Sudden cardiac arrest during marathons among young, middle-aged, and senior runners.

AIM: There is limited evidence showing a relationship with age in the incidence of sudden cardiac arrest during exercise, which can be lethal at times. METHODS: To examine the incidence by age group in sudden cardiac arrest, we prospectively observed cases of sudden cardiac arrest in full marathons certified by the Japan Association of Athletics Federations held between April 2011 and March 2019. Sudden cardiac arrest was defined as collapse during or 1 h after the race requiring chest compressions or the use of an automatic electrical defibrillation. RESULTS: 4.1 million persons ran in 516

The Measurement and Application of Critical Speed and D' in Running: A Scoping Review.

Critical speed (CS) represents the maximal metabolic steady state in running, and D' represents the finite distance capacity above CS. However, there is currently no consensus on the optimal measurement and modelling methods to obtain CS and D' estimates, how CS and D' influence running performance, or how CS and D' have been applied within a training environment. The aims of this scoping review were to determine the specific protocols and modelling methods that have been implemented to quantify CS and D' in running; the deterministic potential of CS and D' for running race performance; and th

An Unexpected Cause of Marathon Dyspnea: A Diagnostic Challenge in an Amateur Athlete.

BACKGROUND Above-average exercise capacity and a high threshold for symptoms such as dyspnea or chest pain in athletes can create false reassurance regarding the absence of clinically significant coronary artery disease (CAD). The aim of this report is to present the diagnostic reasoning and therapeutic management of an amateur endurance athlete who developed exertional dyspnea during a marathon run. CASE REPORT A 50-year-old man who was an amateur long-distance runner with a history of hypertension, hyperlipidemia reported recurrent exertional dyspnea with the onset approximately at the 20th

Tracking sleep in ultramarathon male runners before, during, and after a mountain race: Actigraphic evidence of sleep restriction.

OBJECTIVES: This study aimed to analyze athletes' sleep before, during, and after a race using actigraphy. METHODS: We evaluated 15 male athletes (45.1 ± 10.5 years), who participated in Brazil 135 Ultramarathon (135 km: n = 4; 217 km: n = 11). Wristwatch actigraph measured sleep before (7 days), during, and after the race (7 days). Sleep data were available for 13 athletes. RESULTS: Actigraphy indicated short sleep duration and suboptimal sleep parameters throughout the pre- and post-race periods, since most slept less than 8 hours, had low sleep efficiency, and high Wake After Sleep Onset.

LDH isoenzyme 5 is an index of early onset muscle soreness during prolonged running.

BACKGROUND: Muscle soreness is also induced during prolonged running such as a full marathon, and muscle soreness and increased damage markers are detected immediately after such a running. We named this muscle soreness, early onset muscle soreness (EOMS). Additionally, lactate dehydrogenase (LDH) level which has some isoenzyme is increased immediately after prolonged exercise. However, it is unclear that EOMS is related to muscle damage markers on prolonged running. This study aimed to determine at which point EOMS, and muscle damage markers are related to EOMS during prolonged running. METH

Reliability of medial versus lateral video analysis for footstrike assessment in treadmill and overground running.

Although video analysis is widely used to assess footstrike pattern (FSP) and footstrike angle (FSA), the reliability of these measures from medial and lateral views remains poorly understood, particularly across treadmill and overground running. This study evaluated the intra-rater, inter-rater and inter-side reliability of FSP classification and FSA measurement between treadmill and overground running. This study involved seven novice runners on an indoor treadmill and nine experienced runners during an outdoor marathon-distance run. Synchronized medial- and lateral-view videos were recorded

Hata-Yanagiya physical activity calculation system: a novel global positioning system-based method for accurate estimation of oxygen consumption during walking and running.

INTRODUCTION: Marathon running has become increasingly popular among amateur athletes, many of whom maintain speeds of 8-9 km/h. However, existing methods for estimating oxygen consumption (VO2) during running and walking-such as the American College of Sports Medicine (ACSM) equations and commercial activity monitors-often lack accuracy and transparency. This study introduces the Hata-Yanagiya Physical Activity Calculation (HYPAC) system, a novel approach for estimating VO2 using Global Positioning System (GPS) and map data. METHODS: The HYPAC system was developed through regression equation

Left Ventricular Hypertrabeculation (LVHT) in Athletes: A Negligible Finding?

Left ventricular hypertrabeculation (LVHT) used to be a rare phenotypic trait. With advances in diagnostic imaging techniques, LVHT is being recognised in an increasing number of people. The scientific data show the possibility of the overdiagnosis of this cardiomyopathy in a population of people who have very high levels of physical activity. We describe the case of a young athlete with no medical history, who presented with syncope during a marathon running race. Initial evaluation showed elevated troponin I; transthoracic echocardiography showed a trabeculated ventricle and subsequent cardi

The exploration of immune system function changes in marathon athletes after high-intensity training by Agent-Based Model.

BACKGROUND: Marathon running imposes substantial metabolic demand. While the acute effects of exercise are well-documented, the chronic immunometabolic alterations associated with intensified training blocks in elite populations remain characterized by complex, non-linear dynamics. Contemporary debates persist regarding whether post-exercise lymphopenia represents immunosuppression (the "open window") or a functional redistribution of immune effectors. This study explored the chronic effects of a four-week high-intensity training (HIT) block on immune markers in professional marathon athletes

Effects of vitamin D3 supplementation on cardiac, muscle, and immune responses after a marathon race: a single-blind, placebo-controlled trial.

BACKGROUND: Strenuous endurance exercise imposes substantial physiological stress on the cardiovascular system and has been associated with transient elevations in cardiac biomarkers. Vitamin D₃ has been suggested to influence oxidative stress and immune responses. In this study, we investigated the effects of vitamin D₃ supplementation on biomarkers of cardiac, muscle, and immune responses following a marathon race. METHODS: Twenty-one amateur runners were allocated to either a vitamin D₃ supplementation group (receiving vitamin D₃ for 8 weeks) or a placebo group. All participants completed

[Hyponatraenmia during a long-distance run: due to excessive fluid intake].

Hyponatraemia during a long-distance run, such as a marathon, is usually mild and asymptomatic. However, in runners who present with symptoms at a healthcare station, the diagnosis of hyponatraemia can have significant consequences. The complaints are usually aspecific, but in more severe hyponatraemia, signs of cerebral dysfunction due to incipient to severe cerebral oedema predominate. The most important aetiological factor is an excessive electrolyte-free fluid intake. The most important risk factors for the development of hyponatraemia are: a long duration of the run, female gender, recent

Thirty days of double-strain probiotic supplementation increases monocyte phagocytosis in marathon runners.

Marathon runners, subjected to intense training regimens and prolonged, exhaustive exercises, often experience a compromised immune response. Probiotic supplementation has emerged as a potential remedy to mitigate the impact of prolonged exercise on athletes. Consequently, this study sought to assess the influence of probiotic supplementation on monocyte functionality both before and after the official marathon race. Twenty-seven runners were randomly and double-blindly assigned to two groups: placebo (n 13) and probiotic (PRO) (n 14). Over 30 d, both groups received supplements - placebo sach

Athletes' medical preventive behaviors: the case of oral health and ultraendurance trail runners.

Ultra-endurance runners are particularly exposed to caries risk factors due to the continuous intake of sugars in liquid or sticky forms during long-distance exercise. The risk of caries increases due to a failure to perform oral hygiene during races. Ultra-endurance runners had good oral health status despite these particularities. Our hypothesis is that high compliance with preventive oral health recommendations (toothbrushing and preventive or regular dental visits) counterbalance the risks associated with their exposure to caries. We aimed to gain a better understanding of preventive denta

Inner Dialogues and Nutritional Anxiety in Sports Tourism: Understanding Runners' Habits in Pre-Race Food-Related Stress Abroad.

Background/Objectives: For runners competing abroad in sports events, the hours before a race are marked by heightened psychological tension, where even food choices can feel crucial to success. While pre-race nutrition is often addressed in terms of physiological needs, little is known about the inner psychological processes that accompany food decisions in unfamiliar cultural and environmental contexts. This study explores the inner dialogues, anxieties, and coping mechanisms of international runners facing the question of whether and what to eat before competition. Methods: A qualitative st
