Recommendations From the Women's Preventive Services Initiative on Breast Cancer Screening for Women at Average Risk and Patient Navigation Services for Breast and Cervical Cancer Screening.

The Women's Preventive Services Initiative (WPSI) expanded its previous breast cancer screening recommendation-initiate annual or biennial mammography screening for women at average risk of breast cancer between the ages of 40 and 50 years-by including additional imaging and pathology evaluation as part of the screening process if needed. Consistent with the previous recommendation, screening should continue through at least age 74 years, and age alone should not be the basis for discontinuing screening. To increase utilization of screening recommendations, the WPSI also issued a new recommend

"The uncharted waters of completely not knowing what's next": rare cancer patients' experiences regarding navigating uncertainty.

PURPOSE: A rare cancer diagnosis can cause a great deal of uncertainty. However, rare cancer survivors' experience with illness uncertainty is underreported in the literature. This study aimed to explore how people diagnosed with a rare cancer experience and manage uncertainty throughout their cancer experience. METHODS: A secondary analysis of interview data was undertaken. Data were obtained from semi-structured interviews conducted with 20 rare cancer survivors between November 2023 and February 2024. Interviews were recorded and transcribed verbatim. Data were analysed in terms of how par

A nurse navigator program for the management of hepatitis C virus in patients with severe mental disorder. A feasibility trial.

People with severe mental disorders (SMD) face higher hepatitis C virus (HCV) infection risks and lower treatment adherence compared to the general population. Traditional care pathways often fail due to fragmented coordination and patient-related barriers. This study evaluates the feasibility and acceptability of an individualized nurse navigation program (NNP) designed to facilitate adherence to medical appointments and pharmacological treatment in this population. A feasibility randomized open parallel trial was conducted. The intervention simplified the clinical pathway through intensive n

Navigating Diagnostic Uncertainty: A Case Report on Anesthetic Management of Mechanical Thrombectomy in a Patient With Occult Fibromuscular Dysplasia.

Fibromuscular dysplasia (FMD) is a rare, nonatherosclerotic vascular disease that predisposes patients to arterial stenosis, dissection, and stroke, often with delayed diagnosis due to nonspecific symptoms. We present a 30-year-old female with a history of migraines and recent whiplash injury who developed acute ischemic stroke from right internal carotid artery occlusion. Following neurological deterioration, she underwent emergent mechanical thrombectomy under general anesthesia. Airway management incorporated manual in-line stabilization to minimize cervical vessel stress, and hemodynamics

Community navigation intervention for social needs in patients with abnormal mammography: a randomized controlled trial study protocol.

BACKGROUND: Effective follow-up care plays a critical role in enhancing breast cancer outcomes, as diagnosing breast cancer in earlier stages can improve mortality and ease treatment burden. Individuals with unmet social needs face greater barriers in completing their follow-up care (e.g., additional imaging, biopsies, and surgery). They are two times less likely to complete follow-up appointments. When patients do not complete follow-up appointments, they are at higher risk of detecting breast cancer at a later stage. We aim to examine how a social needs intervention via community navigation

DRESS syndrome in a patient with rheumatoid arthritis: Navigating the intersection of drug-induced hypersensitivity and immune-mediated inflammation.

Drug reaction with eosinophilia and systemic symptoms (DRESS) syndrome is a highly severe drug-induced hypersensitivity reaction with significant morbidity. Its diagnosis can be particularly challenging in patients with autoimmune diseases receiving multiple medications. We present the case of a woman in her late 30s with recently diagnosed rheumatoid arthritis and hyperthyroidism who developed high-grade fever, diffuse rash and facial oedema following treatment with allopurinol, leflunomide and carbimazole. Her symptoms were attributed to DRESS syndrome, supported by findings of eosinophilia,

Noninvasive mapping of visuospatial attention by navigated repetitive transcranial magnetic stimulation in patients with parietal lobe tumors.

OBJECTIVE: Visuospatial neglect corresponds to a burdening cognitive deficit with reduced space attention and disturbed stimuli detection of the contralateral side. Unilateral strokes, tumor lesions, or intracerebral hemorrhage may cause it. Identifying specific areas responsible for the onset of visuospatial neglect has proven difficult. The authors hereby aimed to detect neglect-positive areas in patients with parietal gliomas undergoing tumor resection through navigated repetitive transcranial magnetic stimulation (nrTMS). METHODS: The authors performed a monocentric prospective study that

Efficacy of Tezepelumab in Patients with Severe, Uncontrolled Asthma Across Multiple Clinically Relevant Subgroups in the NAVIGATOR Study.

INTRODUCTION: Many patients with severe asthma continue to experience symptoms and exacerbations despite treatment with standard-of-care therapy. In the phase 3 NAVIGATOR study, tezepelumab significantly reduced exacerbations over 52 weeks compared with placebo in patients with severe, uncontrolled asthma. This analysis assessed the efficacy of tezepelumab in reducing asthma exacerbations in various clinically relevant subgroups of patients in NAVIGATOR. METHODS: NAVIGATOR was a phase 3, multicentre, randomized, double-blind, placebo-controlled study. Participants (12-80 years old) with sever

Navigating Countertransference in Inpatient Settings: Optimizing Interventions for Patients with Borderline Personality Disorder and Repeated Acute Hospitalizations.

Over the years, the psychiatric inpatient treatment paradigm has shifted to more brief stays focused on acute stabilization and psychopharmacologic-focused interventions, rather than individual psychotherapeutic engagement. Unfortunately, this has allowed patients with complex interpersonal dynamics, particularly borderline personality disorder, to slip through the cracks of effective treatment. This can contribute to repeated inpatient admissions, where both patients and clinicians feel trapped in a maladaptive, unhelpful cycle. In this article, we examine the evolution of inpatient treatment

Navigating the Diagnostic Challenge of Aspergillus Spinal Epidural Abscess in an Immunocompetent Patient: A Case Report and Literature Review.

Aspergillus spinal epidural abscess (ASEA) is a rare, life-threatening condition that can cause spinal cord compression with neurologic deficits. The diagnosis of ASEA can be challenging due to the atypical clinical presentation and low prevalence. We describe the successful management of a rare, immunocompetent, 85-year-old male with ASEA at the T12-L1 and L1-L2 levels and present a review of the literature. Based on most case reports and our knowledge, this is a rare presentation of ASEA in a patient without systemic symptoms, leukocytosis, or a history of immunosuppressive status due to chr

Outcomes of Total Knee Arthroplasty in Patients with Prior Hardware: A Case-Control Study Using Handheld Navigation.

Presence of hardware in juxta-articular location poses challenge during total knee arthroplasty (TKA). When present in distal femur, it precludes the use of an intramedullary femoral jig during TKA often necessitating removal of hardware leading to prolonged surgery, higher risk of complications, and inferior results. We conducted a case-control study to assess the outcome of TKA among patients with post-traumatic arthritis using a handheld navigation system to perform bone cuts allowing retention of hardware in situ. In 15 patients with post-traumatic arthritis and hardware around the knee (G

Evaluation of 3D fat-navigator based retrospective motion correction in the clinical setting of patients with brain tumors.

PURPOSE: A 3D fat-navigator (3D FatNavs)-based retrospective motion correction is an elegant approach to correct for motion as it requires no additional hardware and can be acquired during existing 'dead-time' within common 3D protocols. The purpose of this study was to clinically evaluate 3D FatNavs in the work-up of brain tumors. METHODS: An MRI-based fat-excitation motion navigator incorporated into a standard MPRAGE sequence was acquired in 40 consecutive patients with (or with suspected) brain tumors, pre and post-Gadolinium injection. Each case was categorized into key anatomical landma
