Intralabyrinthine schwannomas : Surgical management and hearing rehabilitation with cochlear implants.

Intralabyrinthine schwannomas (ILS) are a rare differential diagnosis of sudden hearing loss and vertigo. In an own case series of 12 patients, 6 tumors showed an intracochlear, 3 an intravestibular, 1 a transmodiolar including the cerebellopontine angle (CPA), 1a transotic including the CPA, and 1 a multilocular location. The tumors were removed surgically in 9 patients, whereas 3 patients decided for a "wait-and-test-and-scan" strategy. Of the surgical patients, 3 underwent labyrinthectomy and cochlear implant (CI) surgery in a single-stage procedure; 1 patient had extended cochleostomy with

A comparison of two word-recognition tasks in multitalker babble: Speech Recognition in Noise Test (SPRINT) and Words-in-Noise Test (WIN).

BACKGROUND: The Speech Recognition in Noise Test (SPRINT) is a word-recognition instrument that presents the 200 Northwestern University Auditory Test No. 6 (NU-6) words binaurally at 50 dB HL in a multitalker babble at a 9 dB signal-to-noise ratio (S/N) (Cord et al, 1992). The SPRINT was developed by and used by the Army as a more valid predictor of communication abilities (than pure-tone thresholds or word-recognition in quiet) for issues involving fitness for duty from a hearing perspective of Army personnel. The Words-in-Noise test (WIN) is a slightly different word-recognition task in a f

Mucoepidermoid carcinoma of the external auditory canal (EAC).

We present the third case of mucoepidermoid carcinoma of the external auditory canal (EAC) in the English literature, and discuss the management of this lesion. The patient underwent a wide local resection, superficial parotidectomy, and selective neck dissection. Although intraoperative frozen section margins were negative, permanent histopathologic examination demonstrated tumor in the medial margin, and the tumor was upgraded to a high-grade mucoepidermoid carcinoma. The patient returned to the operating room for a wider local resection, and EAC reconstruction with a temporoparietal pedicle

Auditory disorder in central nervous system miliary tuberculosis: case report.

We evaluated a 28-year-old female with a unilateral hearing loss of unusual pathogenesis, that of central nervous system miliary tuberculosis. Audiologic and otologic findings were consistent with left retrocochlear disorder, characterized by a profound hearing sensitivity loss, absent acoustic reflexes, normal otoacoustic emissions, and the presence of only wave I of the auditory brainstem response. Imaging studies revealed the presence of multiple punctate lesions, one of which was extra-axial and located at the left cerebellopontine angle. The pattern of audiometric test results, particular

A new stapedotomy prosthesis: preliminary report.

OBJECTIVE: Rheumatoid arthritis (RA) can be described as a chronic, inflammatory, progressive, autoimmune disorder characterized by generalized inflammation of the synovial joints, which hereby triggers the progressive erosion of both cartilage and bone. Anakinra is a recombinant form of human IL-1 receptor antagonist which targets the type I IL-1 receptor. In the present systematic review, we intend to evaluate the effectiveness and adverse effects of interleukin-1 antagonists in the treatment of rheumatoid arthritis. MATERIALS AND METHODS: The database search was carried out using PubMed (M

[Meniere's disease. The clinical effect of gentamicin treatment].

In patients with Menière's disease attacks of vertigo can be invalidating. We have tested the effect of instillation of gentamycin in the middle ear to minimize or stop the attacks of vertigo. The results of a prospective investigation of 14 patients with Menière's disease treated with gentamycin in the period from 1993 to 1996 are presented. The patients were followed up to 53 months after the treatment and a telephone follow-up was performed. At the last follow-up nine out of 12 patients were free of attacks of vertigo. Two improved and one patient was unchanged. In two patients deafness dev

Clinical manifestations of aural fullness.

PURPOSE: Even though aural fullness is ubiquitous among patients presenting to otolaryngology clinics, the association between aural fullness and disease development has not yet been clearly determined. MATERIALS AND METHODS: Our study was performed on outpatients from June 2006 to February 2010 whose major complaint was "ear fullness", "aural fullness", or "ear pressure". We assessed their demographic and clinical characteristics, including sex, associated diseases, symptoms, otoscopic findings, audiology test results, and final diagnoses. RESULTS: Among 432 patients, 165 (38.2%) were males

Perilymphatic fistulas in children: rationale for therapy.

We report 26 consecutive patients (32 ears) who were identified in a 2 year period (July 1, 1985-June 30, 1987) with unexplained sudden, fluctuating, or progressive sensorineural hearing loss (SNHL). All patients underwent an exploratory tympanotomy and a perilymphatic fistula was identified in 13 patients (14 ears). The mean change of 14 +/- 27 dB in speech reception threshold before and after surgery was significant at p = 0.08 among children with fistula and ranged from -30 to 80 dB. In children with sudden, progressive or fluctuating SNHL and multiple sensory deficits, including blindness

Comparison of One-Year auditory rehabilitation outcomes by etiology in pediatric patients with bilateral severe hearing loss (70-90 dB): enlarged vestibular aqueduct vs. Other causes.

PURPOSE: This study aimed to compare short-term language outcomes following hearing aid rehabilitation in pediatric patients with severe bilateral hearing loss (70-90 dB), with a particular focus on differences according to etiology. We hypothesized that children with enlarged vestibular aqueduct (EVA) exhibit more favorable speech development compared to those with other genetic or structural causes of hearing loss, and explored the potential presence of a "hidden" air-bone gap associated with EVA. METHODS: We retrospectively reviewed 36 children under five years of age diagnosed with bilate

[Evaluation of deprivation manifestations of the auditory system in patients with unilateral middle ear deafness before and after surgical therapy].

BACKGROUND: This paper will discuss whether long term conductive hearing loss leads to late onset auditory deprivation as described in the literature in patients suffering from bilateral sensorineural hearing loss and fitted with monaural hearing aids. METHOD: The patients were examined using the Freiburg speech test in the undistorted and distorted forms (according to Dieroff 1985), and the pure tone threshold was ascertained for each patient. Thirty-six preoperative und postoperative results after tympanoplasty and stapes surgery were evaluated. Follow-up was performed six weeks and six mon

An exploratory look at pediatric cochlear implantation: is earliest always best?

OBJECTIVES: Since the advent of cochlear implants, age at implantation has declined as investigators report greater benefit the younger a child is implanted. Infants younger than 12 mos currently are excluded from Food and Drug Administration clinical trials, but have been implanted with Food and Drug Administration-approved devices. With a chance that an infant without profound hearing loss could be implanted because of the limitations of the diagnostic measures used with this population and the potential for additional anesthetic risks to infants younger than 1-yr-old, it is prudent to evalu

Unexpected complication of posterior canal occlusion surgery for benign paroxysmal positional vertigo.

OBJECTIVE: The purpose of this report was to illustrate how an unusual complication of posterior canal occlusion surgery for benign paroxysmal positional vertigo (BPPV) may be recognized and prevented. Recurrence of BPPV after occlusion surgery of the posterior semicircular canal has not previously been reported in the literature, to the authors' knowledge. Failure of occlusion not only permits the continued symptoms of BPPV but also burdens the patient with the additional morbidity of a fistula of the PSCC. The authors describe the successful treatment of a patient with recurrent and incapaci

Speech-in-Noise Hearing Impairment and Incident Dementia: A UK Biobank Cohort Study Across Polygenic, APOE ε4, and Familial Risk Strata.

OBJECTIVE: Hearing impairment is considered an important modifiable risk factor for dementia, but whether its association with incident dementia is consistent across inherited susceptibility profiles remains unclear. METHODS: This prospective cohort study included 80,287 UK Biobank participants aged 40 to 69 years at baseline with valid Digit Triplet Test measurements, genetic data, linked follow-up data, and complete prespecified covariates. Hearing function was quantified using the better-ear speech reception threshold (SRT). Incident dementia was ascertained from linked hospital admission

Relationship of Hearing Loss to Parkinson's Disease, Dementia, and APOE Genotype in Adults.

Background: Hearing loss has been recognized as a risk factor for dementia and non-motor features of Parkinson's disease (PD). The apolipoprotein E (APOE) protein contributes to maintenance and repair of neuronal cell membranes, causing age-related disorders. This study aimed to analyze the impact of hearing loss on cognitive impairment, PD severity, and APOE gene expression in these patients. Methods: A total of 72 out-patients diagnosed with either PD or hearing loss were enrolled in this study. The hearing assessment included pure-tone audiometry, speech reception thresholds, and speech dis

Binaural pre-processing for contralateral sound field attenuation can improve speech-in-noise intelligibility for bilateral hearing-aid users.

We have recently proposed a binaural sound pre-processing method to attenuate sounds contralateral to each ear and shown that it can improve speech intelligibility for normal-hearing (NH) people in simulated "cocktail party" listening situations (Lopez-Poveda et al., 2022, Hear Res 418:108,469). The aim here was to evaluate if this benefit remains for hearing-impaired listeners when the method is combined with two independently functioning hearing aids, one per ear. Twelve volunteers participated in the experiments; five of them had bilateral sensorineural hearing loss and seven were NH listen
