Micro-magnetic resonance imaging of the inner ear in the guinea pig.

We applied a magnetic resonance microscopy at 7.05 T with a gradient coil unit to image the fine structure of the guinea pig cochlea. First, a three-dimensional MR image of the surface of the cochlea was obtained to select the location of cross-sectional images. Then, cross-sectional images of the basal turn, the second turn and the apical turn of the cochlea were obtained. Based upon the different protein concentrations of the endolymph vs the perilymph, the scala vestibuli, scala tympani and the cochlear duct could be clearly distinguished. This allowed a determination of the location of bot

Lidocaine in the middle ear.

Anesthesia of the ear canal is produced by injection of lidocaine hydrochloride into the skin of the lateral external ear canal. Ear canal, tympanic membrane, and middle ear surgeries are performed with this injection. The fluid found in the middle ear during tympanotomy was collected and analyzed. The percentage of lidocaine in the fluid was calculated by an enzyme immunoassay technique. Fifteen surgical cases were undertaken in which perilymphatic fluid in the middle ear would not be suspected, such as tympanotomy for otosclerosis. Lidocaine was found in all middle ears in which there was su

The role of acidic fibroblast growth factor in recovery of acoustic trauma.

We administered acidic fibroblast growth factor (aFGF) to the perilymph of the guinea pig cochlea after exposure to intense sound to investigate its effect on the process of recovery after acoustic trauma. We assessed auditory brain stem response (ABR) thresholds to evaluate cochlear function and observed the sensory epithelium using confocal laser-scanning microscopy. After noise exposure (120 dB SPL, 5 h), the ABR threshold showed an increase of approximately 50 dB SPL that recovered after 14 days. Cochlear function in aFGF treated ears recovered more quickly than that in control ears. These

Perilymphatic and endolymphatic pressures during endolymphatic hydrops.

We analysed the fluid pressure in the perilymphatic and endolymphatic spaces of the cochlea in eight guinea pigs after endolymphatic hydrops (EH) was surgically created. The control experiments were done in the opposite ears. EH was histologically confirmed after the conclusion of the pressure measurements. No statistically significant pressure difference between the scala media and the scala tympani was found, either in ears with EH or the control ears. However, the endocochlear potential, which we evaluated for position verification of the measurement pipette in the cochlea, was statisticall

[Changes of glucose uptake in vestibular and auditory nuclei induced by high perilymphatic potassium ion concentration--autoradiographic study].

Characteristic nystagmus could be provoked by introducing potassium ion into the unilateral perilymphatic space of healthy guinea pigs. At first, irritative nystagmus appeared for some time to the K+ introduced ear side, and then, it was followed by paralytic nystagmus which directed to the non introduced ear side. During this experiment, the excitabilities of the vestibular nuclei and the central auditory nuclei were investigated by [14C] deoxyglucose method. In the period of "irritative nystagmus", the increment of glucose uptake in the ipsilateral vestibular nucleus, especially in the super

Experimental round window membrane rupture.

An increase of perilymphatic fluid pressure was found to be an important factor in the aetiology of round window membrane rupture. The critical pressure causing membrane rupture, as determined in anaesthetized cats, was in the range of 10-30 mm Hg (mean, 23.4 +/- 17.1). This value was compared to the pressure increases caused by thoracic and abdominal compression, cervical strangulation, forced Trendelenburg position, coughing, and sneezing.

[Late sequelae following laterobasal fractures, therapeutic and forensic viewpoints].

Report on two cases of temporal bone fracture with unilateral profound deafness and vestibular dysfunction. The other ear was apparently not involved. After 4, respectively 18 years, a sudden hearing loss occurred in the second ear, fluctuating and associated with acute vestibular symptoms. On operation we found in the first case a perilymph fistula in the round window, in the second case a gaping fracture in the promontory. A retrospective study of 527 cases of "sudden hearing loss" revealed 11 more cases with similar histories that must be regarded as sequelae of the head trauma. The latency

[Pathogenesis and clinical aspects of Ménière disease. A study of 739 patients].

The symptomatology of Ménièr's disease is delineated by means of 739 cases observed in a period of 25 years. The dilation of the endolymphatic space accompanied by a temporary mixture of endolymph and perilymph substantiates the idea of a nosologic entity as well as the interpretation of the symptoms: vertigo, fluctuating hearing loss, tinnitus, and fullness of the ear. The medical treatment by infusions of Rheomacrodex, vasodilatory substances, antivertiginosa, and psychosedatives is unsatisfying just as surgical treatment by saccotomy, sacculotomy, labyrinthectomy, and other operations at th

Therapeutics for hearing preservation and improvement of patient outcomes in cochlear implantation-Progress and possibilities.

The emergence of therapeutics targeted at hearing loss holds great promise in the development of novel treatments for this heterogenous condition. Whilst such therapeutics are largely designed to be efficacious in and of themselves, the possibility of combination with devices, namely cochlear implants, could result in much more effective treatment options. Here, we review the otoprotective molecules currently in clinical development, as well as generic steroids, discussing mechanisms of action and mode of delivery to the perilymph of the cochlea. Presenting both preclinical and clinical data,

[Effects of mannitol on the fluids of the internal ear. Use in the treatment of deafness].

The action of mannitol upon the inner ear fluids was studied experimentally by means of kinetics of mannitol entry into cochlear perilymph and endolymph in rats. A daily two hours infusion of 10% mannitol induces an osmotic water flow from cochlear fluids toward plasma. Following this therapeutic protocol, progressive, non-tumoral sensorineural hearing loss is enhanced in 32% of the cases. The rate of hearing improvement is larger than 50% in case of Ménière's disease. For sudden hearing loss, 75% of the cases are improved by mannitol, and 92% of the cases when the delay between the hearing lo

Sudden hearing loss with delayed onset following head trauma.

Eight cases are reported with an identical history: head trauma with immediate profound deafness and vestibular dysfunction in one ear; then after a long latency, sudden hearing loss in the second ear, recurring and fluctuating with vestibular symptoms. Two cases were surgically explored. In one case 15 years after the trauma a perilymph fistula of the round window was found. In the other case a broad fracture of the basal coil of the cochlea was demonstrated, which had been asymptomatic for 18 years. In a retrospective study, 6 more cases with an identical course could be found. Latency betwe

Ear Cubes for local controlled drug delivery to the inner ear.

A new type of advanced drug delivery systems is proposed: Miniaturized implants, which can be placed into tiny holes drilled into (or close to) the oval window. They consist of two parts: 1) A cylinder, which is inserted into the hole crossing the oval window. The cylinder (being longer than the depth of the hole) is partly located within the inner ear and surrounded by perilymph. This provides direct access to the target site, and at the same time assures implant fixation. 2) A cuboid, which is located in the middle ear, serving as a drug reservoir. One side of the cuboid is in direct contact

Hydrogen Inhalation Protects against Ototoxicity Induced by Intravenous Cisplatin in the Guinea Pig.

Introduction: Permanent hearing loss and tinnitus as side-effects from treatment with the anticancer drug cisplatin is a clinical problem. Ototoxicity may be reduced by co-administration of an otoprotective agent, but the results in humans have so far been modest. Aim: The present preclinical in vivo study aimed to explore the protective efficacy of hydrogen (H2) inhalation on ototoxicity induced by intravenous cisplatin. Materials and Methods: Albino guinea pigs were divided into four groups. The Cispt (n = 11) and Cispt+H2 (n = 11) groups were given intravenous cisplatin (8 mg/kg b.w., injec

[Direct acoustic cochlear stimulation for therapy of severe to profound mixed hearing loss: Codacs™ Direct Acoustic Cochlear Implant System].

OBJECTIVE: Implantable hearing aids have become a valid option for the therapy of various forms of hearing loss. The Codacs™ Direct Acoustic Cochlear Implant System is the first vibratory implant available for patients with severe to profound mixed hearing loss (MHL). By directly coupling sound energy into the perilymph, a very high maximum power output (MPO) is achieved over a broad frequency range. Via a conventional stapedotomy, the vibratory energy of the electromagnetic actuator is transferred directly to the perilymph through the oval window. PATIENTS AND METHODS: This article describes

Totally implantable active middle ear implants: ten years' experience at the University of Tübingen.

Active middle ear implants do not produce acoustic sounds but, rather, micromechanical vibrations. The stimulating signal does not leave the transducer as sound, but as a mechanical vibration, directly coupled to the auditory system and bypassing the normal route via air. In this paper, we review our experience with the TICA and the Carina middle ear implants. Both are totally implantable and are coupled to the ossicular chain or to perilymph. The design requirements for electronic hearing implants for patients with conductive hearing loss differ from those for sensorineural hearing loss. Cond
