Global tuberculosis trends: a reflection of changes in tuberculosis control or in population health?

BACKGROUND: Many international organizations are advocating for new funds for tuberculosis (TB) specific interventions. Although this approach should help reduce TB incidence, improvements in population health may also be important. We have analyzed the association between changes in population health and health service indicators, and concomitant changes in TB incidence between 1990 and 2005. METHODS: Country level data on population health and health services, economic and epidemiologic indicators were obtained for 165 countries. Regression methods were used to estimate the association of c

Family environment is associated with endemic Burkitt lymphoma: a population-based case-control study.

Endemic Burkitt's lymphoma (eBL) has been linked to Epstein-Barr virus and holoendemic Plasmodium falciparum malaria. These co-infections, however, are insufficient to explain the non-random occurrence of Endemic Burkitt's lymphoma within Equatorial Africa. To explore whether this distribution could be explained by household characteristics and family environment, we conducted a case-control study using 41 hospitalized incident endemic Burkitt's lymphoma cases and 91 healthy controls identified through a population-based multistage cluster-sampling scheme in Nyanza Province, Kenya. In a multiv

Urinary lead exposure and breast cancer risk in a population-based case-control study.

BACKGROUND: Lead is a toxic nonessential metal with widespread exposure starting in utero. Lead has been reclassified in 2004 by the International Agency for Research on Cancer Working Group from a "possible" to a "probable" human carcinogen. Lead may be a facilitative or permissive carcinogen, which means that lead may permit or augment the genotoxic effects of other exposures. METHODS: This population-based study in Wisconsin gathered survey data and home-collected urine specimens from 246 women, ages 20 to 69 years, with incident invasive breast cancer identified from the Wisconsin state r

Vaccination history and risk of non-Hodgkin lymphoma: a population-based, case-control study.

OBJECTIVE: As factors that alter the immune system have been implicated in non-Hodgkin lymphoma (NHL) etiology, it is of interest to explore the association between vaccination and risk of NHL. Results of few epidemiologic studies conducted thus far are inconsistent, and only one has examined the association by histologic subtype. SUBJECTS: A population-based, case-control study of 387 patients with NHL and 535 controls conducted in Nebraska between 1999 and 2002. METHODS: Information on vaccination for tetanus, polio, influenza, smallpox, and tuberculosis, as well as important environmental

Antibiotic use and risk of non-Hodgkin's lymphoma: a population-based case-control study.

Antibiotic use in 759 non-Hodgkin's lymphoma (NHL) patients and 589 controls was compared. Neither total antibiotic use (odds ratio=0.7, 95% confidence interval=0.5-1.2), nor antibiotic use by site, was associated with total NHL, or NHL subtypes. There were no trends with frequency or age at first use (P trend=0.23 and 0.26, respectively).

From population structure to genetically-engineered vectors: new ways to control vector-borne diseases?

Epidemiological studies on vectors and the pathogens they can carry (such as Borrelia burgdorferi) are showing some correlations between infection rates and biodiversity highlighting the "dilution" effects on potential vectors. Meanwhile other studies comparing sympatric small rodent species demonstrated that rodent species transmitting more pathogens are parasitized by more ectoparasite species. Studies on population structure and size have also proven a difference on the intensity of the parasitic infection. Furthermore, preliminary results in genetic improvement in mosquitoes (genetic marke

Population-based case-control study of measles, mumps, and rubella and inflammatory bowel disease.

BACKGROUND: Previous controversy was generated over the hypothesis that a paramyxovirus such as measles or vaccination against such viruses might be causally associated with inflammatory bowel disease (IBD). We aimed to determine if Crohn's disease (CD) or ulcerative colitis (UC) subjects are more likely to be seropositive for measles, mumps, or rubella than controls. METHODS: Using our population-based University of Manitoba IBD Research Registry we recruited CD (n = 235) and UC (n = 137) subjects ages 18-50 years for a study involving detailed questionnaires and venipuncture. We accessed th

Cigarette smoking and risk of non-Hodgkin's lymphoma--a population-based case-control study.

BACKGROUND: Epidemiologic evidence of an association between tobacco smoking and non-Hodgkin's lymphoma has been conflicting. This may reflect that non-Hodgkin's lymphoma comprises several distinct disease entities with different etiologies, as some studies have indicated an association between smoking and follicular lymphoma. OBJECTIVE: To investigate the association between cigarette smoking and non-Hodgkin's lymphoma risk, overall and by subtype. METHODS: As part of a nationwide Danish-Swedish population-based case-control study, we interviewed 3,055 incident non-Hodgkin's lymphoma patien

Aspirin and the risk of Hodgkin's lymphoma in a population-based case-control study.

BACKGROUND: Regular use of nonsteroidal anti-inflammatory drugs (NSAIDs) is associated with decreased risk of several malignancies. NSAIDs may prevent cancer development by blocking the cyclooxygenase-catalyzed synthesis of proinflammatory prostaglandins. Aspirin may also protect against Hodgkin's lymphoma by inhibiting transcription factor nuclear factor kappaB (NF-kappaB), which is necessary for immune function and the survival of Hodgkin's lymphoma cells. We examined the association between regular analgesic use and the risk of Hodgkin's lymphoma. METHODS: A population-based case-control s

Population-based case-control investigation of risk factors for leptospirosis during an urban epidemic.

Between March and October 2000, 157 suspected cases of leptospirosis hospitalized with complications of Weil's syndrome and a mortality of 8% were identified in Salvador, Brazil. We conducted a population-based case-control study to identify risk factors for acquisition of leptospirosis in neighborhoods with high endemicity during the rainy season-associated urban epidemic. Sixty-six (65%) of 101 laboratory-confirmed cases and 125 age and sex-matched healthy neighborhood controls were interviewed. Residence in proximity to an open sewer (matched odds ratio [OR] = 5.15, 95% confidence interval

Aspirin and risk for gastric cancer: a population-based case-control study in Sweden.

While aspirin and other non-steroid anti-inflammatory drugs (NSAIDs) are associated with gastric mucosal damage, they might reduce the risk for gastric cancer. In a population-based case-control study in 5 Swedish counties, we interviewed 567 incident cases of gastric cancer and 1165 controls about their use of pain relievers. The cases were uniformly classified to subsite (cardia/non-cardia) and histological type and information collected on other known risk factors for gastric cancer. Helicobacter pylori serology was tested in a subset of 542 individuals. Users of aspirin had a moderately re
