Activation of α1A -adrenoceptors desensitizes the rat aorta response to phenylephrine through a neuronal NOS pathway, a mechanism lost with ageing.

BACKGROUND AND PURPOSE: A NO-mediated desensitization of vasoconstrictor responses evoked by stimulation of α1 -adrenoceptors has been reported in different vessels. We investigated the involvement of each α1 -adrenoceptor subtype and constitutive NOS isoforms and the influence of ageing and hypertension on this process. EXPERIMENTAL APPROACH: Wistar and spontaneously hypertensive rats (SHR), 16, 32, 52 and 72 weeks-old, were used to evaluate the desensitization process. Expression of α1 -adrenoceptor subtypes, endothelial NOS (eNOS) and neuronal NOS (nNOS) were determined in rat aorta and le

Determination of the 90% effective dose (ED90) of phenylephrine for hypotension during elective cesarean delivery using a continual reassessment method.

OBJECTIVE: The purpose of this study was to determine, by continual reassessment, the 90% effective dose (ED90) of phenylephrine for hypotension after combined spinal-epidural anesthesia. STUDY DESIGN: Term pregnant women scheduled for elective cesarean delivery received combined spinal epidural anesthesia. Subjects received phenylephrine at one of 6 incremental doses ranging from 60 to 160μg (n=3 for each dose). While the first cohort received a conservative, predetermined dose of 60μg, subsequent cohorts received phenylephrine doses determined using Bayesian-based software. One of the prede

Differential roles of MAPKs and MSK1 signalling pathways in the regulation of c-Jun during phenylephrine-induced cardiac myocyte hypertrophy.

Gq-protein-coupled receptor (GqPCR) signalling is associated with the induction of cardiac myocyte hypertrophy, which is characterized by an increase in expression of immediate early genes via activation of pre-existing transcription factors. Here, we explore the role of MSK1 and MAPK signalling pathways in the regulation of the immediate early gene c-jun. The results provide further support for the role of MSK1 in cardiac myocyte hypertrophy and indicate that PE activates distinct signalling mechanisms which culminate with a complex activation of c-jun. ERK1/2 and JNKs are the principal kinas

Experimental hyperlipidemia prevents the protective effect of ischemic preconditioning on the contractility and responsiveness to phenylephrine of rat-isolated stunned papillary muscle.

This study was designed to establish a hyperlipidemic diet (significant increase in the cholesterol and triglycerides blood levels, but without atherogenic changes in heart muscle and coronary vessels) and to investigate the influence of experimental hyperlipidemia on the effects of ischemic preconditioning (PC) of rat-isolated papillary muscle on the time course of contractility during simulated ischemia and reperfusion and responsiveness to phenylephrine under such a condition. The animals were divided in four experimental groups: standard diet-fed control group (SD), SD underwent ischemic p

The effect of cold storage on the inhibitory action of isoprenaline, phenylephrine and nicotine on the mechanical and membranal activities of guinea-pig taenia caecum.

1. The effects of prolonged cold storage on the mechanical and membranal responses to stimulation of alpha- and beta-adrenoceptors by phenylephrine and isoprenaline, respectively, were studied on the guinea-pig taenia caecum.2. Cold storage invariably caused a decrease in the resting membrane potential, and this effect was enhanced as the duration of treatment was prolonged.3. After cold storage (18 days) the tissue potassium ion content (89.7 +/- 1.7 mmol/kg wet wt.) was decreased to 30.5 +/- 1.9 mmol/kg wet wt. whereas that for sodium (69.2 +/- 1.4 mmol/kg wet wt.) increased to 134.0 +/- 2.3

Differential effects of phenylephrine on brain versus skeletal muscle microvascular perfusion: an in vivo laboratory study using translational rodent models.

PURPOSE: Hypotension, or low mean arterial blood pressure (MAP), has been associated with adverse outcomes in perioperative patients. A primary goal of treating hypotension during surgery is to preserve vital organ perfusion by maintaining intravascular volume and the use of vasoactive medications, including phenylephrine (PE), to support MAP. Phenylephrine, a vasoconstrictor with α1-adrenergic agonist activity, acts on resistance arterioles and veins to increase vascular resistance and reduce venous capacitance, thereby increasing MAP. It predominantly acts on skeletal muscle resistance arter

Association between continuous phenylephrine infusion and hemodynamic stability in patients undergoing gastrointestinal endoscopy under remimazolam sedation: a retrospective cohort study.

BACKGROUND: Hypotension is an important complication in the procedural sedation for the gastrointestinal endoscopy despite the use of remimazolam, which has superior hemodynamic stability compared to propofol. Prophylactic vasopressor therapy may preserve cerebral perfusion and enhance the outcome of recovery. This study evaluated the association between continuous infusion of phenylephrine and hemodynamic stability and recovery of cognition in patients undergoing endoscopy under remimazolam sedation. METHODS: This retrospective cohort study analyzed clinical records of 108 adult patients (Am

A Comparison of Phenylephrine and Norepinephrine Infusions in Preventing Hypotension After Subarachnoid Block in Caesarean Section: A Randomized, Double-Blind Study.

Background Prophylactic vasopressor infusion is commonly recommended during cesarean section under spinal anesthesia to prevent hypotension. Norepinephrine is gaining recognition as a viable alternative to phenylephrine in obstetric anesthesia, primarily due to its lower propensity to cause reflex bradycardia. This study compares the efficacy and safety of prophylactic intravenous infusions of norepinephrine and phenylephrine, administered at varying infusion rates, on maternal hemodynamics and neonatal outcomes during cesarean delivery. Methods In this prospective, randomized, double-blinded

Blockade of endothelial Mas receptor restores the vasomotor response to phenylephrine in human resistance arterioles pretreated with captopril and exposed to propofol.

BACKGROUND: Hypotension that is resistant to phenylephrine is a complication that occurs in anesthetized patients treated with angiotensin converting enzyme (ACE) inhibitors. We tested the hypothesis that Ang 1-7 and the endothelial Mas receptor contribute to vasodilation produced by propofol in the presence of captopril. METHODS: The internal diameters of human adipose resistance arterioles were measured before and after administration of phenylephrine (10-9 to 10-5 M) in the presence and absence of propofol (10-6 M; added 10 min before the phenylephrine) or the Mas receptor antagonist A779

The effects of bupivacaine, L-nitro-L-arginine-methyl ester, and phenylephrine on cardiovascular adaptations to asphyxia in the preterm fetal lamb.

UNLABELLED: The preterm fetal lamb that is exposed to clinically relevant plasma concentrations of lidocaine loses its cardiovascular adaptations to asphyxia, and its condition deteriorates further. Nitric oxide (NO) is an important regulator of vascular tone, and local anesthetics are known to inhibit endothelium-dependent vasodilation. The purpose of the present study was to determine whether the adverse effects of lidocaine noted in the preterm fetal lamb also occur with bupivacaine and whether the inhibition of NO results in effects similar to those of bupivacaine. Thirty-two chronically p
