Adverse reactions with levamisole vary according to its indications and misuse: A systematic pharmacovigilance study.

UNLABELLED: Levamisole was initially prescribed for the treatment of intestinal worms. Because of immunomodulatory properties, levamisole has been used in inflammatory pathologies and in cancers in association with 5-fluorouracil. Levamisole is misused as a cocaine adulterant. Post-marketing reports have implicated levamisole in the occurrence of adverse drug reactions (ADRs) and its use is now limited in Europe and North America. In contrast, all other parts of the World continue to use single-dose levamisole as an anthelmintic. The aim of this study was to identify ADRs reported after levami

Exploring Neuroprotective Agents for Sepsis-Associated Encephalopathy: A Comprehensive Review.

Sepsis is a life-threatening condition resulting from an inflammatory overreaction that is induced by an infectious factor, which leads to multi-organ failure. Sepsis-associated encephalopathy (SAE) is a common complication of sepsis that can lead to acute cognitive and consciousness disorders, and no strict diagnostic criteria have been created for the complication thus far. The etiopathology of SAE is not fully understood, but plausible mechanisms include neuroinflammation, blood-brain barrier disruption, altered cerebral microcirculation, alterations in neurotransmission, changes in calcium

Post-COVID-19 pure red cell aplasia: A case report and literature review.

Pure red cell aplasia is a rare hematologic disorder characterized by severe normocytic anemia, reticulocytopenia, and absence of erythroid precursors. Since the COVID-19 pandemic, pure red cell aplasia has emerged as an uncommon post-infectious complication, likely driven by immune dysregulation. A 72-year-old man developed transfusion-dependent anemia several weeks after a mild COVID-19 infection. Laboratory evaluation revealed normocytic anemia (hemoglobin of 7.0 g/dL) with profound reticulocytopenia, and bone marrow biopsy showed markedly reduced erythropoiesis with preserved granulopoiesi

Primary immunodeficiency-related bronchiectasis in adults: comparison with bronchiectasis of other etiologies in a French reference center.

BACKGROUND: Bronchiectasis is a heterogeneous disease depending on etiology. It represents the most frequent non-infectious pulmonary complication of primary immunodeficiencies (PID). We investigated whether bronchiectasis associated with PID had a distinct course in comparison to bronchiectasis of other causes. METHODS: Retrospective single-center study of adult patients diagnosed with non-cystic fibrosis bronchiectasis with more than 5 years of follow-up and at least 4 pulmonary functional tests available at one year apart. They were divided into three groups: PID- related bronchiectasis, i

Transplantation-Associated Altered Mentation and Encephalopathy: A New Classification for Acute Neurocognitive Changes Associated with Hematopoietic Cell Transplantation from the ASTCT Committee on Practice Guidelines.

Acute encephalopathy, manifesting clinically as delirium, is a common but often unrecognized complication of hematopoietic cell transplantation (HCT). Delirium can occur in patients of any age and is observed after autologous or allogeneic HCT. Although delirium has been studied primarily during initial HCT hospitalizations in recipients of myeloablative conditioning, recent investigations have identified delirium later post-transplantation and in recipients of reduced-intensity conditioning. Acute encephalopathy can be driven by infectious complications, medications, tissue damage, and/or org

Guillain-Barré syndrome: report of two rare clinical cases occurring after allergenic bone grafting in oral maxillofacial surgery.

Guillaine-Barré Syndrome (GBS), also known as post-infectious polyneuropathy or acute idiopathic polyneuritis, is an infrequent disorder of the peripheral nervous system. The cause of GBS is unknown. It has been associated in the past with microbial infections, vaccinations, surgical procedures and debilitation of the patient. The classic signs of GBS occurring in the two patients being reported here are muscle weakness, motor and sensory impairment and ascending paralysis with respiratory involvement. The documented cases involved GBS syndrome following oral and maxillofacial surgery in which

Painful small-fibre multifocal mononeuropathy and local myositis following influenza B infection.

A 47-year-old man experienced multifocal mononeuropathy and putative ganglionopathy associated with influenza B infection, characterized by aching and dysesthesia in the right arm and left leg with normal deep sense. He displayed muscle atrophy in the affected limbs, which might have resulted from local myositis or a disorder similar to neuralgic amyotrophy. Sural nerve biopsy revealed a severe loss of unmyelinated and thinly myelinated fibres, consistent with a small fibre neuropathy, without evidence of angiopathy or inflammation. We could not detect any other cause of the neuropathy except

Burden of infectious diseases, substance use disorders, and mental illness among Ukrainian prisoners transitioning to the community.

BACKGROUND: The epidemics of incarceration, substance use disorders (SUDs), and infectious diseases are inextricably intertwined, especially in the Former Soviet Union (FSU). Few objective data documenting this relationship regionally are available. We therefore conducted a comprehensive, representative country-wide prison health survey in Ukraine, where one of the world's most volatile HIV epidemics persists, in order to address HIV prevention and treatment needs. METHODS: A nation-wide, multi-site randomly sampled biobehavioral health survey was conducted in four Ukrainian regions in 13 pri

Study on the Role of Gastrointestinal Parasite in Irritable Bowel Syndrome Patients in a Tribal Region of India.

Background Irritable bowel syndrome (IBS) is a functional gastrointestinal (GI) disorder in which abdominal pain is associated with a change in bowel habits. Gut inflammation might be one of the mechanisms of pathogenesis. However, the cause of IBS is not clearly understood. Post-infectious IBS (PI-IBS) is the onset of IBS after an episode of infectious gastroenteritis. While the exact pathophysiology of PI-IBS is not established, the mechanism might be an altered serotonin signaling activity, inflammation, malabsorption, and small intestinal bacterial overgrowth. Various parasites such as Bla

Changes in the symptom pattern and the densities of large-intestinal endocrine cells following Campylobacter infection in irritable bowel syndrome: a case report.

BACKGROUND: Irritable bowel syndrome (IBS) is a common chronic functional gastrointestinal disorder. Post-infectious IBS (PI-IBS) is a subset of IBS that accounts for a large proportion of IBS patients. The PI-IBS symptoms meet the Rome criteria for IBS with diarrhoea (IBS-D) or IBS with mixed bowel habits (IBS-M). A low-grade inflammation has been reported to occur in PI-IBS. Abnormalities in intestinal endocrine cells have been reported in both sporadic IBS and PI-IBS. CASE PRESENTATION: A 20-year-old female with a diagnosis of IBS with constipation (IBS-C), according to Rome III criteria,

Microbial signatures in post-infectious irritable bowel syndrome--toward patient stratification for improved diagnostics and treatment.

Irritable bowel syndrome (IBS) is a multifactorial and heterogeneous disorder estimated to affect over 10% of the Western population. A subset of the patients reports the start of the disease after an episode of gastroenteritis. The alterations in the intestinal microbiota of the post-infectious IBS (PI-IBS) patients were recently investigated in a British cohort and shown to differentiate from the healthy controls and resemble that of diarrhea-predominant IBS (IBS-D) patients. The altered 27 genus-like groups created a microbial signature, which could be used to objectively stratify patients

Clinical diagnostic accuracy of respiratory failure in critically ill hematopoietic stem cell transplant patients.

RATIONALE: Acute respiratory failure (ARF) is a frequent complication following hematopoietic cell transplantation (HCT). We aimed to characterize the etiologies of ARF in patients who died in the intensive care unit following HCT based on autopsy findings. We then evaluated agreement between the clinical and pathologic diagnosis. METHODS: We performed a chart review of all HCT patients who died and underwent autopsy in our ICU between 2006-2016. We evaluated the presumed clinical diagnosis and confidence in the diagnosis by chart review, the pathologic diagnosis on autopsy, and whether the c

Acute Gastroenteritis Is a Risk Factor for the Development of Disorders of Gut-Brain Interaction in Children.

INTRODUCTION: Acute gastroenteritis (AGE) is the most common disease predisposing to the development of disorders of gut-brain interactions (DGBIs) in adults (postinfectious DGBI [PI-DGBI]). There is paucity of data on incidence and risk factors for the development of PI-DGBI in children. The aims of this study are to (i) assess whether AGE predisposes children to the development of PI-DGBI and (ii) assess whether the severity of AGE is associated with the development of PI-DGBI. METHODS: This was a prospective, controlled, cohort study. Children with recent AGE (cases) and siblings (controls

Third nerve palsy as first presenting symptom of Guillain-Barre Syndrome spectrum clinical variant.

PURPOSE: To report an unusual presentation of a Guillain-Barre Syndrome (GBS) spectrum clinical variant with an initial isolated, unilateral third nerve palsy prior to development of areflexia and lower extremity weakness. OBSERVATIONS: A 16-month-old presented with a pupil-involving right third nerve palsy and normal lower extremity strength and reflexes. She was treated with a short oral steroid taper for presumed post-infectious cranial nerve (CN) palsy and had initial improvement. Soon after completing the taper, she developed an inability to sit, stand, or crawl with lower extremity aref

Widespread haemorrhages in infants post-shunting (WHIPS): clinical features, risk factors and neuroimaging characteristics of a rare and under-recognised phenomenon.

PURPOSE: Infants undergoing CSF shunting procedures face a rare complication which we propose to rename "Widespread Haemorrhages in Infants Post-Shunting" (WHIPS) to better capture this unique phenomenon specific to infants undergoing CSF diversion. Our objective is to analyse the risk factors for WHIPS development and provide a detailed neuroradiological description of these haemorrhages. MATERIALS AND METHODS: A radiology information system (RIS) was searched using the search terms "shunt" and/or "catheter" and/or "drain" and/or "ventriculoperitoneal" and/or "VP" between September 2008 to J

ReVita: home-based transcutaneous auricular vagus nerve stimulation for reducing pathological fatigue in chronic neurological and post-infectious conditions-study protocol for a randomized, double-blind, sham-controlled trial.

BACKGROUND: Pathological fatigue is one of the most prevalent and disabling symptoms across chronic neurological, post-infectious, and inflammatory diseases, yet effective treatments remain limited. Transcutaneous auricular vagus nerve stimulation (taVNS) is a safe, non-invasive neuromodulatory intervention that targets neuroimmune and autonomic pathways implicated in the development and persistence of fatigue. This randomized controlled trial investigates whether home-based taVNS can reduce fatigue severity in a transdiagnostic population. METHODS: This study is a randomized, double-blind, s
