Ultrasound as a predictor of time-loss injury for the patellar tendon, Achilles tendon and plantar fascia in division I collegiate athletes.

OBJECTIVES: Tendinopathy and fasciopathy are common conditions that can result in time-loss injury in athletes. This study aimed to determine if preseason sonographic abnormalities of the patellar tendon, Achilles tendon and plantar fascia are associated with future time-loss injuries in collegiate athletes. METHODS: National Collegiate Athletic Association Division I athletes from three institutions participated in this 3-year prospective, observational study. Each athlete completed a questionnaire, self-reporting current and prior symptoms and underwent an ultrasound examination of bilatera

A 2-free-end flexor carpi radialis tendon graft for treating a complex index finger extensor tendon injury.

BACKGROUND: Volar plating has increasingly become the most used technique for the treatment of unstable distal radius fractures due to the low soft-tissue disturbance and its biomechanical reliability, which allows the early mobilization of the wrist. One of the main goals of the volar locking compression plates design is to avoid those soft tissue complications historically associated to the dorsal plating. However, extensor tendon complications can not be completely excluded. METHOD: The authors report the case of a patient with a complete rupture of the index finger extensor tendons after

Late rupture of a flexor tendon after electrical injury: tendon localization using magnetic resonance imaging. A case report.

Rupture of a flexor tendon in the nonrheumatoid population is rare. Localizing the level of the tendon rupture can be a difficult task. We describe an individual who presented with the sudden inability to flex the index finger of the hand, which had been subjected to a severe electrical injury. Physical exam demonstrated lack of index finger flexion at the distal joint. The patient denied any pain or history of recent trauma. Preoperative magnetic resonance imaging (MRI) successfully located the proximal and distal ends of the ruptured flexor digitorum profundus tendon in the forearm. MRI allo

Combined effects of photobiomodulation and platelet-rich plasma on healing of acute Achilles tendon injuries in rats.

Tendon injuries are challenging to treat due to limited intrinsic healing, and although photobiomodulation (PBM) and platelet-rich plasma (PRP) show therapeutic potential, their combined effects remain unclear. This study evaluated the individual and combined efficacy of PBM and PRP on Achilles tendon repair in rats. A total of 144 rats were assigned to six groups and treated with PBM, PRP, or combined PBM + PRP therapy; tissues were collected at 1 and 8 weeks for biochemical and histological assessment. Both PBM and PRP reduced IL-1β and TNF-α levels and improved collagen organization, while

Applicability of combined high-frequency and contrast-enhanced ultrasound in finger extensor tendon injuries: three case reports.

BACKGROUND: By combining high-frequency and contrast-enhanced ultrasound (CEUS), the position of the severed end of a finger extensor tendon injury and the injury classification can be determined as part of a comprehensive preoperative evaluation in clinical practice. However, there have been no reports of high-frequency ultrasound combined with CEUS for the preoperative diagnosis of human finger extensor tendon injury. CASES PRESENTATION: One case of complete rupture of the extensor tendon was diagnosed by ultrasound, which was completely consistent with the surgery; one case of incomplete r

Up to five-week delay in primary repair of Zone 2 flexor tendon injuries: outcomes and complications.

We report the outcomes of delayed primary repair of flexor tendons in Zone 2 in 31 fingers and thumb (28 patients) averaging 15 days (range 4-37) after injury in 2020. The delay was longer than usual due to the COVID-19 pandemic. The tendons were repaired with a 6-strand core suture (M-Tang method) or a double Tsuge suture and a peripheral suture. This was followed by an early, partial-range, active flexion exercise programme. Adhesions in four digits required tenolysis. These patients were not with longest delay. Outcomes of two improved after tenolysis. The other two patients declined furthe

Outcomes After Primary Repair and Staged Reconstruction of Zone I and II Flexor Tendon Injuries in Children.

BACKGROUND: Delayed diagnosis of flexor tendon injury in children is common, and consequent flexor sheath scarring may necessitate a 2-stage reconstruction. Previous studies show variable outcomes after 2-stage flexor reconstruction in children, especially those below 6 years old. We evaluated functional and subjective outcomes of primary repair and staged reconstruction of zone I and II tendon injuries in children under 6 years of age. METHODS: A retrospective chart review identified 12 digits in 10 patients who had undergone surgical treatment of a zone I or II flexor tendon injury. Seven d

[Analysis of Musculoskeletal Systems and Their Diseases. Pathology and treatment for injuries of the tendon and ligament].

Tendons and ligaments are dense connective fibers mainly composed of type I collagen. However, the structure, function, and cellular behavior are different between tendons and ligaments. Anatomic, biological, and developmental studies have revealed the distinct differentiation process of tendons, ligaments, and these attachments to the bones (so-called entheses). In addition, arthroscopic devices, surgical materials, and operative techniques for the treatment of tendon/ligament injuries have been dramatically improved. Based on these backgrounds, the treatment strategies of tendon/ligament dis

High-resolution 3-T MRI of the fingers: review of anatomy and common tendon and ligament injuries.

OBJECTIVE. With high-resolution 3-T MRI, the complex anatomy of the fingers can be imaged in exquisite detail to provide an accurate diagnosis of clinically important ligament and tendon injuries. CONCLUSION. We present our 3-T MRI protocol using a dedicated hand-and-wrist coil and review normal MRI anatomy of the fingers. We emphasize a systematic approach to the interpretation of finger MRI examinations and illustrate this approach with examples of tendon and ligament abnormalities.

Suturing of the flexor digitorum profundus tendon to the entire volar plate in distal zone I injuries.

PURPOSE: The author conducted an anatomical and a prospective clinical study to address the technique and complications of using the entire volar plate (VP) as a distally-based flap for distal Zone I flexor digitorum profundus (FDP) tendon repair. METHODS: In the anatomical study, eight fresh finger specimens were dissected to study the anatomy of the VP as well as the stability of the distal interphalangeal joint (DIPJ) after raising the entire VP as a distally-based flap. In the clinical series, six patients underwent repair of distal Zone I FDP lacerations using the VP flap technique and w

Transient bone loss of distal radius and ulna following clean-cut tendon injuries, repair and passive mobilisation.

The aim of this study was to investigate whether there is any significant bone loss of the ulna and radius following acute tendon-artery-nerve clean-cut injuries at the wrist level which were repaired and rehabilitated by early passive mobilisation. Fifty-eight patients who underwent such operation were enrolled in this study. Patients in Group I (n=28) had primary tendon repairs alone, in Group II (n=15) primary tendon and nerve repairs and in Group III (n=15) primary tendon, nerve and artery repairs. Bone mineral density (BMD) measurements of the ulna and radius were obtained during the firs

Running Interlocking Horizontal Mattress-A New Technique versus Modified Kessler in Extensor Tendon Injuries: A Comparative Study.

Introduction  Pursuit for a new technique, which could be placed on flat extensor tendons and strong enough to withstand the tension forces of early mobilization to prevent stiffness, started few decades ago. We evaluated the new technique of tendon repair using running interlocking horizontal mattress (RIHM) sutures followed by early controlled motion protocol in traumatic extensor tendon injuries and compared it to modified Kessler technique. Materials and Methods  We conducted a prospective randomized interventional study of 18 months' duration, with 30 patients. A total of 103 tendons were

Imaging Evaluation of Medial and Lateral Elbow Pain: Acute and Chronic Tendon Injuries of the Humeral Epicondyles.

Medial and lateral elbow pain are often due to degenerative tendinosis and less commonly due to trauma. The involved structures include the flexor-pronator tendon origin in medial-sided pain and the extensor tendon origin in lateral-sided pain. Multimodality imaging is often obtained to verify the clinically suspected diagnosis, evaluate the extent of injury, and guide treatment decisions. Image-guided procedures can provide symptom relief to support physical therapy and also induce tendon healing. Surgical debridement and repair are typically performed in refractory cases, resulting in good t

Oblique incisions in hamstring tendon harvesting reduce iatrogenic injuries to the infrapatellar branch of the saphenous nerve.

PURPOSE: Iatrogenic injury to the infrapatellar branch of the saphenous nerve (IPBSN) is associated with many surgical interventions to the medial aspect of the knee, such as anterior cruciate ligament (ACL) reconstruction. Different types of surgical incisions during hamstring tendon harvesting for ACL reconstruction are related to a variable risk of IPBSN injury. This study aimed to evaluate the risk of iatrogenic IPBSN injury during hamstring tendon harvesting for ACL reconstruction with different incision techniques over the pes anserinus. METHODS: This study was performed on 100 cadavers

Use of platelet-rich plasma for patellar tendon and medial collateral ligament injuries: best current clinical practice.

Platelet-rich plasmas (PRPs) are complex molecular therapies prepared from the patient's own blood through minimal manipulation. Clinical studies examining the efficacy of PRPs to manage patellar tendinopathy and medial collateral ligament (MCL) injuries have been reviewed. We found three controlled trials, two of them randomized, and seven case series in the management of patellar tendinopathy. In addition, three other randomized studies showed that PRPs help to regenerate the patellar tendon harvest site for anterior cruciate ligament reconstruction and to reduce patellar donor site morbidit
