Clinical and Histopathologic Features Can Help Target Immunohistochemical Stain Use in the Diagnosis of Viral Esophagitis.

OBJECTIVES: Herpes simplex virus (HSV) and cytomegalovirus (CMV) immunohistochemical stains (IHC) are frequently applied on esophageal biopsies. Our aims were to identify IHC use patterns in viral esophagitis (VE), and clinicopathologic features of VE that could guide IHC use. METHODS: We included 58 VE cases and 60 controls, defined as patients with negative HSV/CMV IHC between January 2006 and July 2017. Biopsies were reviewed and histologic features and clinical data recorded. RESULTS: Thirteen cases required IHC for diagnosis. IHC was performed in 13 HSV and 5 CMV cases where diagnostic

High level antibodies to TORCH in the IVIG preparation from Taiwanese.

BACKGROUND: Congenital TORCH (toxoplasmosis, other viruses [varicella-zoster virus, VZV, etc.], rubella, cytomegalovirus [CMV], Herpes simplex virus [HSV]) infections are major causes of prenatal, perinatal, and postnatal morbidity and mortality. Although treatment or prevention strategies are available for these pathogens, all drugs may not be safe during the pregnancy. The aim of this study is to measure the antibodies (Abs) concentration in the intravenous immunoglobulin (IVIG) preparation to evaluate the therapeutic potential for TORCH infection. METHODS: We tested the only one commercial

An Adult Case of Herpes Simplex Virus-associated Granulomatous Encephalitis.

Brain granuloma occurs under certain conditions. Herpes simplex virus (HSV) causes granulomatous encephalitis in children; however, it has been rarely reported in adults. A 74-year-old man with a history of herpes simplex encephalitis suffered recurrent seizures. Brain magnetic resonance imaging revealed a mass lesion and resection was performed. A polymerase chain reaction using a brain biopsy specimen was positive for HSV DNA; thus, the patient was diagnosed with HSV-associated granulomatous encephalitis. After administering acyclovir, the patient showed improvement. HSV can cause granulomat

Burden of herpes simplex virus encephalitis in the United States.

Herpes simplex virus encephalitis (HSVE) is a disease of public health concern, but its burden on the healthcare of United States has not been adequately assessed recently. We aimed to define the incidence, complications and outcomes of HSVE in the recent decade by analyzing data from a nationally representative database. Healthcare Cost and Utilization Project databases were utilized to identify patients with primary discharge diagnosis of HSVE. Annual hospitalization rate was estimated and several preselected inpatient complications were identified. Regression analyses were used to identify

Herpesvirus transport to the nervous system and back again.

Herpes simplex virus, varicella zoster virus, and pseudorabies virus are neurotropic pathogens of the Alphaherpesvirinae subfamily of the Herpesviridae. These viruses efficiently invade the peripheral nervous system and establish lifelong latency in neurons resident in peripheral ganglia. Primary and recurrent infections cycle virus particles between neurons and the peripheral tissues they innervate. This remarkable cycle of infection is the topic of this review. In addition, some of the distinguishing hallmarks of the infections caused by these viruses are evaluated in terms of their underlyi

Expression of herpes simplex virus DNA fragments located in epidermal keratinocytes and germinative cells is associated with the development of erythema multiforme lesions.

Skin from acute and healed herpes simplex virus or herpes simplex virus-associated erythema multiforme (HAEM) lesions was examined by polymerase chain reaction with primers for DNA polymerase, ICP8, thymidine kinase (5' end of herpes simplex virus genome), and ICP27 (3' end of herpes simplex virus genome). The primers were herpes simplex virus specific and equally sensitive. The four herpes simplex virus genes were seen in acute herpes simplex virus lesions, but except for one patient, only polymerase (or polymerase and ICP8) were seen in 7-d healed lesional skin. Herpes simplex virus DNA was

[Central nervous system infection due to Herpes simplex virus in AIDS].

Infections of the central nervous system by Herpes simplex viruses (Herpes simplex type 1 and Herpes simplex type 2) are uncommon in acquired immune deficiency syndrome and are often clinically and pathologically atypical. We have collected 11 cases of herpes simplex encephalomyelitis in AIDS patients reported in the literature. Only 3 of these cases presented with a typical, necrotizing, limbic encephalitis. Other clinicopathological patterns included ventriculitis, rhombencephalitis and myelitis. Ventriculitis and rhombencephalitis were usually due to infection by HSV-1, whereas myelitis was

Herpes simplex virus in chronic relapsing keratoconjunctivitis.

In a patient with a 5-month history of chronic relapsing keratoconjunctivitis, herpes simplex virus antigen was disclosed by immunofluorescence test in material obtained by conjunctival scraping, and the symptoms resolved following acyclovir (Zovirax) treatment. The diagnostic difficulties with patients presenting without symptoms suggestive of herpes simplex virus may be responsible for the lack of similar reports.

Distinct monoclonal antibodies separately label the hexons or the pentons of herpes simplex virus capsid.

The surface shell of the capsid of herpes simplex virus type 1 (HSV-1) is 15 nm thick and 125 nm in outer diameter and has the form of an icosahedral (T = 16) surface lattice, composed of 150 hexons and 12 pentons. Hexons are traversed by axial channels and have six-fold symmetric external protrusions, separated by triangular nodules ("triplexes"). Pentons resemble hexons morphologically, apart from their different order of symmetry. To localize VP5, the major capsid protein, in the shell structure and to investigate whether pentons are composed of the same molecules as hexons, we have perform

[Evaluation of the method of DNA amplification (PCR, polymerase chain reaction) for diagnosis of superficial ocular herpes].

We evaluated the polymerase chain reaction (PCR) as a diagnostic method in superficial corneoconjunctival herpes and compared it to viral culture. A total of 38 patients were included and divided into 3 groups according to the clinical aspect. Fourteen patients that had a typical clinical aspect for herpes served as reference to evaluate the method. Nine patients had atypical lesions (herpes-suspects) and 15 patients (13 foreign corneal bodies and 2 adenovirus keratoconjunctivitis) made up the control group. All culture positive cases were also positive with the PCR method (in total 10 cases).

Discrimination of herpes simplex virus types 1 and 2 cerebral infections in a rat model.

A rat model was used to evaluate intertypic differences after intracranial (i.c.) inoculation of herpes simplex virus (HSV) types 1 and 2 strains with regard to neurovirulence and neuropathology, and influence of age on susceptibility. In adult rats, HSV-1 strains were more virulent than HSV-2 strains. HSV-1 replicated to higher titers in the central nervous system (CNS) of rats, as compared with HSV-2. In rats of less than 2-3 weeks of age, HSV-1 and HSV-2 were equally virulent, but morphological examination of rat brains showed type-specific differences in pathology and viral distribution al

Antiviral activity of Flos verbasci infusion against influenza and Herpes simplex viruses.

The lyophilized infusion from flowers of Verbascum thapsiforme Schrad. (FVI) showed antiviral activity in in vitro studies against Fowl plague virus, several influenza A strains, influenza B strain as well as Herpes simplex virus. Influenza viruses titer decreased by 1-3 log units, while of H. simplex virus by 2.3 log. FVI has shown virucidal activity on H. simplex virus at 300 micrograms/ml, but did not inactivate influenza viruses. Phytochemical investigations of FVI have shown the presence of flavonoids, iridoids, phenolic acids, saponins, amino acids and free sugars.

Anogenital warts in children. Clinical and virologic evaluation for sexual abuse.

Seventy-three children with anogenital warts were examined for sexual abuse during a 2-year period. Our data suggest that nonsexual transmission is common, particularly in children under 3 years of age. Approximately 25% of these children were younger than age 1 year, and another 50% were between the ages of 1 and 3 years. No evidence of sexual abuse was detected in 66 children.

Serological assessment of acyclovir treatment of herpes genitalis.

Oral acyclovir was given to 60 patients with herpes genitalis--20 experiencing a first attack and 40 with recurrent attacks. All patients were followed up for 1 year. Serial serum samples from the patients as well as from 20 controls were studied to determine the effect of therapy on the immune response to herpes simplex virus (HSV). No toxicity was observed, and very few patients had rather insignificant side effects (e.g., diarrhea). The frequency of recurrence (number of recurrences per year) of genital herpes in acyclovir-treated patients was found significantly lower than in controls. Mor
