Ultrasound-Guided Intervention for Treatment of Trigeminal Neuralgia: An Updated Review of Anatomy and Techniques.

Orofacial myofascial pain is prevalent and most often results from entrapment of branches of the trigeminal nerves. It is challenging to inject branches of the trigeminal nerve, a large portion of which are shielded by the facial bones. Bony landmarks of the cranium serve as important guides for palpation-guided injections and can be delineated using ultrasound. Ultrasound also provides real-time images of the adjacent muscles and accompanying arteries and can be used to guide the needle to the target region. Most importantly, ultrasound guidance significantly reduces the risk of collateral in

[Surgical treatment of chronic maxillary sinusitis. Report of a new surgical technic and an evaluation of results of traditional surgery].

During recent years, a new operative procedure for treatment of chronic maxillary sinusitis has been strongly recommended as being more effective and more physiological than the traditional Caldwell-Luc operation. The principle of the method is endoscopically guided endonasal resection of the anterior ethmoid cells and enlargement of the natural ostium of the maxillary sinus in the middle meatus. In contrast, the principle in the Caldwell-Luc operation is opening into the sinus in the canine fossa, providing direct approach to the lumen, and finally creation of a fenester towards the inferior

Herpes zoster of the trigeminal nerve following microvascular decompression.

A patient developed herpes zoster of the maxillary division of the trigeminal nerve after microvascular decompression. Varicella zoster virus lies dormant in the Gasserian ganglion until reactivation and can cause herpes zoster ophthalmicus. This can result in serious ocular complications including blindness. Antiviral agents are effective if commenced promptly.

Unreported variant foramen of the frontal bone: the latero-orbital foramen.

Variant foramina of the skull can lead to misdiagnosis on medical imaging and potentially, intraoperative complications if not appreciated. Here, we report an unusual foramen found superior to the frontozygomatic suture. The foramen was located on the left side at the superolateral rim of the orbit, 2.36 cm inferolateral to the supraorbital foramen. It was positioned 2.5 mm superior to the frontozygomatic suture. The foramen had a length of 3.1 mm and a width of 1.3 mm. The internal opening of the foramen was located 1.45 cm superolateral to the zygomaticotemporal foramen. We suggest that this

Changing the topographical anatomy among the maxilla, palatine bone, and greater palatine nerve: a histological study using human fetuses.

PURPOSE: The palatine bone (PAL) rides over the maxilla (MX) without an end-to-end suture in the bony palate of fetuses. However, changes in the topographical relationship among bones was unknown at and along the pterygopalatomaxillary suture, including the palatine canals. METHODS: Using sagittal, frontal, and horizontal histological sections of the head from 15 midterm fetuses to 12 near-term fetuses, we depicted the changes in the topographical anatomy of the MX, PAL, and greater palatine nerve (GPN). RESULTS: In the bony greater palatine canal of these fetuses, the medial and posterior w

A variation of the infraorbital nerve: its potential clinical consequence especially in the treatment of trigeminal neuralgia: case report.

OBJECTIVE: We report the case of a seemingly rare variation of a branch of the trigeminal nerve. This unusual finding is of practical importance to the neurosurgeon because ablation of this nerve(s) in such a case would require modification of technique. The existence of a bifurcated infraorbital nerve is also relevant for application of local anesthesia to portions of the face innervated by the maxillary nerve. CLINICAL PRESENTATION: During cadaveric dissection of the face of a male specimen, 2 branches of the infraorbital nerve were identified emanating onto the face. The 2 branches entered

The Endoscopic Prelacrimal Recess Approach to the Pterygopalatine Fossa and Infratemporal Fossa.

PURPOSE: The authors studied the anatomic importance of the endoscopic prelacrimal recess approach (PLRA) to the pterygopalatine fossa (PPF) and infratemporal fossa (ITF). METHODS: Ten adult heads (20 sides) from cadavers fixed in formalin were dissected using the PLRA. Anatomic dissections were detailed and several crucial landmarks measured. RESULTS: Identification of the infraorbital neurovascular bundle is the crucial step for the detection of other branches of the maxillary artery. The distance from the base of columella to inferior orifice of the nasolacrimal duct, sphenopalatine foram

Penetrating foreign body in the masticator space with injury to the internal maxillary artery: a surgical challenge.

INTRODUCTION Foreign bodies (FBs) in the masticator space (MS) are a unique problem because of the difficulty of accessing this deep compartment within the head and neck. In addition, MS contents include critical structures such as the internal maxillary artery (IMA) and mandibular nerve. CASE HISTORY A 39-year-old tradesman was involved in a construction accident whereby a metallic projectile from a machinery drill penetrated his left cheek. Computed tomography revealed a metallic object of dimension 1.9 ×1.2 cm within the MS, with concomitant fracture of left maxillary anterior and lateral w

A 7th nerve palsy in a child with Langerhans histiocytosis.

A 7-year-old girl developed an osteolytic soft tissue mass extending into the right orbital floor and maxillary sinus which was confirmed to be Langerhans cell histiocytosis. Partial lower motor 7th nerve palsy had developed, likely due-to a combination of local swelling from the tumour and the steroids.

Endoscopic Vascular Decompression in Trigeminal Neuralgia.

AIM: Microscope may fail to detect culprit vessel at the root entry zone or distally, especially when the suprameatal tubercle is prominent and when the compressing vessel is lying anteriorly to the trigeminal nerve without using significant brain retraction. Endoscopic techniques allow better visualization of the nerve and vascular conflict. MATERIAL AND METHODS: A retrospective study of 178 patients of endoscopic vascular decompression without the use of microscope was done. The follow-up period ranged from 12 to 108 months (average 58 months). RESULTS: The age of the patients ranged from

Regenerative facial reconstruction of terminal stage osteoradionecrosis and other advanced craniofacial diseases with adult cultured stem and progenitor cells.

BACKGROUND: Treatment options in cases of severe craniofacial disorders with bone loss and tissue damage are usually limited to vascularized and nonvascularized tissue transfers, allografts, mechanical devices and, more recently, facial transplantation. Despite the therapies available, the demand for new approaches is realized in cases where current therapies are unable to resume form and function. This study presents the feasibility of alternative treatments based on cultured bone marrow cells that yield mixed populations of mesenchymal, hematopoietic, and endothelial lineages at very early s

Safety and efficacy of percutaneous radiofrequency thermocoagulation with varying puncture trajectories for recurrent maxillary trigeminal neuralgia: a prospective study.

BACKGROUND: Recurrence of maxillary trigeminal neuralgia (TN) poses a significant clinical challenge. Percutaneous radiofrequency thermocoagulation (PRT) has been established as an effective treatment for maxillary TN. However, the safety and efficacy of different puncture pathways, particularly in the context of pain recurrence, may vary. OBJECTIVE: This study aimed to compare the safety and efficacy of PRT via the foramen ovale (FO) and pterygopalatine fossa (PF) approaches for recurrent maxillary TN. METHODS: This prospective, non-randomized controlled study included 46 patients with recu

Anatomic Study of Accessory Infraorbital Nerves and Foramina: Application for a Better Understanding of Complications of Le Fort Fractures and Osteotomy.

PURPOSE: Neurosensory disturbance of the infraorbital nerve is one of the many complications of the Le Fort osteotomy. An accessory infraorbital foramen (AIOF) can be one reason for such complications. Therefore, the purpose of the present study was to elucidate the distribution of the accessory infraorbital nerve (AION) arising from the AIOF. MATERIALS AND METHODS: We dissected 60 sides from 30 fresh-frozen cadaveric heads. RESULTS: An AIOF was found in 36.7% of the heads examined. When an AIOF was detected, the nerve was traced distally and proximally. The mean horizontal and vertical diam

Sphenopalatine ganglion block ... a new treatment for burning mouth syndrome?: a case report.

Burning mouth syndrome is a poorly understood entity for which current treatment modalities fail to provide effective relieve. Branches of the maxillary and mandibular nerves are responsible for the innervation of the affected area. These are also the nerves involved in trigeminal neuralgia, an entity where sphenopalatine block has proved to be effective. We present a case of a patient with burning mouth syndrome in whom a bilateral sphenopalatine ganglion block was successfully performed for pain treatment. It is an easy and safe technique that can be a valuable treatment option for these pat

Bioimpedance Sensing Surgical Drill - Computational Modelling and Experimental Validation.

Surgical drilling to fixate dental implants is associated with high risk of injury to the inferior alveolar nerve (IAN) and the maxillary sinus. Current common practice is to use pre-operative radiographs to plan and drill with no real-time feedback of drill tip position with respect to these critical structures. Real-time proximity sensing of the IAN and maxillary sinus by measuring the electrical impedance properties of tissues, directly from the drill tip, while drilling may reduce and eventually eliminate this risk. Sensing impedance to detect tissue boundaries needs sensor geometry optimi

[Origin of olfactory and rhinosensory evoked cortical potentials in diseases of the central nervous system].

This is the first report to be published on olfactory evoked potentials in patients with well-defined lesions of the central nervous system and the trigeminal nerve. Absence of olfactory evoked potentials is seen in post-central and parietotemporal lesions. The first peak of the so-called olfactory evoked twin potential is absent in lesions of the basal nuclei and sectioning of the trigeminal or ophthalmic nerve, whereas there is no second peak in subcortico-frontal and cortico-temporal lesions. Tumours of the corpus callosum and sectioning of the maxillary and mandibular nerves do not disturb

[Surgery of the odontogenic-diseased maxillary sinus. Technic and experiences with the bone cover method].

In the well known extranasal surgical methods described by Caldwell-Luc and Denker, the opening of the bone of the facial wall created during surgery remains open. Scar tissue contract around the infraorbital nerve, fistulae, invasion of soft cheek tissue through the bone defect, and closure of the drainage window may result. The "Bone Lid" method avoids these disadvantages. During osteoplastic surgery of the maxillary sinus routinely performed by the author in more than 700 cases since 1973 a piece of bone (the "Bone Lid") including the attached mucosa is cut out of the anterior wall of the m
