Within the past decade, knowledge of the molecular basis of inflammatory bowel disease (IBD), including Crohn disease (CD) and ulcerative colitis, has advanced owing to the explosive growth of research involving the human genome. Furthermore, a shared interest between molecular biologists and clinical researchers has contributed to an emerging understanding of IBD. Nucleotide-binding oligomerization domain 2 (NOD2) belongs to an apoptotic regulatory family of genes and has been linked to CD. In addition, research into nuclear factor kappa B (NF-kappa B), the proteasome, interleukins, and tumor

It is to Wilhelm Conrad Röntgen, the first elected "radiologist" of our Academy and the first Nobel Prize winner in physics, that we owe the transparency of the hand. We celebrate today the centenary of the great scientific discovery which was to revolutionize the diagnosis, and thereby the treatment, of a large number of illnesses the discovery of X-rays. It would be unjust not to link the name of this scientist with that of his wife, Bertha, who, ignorant of the dangers of all "novel medical inventions" volunteered her own hand for his research experiments: the hand which was to bring to the

Questions have been used as a teaching tool to assess the learner's knowledge, comprehension, and critical thinking. Thoughtfully crafted questions can lead to new discoveries that explore the subject matter, whereas poorly worded questions can lead to decreased learning, confusion, intimidation, and limited thought processing. This column is the first in a series exploring the use of questions. We will explore strategies, examples, and ideas that increase learning and critical thinking within professional development and during interactions with nurses. Our patient initially presented with 6 months of left jaw pain and gingival bleeding, leading to the discovery of a radiolucent left maxillary mass on dental evaluation. A biopsy confirmed clear cell odontogenic carcinoma, and the patient was treated with definitive surgery and radiation for localised disease. Unfortunately, the patient was found to have pulmonary metastases 3 months after initial management and was subsequently treated with a combination of cytotoxic chemotherapy and immunotherapy with a partial response. To our knowledge, this is the first case demonstrating the successful u

One hundred years ago, in 1924, the first description of a patient with a disease, now known as thrombotic thrombocytopenic purpura (TTP) was published by Dr. Eli Moschcowitz. In honor of this report, this article, written by distinguished specialists in TTP, reviews the increase in scientific knowledge on this disease during the last 100 years. It covers the scientific progress from plasma therapy, the first beneficial treatment for TTP, to the elucidation of the pathophysiology, the discovery of ADAMTS13, the development of assays and targeted therapies up to the modern treatment concepts, t

Mast cells (MCs) contribute prominently to all allergic diseases, yet are still poorly understood owing to their exclusive residence in tissues. Recently, the use of RNA-sequencing, proteomics, and other technological advances have accelerated the acquisition of new knowledge. This includes an expanded definition of MC heterogeneity and developmental origins, previously unrecognized functions for MCs, discoveries of genetic causes of MC-related disorders, the introduction of new therapies for clonal MC disease, and the identification of new potential target for treatments. This issue of Advanc

We report an extremely rare case of splenic artery arising from hepatic artery proper in a patient with celiacomesenteric trunk variant. This anatomical variation was detected angiographically during hepatic mapping prior to transarterial radioembolization (TARE) for hepatocellular carcinoma in an 84-year-old man. TARE of hepatic tumors is one of the frequent procedures done by interventional radiologists. The identification of such rare vascular aberrations is of great importance not only in current interventional radiology procedures such as radioembolization but also in surgery and diagnost

The formation and accumulation of crystalline material in tissues is a hallmark of many metabolic and inflammatory conditions. The discovery that the phase transition of physiologically soluble substances to their crystalline forms can be detected by the immune system and activate innate immune pathways has revolutionized our understanding of how crystals cause inflammation. It is now appreciated that crystals are part of the pathogenesis of numerous diseases, including gout, silicosis, asbestosis, and atherosclerosis. In this review we discuss current knowledge of the complex mechanisms of cr

Although the disease today known as Huntington's was described as early as the mid-19th century, knowledgeable physicians despaired of finding successful therapies and affected families largely kept it hidden. Starting in the late 1960 s, the confluence of grass-roots advocacy by HD family members, advances in Parkinson's treatment, and the development of molecular genetics and neuroscience helped turn HD into a focus of growing biomedical research. While therapies lag behind laboratory discoveries, disease altering interventions are now moving closer to the clinic. The Journal of Huntington's

Patients with large omphaloceles are subject to anatomical displacement and morphological distorsion after surgical repair. We report 2 such complications in an infant after repair of a giant omphalocele that contained both liver and bowel. Esophageal obstruction resulted from sharp, anterior angulation of the distal esophagus that hindered placement of a feeding tube in the neonatal period. At 1 year of age, routine cardiac evaluation led to the discovery of a "mass" within the heart caused by invagination of a tongue of liver tissue into the right atrium. Knowledge of these less well-recogni

Pneumocystis pneumonia and AIDS have been linked together for many years. In the 1980s and 1990s, these diseases often resulted in admission to the critical care unit for many patients. Since the discovery of antiretroviral therapy and Pneumocystis prophylaxis, this has been a less frequent occurrence. Knowledge about caring for this patient in the critical care unit is often not available. Psychological and physiological needs common to this population are different from other populations and must be addressed. Pharmacological challenges are common and may go unrecognized until complications

In a patient treated with high doses of corticosteroids for giant cell arteritis, a renewed rise in sedimentation rate accompanied by painful limitation of mobility of the shoulder led to the discovery of an intra-deltoid fluid collection. Aspiration yielded pus, culture showing the presence of Nocardia asteroides (NA). No portal of entry nor extramuscular sites of infection were found, justifying a diagnosis of primary nocardial muscle abscess. To the best of our knowledge, this is the first reported case. The opportunity is taken to review the epidemiological and clinical characteristics of

The understanding and treatment of vaginismus and dyspareunia have been greatly advanced by discoveries in sexual physiology and by the integration of the psychological sciences into clinical medicine. Historical evaluation of these problems should attend to their chronology, the impact on the individual and her relationships, and prior attempts at solution; and one should keep in mind common theories of etiology of sexual dysfunction. Physical examination, both as an educational instrument for the patient and as a means of localizing pain, can be most informative when undertaken with a thorou

Classical immunological research has been mainly devoted to natural defense mechanisms against infections and to the development and action of vaccines. With the discovery of immune tolerance and following investigations on transplantation immunity, the concept of the immune response was generalized as the higher animal's ability to discriminate between "self" and "notself". Since then research has concentrated on cells and their products involved in these immune phenomena. Application of the vast knowledge in the field of modern immunology should aid greatly in the solution of many actual cli

Metabolic dysfunction-associated steatotic liver disease (MASLD, formerly known as non-alcoholic fatty liver disease) is a leading cause of chronic liver disease worldwide. MASLD can progress to metabolic dysfunction-associated steatohepatitis (MASH, formerly known as non-alcoholic steatohepatitis) with subsequent liver cirrhosis and hepatocellular carcinoma formation. The advent of current technologies such as single-cell and single-nuclei RNA sequencing have transformed our understanding of the liver in homeostasis and disease. The next frontier is contextualizing this single-cell informatio

During the last two decades, our knowledge on the genetic bases of Mendelian forms of dystonia has expanded significantly. This has translated into the generation of multiple cell and animal models to explore the neurobiological bases of this hyperkinetic movement disorder. A majority of these studies have focused on DYT1 dystonia, caused by dominant mutations in the gene encoding for the protein torsinA. Since its discovery, work in multiple laboratories helped identify the subcellular localization of torsinA, key structural features, functionally important physical interactions and biologica

BACKGROUND: Spine surgery relies heavily on imaging, with radiography-based devices being the major operating room imaging modality. Radiation exposure is an occupational risk historically recognized shortly after the discovery of radiation itself. Exposure of both patients and operating room staff is of increasing concern as the knowledge regarding the hazards of radiation is steadily accumulating.
METHODS: We conducted a literature review of the history of radiation exposure limits and updates on current studies showing the risks of low-dose exposures.
RESULTS: Multiple studies reporting o

Congenital heart disease (CHD) is a common type of birth defect and a leading cause of infant and childhood mortality. Although recent advancements in genetic technologies have allowed for the discovery of new genomic variation associated with CHD, the prioritization and interpretation of variants for pathogenicity remain a challenge. Further, the underlying molecular genetic mechanisms for CHD remain incompletely defined. In this article, we summarize established genetic etiologies of CHD and highlight advances in knowledge of the underlying genetic architecture of CHD, along with challenges

Recent advances in the understanding of the molecular basis of genetic lipodystrophies have promoted understanding of how adipose tissue disorders can cause the metabolic syndrome and its complications. These discoveries hold promise for elucidating pathways and mechanisms by which common disorders of obesity cause metabolic complications. Novel therapeutic approaches for patients with lipodystrophies also may have implications for treatment of the metabolic syndrome in patients with regional adiposity. This article reviews these recent advances in our knowledge of the clinical features, metab

The rheumatologic disorders associated with diabetes mellitus have been reviewed. From the evidence presented, it can be concluded that neuroarthropathy and osteolysis are definitely assoicated with diabetes. Ankylosing hyperostosis and periarthritis probably represent valid associations, and possible, but still unproven associations exist for gout, pseudogout, the carpal tunnel syndrome, osteoarthritis, Dupuytren's contracture and joint contractures. Despite the lack of a proven pathophysiologic basis these interrelationships may be clinically relevant. The discovery of one of these disorders
