Arthroscopic technique for patch augmentation of rotator cuff repairs.

The patient is placed in the lateral position, and an arthroscopic cuff repair is performed according to standard techniques. The line of repair is usually in the shape of a "T" or an "L." The repair is viewed through the lateral portal, with fluid inflow through the scope. Mattress sutures are placed in the anterior and posterior portions of the cuff, with respect to the line of repair, just medial to the most medial point of the tear. The sutures are placed in accordance with margin convergence suture passing methods. Next, 2 double-stranded suture anchors are placed into the lateral aspect

[Rotator cuff tear--an occupational disease? An epidemiological analysis].

BACKGROUND: In literature there are only few data which describe the influence of occupation on the development of rotator cuff disease. METHODS: In a retrospective study, 760 open rotator cuff repairs were analysed and related to the profession and occupational load. Exclusion criteria were traumatic tears and sports injuries. All male persons were statistically analysed and the data compared with occupational patterns of the region, obtained from the Federal Statistical State Office. RESULTS: Rotator cuff repairs were performed in 472 males who had no evidence for a traumatic origin. After

[Arthroscopic treatment of calcifying tendinitis of the rotator cuff].

The treatment of symptomatic calcifying tendinitis of the rotator cuff is usally medical. Whereas, chronic and painfull features can beneficiate of a surgical treatment. With shoulder arthroscopy it's possible to remove the type A and B calcifications and to perform a bursectomy and acromioplasty in type C uncollected. The clinical and radiological results with one year of follow-up upgrate 90% of good and excellent results. Calcifying tendinitis reatment appear like one of the best indications of the shoulder arthroscopy.

Evaluation of rotator cuff tears with magnetic resonance arthrography.

The size and morphologic features of rotator cuff tears may influence treatment selection and affect final outcomes. Magnetic resonance arthrography allows observation of these features and other intraarticular structures. To assess the utility of magnetic resonance imaging in assessing size and morphologic features, we retrospectively reviewed observations on 41 shoulders in 37 consecutive surgically treated patients (mean age, 63.2 years) who had magnetic resonance imaging followed by magnetic resonance arthrography. The maximum rotator cuff defect size in the anteroposterior direction defin

Management of irreparable rotator cuff tears and glenohumeral arthritis.

Glenohumeral arthritis with irreparable rotator cuff tears remain a difficult entity to treat. Varied causes include rotator cuff tear arthropathy, osteoarthritis, or rheumatoid arthritis with irreparable cuff tear. Common symptoms are progressive pain and dysfunction. Physical examination may reveal pain, crepitance, rotator cuff weakness, and loss of motion and function. Radiographs may reveal varying degrees of osteophyte formation, sclerotic bone, superior humeral head migration, and bony erosion. Additional imaging modalities may reveal cuff tear size, retraction, atrophy, and fatty infil

Rotator cuff impingement due to enchondroma of the acromion.

We report the case of a patient with a long standing clinical history of subacromial impingement, in which plain radiographs and CT-scan revealed a well-circumscribed lucent lesion expanding the undersurface of the acromion. MRI showed the subacromial fat to be completely obliterated and the infraspinatus tendon deformed due to compression by a mass localised at the acromion. The acromial lesion was curetted, and histopathologic analysis was consistent with the diagnosis of enchondroma. Tumours located in the coracoacromial arch have been very rarely reported as a cause of subacromial impingem

Pseudoaneurysm after interscalene block for a rotator cuff repair.

We present a case of a 68-year-old female patient who had an interscalene nerve block (ISB) complicated by compression of her brachial plexus by a pseudoaneurysm. The complication occurred after the patient received an ISB as anesthesia for an outpatient shoulder procedure. Review of this complication should alert surgeons to consider this diagnosis as a possibility in patients with postoperative pain and/or neurologic compromise after receiving an ISB.

Reversed arthroscopic subacromial decompression for massive rotator cuff tears.

This prospective study evaluates the results of a procedure for massive rotator cuff tears that we term reversed arthroscopic subacromial decompression (ASD). The procedure includes an arthroscopic debridement of the subacromial space and glenohumeral joint, an arthroscopic tuberoplasty, and depending on the pathologic condition of the long head of the biceps, a biceps tendon tenotomy. Reversed ASD avoids a classic acromioplasty in order to preserve the integrity of the coracoacromial arch. Twenty-three patients with a mean age of 69 years underwent this procedure. After a mean follow-up of 40

Passive tension and gap formation of rotator cuff repairs.

The objectives of this study were to quantify the relationship between passive tension of rotator cuff repairs and arm position and to examine the effect of this tension on repair gap formation. Five patients undergoing open surgical rotator cuff repair of the supraspinatus tendon were recruited. Tendon tension was recorded as the supraspinatus was advanced into a bone trough and secured. The relationship between arm position and repair tension was then measured. Standardized rotator cuff tears were created in 3 cadaveric shoulders and repaired by use of the intraoperative technique. The diffe

Results of arthroscopic acromioplasty for chronic rotator cuff lesion.

The influence of acromioplasty in long standing rotator cuff deficiency with intractable pain was retrospectively evaluated in a consecutive series of 13 patients who were followed for a mean period of 19 months (range, 12 to 42 months) after arthroscopic acromioplasty. The Constant score improved from 59.3 (range, 39.9 to 90.3) preoperatively to 98.7 (69.1 to 122.7) postoperatively. Pain and motion improved significantly whereas strength did not improve. Arthroscopic acromioplasty in painful chronic rotator cuff avulsion was found to be an effective means to control pain and improve motion; i

Arthroscopic repair of large and massive rotator cuff tears.

PURPOSE: The purpose of this study was to evaluate the outcome of arthroscopically repaired large and massive rotator cuff tears at 1- to 5-year follow-up. TYPE OF STUDY: Retrospective chart review. METHODS: Sixty consecutive patients who had arthroscopically repairable large or massive tears were identified by retrospective chart review. Large tears are defined as defects in the cuff measuring from 3 to 5 cm and massive tears measuring greater than 5 cm. Ten of these patients were lost to follow-up before 1 year postoperatively and were excluded from the study. The remaining 50 were the foc

Acromial spur formation in patients with rotator cuff tears.

In this study we analyzed the acromial spurs of 15 patients with impingement syndrome undergoing open rotator cuff repair. Mineral apposition analysis and quantitative cytochemical techniques for glucose-6-phosphate dehydrogenase (G6PD) activity (pentose phosphate pathway), alkaline phosphatase (ALP) activity (osteoblast activity), and tartrate-resistant acid phosphatase (TRAP) activity (osteoclast phenotype) were used to examine the distribution and level of activity of selected marker enzymes within the acromial spur insertion into the coracoacromial ligament in order to establish whether lo

Chronic rotator cuff injury and repair model in sheep.

BACKGROUND: Most rotator cuff surgery is performed on chronic tears. As there is no animal model in which to examine the physiology of muscle and tendon injury and repair in this setting, we developed a chronic rotator cuff injury model in sheep. METHODS: The infraspinatus tendon was released in thirty-six female sheep. Biopsy specimens were obtained from the muscle and were analyzed for fat content. The force generated by the muscle with supramaximal stimulation was recorded intraoperatively. A control group (twelve sheep) underwent an immediate tendon repair. In the remaining twenty-four sh

Failed surgical management of partial thickness rotator cuff tears.

Some authors have recommended that rotator cuff tears <50% of tendon thickness be debrided and those involving >50% of the tendon be treated with miniopen repair. We hypothesize that if indications for selecting between simple debridement and tendon repair were appropriate, then both groups should have comparable outcomes. Thirty-nine patients with partial rotator cuff tears met inclusion criteria and were available for retrospective analysis. Twenty-six percent of patients who underwent debridement and 12.5% of patients who had mini-open repair had unsatisfactory results according to Neer's c

Open versus arthroscopic treatment of chronic rotator cuff impingement.

We report the results of 238 consecutive patients who underwent in total 261 acromioplasties because of chronic rotator cuff impingement. The procedure was performed either in conventional open technique (80) or arthroscopically (181). Two years (1-10) after the operation 68% of the patients treated with the open technique had an excellent or good result compared 82% of the patients treated arthroscopically by an experienced arthroscopic surgeon. Compared to the open technique, the arthroscopic procedure had a statistically significant superior result concerning outcome, operating time and hos

Management of chronic deep infection following rotator cuff repair.

BACKGROUND: Deep infection of the shoulder following rotator cuff repair is uncommon. There are few reports in the literature regarding the management of such infections. METHODS: We retrospectively reviewed the charts of thirteen patients and recorded the demographic data, clinical and laboratory findings, risk factors, bacteriological findings, and results of surgical management. RESULTS: The average age of the patients was 63.7 years. The interval between the rotator cuff repair and the referral because of infection averaged 9.7 months. An average of 2.4 procedures were performed prior to

[Histological and electron microscopic study of rotator cuff tear].

OBJECTIVE: To understand the changes of rotator cuff during tear and observe the pathologic and ultramicroscopic structure of rotator cuff. METHOD: Histological and ultra thin slides of samples of 36 cases of rotator cuff were made. RESULT: The degeneration of rotator cuff happened in synovium side, and then extended to the middle and deep sides of tendon. The deep side of cuff tendon could degenerate without synovium. The repair of tendon could be achieved with two types of healing. CONCLUSION: It would be difficult to achieve full repair when complete tear and deep side tear occurred to t

The rotator cuff. Full-thickness tears. Mini-open repair.

Arthroscopically assisted rotator cuff repair, through a mini-open, deltoid-splitting approach, is an effective treatment for small- to medium-sized full-thickness tears of the rotator cuff. Advantages over traditional open repair include relative deltoid preservation, the ability to assess and treat coexisting intra-articular pathology, shorter hospitalization, reduced early morbidity, easier and quicker rehabilitation, and a smaller scar. Standard open repair is preferred for larger tears with a greater degree of tendon retraction or when the subscapularis tendon is involved in the tear.
