Reasons for miniplate removal following maxillofacial trauma: a 4-year study.

INTRODUCTION: There is still no consensus on the need for routine removal of titanium miniplates in the maxillofacial skeleton. The purpose of this 4-year prospective study was to evaluate the reasons for removal of titanium miniplates ("2.0mm") following maxillofacial trauma. MATERIAL AND METHODS: Records of 280 patients were evaluated concerning the number of plates inserted, the site of plating, the number of patients in whom plates were removed, the site of removal and the reasons for which removal of plates was indicated. RESULTS: In the 280 patients with facial trauma, 599 miniplates w

Computer planning and intraoperative navigation in cranio-maxillofacial surgery.

Preoperative computer design and stereolithographic modeling combined with intraoperative navigation provide a useful guide for and possibly more accurate reconstruction of a variety of complex cranio-maxillofacial deformities. Although probably not necessary for routine use, the author's early experience confirms that of other surgeons with more than a decade of experience: computer-assisted surgery is indicated for complex posttraumatic or postablative reconstruction of the orbits, cranium, maxilla, and mandible; total temporomandibular joint replacement; orthognathic surgery; and complex de

Postoperative infections in craniofacial reconstructive procedures.

The rate of, and possible risk factors for, postoperative craniofacial infection is unclear. To investigate this problem, we reviewed 349 cases of craniofacial skeletal procedures performed from 1996 to 1999 at our institution. Infection rate was determined and correlated with the use of implants, operative site, and cause of deformity. The inclusion criteria consisted of all procedures requiring autologous or prosthetic implantation in craniofacial skeletal sites, as well as all procedures involving bone or cartilage resection, osteotomies, debridement, reduction and/or fixation. Procedures t

Simultaneous functional and fixed appliance therapy for growth modification and dental alignment prior to prosthetic habilitation in hypohidrotic ectodermal dysplasia: a clinical report.

This clinical report describes the simultaneous use of functional and fixed appliances to modify the pattern of dentofacial development and align teeth in preparation for prosthodontic habilitation of a growing child with hypohidrotic ectodermal dysplasia. The treatment objective was to create a more favorable starting point for the prosthodontic phase of habilitation by improving the sagittal and vertical skeletal relationships and facial esthetics. This was accomplished through growth modification with functional appliances conducted simultaneously with eruption of maxillary and mandibular m

Nasal reconstruction for maxillonasal dysplasia.

BACKGROUND: Maxillonasal dysplasia, Binder type (Binder syndrome and nasomaxillary hypoplasia), is a spectrum of deficient nasomaxillary osteocartilaginous framework, deficient nasal soft tissues, and a short columella. The correction of these deformities is challenging, and results are often disappointing. Tissue expansion with multiple bone grafts for nasal augmentation from childhood has been advocated as a means to address the constricted soft tissues. However, bone grafts in children have been associated with unpredictable growth and resorption. Agreeing with the principle of serial nasal

An interdisciplinary approach to aesthetic single implant outcomes for young patients.

Clinicians carry a significant obligation to young patients with single missing teeth to ensure that they are prepared for a lifetime of healthy dental function and long-term aesthetic success. Adolescents and young adults in need of implant treatment embody a large and growing patient population that requires distinctly different treatment approaches than those that apply to older patient groups. The primary causes of single missing teeth in young patients are trauma and congenital absence, whereas older populations typically lose teeth due to failure of traditional dental procedures. This ar

Late effects of orbital enucleation and radiation on maxillofacial prosthetic rehabilitation: A clinical report.

The retinoblastoma is one of the most common tumors of the eye diagnosed in childhood. The treatment for patients with retinoblastoma includes surgical removal of the lesion along with adjunctive chemotherapy and radiotherapy. If the tumor is controlled, these treatments can have secondary adverse effects pertaining to the growth and development of orofacial structures in young patients. This clinical report describes the prosthetic rehabilitation of a patient who underwent enucleation followed by radiation therapy and adjunctive chemotherapy to treat the primary diagnosis of retinoblastoma. Epidemiology and management of maxillofacial fractures in an Australian trauma centre.

BACKGROUND AND AIM: Trauma is a leading cause of morbidity and mortality, with a considerable proportion of trauma patients sustaining concomitant maxillofacial (MF) injuries. The purpose of this study was to review and analyse the epidemiology, management and complications of patients with MF fractures managed by the Faciomaxillary Surgery Unit at the Alfred Trauma Hospital in Melbourne. The secondary objective of the study was to determine the risk factors for developing postoperative complications. METHODS: A retrospective records review was performed for 980 patients who were treated for

A long-term retrospective outcome assessment of facial growth, secondary surgical need, and maxillary lateral incisor status in a surgical-orthodontic protocol for complete clefts.

In 1965, the cleft palate team at Children's Memorial Hospital embarked on a new surgical-orthodontic protocol in the habilitation of newborn complete cleft lip and palate cases. It brought the orthodontic effort into focus at birth and in planned sequence to correspond with the surgical procedures of lip closure, maxillary alveolar stabilization by means of an autogenous graft of the authors' design, and complete palate closure, all within the first year of life. The purpose of this investigation is threefold: first, to review the authors' previous publications and assess growth, secondary su

Functional rehabilitation using orthognathic surgery, bone transplants and implants after irradiation of malignancy in early childhood.

Functional rehabilitation has been carried out in four young patients by use of orthognathic surgery, bone grafting and implants. All four patients had been successfully treated for malignant tumours in the midface during early childhood by use of full dose irradiation. Retarded growth of the maxilla together with missing root formation of permanent teeth prevented the alveolar process to develop. Bimaxillary osteotomies with inlay bone graft have been made in two cases and maxillary osteotomy le Fort I with interpositional and inlay grafting in two individuals. Implants surgery has been made

In vivo comparison of hard tissue regeneration with human mesenchymal stem cells processed with either the FICOLL method or the BMAC method.

OBJECTIVE: To compare new bone formation in maxillary sinus augmentation procedures using biomaterial associated with mesenchymal stem cells (MSCs) separated by two different isolation methods. BACKGROUND: In regenerative medicine open cell concentration systems are only allowed for clinical application under good manufacturing practice conditions. METHODS: Mononuclear cells, including MSCs, were concentrated with either the synthetic polysaccharide (FICOLL) method (classic open system--control group, n = 6 sinus) or the bone marrow aspirate concentrate (BMAC) method (closed system--test gro

Implant Treatment after Sagittal Splitting Ramus Osteotomy and Alveolar Ridge Augmentation in Patient with Mandibular Prognathism and Multiple Missing Maxillary Teeth.

An abnormal maxillomandibular ridge relationship frequently hinders oral implant treatment in patients with jaw deformities. Here, we describe a patient who was experiencing difficulty using dentures due to multiple maxillary tooth loss and mandibular prognathism. Treatment comprising sagittal splitting ramus osteotomy and alveolar ridge augmentation using bone grafts harvested from the mandibular ramus followed by implant treatment yielded good outcomes. The patient was a 47-year-old woman presenting with an unstable upper partial denture. Although prior prosthetic treatment for mandibular pr

Pedicled myofascial temporalis flap for closure of large maxillary defects after medication-related osteonecrosis of the jaws. A case series.

BACKGROUND: Medication-related osteonecrosis of the jaws (MRONJ) located in the maxilla may lead to challenging oro-antral and oro-nasal defects too extensive to be predictably closed with local soft tissue flaps. The use of a pedicled myofascial temporalis flap (PMTF) is, however, well-established for closing large maxillary defects following ablative craniomaxillofacial surgery. OBJECTIVES: The purpose of the present study was to evaluate the use of PMTF for maxillary defect closure in patients with stage 3 MRONJ. METHODS: A retrospective cohort study was conducted based on data from the C

Clinical analysis of implant losses in oral tumor and defect patients.

In the period between 1990 and 1996, 279 endosteal dental Bone-Lock implants were placed in 79 patients. Of them 63 have been treated with ablative tumor and reconstructive surgery in the oral cavity, the rest presented with maxillo-mandibular defects of different origin. The circumstances of implant loss were noted down for descriptive analysis concerning age, sex, topography, implant dimensions, loading, time in place and type of superstructure. Failure analysis was done concerning the implants and the patients. Five causes for implant loss could be detected: lacking primary osseointegration
