Resolving the sexual intimacy paradox: a developmental model for the treatment of sexual desire disorders.

Although relationship factors are an important component in the evaluation and treatment of hypoactive sexual desire (HSD) disorders, a sexual intimacy paradox often occurs wherein many couples increase the intimacy in their relationship while their sexual desire continues to decline. To resolve the apparent paradox and to further our understanding of the relationship between intimacy and eroticism, we offer a developmental model of sexual intimacy as a guide to treating the interpersonal aspects of HSD and other sexual dysfunctions. This model expands Erikson's developmental phase of intimacy

Efficacy of treatments for anxiety disorders: a meta-analysis.

To our knowledge, no previous meta-analysis has attempted to compare the efficacy of pharmacological, psychological and combined treatments for the three main anxiety disorders (panic disorder, generalized anxiety disorder and social phobia). Pre-post and treated versus control effect sizes (ES) were calculated for all evaluable randomized-controlled studies (n = 234), involving 37,333 patients. Medications were associated with a significantly higher average pre-post ES [Cohen's d = 2.02 (1.90-2.15); 28,051 patients] than psychotherapies [1.22 (1.14-1.30); 6992 patients; P < 0.0001]. ES were 2

Reflective functioning skills mediating the relationship between attachment security and therapeutic alliance in mentalization-based treatment for children.

OBJECTIVES: The therapeutic alliance is a critical predictor of psychotherapy outcomes, yet little is known about the child-specific interpersonal factors influencing its early development in child psychotherapy. This study aimed to examine whether children's pre-treatment attachment security and reflective functioning (RF) skills predict the strength of early therapeutic alliance. DESIGN: The study was part of a randomized controlled trial measuring the effectiveness of Mentalization-Based Treatment for Children (MBT-C). METHODS: Participants were 71 children aged 7-12 who received the MBT-

Neurodiversity as an epistemic stress test for psychotherapy and health care: a neuroaffirmative conceptual analysis of relational-proximity priors in clinical, institutional, and AI-supported models.

Neurodiversity-affirming mental health care requires not only adapting support to neurodivergent people but also examining the relational assumptions through which alliance, empathy, cooperation, progress, and risk are interpreted. This Conceptual Analysis develops the heuristic concept of relational-proximity priors: implicit assumptions that emotional closeness, rapid reciprocity, eye contact, visible warmth, and ready acceptance of help are generally reliable indicators of therapeutic fit or improvement. Drawing primarily on autism research and, more cautiously, on broader neurodivergence-i

Transference interpretation and psychotherapy outcome: a systematic review of a no-consensus relationship.

Despite its well-established importance in psychoanalytic theory, there is a scarcity of empirical evidence on the relationship between a therapist's transference interpretation (TI) and therapeutic outcome. The current scientific literature shows no consensus on the existence and nature of such an association. Therefore, the present study aimed to systematically review the literature on the link between TI and outcomes in psychodynamic psychotherapies. The American Psychological Association PsycInfo, MEDLINE, and the Web of Science Core Collection were selected as the primary databases for th

A Functional Connectivity Study to Investigate the Role of the Right Anterior Insula in Modulating Emotional Dysfunction in Borderline Personality Disorder.

OBJECTIVE: Previous imaging studies in patients with borderline personality disorder (BPD) have detected functional brain dysfunctions. Mindfulness training may improve the symptoms of BPD, although the neural mechanisms involved remain poorly understood. This study had several key aims: a) to investigate the role of right anterior insula (rAI) functional connectivity in modulating baseline emotional status in BPD, b) to compare differences in connectivity changes after mindfulness training versus interpersonal effectiveness intervention, and c) to explore the correlation between longitudinal

Dimensional assessment of self- and interpersonal functioning in adolescents: implications for DSM-5's general definition of personality disorder.

Central to the proposed DSM-5 general definition of personality disorder (PD) are features of self- and interpersonal functioning. The Social Cognition and Object Relations Scale-Global Rating Method (SCORS-G) is a coding system that assesses eight dimensions of self- and relational experience that can be applied to narrative data or used by clinically experienced observers to quantify observations of patients in ongoing psychotherapy. This study aims to evaluate the relationship of SCORS-G dimensions to personality pathology in adolescents and their incremental validity for predicting multipl

Maternal care and paternal protection influence response to psychotherapy treatment for adult depression.

BACKGROUND: Adverse childhood experiences of neglect, overprotection and abuse, well-recognized risk factors for the development of adult psychopathology, were examined as predictors of response to psychotherapy treatment for adults with depression. METHODS: Outpatients in a randomized clinical trial of interpersonal psychotherapy (IPT) or cognitive-behavioral therapy (CBT) completed the parental bonding instrument (PBI) at baseline to establish levels of care and protection. Childhood abuse was asked about using clinical interviews. The PBI variables were examined in tertiles while the abuse

Psychotherapy with Survivors of Sexual Abuse and Assault.

Sexual assault and abuse can result in severe physical and emotional trauma to the victim. Deploying targeted psychotherapeutic treatment that is individualized for the survivor is important to achieving optimal patient outcomes. There are several valid and evidence-based treatments available for posttraumatic stress disorder (PTSD) and interpersonal difficulties that can result from sexual abuse and assault. In this article, the authors discuss psychodynamic psychotherapy, trauma-focused cognitive behavioral therapy (TF-CBT), and eye movement desensitization and reprocessing therapy (EMDR) fo

Phobias of attachment-related inner states in the psychotherapy of adult survivors of childhood complex trauma.

The clinical case described in this article illustrates the value of taking into account the dynamics of disorganized attachment in the assessment of attachment-related phobias (phobia of attachment and phobia of attachment loss) during the psychotherapy of chronically traumatized patients. These seemingly opposite phobias typically coexist in the same patient, appear as phobias of both inner states (affect phobias) and relational experiences, and are linked to dissociated representations of self-with-other. Theory and research on attachment disorganization provide a clinician-friendly concept

Treating complicated grief: effects on sleep quality.

Complicated grief (CG) is characterized by recurrent distressing emotions and intrusive thoughts related to the loss of a loved one and avoidance of reminders of the deceased. Poor sleep quality characterizes CG. Cognitive-behavioral therapy and interpersonal psychotherapy reduce CG symptom severity. The effect of these treatments on sleep quality is not known, however. We investigated whether complicated grief treatments (CGTs) improve sleep quality, as assessed by the Pittsburgh Sleep Quality Index. Data collected from 95 adults who participated in a randomized controlled trial were reviewed

The impact of attachment style on posttraumatic stress symptoms and treatment response in survivors of sexual assault.

Background: Sexual assault (SA) is associated with a high risk of posttraumatic stress disorder (PTSD), and delayed disclosure of SA may exacerbate psychological distress. Attachment theory provides a useful framework for understanding individual differences in trauma response. However, little is known about how attachment styles impact symptom severity and psychotherapy outcomes in late disclosers of SA. This study aimed to examine the relationship between adult attachment styles, posttraumatic stress symptoms, and psychotherapy outcomes in a clinical sample of adult survivors of SA.Objective

[Dependent patient and interpersonal dependency: psychotherapeutic strategies].

OBJECTIVES: This article is a review of psychotherapies for patients suffering from dependent personality and interpersonal dependency. METHOD: We synthesized articles making reference to this question, notably those written by Bornstein, author who refers to the dependent personality. We highlighted the psychotherapies that have been the object of an evaluation. The research on the subject is sparse: only eight studies permitting assessment of psychotherapies in this indication in 2005. Besides these psychotherapies, we detailed other approaches which are used by practitioners in these indic

Dropping out of therapy in context of an RCT: a qualitative analysis.

OBJECTIVE: This study explored the multifaceted processes underlying psychotherapy dropout by conducting a multi-method qualitative analysis of logbooks, therapist reports and post-interviews of 46 cases of dropout from a randomized controlled trial. METHOD: The data from the 46 cases were subjected to descriptive-interpretative qualitative research to identify common themes and constructs, and to an ideal-type analysis to discover case-wise variation. This triangulation of methods facilitated the development of a layered understanding of dropout. RESULTS: The descriptive-interpretative qual

Gift wrapping: An element of Hypnotically Informed Psychotherapy.

Crafting clinical interventions that are effective is of paramount importance in the therapeutic endeavor. How one conceptualizes people, intra and interpersonal factors, and the interplay between mind, body, and spirit, informs, and influences how one intervenes. In this journal paper, the concept of "gift wrapping" is employed to convey a way of conceptualizing aspects of the therapeutic encounter and the presentation of therapeutic interventions. It is rooted in Hypnotically Informed Psychotherapy (HIP) (Staffin, 2024a, in press). With HIP, the conscious mind is depotentiated, thus freeing

An overview of the treatment of eating disorders in adults and adolescents: pharmacology and psychotherapy.

PURPOSE: This article provides an overview of current treatment options for adults and adolescents suffering from eating disorders (ED). VIEWS: ED are prevalent public health problems that considerably impair physical health and disrupt psychosocial functioning. In primary care settings, anorexia nervosa, bulimia nervosa, and binge eating disorder represent the most frequently seen types of eating disorders, in both adults and adolescents. To address these maladaptive eating-related behaviors and concurrent psychiatric symptoms, various pharmacological interventions and specialized psychologi

Nonverbal synchrony predicts premature termination of psychotherapy for social anxiety disorder.

Premature termination is a problem in psychotherapy. In addition to the examination of demographic and clinical variables as predictors of dropout, research indicates the importance of dyadic variables. Nonverbal synchrony (e.g., movement synchrony) operationalizes the coordination of patient and therapist and is a promising candidate for predicting premature termination. This secondary data analysis included data on patients with social anxiety disorder (N = 267) that were treated with > 20 sessions of cognitive-behavioral therapy or psychodynamic therapy. Therapy outcome was measured by the

Treatment of paranoid personality disorder with cognitive analytic therapy: a mixed methods single case experimental design.

UNLABELLED: Paranoid personality disorder (PPD) presents as chronic and widespread interpersonal distrust, whereby the actions of others are interpreted as malevolent and malicious. This research details the assessment, formulation and treatment of a case of PPD within a 24-session contract of cognitive analytic therapy (CAT). The outcome methodology was an A/B with extended follow-up single case experimental design (SCED). The SCED was supplemented with qualitative patient interviewing via the Change Interview regarding their experience of CAT, whether change had taken place and detailing of
