Hip resurfacing arthroplasty for end-stage arthritis caused by childhood hip disease.

INTRODUCTION: Patients with hip arthritis due to Legg-Calvé-Perthes (LCP) and slipped capital femoral epiphysis (SCFE) pose altered femoral anatomy, making hip resurfacing arthroplasty (HRA) technically complicated. We examined implant survival and clinical symptoms in patients with a history of LCP or SCPE who underwent HRA for end-stage osteoarthritis. METHODS: Data was collected for patients who underwent HRA for osteoarthritis due to LCP (n = 59) or SCFE (n = 32). Harris Hip Scores (HHS), UCLA activity scores, and radiographs were evaluated pre and postoperatively. Wilcoxon Signed-Rank Te

Radiographic hip involvement in ankylosing spondylitis: factors associated with severe hip diseases.

OBJECTIVE: To determine the factors associated with severe radiographic hip involvement in patients with ankylosing spondylitis (AS). METHODS: A cross-sectional retrospective study was performed. The patients were classified into 3 groups based on the Bath Ankylosing Spondylitis Radiology Hip Index (BASRI-hip): minimal hip disease, moderate hip disease, and severe hip disease. Demographic, clinical, radiographic, and laboratory data were collected and analyzed. To identify factors associated with severe hip disease, ordinal regression analyses were performed. RESULTS: A total of 256 patients

Total hip arthroplasty in high dislocated and severely dysplastic septic hip sequelae.

The authors analyzed a consecutive series of 20 total hip arthroplasties performed using a cementless conical stem with shortening osteotomy combined with greater trochanter transfer in cases with a highly dislocated hip secondary to sequelae of a septic hip in childhood. Mean patient age was 47.3 years and the mean follow-up period was 3.4 years. An acetabular metal cup was inserted in 5 cases, and only a liner was inserted after cementing in 15 cases. Mean Harris hip score improved from 42.4 preoperatively to 84.2 at final follow-up. Mean leg lengthening was 36.5 mm, and time to greater troc

Incidental "rotator cuff tear of the hip" at primary total hip arthroplasty.

Gluteus medius tendon tears are occasionally noted during primary total hip arthroplasty. In this study, we reviewed the cases of 513 total hip arthroplasty patients to determine the incidence of these tears and to report clinical outcomes. We found 8 patients (8 hips) with incidental gluteus medius tendon tears for an incidence of 1.6%. After surgical repair, no patient had a perceptible limp or a Trendelenburg sign postoperatively.

Functional outcomes of bilateral hip necrosis: total hip arthroplasty versus extracorporeal shockwave.

INTRODUCTION: This study compared the functional outcomes of total hip (THA) in one and shockwave (ESWT) in the other in 17 patients with bilateral hip necrosis. METHOD: In THA side, only one type of prosthesis was used and all components were press fixed. In ESWT side, each hip received 6,000 impulses of shockwave at 28 kv. RESULTS: The evaluations included pain score and Harris hip score, radiographs and MR images. The magnitudes of improvement in pain and function favored the ESWT side. Thirteen patients rated ESWT better than THA; four patients reported comparable results between THA and

Unilateral hip osteoarthritis: can we predict the outcome of the other hip?

OBJECTIVE: The objective of this study was to define, in unilateral hip osteoarthritis (OA), factors predicting the outcome of the other hip. MATERIALS AND METHODS: We examined the anteroposterior radiographs of the pelvis of 95 white patients with unilateral idiopathic (56 patients) or secondary to congenital hip diseases (39 patients) OA. The other hip was free from symptoms (pain or limping) at the initial examination and without radiographic evidence of OA; it was what we call a "normal" hip. Two parameters were evaluated: (1) the type of osteoarthritis of the involved hip and (2) the ran

Hip arthroscopy without a perineal post: a safer technique for hip distraction.

Pudendal nerve palsy is a reported complication of hip arthroscopy. We report a technique using a deflated taped beanbag rather than a perineal post. The patient is placed in the supine or lateral position on a fracture table. The beanbag is contoured around the patient's flank and thorax. The distal aspect of the beanbag is placed no further than the iliac crest, and care is taken to avoid compression of the posterior aspect of the axillary region or the posterior humerus. The molded beanbag is deflated, a blanket is positioned over the abdomen and lower thorax, and with the use of 3-in-wide

Hip arthroscopy for osteochondral loose body removal after a posterior hip dislocation.

We report on the application of hip arthroscopy to remove an osteochondral fragment created by a posterior hip dislocation. Preoperative and postoperative radiographs and computed tomography scans correlate with intraoperative arthroscopic photographs and are presented with this report. Arthroscopy allowed excellent visualization of the joint and facilitated straightforward removal of the fragment. We were able to avoid the larger incision required by an arthrotomy and decreased the patient's overall morbidity from this condition.

Temporal gait analysis of hip osteoarthritic patients operated with cementless hip replacement.

OBJECTIVE: This study attempts to identify temporal gait parameters, which characterize the pre- and postoperative course of hip monoarthritic patients who undergo cementless hip replacement. BACKGROUND AND METHODS: Twenty-five female hip monoarthritic subjects (mean age 51, SD 5.0) were studied preoperatively; 20 out of them were studied at 1 and 8-10 years after the operation with a 22-m conductive walkway system using telemetry. A gender-matched control group was also studied (mean age 60, SD 2.0). The assessed parameters are the gait cycle duration, the single and double support, the sing

Can ischemic hip disease cause rapidly destructive hip osteoarthritis? A case report.

Avascular osteonecrosis of the femoral head (AONFH) usually goes through the four stages described by Arlet and Ficat: normal radiographs, heterogeneity and sclerosis of the femoral head, subchondral fracture with an individualized sequestrum, and secondary osteoarthritis. Arlet and Ficat individualized a specific pattern of AONFH which they called ischemic hip disease, in which cartilage damage seen as concentric joint space loss precedes the bony alterations. Although radiological and pathological studies of ischemic hip disease have been published, no clinical data are available. We report

Bipolar versus total hip arthroplasty for hip osteonecrosis in the same patient.

The authors studied 28 patients with bilateral avascular necrosis of the femoral head who were treated with a cementless bipolar endoprosthesis in one hip and cementless total hip arthroplasty in the other. All the hips selected for bipolar endoprostheses were classified as having avascular necrosis of the femoral head Ficat Stage III, and all the hips selected for total hip arthroplasty were classified as having Ficat Stage IV avascular necrosis. After a midterm followup of an average of 6.4 years (range, 4-12 years), 24 of 28 hips that received bipolar endoprostheses were considered satisfac

Results of total hip arthroplasty for Crowe Type III developmental hip dysplasia.

From 1969 through 1980, 90 hips in 82 patients had cemented total hip arthroplasty for Type III developmental hip dysplasia. Seventy hips were reviewed at an average of 16.6 years (range, 5-23 years) after operation. Aseptic loosening developed in 53% of acetabular cups and 40% of femoral stems. Despite attempts to place acetabular components in the anatomic center, 18 cups (25.7%) were placed outside that area. Using a measurement method to determine the true acetabular region and approximate femoral head center, final acetabular loosening strongly correlated with initial cup placement. Loose

Individual risk factors for hip osteoarthritis: obesity, hip injury, and physical activity.

Hip osteoarthritis is a major cause of pain and disability. The authors explored individual risk factors for hip osteoarthritis in a population-based case-control study. The study was performed in two English health districts (Portsmouth and North Staffordshire) from 1993 to 1995. A total of 611 patients (210 men and 401 women) listed for hip replacement because of osteoarthritis over an 18-month period were compared with an equal number of controls selected from the general population and individually matched for age, sex, and family practitioner. Information about suspected risk factors was

Hip fractures. Epidemiology, risk factors, falls, energy absorption, hip protectors, and prevention.

The present review summarizes the pathogenic mechanisms leading to hip fracture based on epidemiological, experimental, and controlled studies. The estimated lifetime risk of hip fracture is about 14% in postmenopausal women and 6% in men. The incidence of hip fractures increases exponentially with aging, but the time-trend in increasing age-specific incidence may not be a universal phenomenon. Postmenopausal women suffering earlier non-hip fractures have an increased risk of later hip fracture. The relative risk being highest within the first years following the fracture. Total hip replacement with solid autologous femoral head graft for hip dysplasia.

Acetabuloplasty with solid autologous femoral head graft for the treatment of hip dysplasia is an established method which creates the contained cavity needed to accommodate the artificial cup. In order to evaluate the medium- and long-term results of this method using a cementless hemispheric cup, 34 patients (39 hips) operated on between 1979 and 1986 were clinically and radiologically reviewed. The minimal follow-up was 5 years (average 7.6 years). The Harris hip score increased from 36 points preoperatively to 89 points 1 year postoperatively and to 85.1 at the last follow-up.
