Long-Term Velopharyngeal Insufficiency-Related Quality of Life in Cleft Palate Patients: Speech and Surgical Factors.

INTRODUCTION AND AIMS: Velopharyngeal insufficiency (VPI) is a common functional complication in patients with cleft palate, often resulting in impaired speech, social difficulty, and psychosocial distress. This study aims to assess VPI-related quality of life (VPI-QOL) in cleft palate patients after primary palatoplasty, and to identify clinical and functional predictors, particularly speech outcomes and postoperative complications, that significantly affect the perceived QOL. METHODS: This cross-sectional study involved 170 cleft palate patients assessed using the validated Chinese version

Transcutaneous cervical vagus nerve stimulation improves speech comprehension in noise: A crossover, placebo-controlled study.

Speech comprehension in noisy environments remains a significant challenge, even among individuals with clinically normal hearing and users of hearing aids and cochlear implants. While conventional assistive hearing devices address limitations in the auditory periphery, they do not directly enhance the brain's capacity to segregate speech from background noise. Because tonic vagus nerve stimulation (VNS) has demonstrated potential for rapidly improving central sensory processing, this study investigated whether tonic transcutaneous cervical VNS (tcVNS) can augment speech-in-noise intelligibili

Amplification rationale for hearing aids based on characteristics of the Japanese language.

OBJECTIVE: Hearing aid amplification rationales have typically been developed by using global averages of the long-term average speech spectrum (LTASS) from Western European languages. However, there are few reports on hearing-aid amplification based on acoustic-phonetic characteristics of the Japanese language. This study's objective is to investigate the LTASS for Japanese, and to compare a typical amplification rationale originally developed mainly for Western European languages with an amplification rationale specifically adjusted to the LTASS for Japanese. METHODS: LTASS for two speech m

The prevalence of verbal communication disability in patients with Parkinson's disease.

Previous studies of speech dysfunction in patients with Parkinson's disease (PD) consisted of postal surveys, which rely on the subjective opinions of patients, or were primarily focused on the detailed assessment of impairments without addressing the disabilities resulting from them. Similarly, the impact of the communication disability on the patient's everyday life was not assessed. The aim of the present study was to examine the prevalence of verbal communication disability in these patients, using a validated objective measurement tool. A secondary aim was to establish the patients' aware

Comparison of in-the-ear and over-the-ear hearing aid fittings.

A custom in-the-ear (ITE) hearing aid fitting was compared to two over-the-ear (OTE) hearing aid fittings for each of 9 subjects with mild to moderately severe hearing losses. Speech intelligibility via the three instruments was compared using the Speech Intelligibility Rating (SIR) test. The relationship between functional gain and coupler gain was compared for the ITE and the higher rated OTE instruments. The difference in input received at the microphone locations of the two types of hearing aids was measured for 10 different subjects and compared to the functional gain data. It was conclud

Boundary conditions for cortical speech tracking as an objective speech-in-noise marker: a three-dataset MEG/EEG benchmark.

Objective electrophysiological markers are often proposed for speech-in-noise assessment, but their validation targets are not always explicit: a marker may calibrate with intelligibility, distinguish acoustic conditions, decode attention, or predict an individual speech reception threshold (SRT). We used three public MEG/EEG datasets to test these endpoints within one validation framework. In MEG-SCANS (22 normal-hearing listeners), delta-band sentence-level envelope reconstruction increased with target intelligibility (Friedman chi-square=31.27, p = 8.3e-6). However, individual curves were r

Speech Analysis Using Artificial Intelligence as a Peri-Operative Evaluation: A Case Report of a Patient with Temporal Lobe Epilepsy Secondary to Tuberous Sclerosis Complex Who Underwent Epilepsy Surgery.

BACKGROUND: Improved conversational fluency is sometimes identified postoperatively in patients with epilepsy, but improvements can be difficult to assess using tests such as the intelligence quotient (IQ) test. Evaluation of pre- and postoperative differences might be considered subjective at present because of the lack of objective criteria. Artificial intelligence (AI) could possibly be used to make the evaluations more objective. The aim of this case report is thus to analyze the speech of a young female patient with epilepsy before and after surgery. METHOD: The speech of a nine-year-old

Improving hearing-aid gains based on automatic speech recognition.

This study provides proof of concept that automatic speech recognition (ASR) can be used to improve hearing aid (HA) fitting. A signal-processing chain consisting of a HA simulator, a hearing-loss simulator, and an ASR system normalizing the intensity of input signals was used to find HA-gain functions yielding the highest ASR intelligibility scores for individual audiometric profiles of 24 listeners with age-related hearing loss. Significantly higher aided speech intelligibility scores and subjective ratings of speech pleasantness were observed when the participants were fitted with ASR-estab

Experimental Investigation of Acoustic Features to Optimize Intelligibility in Cochlear Implants.

Although cochlear implants work well for people with hearing impairment in quiet conditions, it is well-known that they are not as effective in noisy environments. Noise reduction algorithms based on machine learning allied with appropriate speech features can be used to address this problem. The purpose of this study is to investigate the importance of acoustic features in such algorithms. Acoustic features are extracted from speech and noise mixtures and used in conjunction with the ideal binary mask to train a deep neural network to estimate masks for speech synthesis to produce enhanced sp
