Metformin and Inflammation: Its Potential Beyond Glucose-lowering Effect.

Metformin is an oral hypoglycemic agent which is most widely used as first-line therapy for type 2 diabetes. Metformin improves hyperglycemia by suppressing hepatic glucose production and increasing glucose uptake in muscle. Metformin also has been shown to reduce cardiovascular events in randomized controlled trials; however, the underlying mechanism remains to be established. Recent preclinical and clinical studies have suggested that metformin not only improves chronic inflammation through the improvement of metabolic parameters such as hyperglycemia, insulin resistance and atherogenic dysl

Pilot study of oral metformin in cancer-bearing cats.

A prospective dose escalation pilot study was performed in cancer-bearing cats to assess toxicity and surrogate biomarkers of pharmacologic activity of oral metformin hydrochloride. Nine cats with measurable spontaneous cancer were treated with oral metformin for 14 days. Monitoring included complete blood count (CBC), serum biochemistry, lactate, pH, insulin-like growth factor-1, and vascular endothelial growth factor serially until study completion. At the maximum tolerated dose of 10 mg kg-1 q12 h side effects were primarily mild to moderate gastrointestinal upset (anorexia, vomiting, and/o

Metformin and other antidiabetic agents in renal failure patients.

This review mainly focuses on metformin, and considers oral antidiabetic therapy in kidney transplant patients and the potential benefits and risks of antidiabetic agents other than metformin in patients with chronic kidney disease (CKD). In view of the debate concerning lactic acidosis associated with metformin, this review tries to solve a paradox: metformin should be prescribed more widely because of its beneficial effects, but also less widely because of the increasing prevalence of contraindications to metformin, such as reduced renal function. Lactic acidosis appears either as part of a

Metformin and other insulin sensitizers in polycystic ovary syndrome.

Polycystic ovary syndrome (PCOS) is a complex disorder affecting a substantial group of women from adolescent to menopausal age groups. A substantial subgroup of these women exhibits increased insulin resistance. Insulin resistance is difficult to establish in clinical practice and many surrogate tests are available, although their value in the clinical setting is uncertain. The QUICKI method is an inexpensive and easy test to administer and is probably the best test to be used in PCOS patients, preferably in a clinical trial context. There is considerable evidence that insulin-sensitizing age

Beyond aspirin-cancer prevention with statins, metformin and bisphosphonates.

Cancer risk reduction using pharmacological means is an attractive modern preventive approach that supplements the classical behavioural prevention recommendations. Medications that are commonly used by large populations to treat a variety of common, non-cancer-related, medical situations are an attractive candidate pool. This Review discusses three pharmacological agents with the most evidence for their potential as cancer chemopreventive agents: anti-hypercholesterolaemia medications (statins), an antidiabetic agent (metformin) and antiosteoporosis drugs (bisphosphonates). Data are accumulat

Metformin exposure affects human and mouse fetal testicular cells.

BACKGROUND: Metformin is a drug used in the treatment of diabetes and of some disorders related to insulin resistance, such as polycystic ovary syndrome. Gestational diabetes can cause complications for both mother and child, and some studies have shown a beneficial effect of metformin during pregnancy without an increase in perinatal complications. However, the effects on the gonads have not been properly studied. Here we investigated the effect of metformin administered during pregnancy on the development and function of the fetal testis. METHODS: A dual approach in vitro and in vivo using

Metformin use and diabetic pregnancy-has its time come?

The prevalence of Type 2 diabetes in women of childbearing age continues to grow as the incidence of Type 2 diabetes increases. Recent evidence shows that treatment of gestational diabetes ensures the best possible outcome for pregnancy complicated by gestational diabetes. Metformin is a logical treatment in these circumstances but there has always been concern about its safety for the fetus, particularly as it crosses the placenta and it may increase the risk of teratogenesis. Although evidence is accumulating that metformin is useful and has a role in polycystic ovary syndrome, a condition o

Outcome of clinical pregnancies after ovulation induction using metformin.

The objective of this study was to review the first 50 clinical pregnancies of women with polycystic ovarian syndrome (PCOS) who had ovulation induced either with metformin alone, or in combination with clomifene. The study was confined to women with PCOS attending our infertility service. A register of clinical pregnancies was maintained of women who conceived after metformin therapy. The metformin was continued throughout the first trimester. The outcome of pregnancy was determined by individual chart review. Of the 50 women, 21 conceived with a combination of clomifene and metformin, and 29

Metformin-associated lactic acidosis: case reports and literature review.

BACKGROUND: Lactic acidosis is a widely recognized, though rare, side effect of metformin. This paper describes five patients admitted to Chang Gung Memorial Hospital from 1 September 1998 to 31 May 2001 suffering severe lactic acidosis caused by metformin, and reviews the literature. PATIENTS: Five cases diagnosed as having meftormin-associated lactic acidosis (MALA) were discovered during the study period. Three had normal renal function before the onset of MALA and two had attempted suicide bytaking large amounts of metformin. One patient with end-stage renal disease developed MALA despite

Acute sympathoinhibitory actions of metformin in spontaneously hypertensive rats.

Chronic treatment with the antihyperglycemic agent metformin prevents hypertension in spontaneously hypertensive rats. This effect has been ascribed to normalization of plasma insulin levels. However, whether metformin affects arterial pressure via changes in sympathetic nerve activity is unknown. Therefore, the objective of this study was to examine whether acute administration of metformin produces changes in mean arterial pressure, heart rate, or efferent renal sympathetic nerve activity in spontaneously hypertensive rats. Rats were anesthetized with alphaxalone-alphadolone (Saffan), paraly

Metformin restores insulin secretion altered by chronic exposure to free fatty acids or high glucose: a direct metformin effect on pancreatic beta-cells.

Because metformin affects glucose and free fatty acid (FFA) metabolism in peripheral insulin target tissues, we investigated the effect of this drug in restoring a normal secretory pattern in rat pancreatic islets whose function has been impaired by chronic exposure to elevated FFA or glucose concentrations. We cultured rat pancreatic islets with or without FFA (2 mmol/l oleate/palmitate 2:1) or high glucose (16.7 mmol/l) concentrations in the presence or absence of metformin (0.25-12.5 microg/ml) and then measured insulin release, glucose utilization, glucose, and FFA oxidation. When compared

The coordinated actions of metformin in the intestine and liver improves hyperglycemia.

Metformin is the first-line oral anti-diabetic agent. Metformin concentrations in the intestine can reach up to 1.3 mM, while those in the portal vein are approximately 0.075 mM. It is unclear whether this metformin concentration difference contributes to metformin's antidiabetic effects. Here, we showed that high metformin concentrations upregulate G6PC expression through AMP-activated protein kinase (AMPK) activation to prevent glucose release in intestinal epithelial cells (IECs). The inhibition of mitochondrial activity by high metformin concentrations leads to drastically increased glucos

Effects of metformin on head and neck cancer: an umbrella review.

BACKGROUND: Metformin is a commonly used antihyperglycemic drug to treat type 2 diabetes mellitus worldwide. However, its effectiveness was recently documented beyond diabetic treatment, such as in the case of cancer. Recent epidemiological evidence suggests that the use of metformin has shown benefits in breast and colorectal cancer; however, its effectiveness was inconclusive in the case of head and neck cancer. This umbrella review aimed to evaluate the effectiveness of metformin among head and neck cancer patients. METHODS: Three electronic databases and additional sources (Google Scholar

Metformin: A Promising Candidate for Slowing the Progression of Cardiovascular Aging.

Cardiovascular aging, characterized by endothelial dysfunction, arterial stiffening, and myocardial remodeling, is a major contributor to the development of age-related cardiovascular disease. Metformin, a first-line therapy for type 2 diabetes, has attracted interest as a promising geroprotective agent due to its ability to target several fundamental aging pathways. Mechanism: Beyond glucose-lowering effects, metformin may confer cardiovascular benefits through activation of AMP-activated protein kinase and inhibition of mitochondrial complex I. Preclinical work suggests these pathways enhanc

Metformin and Placental Ferroptosis in Gestational Diabetes: A Mechanistic Study.

Gestational diabetes mellitus (GDM) is a major contributor to adverse pregnancy outcomes. Although the involvement of ferroptosis in GDM pathogenesis has been recognized, the precise mechanisms remain incompletely understood. This study is aimed at investigating the potential regulatory effects of metformin (Met) on ferroptosis in GDM. Patients with GDM were stratified into four groups: diet-controlled (Group 1), nonmedicated (Group 2), Met-treated (Group Met), insulin-treated (Group Ins), alongside healthy controls (Group N). Placental trophoblasts and HTR-8/SVneo cells were subjected to anal

Exploring the impact of metformin on testicular aging in Syrian hamsters.

Biochemical and molecular mechanisms associated with testicular aging are still poorly understood. Here, using the Syrian hamster as a natural model of aging, we observed a disturbed spermatogenesis with reduction of the testicular weight and the gonadosomatic index, altered histology including tubular wall fibrosis, increased collagen deposition, and diminished steroidogenesis in testes of aged animals. These changes took place in parallel with an increase in the levels of inflammatory and oxidative stress markers and a reduction in the cell proliferative, survival, DNA repair, and autophagic
