Intradural extramedullary epidermoid cyst of the spinal canal: report of one case.

A 9-year-old girl developed a progressive unsteady gait for one month. Hyperactive reflexes of the four extremities and a spasticity of the lower extremities were found. Bilateral Babinski responses were elicited. An intradural extramedullary mass of upper thoracic spinal cord was demonstrated using magnetic resonance imaging and was confirmed as an intradural extramedullary epidermoid cyst at surgery. This epidermoid cyst was from congenital origin. No congenital anomalies such as spina bifida or dermal sinus were associated. The epidemiology, clinical features, radiology, surgical treatment

Myeloscopic observation of adhesive arachnoiditis in patients with lumbar spinal canal stenosis.

In this study a myeloscope was used to assess the influence of adhesive arachnoiditis on the surgical outcome of patients with lumbar spinal canal stenosis (LSS). The presence or absence and the degree of the complication of adhesive arachnoiditis were preoperatively determined by myeloscopy in 36 patients with LSS, and these findings were compared with the postoperative results. Various degrees of adhesive changes in the cauda equina were observed in all 36 patients. Patients with marked adhesions, which may indicate a blocked cauda equina, had distinctly worse operative results than did pati

Electrophysiologic evaluation of intermittent sacral nerve dysfunction in lumbar spinal canal stenosis.

Intermittent sacral nerve dysfunction, induced by walking in lumbar spinal canal stenosis, may cause symptoms including hyperesthesia and pain in perineum, urinary bladder incontinence, and penile erection. However, it is difficult to objectively document these symptoms. Evoked external urethral sphincter potentials by conus medullaris stimulation were measured both before walking and after walking in five patients who complained of sexual organ or urinary dysfunction during walking, and were compared with simultaneously induced sacral nerve symptoms. These potentials were also measured during

The central (spinal) canal in congenital murine hydrocephalus: morphological and physiological aspects.

Obstruction of cerebrospinal fluid pathways produces changes in the parenchyma of the brain and spinal cord. A murine model of hydrocephalus (hy-3) used to study changes in the central canal of the spinal cord and these changes are correlated with events occurring in the intracranial compartment. The central canal is enlarged and subependymal interstitial edema is produced by cerebrospinal fluid at the same time as the intracranial ventricular system is expanding. Plugging of the obex results in collapse of the central canal. Ferritin tracer studies demonstrate transependymal flow of cerebrosp

Gouty tophi in the thoracic spinal canal with incomplete paraplegia: a case report.

Gout manifesting in the spine is a relatively rare occurrence. Involvement of the lumbar spine is more common in cases of spinal gout, while reports of gouty tophi forming within the thoracic spinal canal are exceedingly rare. This article reports a case of incomplete paraplegia caused by gouty tophi in the thoracic spinal canal. The patient, a 56-year-old man, returned to normal after undergoing surgical treatment and was monitored for ten months postoperatively. This case highlights the importance of early diagnosis of spinal gout to prevent the progression of neurological symptoms. Surgical

The Impact of Frailty on Surgical Outcome of Patients with Lumbar Spinal Canal Stenosis.

INTRODUCTION: Frailty is an important factor in surgical outcomes. The current study aimed to evaluate the effect of preoperative frailty on postoperative outcomes in older patients with lumbar spinal canal stenosis (LSCS). METHODS: We retrospectively examined 209 patients aged ≥65 years who underwent surgery for LSCS. Health-related quality-of-life (HRQOL) tools, including the Roland-Morris Disability Questionnaire (RDQ), Zurich Claudication Questionnaire (ZCQ), and Japanese Orthopaedic Association Back Pain Evaluation Questionnaire (JOABPEQ), were used in the assessment conducted before sur

Gravity determines the direction of nerve roots sedimentation in the lumbar spinal canal.

OBJECTIVES: To investigate the role of gravity in the sedimentation of lumbar spine nerve roots using magnetic resonance (MR) imaging of various body positions. METHODS: A total of 56 patients, who suffered from back pain and underwent conventional supine lumbar spine MR imaging, were selected from sanmen hospital database. All the patients were called back to our hospital to perform MR imaging in prone position or lateral position. Furthermore, the sedimentation sign (SedSign) was determined based on the suspension of the nerve roots in the dural sac on cross-sectional MR images, and 31 case

Effects of a single-bolus bupivacaine injection into the coccygeal spinal canal of rabbits.

It has been reported that drugs intended for epidural administration through the lumbosacral junction are accidentally administered into the subarachnoid space frequently in rabbits. Therefore, we evaluated the epidural single-bolus injection technique for the administration of bupivacaine into the coccygeal spinal canal of rabbits. After epidural distribution was confirmed by the injection of iohexol into the coccygeal spinal canal, 0.3 ml/kg 0.5% bupivacaine or 0.3 ml/kg normal saline was injected via the same needle. After the first attempt of iohexol injection, although the contrast was fo

Adrenal Cortical Adenoma in the Spinal Canal: A Case Report and Review of the Literature.

Ectopic adrenal cortical neoplasms of the spinal cord are extremely rare. To date only 10 such cases have been described. We present a case of a 46-year-old woman with lower back pain radiating to the right gluteal and posterior femoral regions, without a history of traumatic injury. Magnetic resonance imaging (MRI) of the thoracic and lumbar spine showed an intradural, extramedullary, well-circumscribed, contrast-enhancing lesion located in the T12-L1 region, hypo- to isointense on T2-weighted imaging, and isointense on T1. Complete surgical removal of the lesion, measuring 3 × 2.5 × 1 cm, wa

Automated boundary extraction of the spinal canal in MRI based on dynamic programming.

The spinal cord is the only communication link between the brain and the body. The abnormalities in it can lead to severe pain and sometimes to paralysis. Due to the growing gap between the number of available radiologists and the number of required radiologists, the need for computer-aided diagnosis and characterization is increasing. To ease this gap, we have developed a computer-aided diagnosis and characterization framework in lumbar spine that includes the spinal cord, vertebrae, and intervertebral discs. In this paper, we propose two spinal cord boundary extraction methods that fit into

Measurement of occlusion of the spinal canal and intervertebral foramen by intervertebral disc bulge.

BACKGROUND: Disc protrusion has been proposed to be a possible cause of both pain and stenosis in the lower spine. No previous study has described the amount of disc occlusion of the spinal canal and intervertebral foramen that occurs under different loading conditions. The objective of this study was to quantitatively assess the percent occlusion of the spinal canal and intervertebral foramen by disc bulge under different loading conditions. METHODS: Spinal canal depth and foraminal width were measured on computed tomography-scanned images of 7 human lumbar spine specimens. In vitro disc bul

The anatomy of the thoracic spinal canal investigated with magnetic resonance imaging (MRI).

BACKGROUND AND OBJECTIVES: Anesthesiologists are reluctant to consider higher levels for spinal anesthesia, largely due to direct threats to the spinal cord. The goal of this study is to investigate, with magnetic resonance imaging (MRI), the distances between the relevant structures of the spinal canal (spinal cord, thecal tissue, etc.) to determine modal anatomical positions for neuraxial anesthesia. METHOD: A group of 19 patients were imaged with an MRI scanner in supine position. Medial sagittal slices of the thoracic and lumbar spine were measured for the relative distances between anato

Spinal canal size in ossification of the posterior longitudinal ligament of the cervical spine.

BACKGROUND: The size of the spinal canal is a factor that contributes to the neurologic deficits associated with cervical ossification of the posterior longitudinal ligament (OPLL). METHODS: Bone-window computed tomography (CT) examinations of the cervical spine in 64 patients with cervical OPLL were reviewed. Forty-two patients underwent surgical treatment (anterior decompression: 16 patients, posterior decompression: 26 patients). The remaining 22 patients were managed conservatively. Selection of the surgical approach, anterior or posterior, was based on the longitudinal extent of cord com

Posttraumatic sagittal osseous bar in the spinal canal of an adult: a case report.

Diastematomyelia is a congenital anomaly wherein the distal spinal canal is bisected by a longitudinally oriented septum made up of fibrous tissue, cartilage, or bone. The main lesion is expressed by characteristic gait disturbances and dysfunction of the anal and vesical sphincters resulting from damage to the cord or cauda equina. Symptoms almost invariably begin during childhood. By the time symptoms appear, the damage is largely irreversible. In rare instances, the neuropathic expression of diastematomyelia is delayed until adult life. Only a dozen such cases have been reported. Described
