Acute hyponatremia, following neurosurgery, results from inappropriate antidiuretic hormone secretion (SIADH) or cerebral salt wasting (CSW). CSW is due to abnormally high atrial or brain natriuretic peptides (ANP, BNP), which block all stimulators of zona glomerulosa steroidogenesis, resulting in mineralocorticoid deficiency. A case of lymphocytic hypophysitis is described in a patients with Graves' disease and diabetes mellitus. The 62-year-old man was admitted to hospital with the complaints compatible with hyperthyroidism in April 1993. His medical history, physical examination, thyroid function tests, thyroid scintigraphy and thyroid ultrasonography revealed Graves' disease. To investigate pituitary effects on the vitamin K-dependent coagulation factors, female rats were hypophysectomized (hypox) and treated with growth hormone (GH), cortisone, thyroxine, vitamin K, or saline. In one report, the control of central pain (thalamic pain syndrome) was also approached with chemical hypophysectomy. We describe here a unique case of lymphocytic hypophysitis accompanied by pustulosis palmaris et plantaris and eosinophilia. The patient also suffered from panhypopituitarism with hyperprolactinemia and pituitary diabetes insipidus caused by lymphocytic hypophysitis. Complications of pustulosis palmaris et plantaris and eosinophilia with lymphocytic hypophysitis have not been reported previously. In the present case, the activities of the three diseases correlated well throughout the patient's course, suggesting that a common mechanism might possibly participate in their pathogenesis.

We have examined the anterior pituitary hormone messenger (m) RNA species contained in biopsies of 41 pituitary tumours obtained at hypophysectomy using in-situ hybridization histochemistry.
