Building a virtual reality temporal bone dissection simulator.

The temporal bone is one of seven bones that comprise the human skull, and has an intimate relationship with many vital structures. Anatomically, its three-dimensional relationships make it one of the most challenging areas for surgeons to understand and master. In addition, the temporal bone contains minute structures that are among the most sophisticated and delicate in the human body. These structures include the cochlea and vestibular organs, which are responsible for hearing and balance; the middle ear, including the ossicles, which conduct acoustic energy to the cochlea; and the facial n

Giant cell tumor of the temporal bone (osteoclastoma).

A 49-year-old white female patient was referred to the ENT Department, Zentralkrankenhaus, Bremen, because of conductive hearing loss. Widespread tumor formation was found in the left temporal bone and was identified by biopsy to be an osteoclastoma. The tumor was extirpated surgically and reconstruction was performed. Periodic clinical follow-ups during a 36-month period after surgery have included control computed tomographic scans and have shown no evidence for recurrent disease. The specific pathology of the osteoclastoma is presented and different ways for treatment are discussed.

Fibrous dysplasia of the temporal bone: imaging findings.

Lichtenstein [1] in 1938 coined the term fibrous dysplasia to describe a disorder characterized by the progressive replacement of normal bone elements by fibrous tissue. Histopathologically, these lesions consist of an abnormal proliferation of fibrous elements intermixed with haphazardly arranged trabeculae of woven bone. The disease can involve any bone in the body. In the head and neck, the skull and facial bones are involved in 10-25% of cases of monostotic fibrous dysplasia and in 50% of the polyostotic variety. Involvement of the temporal bone, however, is relatively rare, and only 53 ca

Transpetrosal access to meningiomas juxtaposing the temporal bone.

Meningiomas arising from the petroclival dura have presented a challenge to both otolaryngologists and neurosurgeons. Access by means of subtemporal, suboccipital, translabyrinthine, or transcochlear routes have inherent limitations both surgically as well as from resultant morbidity. Since 1988, the authors have used a transpetrosal transtentorial (combined subtemporal suboccipital presigmoid) approach to these lesions in selected cases. The goals of this technique have been to preserve function of the cranial nerves, to avoid excessive retraction of the temporal lobe and cerebellum, and to e

[Extracanalicular osteomas of the temporal bone: three cases].

Osteoma is a slow-growing tumor formed by mature bone tissue. In the temporal bone, intracanalicular osteomas are more frequent than extracanalicular osteomas. In Spain, only twelve cases of extracanalicular tumors of the temporal bone have been described in the literature. We present three new cases of extracanalicular osteoma (mastoids). The clinical and radiological features of each case are described and the relevant literature is reviewed.

Temporal bone pathology in patients without caloric response.

Pathologies of serial sections of 9 temporal bones from 6 patients without caloric response were studied. The patients died of epipharyngeal cancer, middle ear cancer, chronic renal failure with DIC, pancreatic cancer with DIC, multiple brain metastasis of pancreatic cancer, and leukemia. Under light microscopy, 8 ears showed pathologies in the vestibular end organs and the nerves. Possible causes of the lack of caloric responses in the 8 ears were endolymphatic hemorrhage, labyrinthitis, leukemic infiltration in the labyrinth, degeneration or disappearance of the vestibular sensory cells, red

Otosclerosis: the University of Minnesota temporal bone collection.

A study of 1452 human temporal bones revealed a previously unpublished material of 144 bones with otosclerosis. After exclusion of infants and individuals of races other than white, the incidence of otosclerosis was 12.75%. Of the bones with otosclerosis, 56.1% belonged to men and 43.9% to women. The incidence of clinical and histologic otosclerosis was practically the same for men (44.7% to 55.3%) as for women (47% to 53%). However, the incidence of bilateral otosclerosis was higher in women (88.9%) than in men (65.2%). Bilateral otosclerosis was present in 75.6%, whereas it was unilateral in

Total temporal bone resection for squamous cell carcinoma.

Because of unsatisfactory results in treatment of malignant conditions of the temporal bone, a technique for total en bloc resection of the temporal bone and carotid artery was reported by Graham et al. in 1984. The procedure involves resection of the internal carotid artery, cranial nerves VI through XII, and structures adjacent to the temporal bone. Experience with two additional cases led to numerous modifications in the recommended procedure, as reported by Sataloff and Myers. Additional clinical experience with this technique and its complications has resulted in further modification. Add

Auditory and temporal bone abnormalities in CHARGE association.

CHARGE association is a recently described cluster of congenital defects including ocular coloboma, heart disease, choanal atresia, retarded development and/or CNS abnormalities, genital hypoplasia, and ear anomalies. Although congenital hearing loss has been reported in CHARGE association, no information regarding the underlying temporal bone disease is available in the literature to date. The authors evaluated four patients with multiple anomalies consistent with CHARGE syndrome. Two surviving patients have bilateral severe hearing loss on auditory brain stem response testing. Two patients d

Cockayne syndrome--an audiologic and temporal bone analysis.

Cockayne's syndrome is a triad of dwarfism, retinal atrophy, and deafness. Over thirty cases have been presented in the literature. We have examined and audiometrically tested three patients (ages 13 to 17) with confirmed Cockayne's syndrome and have analyzed the temporal bones of another who died at age 24. To our knowledge this is the first reported temporal bone analysis of a patient with Cockayne's syndrome. Audiograms revealed bilateral symmetric sensorineural hearing loss that was greatest in the high frequencies. Temporal bone examinations revealed inner and outer hair cell losses in th

The petrous temporal bone (including the cerebellopontine angle).

Lower resolution CT scanners play an important role in the evaluation of the cerebellopontine angle, but careful integration along with the older conventional examinations is often necessary. A high-resolution CT scanner may provide all the necessary information. With proper techniques, bony changes are most clearly analyzed, and small tumors of the acoustic nerve may be directly seen in the cerebellopontine angle, and even in the internal auditory canal, with only a contrast-enhanced scan. If necessary CT-air cisternography provides the most sensitive and least morbid of the definitive, invas

High resolution computed tomography of the temporal bone.

The radiation dose and the anatomic detail of computed tomography (CT) and conventional tomography were studied in 18 patients with suspected acoustic neuroma, cholesteatoma, malignant tumor, and temporal bone fracture. It is concluded that CT is the technique of choice in patients suspected of having acoustic neuromas or malignant tumors of the temporal bone, while hypocycloidal tomography should be used to evaluate pateints with otosclerosis, small cholesteatomas, and inflammatory disease of the middle ear. For diagnosing fractures and large cholesteatomas, CT and conventional tomography are

Temporal bone histopathological findings in trisomy 13 syndrome.

This study reports the histopathological findings of 14 temporal bones from infants with trisomy 13 syndrome. The most primitive anomalies in the structures of the inner and middle ears in the present series are those of the semicircular canals, particularly of the horizontal canals: flattened horizontal canal cristae, absence or opening of the utricular endolymphatic valve, small facial nerve, and obtuse angle of the geniculate area of the facial nerve. Each ear demonstrated more than one of those anomalies. The anomalies present features similar to those found in the structures of the normal

Bilateral Thinning of the Temporal Bone: A Case Rep.

Bone mass is maintained by a balance between bone formation by osteoblasts and resorption by osteoclasts. Calvarial thinning can occur because of various factors. However, no previous studies have described bilateral temporal thinning (BTT) of the skull. This report presents a case of a squamous part of the bilateral temporal bone in a patient with alcohol-induced liver cirrhosis. This is the first case of BTT exhibiting an appearance similar to that of bilateral parietal thinning (BPT) in a patient with osteoporosis caused by liver cirrhosis. Although the precise pathogenic mechanism underlyi
