First generation H₁ histamine receptor antagonists, such as d-chlorpheniramine (d-CPA) and diphenhydramine, produce drowsiness in humans. They are currently used as over-the-counter sleep aids. However, the mechanisms underlying drowsiness induced by these H₁ histamine receptor antagonists remain obscure because they produce heterogeneous receptor-independent actions. Ketotifen is a second generation H₁ histamine receptor antagonist which is more permeable to the brain than newer H₁ histamine receptor antagonists. Therefore, to access sleep-inducing profiles by H₁ histamine receptor blocking a

OBJECTIVE: Memory impairment is common in people with temporal lobe epilepsy (TLE). Recent studies in healthy subjects showed a positive correlation between sleep spindles coupled to slow waves (SWs) and memory performance. We aimed to determine differences in spindle-SW coupling in TLE patients compared to healthy controls using combined high-density electroencephalography and polysomnography.
METHODS: The study population consisted of 20 patients (12 female, 36.5 ± 9.9 years old) with unilateral drug-resistant TLE (10 left temporal) and 20 age- and sex-matched controls (12 female, 31.2 ± 6.

Sleep electroencephalogram (EEG) and cranial computed tomography (CT) were performed in 15 drug-free inpatients with an anxiety disorder. The sleep EEG of the patients was characterized by a decreased sleep efficiency and a shortened REM latency. Under visual inspection, four (27%) patients showed ventricular enlargement, while an increased ventricular brain ratio (VBR) was measured in five (33%). No clear associations between EEG sleep measures and ventricular size became obvious, although there is good evidence in the literature that sleep continuity, slow-wave sleep, or REM sleep are closel

We sought to clarify the effect of short-acting benzodiazepine hypnotic on the relationship of arterial blood pressure and arterial partial pressure of carbon dioxide (Paco2) to regional cerebral blood flow (rCBF) during human non-rapid-eye-movement (non-REM) sleep. Nine young normal volunteers were treated in a randomized, crossover design with triazolam or placebo and underwent positron emission tomography at night. During wakefulness and stage 2 and slow wave (stages 3 and 4) sleep, we measured mean arterial blood pressure (MAP), Paco2, and absolute CBF. With triazolam compared to placebo,

Christianson syndrome (CS) is an X-linked intellectual disorder caused by mutations in the SLC9A6 gene. Clinical features of CS include an inability to speak, truncal ataxia, postnatal microcephaly, hyperkinesis, and epilepsy. Almost all patients with CS develop drug-resistant epilepsy-its most serious complication. We report two cases of CS with drug-resistant epilpesy associated with the Lennox-Gastaut syndrome (LGS). One patient experienced generalized tonic seizures since 9 months of age with cognitive regression, which evolved to include atonic seizures at the age of 7 years. Electroencep

OBJECTIVE: The study was performed in early middle-aged African natives with primary open-angle glaucoma to compare the 24-hour intraocular pressure (IOP) variations in healthy versus young glaucoma patients, because IOP follows a circadian (24-hour) oscillation in healthy Caucasians.
DESIGN: Case-control study.
PARTICIPANTS: Sixteen healthy African volunteers (age 24.5 +/- 1 years, mean +/- standard error of the mean) and 11 open-angle glaucoma African patients (age 36.2 +/- 3.3 years).
METHODS: IOP was measured hourly during 24 hours with a Modular One pneumatonometer (Modular One, Digila

Women with severe premenstrual syndrome report sleep-related complaints in the late-luteal phase, but few studies have characterized sleep disturbances prospectively. This study evaluated sleep quality subjectively and objectively using polysomnographic and quantitative electroencephalographic measures in women with severe premenstrual syndrome. Eighteen women with severe premenstrual syndrome (30.5 ± 7.6 years) and 18 women with minimal symptoms (controls, 29.2 ± 7.3 years) had polysomnographic recordings on one night in each of the follicular and late-luteal phases of the menstrual cycle. INTRODUCTION: Sleep-dependent memory consolidation is impaired early in Alzheimer's disease (AD). Closed-loop auditory stimulation (CLAS) of sleep slow oscillations enhances memory consolidation in healthy individuals, but its efficacy in AD remains unknown.
METHODS: We conducted a randomized, double-blind, placebo-controlled crossover trial in 37 patients with biomarker-confirmed early symptomatic AD and an amnestic clinical phenotype. We assessed the effect of one night of CLAS, compared with one night of sham stimulation, on declarative memory retention, sleep electrophysiology, and sleep

Nocturnal sleep of 14 patients with HIV infection was characterized by longer sleep onset latency, shorter total sleep time, reduced sleep efficiency, more time spent awake and in Stage 1. There was significantly less sleep Stage 2 than in healthy controls. REM latency was slightly reduced and correlated negatively with depressive symptomatology, while percentages of REM and slow wave sleep were normal. Patients without complaints at the time of the investigation exhibited similar sleep abnormalities. The results stress the usefulness of polysomnography as a sensitive methodology for detection

Chronic opioid use has been known to cause disturbed sleep as well as excessive daytime sleepiness. During induction and maintenance of opioid use there is a reduction of REM- and slow wave sleep. Central sleep apnea (CSA) has been reported in about 30 % of patients with chronic opioid use. Ataxic breathing and CSA are more prominent in NREM- than REM-sleep. CSA does not seem the sole cause of excessive daytime sleepiness in these patients. Further studies are necessary regarding the effects and consequences of chronic opioid use during sleep.

15 patients with polysomnographically verified syndrome of obstructive apnoea in sleep (apnoea index value was equal to 35.6) were examined. The peculiarities of cardiac rhythm vegetative regulation were investigated with the help of cardiorhythmintervalography technique. The decrease of amplitude of the middle-frequency peak (the slow waves of the first order, SW1) as well as the predominance of the low-frequency peak (slow waves of the second order, SW2) were observed in cardiac rhythm spectrum. These findings reflected the insufficiency in sympathetic-adrenal mechanisms of regulation. Besid

PURPOSE: Reduced rapid-eye movement (REM) sleep has been linked to increased mortality in the general population. We investigated whether diminished REM sleep is associated with higher mortality in adults with coronary artery disease (CAD) and obstructive sleep apnea (OSA).
METHODS: This secondary analysis of the RICCADSA trial included 356 revascularized CAD patients with OSA (apnea-hypopnea index [AHI] ≥ 15 events/h) and total sleep time (TST) ≥ 240 min on baseline polysomnography. Reduced REM sleep was defined as the lowest quartile of REM percentage. Cox proportional hazards models assess

OBJECTIVES: Slow waves of intracranial pressure (ICP) are spontaneous oscillations with a frequency of 0.3-4 cycles/min. They are often associated with pathological conditions, following vasomotor activity in the cranial enclosure. This study quantifies the effects of general anaesthesia (GA) on the magnitude of B-waves compared with natural sleep and the conscious state.
MATERIALS AND METHODS: Four groups of 30 patients each were formed to assess the magnitude of slow waves. Group A and group B consisted of normal pressure hydrocephalus (NPH) patients, each undergoing cerebrospinal fluid (CS

The mechanisms underlying the generation of hippocampal epileptic seizures and interictal events and their interactions with the sleep-wake cycle are not yet fully understood. Indeed, medial temporal lobe epilepsy is associated with hippocampal abnormalities both at the neuronal (channelopathies, impaired potassium and chloride dynamics) and network level (neuronal and axonal loss, mossy fiber sprouting), with more frequent seizures during wakefulness compared with slow-wave sleep. In this article, starting from our previous computational modeling work of the hippocampal formation based on rea

Exteroception is attenuated during sleep, while interoceptive signals may remain active. If so, they can potentially trigger responses in their target areas, including the hippocampus, which is known to receive information from the internal organs. We investigated whether the hippocampus is synchronized to various visceral events in sleep and wakefulness. Activity of hippocampal neurons and local field potentials (LFPs) was coregistered with respiration, heart rate, and myoelectric signals of the stomach and duodenum in two adult female cats over multiple sleep-wake cycles. Drug delivery to the central nervous system remains a major problem due to biological barriers. The blood-brain-barrier can be bypassed by administering drugs intrathecally directly to the cerebrospinal fluid (CSF). The glymphatic system, a network of perivascular spaces promoting fluid exchange between CSF and interstitial space, could be utilized to enhance convective drug delivery from the CSF to the parenchyma. Glymphatic flow is highest during sleep and anesthesia regimens that induce a slow-wave sleep-like state. Here, using mass spectrometry and fluorescent imaging techniques, we show t

Attention-deficit/hyperactivity disorder is the most common neurobehavioral disorder in children and frequently associated with epilepsy. For patients with both conditions, methylphenidate remains a mainstay in the treatment of behavioral problems. Most studies demonstrate that methylphenidate is effective in treating children with well-controlled epilepsy, and that methylphenidate does not increase the risk of having seizures in patients with EEG abnormalities without epilepsy. However, in patients with active seizures, the results are somewhat contradictory. This article presents the case of

Persons in the completely locked-in state (CLIS) suffering from amyotrophic lateral sclerosis (ALS) are deprived of many zeitgebers of the circadian rhythm: While cognitively intact, they are completely paralyzed, eyes mostly closed, with artificial ventilation and artificial nutrition, and social communication extremely restricted or absent. Polysomnographic recordings in eight patients in CLIS, however, revealed the presence of regular episodes of deep sleep during night time in all patients. It was also possible to distinguish an alpha-like state and a wake-like state. Classification of rap

Electrical status epilepticus in sleep (ESES) describes an electroencephalographic pattern showing significant activation of epileptiform discharges in sleep. The terms continuous spike wave in slow-wave sleep (CSWS) and Landau-Kleffner syndrome (LKS) describe the clinical epileptic syndromes seen with ESES. Although there is an overlap between these 2 syndromes, children with CSWS present with a more global regression have more problematic epilepsy and have EEG foci located predominantly in frontotemporal or frontocentral regions. In contrast, children with LKS present with an acquired audito
