Ipsilateral Dural Thickening and Enhancement: A Sign of Isolated Cortical Vein Thrombosis? A Case Report and Review of the Literature.

BACKGROUND: Isolated cortical vein thrombosis (ICVT) being a rare condition (6% of intracranial vein thromboses), no clinical guidelines and few radiologic clues to it have been established. ICVT mostly appears in conjunction with sinus vein thromboses. ICVTs primarily occur during pregnancy and puerperium (35%). The great variability of cortical veins and difficulty identifying small occluded vessels complicate ICVT diagnosis. We present the first case of isolated ipsilateral dural thickening and enhancement as a potential radiologic sign of ICVT shown on magnetic resonance imaging. CASE DES

Major complications of epidural anesthesia: a prospective study of 5083 cases at a single hospital.

BACKGROUND: We undertook a prospective study of non-obstetric epidurals placed in surgical inpatients at a single teaching hospital to evaluate the incidence of and potential risk factors for major complications of continuous epidural anesthesia. METHODS: Demographic information, details of the epidural procedure, and complications (from the pre-anesthetic period through resolution) were recorded for more than 5000 surgical inpatients who underwent continuous epidural anesthesia in our institution between March 2009 and April 2011. The incidence of and risk factors for major complications wer

Isolated Abducens Nerve Palsy Following Spinal Anesthesia.

A healthy 28-year-old lady, para 1, presented to the emergency department with persistent frontal headache, nausea, and vomiting following an emergency cesarean section four days ago. She experienced difficulties with six failed attempts of spinal anesthesia intrapartum before conversion to general anesthesia. A 25-gauge Whitacre needle was utilized for administering spinal anesthesia under a sitting position. The anesthetist noticed a loss of resistance upon needle insertion, but only a negligible amount of cerebrospinal fluid was obtained upon removing the stylet. The patient underwent an em

[Ultrasound-guided combined femoral-obturator nerve block for the knee arthroscopic surgery of meniscal lesions: case report].

We report successful management of anesthesia in two cases of knee arthroscopic surgery of meniscal lesions using ultrasound-guided combined femoral-obturator nerve block with inhalation anesthesia. The blocks were performed with 30 ml of 0.5% ropivacaine under ultrasonographic visualization. The perioperative courses were uneventful and there was no complaint about postoperative pain. Unlike spinal or epidural anesthesia, combined femoral-obturator nerve block has advantages of no muscle weakness in healthy lower limbs, no urinary retention, and no post dural puncture headache. Our technique

Dural puncture-induced intracranial hypotension causing diplopia.

BACKGROUND: Diplopia that occurs after an epidural spinal catheter has been placed for pain control has been attributed to sixth nerve palsy nerve palsy induced by intracranial hypotension. There is sparse information about the factors that confound diagnosis in this setting. METHODS: Review of 6 cases examined over a period of 5 years at a single tertiary care medical center. RESULTS: Six confounders to diagnosis were identified: 1) lack of awareness that an epidural spinal catheter was or had been in place; 2) delayed reporting of diplopia; 3) mild or inapparent ductional deficits; 4) lack

Late Seizures in Postpartum Women After Cesarean Section: Differential Diagnosis Between Postpartum Eclampsia and Post-dural Puncture Eclampsia.

Postpartum eclampsia is an uncommon but potentially life-threatening complication that can occur up to six weeks after delivery, even in women without prior hypertension. Post-dural puncture headache (PDPH), a frequent adverse effect of spinal anesthesia, typically presents as a postural headache and, in rare cases, may progress to seizures. The clinical overlap between these two conditions poses a significant diagnostic challenge. We report the case of a 25-year-old woman who underwent a cesarean section under spinal anesthesia and developed a headache immediately postpartum, which worsened o

Chronic pain and long-term disability following postdural puncture headache in obstetric patients: a five-year prospective follow-up of a case-control cohort.

BACKGROUND: Unintended dural puncture is a recognized complication of neuraxial labor analgesia and has been associated with postdural puncture headache (PDPH). While short-term outcomes are well documented, long-term consequences remain poorly characterized. No previous study has reported follow-up beyond two years. METHODS: This prospective case-control study followed 336 women up to five years after delivery. The cohort comprised 113 women who delivered without labor epidural analgesia, 113 women who delivered with uncomplicated labor epidural analgesia, 54 women with PDPH following an uni

Epidural injection of hydroxyethyl starch in the management of postdural puncture headache.

Epidural blood patch is the standard treatment for postdural puncture headache when symptomatic therapy is ineffective. We report the cases of two patients who received an epidural injection of hydroxyethyl starch when an epidural blood patch was contraindicated; one due to Streptococcus agalactiae bacteraemia and one due to acute leukaemia. Relief of headache was achieved in both patients with no adverse effects. The use of an epidural hydroxyethyl starch injection may be a suitable alternative for treatment of postdural puncture headache if epidural blood patch is contraindicated.

[Complications of spinal anesthesia and how to avoid them].

Spinal anesthesia is a safe procedure. The knowledge of complications may support efforts to minimize risks, speed up the recognition process and lead to adequate timely therapeutic approaches. Pain during insertion of the needle can be a warning signal for potential conus damage. Hypotension caused by spinal anesthesia should be treated by appropriate vasoactive drugs. Timely recognized cardiac arrest situations are usually well treatable. The incidence of postdural puncture headache should be less than 2% of cases. In case of a high degree of suffering the best currently available treatment

The Suprainguinal Fascia Iliaca Block Prolonging Spinal Anesthesia Duration.

Spinal anesthesia is commonly used for lower limb procedures, its duration may be limited with potential complications due to high doses of local anesthetic. This study describes the technique and experience of using suprainguinal fascia iliaca block (SIFI) as an adjunct to spinal anesthesia in an elderly patient undergoing lower extremity surgery. The case presented here involves an 81-year-old female undergoing hip surgery, where a SIFI block was performed prior to the administration of spinal anesthesia. Despite the unexpectedly prolonged surgical duration of approximately 5 hours, the pati

Pain, postdural puncture headache, nausea, and pruritus after cesarean delivery: a survey of prophylaxis and treatment.

BACKGROUND: The need for a cesarean delivery may interfere negatively with the overall experience of childbirth. Several factors related to anesthesiological management such as postoperative pain and discomfort, nausea and pruritus, and postdural puncture headache (PDPH), may lead to dissatisfaction and have a negative impact on early mobilization and a new mother's ability to care for her newborn baby. Optimal prophylaxis and treatment decrease these complications, increase satisfaction, and prevent chronic pain. This survey determined how prophylaxis and treatment of pain, PDPH, nausea, and

Full-Endoscopic Removal of Sheared Lumbar Epidural Catheter Fragment.

BACKGROUND: An epidural nerve block injection is the most common modality of treatment for control of low backache associated with radiating pain. Epidural catheter injections are also widely used by pain physicians to control cancer pain. Epidural catheters could be associated with procedure-related complications, such as epidural hematoma, epidural abscess, and post-dural puncture headache, and rare complications, such as shearing or breakage of the catheter tip. In this report, we describe the full-endoscopic removal of a sheared epidural catheter fragment. CASE DESCRIPTION: A man aged 24

Distribution of intranasal lidocaine and its effect on electrically-evoked calcium signals in mouse trigeminal ganglion neurons.

BackgroundLidocaine applied intranasally has shown promise in treating post-dural puncture headache and has been explored as a potential treatment for cluster headache and migraine. However, the mechanisms underlying its analgesic effects remain unclear. This study aimed to determine the concentration of lidocaine that reaches target tissues after intranasal application and its impact on the excitability of trigeminal ganglion neurons.MethodsA mouse model was used to examine lidocaine (10%) distribution after intranasal administration, employing liquid chromatography-mass spectrometry (LC-MS).
