoagulant therapy during the first trimester of pregnancy are described. These infants all had hypoplasia of the nasal bones, and two had stippling of epiphyses and bones and deformities of the bones of the hand. One child is mentally retarded. It is suggested that these abnormalities may be related to maternal oral anticoagulant therapy during the first trimester.

Bone and soft-tissue sarcomas infrequently develop in the hand and wrist. Given the complex anatomy of this area, wide resection with adequate margins often impairs hand function because of the resection of essential structures, including tendons, bones, and tissues adjacent to the sarcoma. Here, we present a case of primary synovial sarcoma adjacent to the dorsal side of the carpal bones, which were resected with hemi-resection of the carpus and resection of extensor tendons, followed by wrist joint arthrodesis and palmaris longus tendon grafting. Hand function was satisfactory despite some d

"Radialization," a new technique for operative treatment of the radial club hand, is presented. It has been successfully used in 30 hands (23 patients) since 1979. It is named "radialization" because after all fibrotic tissues are excised, the hand and radial carpal bones are placed over the distal end of the ulna; the hand is fixed with a Kirschner wire in a position of moderate ulnar deviation. Usually, no carpal bones need to be removed. The improved mechanical forces are further stabilized by transposition of the radial wrist extensor and flexor to the ulnar side; this favors a better musc

Chondrosarcoma is most frequently present in the pelvis and long bones and rarely seen in the bones of the hand. Traditionally the treatment of choice for involvment of the hand is ray amputation, however this causes significant functional deficit if there is thumb involvement. There are limited cases in literature of resection of thumb chondrosarcoma with restoration of function. We present a case of high grade chondrosarcoma of the first metacarpal treated with resection and free fibular graft reconstruction. We report excellent functional outcome with this procedure.

Congenital anomalies of the carpus associated with congenital hand and upper limb anomalies were investigated from the point of view of the development of the hand. Defects of the carpus were categorized as follows: 1) Radial ray defect: loss of the scaphoid, trapezium and trapezoid. 2) Central ray defect: loss of the capitate and loss of a part of the trapezium and hamate. 3) Ulnar ray defect: loss of the triguetrum, pisiformis and hamate. Deficiencies of the carpus and more distal parts of the hand proved to be secondary to defects of the forearm bones in both the radial and ulnar ray. On th

Ewing sarcoma is a malignant, small, round, blue cell tumor that often affects the long bones and pelvis. It is rarely seen in the bones of the hand. A minority of Ewing sarcoma cases arise in soft tissue. This case report details the clinical and radiological presentation of an extraosseous Ewing sarcoma of the index finger in a 22-year-old woman. Treatment consisted of an index ray amputation followed by adjuvant chemotherapy. After 1 year, the left hand had excellent function with an acceptable cosmetic appearance and no evidence of metastasis or local recurrence.

Complex injuries involving bones, tendons and soft tissues of the hand still represent a difficult challenge for reconstructive surgeons. Hereby, we present a case of a man suffering from a complex crush injury of the right hand that led to a wide three-dimensional defect with loss of two metacarpal bones, extensor tendons and skin. For composite hand defects, the most common surgical approach provides for a one-stage composite reconstruction or for an early restoration of bone and skin, with delayed tendon grafts. For the peculiarity of the defect, a one-stage reconstruction was not feasible.

The deep branch of the ulnar nerve (DBUN) is a pure motor nerve. It passes through a hypothenar fibromuscular tunnel and courses radially on the interossei surface. The DBUN is not frequently considered during hand fracture surgery, despite the anatomical course of the nerve in close relation to the carpal and metacarpal bones, which makes it vulnerable to penetrating injury and being injured during hand surgery fixations. In this article, we describe a case of DBUN injury after percutaneous pinning of the fourth and fifth metacarpal bone fractures complicated by intrinsic muscle wasting of th

Fractures and luxations in the area of the carpal bones are in comparison with other fractures of the upper and lower extremities comparatively rare. Isolated fractures of luxations of the distal row of the carpal bones, by comparison with those of the proximal row, are still rarer incidents. Because of tight ligamentous unions between the carpal bones on the one hand and between the distal row of the carpal and the metacarpal bones on the other hand, a luxation or fracture is connected with a violent contusion or sprain. In the case of seriously or multiply injured persons the carpal injuries

Distraction osteogenesis has become the most used method for bone lengthening and deformity correction including the bones of the hand and foot. The principle techniques used for hand bone lengthening in posttraumatic and congenital conditions using available external fixators are based on this method. We present the technical specifications and surgical techniques with the use of a mini-fixator that was specially designed for short tubular bones. The surgical techniques of lengthening and web space skin stock creation for future web-pasty are supplied with the diagrams of their application to

Treatment and result of burned total hand was reported here in 4 hands (4 cases). According to the treatment and anatomical characteristics, burned hand was divided into 4 areas: dorsum of hand, dorsum of finger, thenar-wrist area and volar-digit area. The treatment of total hand in third degree burns included emergency escharotomy of hand and digits, early escharotomy and immediate skin grafting on the dorsum of hand, thenar-wrist and volar-digit area. But on the dorsum of digit area this sort of hand was often deepen to the phalangeal bones and joints. After healing of early graft, the expos

Periosteal, intracortical, and endosteal resorptive changes were evaluated in both hands and the proximal radius in 161 chronic renal-failure patients by microradioscopy, radiographic morphometry, and photodensitometry. These changes were compared to abnormalities found in skeletal surveys. A combination of microradioscopy and morphometry of hand bones was sufficiently sensitive for early detection of bone loss in renal osteodystrophy in 61% of the patients. Intracortical resorption in metacarpals appeared to be a sign of more advanced renal osteodystrophy than subperiosteal resorption in phal

Cleft hand is a rare and complex malformation in which the central part of the hand is missing. Since the nineteenth century, a very small number of dissections in cleft hand have been reported and very few reports describe the soft-tissue abnormalities. We report a case of an elderly cadaveric specimen with bilateral cleft hand. X-ray, CT-scan with 3D reconstruction and dissection were performed on the right hand and forearm. On both the sides, we noted the absence of the index, middle and ring finger. Malformation and degenerative arthrosis at the carpal bones level have been identified. The

The author describes three prototypes of a miniature distractor for the bones of hand constructed according to his own design, and its uses in clinical practice. The distractors are used for lengthening bones of the hand after injuries involving loss of the thumb or fingers, and for the treatment of patients with congenital developmental defects of the hand. Over a period of three years, the distractors were used in thirteen cases and Ilizarov's original apparatus in three cases: for the lengthening of metacarpus I in seven cases, metacarpus II in two cases, metacarpi II-IV in one case, metaca

Frequency of rare skeletal lesions in Paget's disease is compared with normal localisations. Radiological morphology of rare localisations of Paget's disease of the scapula, of the lower arm bones, of the hand bones, of the patella and the calcaneus are described. Thickening of trabeculae in the spongy bone and porosity of compact bone, deformation and enlargement of the bones are typical; this has to be differentiated and related to a small number of other bone diseases.

One thousand consecutive fractures of the metacarpals, phalanges, and carpal bones have been recorded over a period of about 10 months in Bergen, Norway, to find out the relative incidence of these fractures in an unselected series: this corresponds well with the few other reports of the incidence of hand fractures that we could find. In the total series the metacarpals, phalanges, and carpal bones account for 36%, 46%, and 18% of the fractures, respectively. Fractures of the scaphoid make up 10.6% of the total, fractures of the neck of the fifth metacarpal 9.7%, and Bennett's fractures 1.4%.
