Reclaiming the patient's voice and spirit in dying: an insight from Israel.

In the latter half of the 20th century, Western medicine moved death from the home to the hospital. As a result, the process of dying seems to have lost its spiritual dimension, and become a matter of prolonging material life by means of medical technology. The novel quandaries that arose led in turn to medico-legal regulation. This paper describes the recent regulation of dying in Israel under its Dying Patient Law, 2005. The Law recognizes advance directives in principle, but limits their effect and form through complex medico-legal artifices. It reflects a culture that places high value on

Day hospital programmes for eating disorders: a review of the similarities, differences and goals.

Day hospital (DH) treatments for eating disorders (EDs) provide intensive daily care and allow patients to maintain and test their social relations and coping skills at home and outside. Although widespread, their description is lacking. This review compares the different types of DH described in the literature and presents our DH experience in Turin, Italy. We searched Psychinfo and Pubmed with the following keywords: anorexia nervosa, bulimia nervosa, EDs, DH, day treatment and partial hospitalisation. We found and reviewed the DH programmes of eleven specialised centres, which have some sha

A Patient-Centered Ethical Framework for Irritable Bowel Syndrome Care: Communication, Trust, Nutrition-Sensitive Care, and Self-Management.

Irritable bowel syndrome (IBS) is a prevalent disorder of gut-brain interaction, characterized by a substantial symptom burden, impaired quality of life, and increased healthcare use. Despite advances in diagnostic criteria and treatment strategies, many patients feel dismissed, inadequately informed, or uncertain about the nature and meaning of their symptoms; these experiences may undermine trust and reduce engagement with healthcare professionals. The aim of this narrative review is to synthesize clinical and ethical considerations and propose a patient-centered ethical framework for IBS ma

From antenatal preparedness to enacted agency during labor: Integrated findings from a woman-centered intervention on mobility.

INTRODUCTION: Freedom of movement and upright positions during labor are recommended in maternity care, yet organizational routines may constrain women's mobility, autonomy, and shared decision-making. This study examined how a woman-centered antenatal intervention on mobility during labor operated and under which conditions its effects were enabled or constrained. METHODS: A convergent mixed-methods study was conducted in community-based childbirth preparation programs in Portugal. Quantitative data were collected from low-risk nulliparous women at baseline (n=56), post-intervention (n=38),

Bounded control in digital health: Privacy discourse and algorithmic governance in Apple Health.

OBJECTIVE: This study examines how user control is constructed, communicated, and experienced in digital health tracking, using Apple Health as a case study. Rather than treating control as either fully empowering or simply illusory, the study conceptualizes it as bounded, relational, and discursively produced within platform environments. METHODS: The study uses a qualitative, triangulated design combining critical discourse analysis of Apple's official "Health App and Privacy" text with semi-structured interviews with Apple Watch and Apple Health users (n = 18). The analysis compares corpor

"How Do You Manage It? I Couldn't; I'm Way Too Jealous!" A Qualitative Research on How Polyamorous People Deal with Their Partners' Relationships.

Polyamorous relationships are a form of consensual non-monogamy involving multiple intimate relationships with mutual consent. Despite frequent discussion of jealousy, little is known about how polyamorous individuals experience and navigate their partners' concurrent relationships and the emotions these may evoke. We conducted ad hoc semi-structured interviews with 15 adults living in Italy who were involved in at least one polyamorous relationship (duration: 8 months-17 years; M = 3.9 years). Participants identified as women (n = 6), non-binary spectrum (n = 6), or men (n = 3), with a mean a

'It Takes a Village': Sociocultural Insights From a Qualitative Study on Strategies for Member Engagement and Participation in SMART Recovery.

INTRODUCTION: SMART (self-management and recovery training) recovery is a mutual-help group that supports and sustains recovery from addiction, including substance use. Meetings are moderated by trained facilitators who manage group dynamics and guide members through a 4-point program. Nonetheless, sociocultural factors can influence group processes, shaping facilitation style and delivery of SMART Recovery. Our study explored facilitation strategies used in Singapore to foster member engagement and retention. METHODS: In-depth qualitative interviews with 18 participants (14 members and four

Long-Acting Injectable Buprenorphine in Prison: Insights From a Short-Answer Survey of People in the Criminal Justice System Commencing Long-Acting Injectable Buprenorphine During a Non-Randomised Clinical Trial.

INTRODUCTION: Long-acting injectable buprenorphine (LAIB) is increasingly used in the Criminal Justice System (CJS) for treating people with opioid use disorder; however, little is known about these patients' experiences. We aimed to understand the perspectives of participants commenced on LAIB in prison as a part of the Understanding NSW Long-Active Opioids in Custody-Treatment (UNLOC-T) study. METHODS: Results of an open-ended survey administered by UNLOC-T researchers underwent analysis. Participants were adults, recruited November 2018-July 2019, with moderate-severe opioid use disorder a

Forgiveness and Adverse Childhood Experiences: A Scoping Review.

It is well established that adverse childhood experiences (ACEs) are strongly associated with a range of adverse outcomes across the lifespan. Approximately two-thirds of the U.S. population report experiencing at least one ACE, placing them at significantly higher risk for poorer health outcomes and relational dysfunction. Forgiveness interventions have emerged as an effective treatment for trauma survivors, yet some studies find that forgiveness can contribute to worse outcomes in certain contexts, and many survivors express reluctance or inability to forgive. We conducted a scoping review f

From Preferences to Practice: Clinician-Informed Strategies to Integrate the MyPref Values-Clarification Tool Into Adolescent and Young Adult Oncology Care.

BACKGROUND: Adolescents and young adults (AYAs) with advanced cancer navigate complex medical decisions amid evolving autonomy and family involvement. MyPref, a digital values-clarification tool using adaptive conjoint analysis, helps AYAs articulate care preferences and supports clinicians in initiating preference-based conversations. This study explored pediatric oncology clinicians' experiences with MyPref to identify barriers, facilitators, and strategies for clinical integration. PROCEDURE: We conducted a qualitative study using semi-structured interviews with attending physicians and nu

Stakeholder priorities for advancing integrative oncology across a pluralistic health system: a qualitative study from Singapore.

BACKGROUND: In Singapore, more than half of patients with cancer use traditional or complementary therapies after diagnosis. Such use is often self-directed and undisclosed to oncology clinicians, creating safety risks and care coordination challenges. To support evidence-informed integration of such therapies within conventional cancer care, this study explored stakeholder priorities for advancing integrative oncology (IO) in Singapore's pluralistic health system. METHODS: We conducted a multi-stakeholder roundtable at the National Cancer Centre Singapore. Twenty-two participants were purpos

Building bridges in the emergency room: educational alliances between junior residents and attending physicians.

INTRODUCTION: Emergency rooms (ERs) offer valuable learning opportunities for junior residents, yet the time constraints and complexity of interprofessional collaboration often hinder the development of strong educational relationships with attending physicians. This study aimed to explore how junior residents perceive and build educational alliances with attending physicians during a short-term ER rotation, using the framework of the "educational alliance" (EA). METHODS: We conducted semi-structured interviews with fourteen junior residents who completed a two-month ER rotation at an emergen

On the edge of dependency: how older adults with rheumatoid arthritis experience living with frailty.

OBJECTIVES: Up to one third of people with rheumatoid arthritis (RA) are frail, facing increased vulnerability, comorbidities, and psychological challenges. As people with RA live longer, frailty becomes increasingly relevant to clinical care. This study explores how individuals aged ≥65 years with RA experience frailty and how it affects their daily lives and health management. METHODS: We conducted an explorative qualitative interview study using purposeful sampling. Participants were recruited from a rheumatology outpatient clinic. Inclusion criteria were age ≥65 years, RA diagnosis, a Mul

What Helps and Hinders Recovery from Depression? A Systematic Review and Qualitative Evidence Synthesis of Patient-Identified Recovery Factors.

UNLABELLED: <p>Introduction: Depression is a common mental disorder with often persistent consequences. Even after adequate treatment, recovery may be far from optimal. To enhance outcomes, we aimed to identify and synthesize factors that depressed adults themselves perceived as facilitating or hindering recovery. METHODS: We searched PubMed, PsycINFO, and SocINDEX (last search: February 2, 2025), screened reference lists, and consulted experts. Eligible studies used a qualitative design, published in English or Dutch since 1980, and explored recovery factors in adults (≥18 years) with depres

Gaslighting Experience, Psychological Health, and Well-being: The Role of Self-Compassion and Social Support.

Gaslighting is an understudied form of psychological violence aimed to reduce victims' autonomy, decision-making ability, security, and belief in their own perceptions. Although it leads to negative psychological, relational, and health consequences, few studies have examined the protective variables of this form of violence. The present study aims to analyze the role of self-compassion and social support (from family, friends, and significant others) in predicting gaslighting experiences, psychological health, and eudaimonic well-being among young Italian women. The mediational role of the ga

Individual differences in motivation to change in individuals with eating disorders: A systematic review.

OBJECTIVE: Motivation to change has been suggested to significantly impact treatment outcomes in eating disorders (EDs). This review will investigate factors associated with motivation to change in EDs with the aim of supporting clinicians to be aware and sensitive to factors that might obstruct recovery and to inform motivation-based interventions. METHOD: Using PRISMA guidelines, this article identified 24 studies through database searches meeting eligibility criteria. Only correlates of motivation were identified, limiting the ability of this review to identify causal relationships. Factor

Perceptions of genetic testing in patients with hereditary chronic pancreatitis and their families: a qualitative triangulation.

Hereditary chronic pancreatitis (HCP) is a genetically determined condition characterized by intermittent acute episodes of pancreatitis and long-term impairment of the exocrine and endocrine pancreatic functions. Genetic test results can have substantial psychological and social consequences for the individuals tested and their families. Nevertheless, little is known so far about the subjective experience of individuals genetically tested for HCP. This qualitative study examines the viewpoints of HCP patients and their relatives in order to identify the psychosocial and ethical implications r

(Un)Spoken realities of living with axial spondyloarthritis: a qualitative study focused on couple experiences.

OBJECTIVE: Axial spondyloarthritis is a long-term rheumatic condition. The symptoms, including pain, can impact on the daily life routines and psychological well-being of individuals that are diagnosed with axial spondyloarthritis (axSpA). Partners are often a main source of support for individuals who manage a long-term condition and they can also be affected by the illness experience, often themselves reporting elevated levels of emotional distress. Few qualitative studies have explored the impact of axSpA on partner relationships. This study addresses the social context of axSpA by investig
